1 Glennon 101 Rotation Descriptions Department Pediatric Cardiothoracic Surgery Intern Experience Our Cardiology and Cardiothoracic Surgery program is ranked among the country s top pediatric heart programs in U.S News & World Report s Best Children s Hospitals. Our two cardiothoracic surgeons give us the most experienced team in the region. Faculty will offer a view of heart surgery from the inside. Interns will see firsthand how congenital heart disease is repaired with all the accompanying elements-- anesthesia, cardiopulmonary bypass, exposure of the heart defect and the subsequent repair. Pediatric Gastroenterology and Hepatology Our Gastroenterology and GI Surgery program is ranked among the country s top rated GI and Liver Disease programs in U.S News & World Report s Best Children s Hospitals. Interns will be introduced to the wide range of diseases this specialty encounters. This will include reviewing the health journeys of real patients, introductions to some of the specialized research underway at SSM Health Cardinal Glennon Children s Hospital, and a walk through of the real-life experience of a child undergoing endoscopy.

2 Pediatric Cardiology St. Louis Fetal Care Institute The Division of Pediatric Cardiology evaluates and treats patients with congenital and acquired heart conditions from infancy through young adulthood. The faculty provides expertise in the areas of outpatient cardiology, interventional cardiology, non-invasive imaging (echocardiography, CT, MRI), electrophysiology (cardiac rhythm disorders), cardiac transplantation, and inpatient/post-operative cardiac care. The intern experience may include any or all of the following at the participant s request: 1. Observing the outpatient clinic operations in the Dorothy and Larry Dallas Heart Center. 2. Observing echocardiographic (cardiac ultrasound) procedures and their interpretation by cardiologists. 3. Observing a cardiac catheterization procedure/intervention in the Pediatric Hybrid Cardiac Catheterization Lab, and/or an electrophysiology procedure in the EP Lab. The St. Louis Fetal Care Institute (FCI) is a partnership between SSM Health Cardinal Glennon Children s Hospital, SSM Health St. Mary s Hospital and Saint Louis University School of Medicine. Bringing together maternal-fetal medicine (MFM) specialists, pediatric and fetal surgeons, specialized nurses, technology, and a family-oriented approach, The Institute offers state-of-the-art diagnostic methods and treatment options for families whose unborn babies are facing medical challenges. The intern experience will include a tour of the FCI, review 2-3 case presentations of FCI patients that have undergone fetal surgery with the FCI staff, and possibly view a real-time demonstration of ultrasound.

3 Dan Dierdorf Emergency and Trauma Center, Access Center and Transport Team Pediatric Intensive Care Unit (PICU) As a state-of-the-art, high tech pediatric medical center, we are prepared for the most critical child health emergencies. The Dan Dierdorf Emergency and Trauma Center at SSM Health Cardinal Glennon Children s Hospital is designated a Level 1 Pediatric Trauma Center by Missouri and Illinois. All members of the trauma team are board-certified specialists in pediatric care. Our Access Center provides direct access for physicians to expert care. All calls are answered 24/7 by pediatric transport nurses. They handle physician consultations, emergent transfers, urgent appointments, and direct admissions. Our neonatal and pediatric transport teams have advanced training in the stabilization, treatment and transport of small infants and children of all ages. Our dedicated ambulance and helicopter, Cardinal Glennon Air are equipped with lifesaving equipment to help the most critical patients. Interns will receive an overview of the Emergency Department operations through the eyes of the patient. They will tour the waiting area, which includes our Safety Treehouse, helipad, ambulance bay with our transport team s ambulance, telemedicine room, trauma rooms and Access Center. This will be followed by a question and answer session with the Emergency Department and Access Center directors, as well as the Director for Disaster Preparedness. Spend time with the Intensive Care staff and a senior member of the Critical Care faculty. Learn about programmatic PICU goals, take a guided tour of our PICU, and interact with nursing, physician, respiratory therapy staff and our everpresent trainees. Interns may potentially listen to rounds, watch a patient admission or a minor procedure to gain insights into what goes on in a Pediatric Intensive Care Unit.

4 Pediatric Pulmonary Medicine The Division of Pulmonary Medicine cares for patients with a wide range of respiratory disorders unique to children. This includes children with recurrent or chronic cough or wheeze, asthma, pneumonia, exercise intolerance, disorders of sleep, and chronic lung diseases such as cystic fibrosis. The division also cares for children dependent on technology, i.e. children with tracheostomy tubes who are mechanically ventilated. Interns will have a review of airway anatomy, be acquainted with the many different techniques to assess lung function and evaluate the airway for the smallest premature infant to fully grown adolescents. They will have an opportunity to attend a procedure, review flexible bronchoscopy, and also visit the Sleep Laboratory to review techniques to assess normal and abnormal sleep. Pediatric Plastic Surgery/St. Louis Cleft- Craniofacial Center Pediatric plastic surgery improves the unique human qualities required for social interaction: appearance, speech, and hands. Each of these elements allows communication, which is essential to a child's psychosocial development. Cleft lip-nosepalate are the most common birth defects in the USA, 1 in 590 live births, and are part of the spectrum of pediatric plastic surgery care. Interns will learn about the spectrum of pediatric plastic surgery care, which includes cleft lip-nose-palate, craniosynostosis, external nose and ears and eyelids, jaw surgery, lesions, and trauma to the bones and soft tissues of the face and hands. Photos demonstrate these treatments, and if patients are available, there may be opportunities to observe surgery or meet some of our kids.

5 Pediatric Dermatology Child Life/Shining Star School SSM Health Cardinal Glennon Children s Hospital is one of few children's hospitals that supports full-time, on-site pediatric dermatology expertise for patients in the office, hospital and emergency room, without regards to ability to pay. The Division of Dermatology provides care for all forms of skinrelated problems such as cutaneous birth defects, vascular anomalies, genetic skin disorders, drug reactions, infections and infestations. Their expertise is nationally recognized in the evaluation and treatment of children with severe inflammatory skin diseases, including new drug development for eczema and psoriasis. Interns will have the opportunity to experience our daily routine, integrating clinical service, teaching and research. There will also be time set aside for either a didactic overview presentation or an informal discussion about related topics of special interest to each intern. Interns will spend time with a member of our Child Life team. Child Life Specialists work with patients and families in the hospital to advocate for their psychosocial, developmental, emotional and educational needs. Child Life Specialists strive to create a supportive environment, while helping empower patients and their families to make decisions affecting their care. Interns will see how our Child Life Specialist facilitate various interventions to meet the unique needs of patients in the hospital. Interns will also visit our Shining Star School. Participants will learn about the school s engaging programs and the various ways the school teachers and their volunteers interact with patients, their families, and schools to help children keep up with their school work, stay connected to their regular lives, and receive appropriate services from their schools.

6 Pediatric Endocrinology You will receive an overview of the Division of Endocrinology and its activities. Then, interns will discover what life is like for a child with newly-diagnosed diabetes. They will learn the new skills the family learns such as how to check blood sugars, how to give an insulin injection, and how to count grams of carbohydrate at meals. Subsequently, interns will walk to the ACC to join an office visit with a diabetic patient and/or for a demonstration of the new technology used in treating diabetes, including insulin pumps and glucose sensors. Pediatric Hematology and Oncology The Costas Center uses a comprehensive, team approach to diagnose and treat children with cancer and blood disorders. The outpatient clinic provides a comfortable setting for children to receive prolonged treatments (chemotherapy, transfusions, or other drug treatments) and minor procedures without having to be admitted to the hospital. 4 North is the inpatient unit, specializing in the delivery of comprehensive care to hospitalized Hematology and Oncology patients, and patients who are undergoing or have undergone stem cell transplants. Interns will be given a tour of both the Costas Center and 4 North, and have the opportunity to consult with some of the members of the multi-disciplinary team, including but not limited to physicians, nurse practitioners, staff nurses and Child Life to discuss their roles in the treatment of the patients that are served through both areas. Interns will have opportunity to observe patient care, talk with patients and families, or observe procedures, as available and appropriate such as port accesses, venous punctures or procedures under sedation.

7 Surgical Services The Dana Brown Neonatal Intensive Care Unit (NICU) In September 2007, SSM Health Cardinal Glennon Children s Hospital opened ten new operating rooms where our surgeons are equipped with the latest, state-of-the-art technology. Each 700-square foot OR allows for increased mobility with portable monitors and improved traffic flow with equipment booms that are attached to the ceiling, The suites are equipped with the latest surgical technology, specifically installed for minimally invasive procedures. Interns will see one of these rooms, and get a close-up look at equipment, humanitarian use devices, and flow of the surgical experience and sterile processing department (SPD). Our pediatric surgeons and OR staff will illustrate cost and application of this state-of-the-art technology, and provide a guided opportunity to view a surgical case. The Dana Brown Neonatal Intensive Care Unit (NICU) is recognized as a Neonatal Center of Excellence by the American Accreditation Healthcare Commission, and it is a regional referral center covering 150 mile radius in both Missouri and Illinois. Our 65 comfortable private rooms welcome patients and their families who need the help of the 18 board-certified neonatologists, and multidisciplinary teams of pediatric specialists. Babies with newborn illnesses cared for in the NICU include the extremely premature, babies with birth defects and other serious problems. A low ratio of patients to nurses ensures each patient receives continuous personalized care from dedicated, specially trained neonatology nurses. Interns will have the opportunity to tour the NICU and to round with a neonatologist and nurse, reviewing the diagnosis and treatment plan for two to three patients typically cared for in the NICU. Disciplines involved in the care of these patients will discuss their involvement with the care of the patients.

8 St. Louis Cord Blood Bank Integrated Care Team Social Services Pastoral Care Footprints Volunteer Services In 1994, SSM Health Cardinal Glennon Children s Hospital launched a new initiative to work with medical associates and the community at large to facilitate and encourage cord blood donation as a means of increasing the number and diversity of available stem cell donors. Using the expertise of obstetricians and the labor and delivery staff at 28 hospitals, more than 187,000 families have willingly donated their baby s cord blood after a healthy delivery to the St. Louis Cord Blood Bank, to save the life of another unrelated person. From this First Gift Program, nearly 2,800 of these products have been released for transplantation, domestically and internationally. Outcome data to evaluate the safety and efficacy of this treatment using cord blood has improved overall survival and is changing the standard of care in many institutions for both children and adult patients. Interns will learn about the public health mission of this cord blood donation program. They will tour our center, which is one of the country s premier cord blood manufacturing facilities, and will leave with an understanding of how the SLCBB has led the industry through licensure of this biologic drug. Following in the footsteps of the Franciscan Sisters of Mary and based on our Mission of serving the underserved in our community, the Integrated Care Team of SSM Health Cardinal Glennon Children s Hospital works to provide support to all families, meeting a multiplicity of needs at many levels. Interns will learn about service projects created by the team, including the recently formed Food for Families Program. Additionally, faculty will individually describe the nature of their support services, and current patient/family needs. Interns will be introduced to real-life circumstances that families face while facing serious diagnoses. The rotations will conclude with a brief visit to the chapel.

9 Therapy Services Child Protection In both Physical and Occupational Therapy we do intervention with children from newborn to 21 years of age in both an inpatient and outpatient setting. Within the PT Department, patients are seen with diagnoses head injuries, orthopedic problems, neurological issues and developmental delays. Treatment options include range of motion of the limbs, strengthening exercises, balance activities, gait training and a variety of developmental activities. Within the OT Department, patients are seen with diagnoses of head injuries, sensory issues, feeding problems, neurological issues and developmental delay. Treatment options include range of motion of the limbs, strengthening activities, feeding intervention, splinting and a variety of developmental activities. The Fostering Healthy Children Program is a new medical home for children who find themselves in the foster care system. The program is the first of its kind in Missouri serving children 12 and under and puts our region in line with recommended national standards for foster children. Staff help foster parents and case managers navigate our sometimes complex medical systems. The Child Protection Program is a team of health care providers dedicated to safeguarding our children. The program assists in the assessment of various forms of child maltreatment. Interns will have the opportunity to discuss the scope of abuse in our communities, the types of cases seen at SSM Cardinal Glennon, and the impact of abuse on the future health and development of our children. Interns will learn about reimbursement gaps inherent in our efforts to address abuse.

10 Pediatric Neurosurgery Complex Medical Care The skills and specialized training of our neurosurgery team allows them to successfully perform complex surgeries that others won t attempt. Interns will have the opportunity to observe a complex pediatric neurosurgical operation. Through observation and discussion with Dr. Elbabaa and his clinical nurse, interns will see state of the art technology and learn what is necessary to provide exceptional, comprehensive care related to the child s brain, spinal cord or central nervous system. The Complex Medical Care Program at SSM Health Cardinal Glennon Children s Hospital provides exceptional medical care, peace of mind and psychosocial empowerment for patients with complex medical care needs. Their role is to coordinate the medical care of patients with all aspects of their medical environment in the best interest of the patient and their family. Patients needing complex medical care have individualized patient care plans that are effective, constantly updated, easily accessible in multiple health care environments, by multiple practitioners, and in the home. Care is streamlined between the home, community outpatient care settings, hospitals and emergency rooms, St. Louis area long-term pediatric care facilities and SSM Health Cardinal Glennon. Interns will have the opportunity to meet the team members (physicians, nurse coordinators, social workers, etc.) that make up this incredible, dynamic team unique to Cardinal Glennon and walk through some of their cases with real families (if able) to illustrate the gift of coordination and advocacy this team provides.

11 Danis Pediatric Center Danis Pediatric Center (DPC) is the academic primary care practice of the Department of Pediatrics at Saint Louis University (SLU) School of Medicine, serving as the medical home for approximately 8,800 patients with 16,000 visits per year. It is the principal site for teaching SLU medical students and residents pediatric primary care. The expertise at DPC is providing general pediatric care and psychosocial support for patients and their families with complex medical and/or social problems. Interns will shadow with clinic leaders and learn about the clinic s vision, mission, and multidisciplinary team of professionals. They will join physician staff members and pediatric nurse practitioners in navigating a clinic schedule representative of a "Day in the Life at Danis Pediatric Center." Interns will work with a full team of providers to assist patients and families meet all of their medical and social needs. Working through a simulated patient list, interns will meet a variety of physician and ancillary staff at DPC, and learn about the array of programs and services provided. After completing the patient vignettes, interns will meet for a debriefing with clinic leadership and review the clinical, educational, service, and research impact of staff and services provided at DPC. This will include a discussion of the future plans for continuing the mission and growing DPC into the forefront as a leader in providing high-quality, family-centered pediatric primary care for medically complex children.
