1 24/7 NurseLine Always here for you Health concerns can happen when you least expect them. You might be on vacation or even on a business trip. Or your child may have a fever in the middle of the night. But there s somewhere you can turn for help any time of the day or night. Call the 24/7 NurseLine to talk with a registered nurse about your health concern. Whether it s a question about allergies, fever, types of preventive care or any other topic, nurses are always there to provide support and peace of mind. And, if you want, a nurse will call you later to see how you re doing. Our nurses can help you choose the right place for care if your doctor isn t available and you aren t sure what to do. Do you need to head straight to the emergency room? Is urgent care best? Or do you need to see your doctor? Making the right call can save you time and money and give you access to the best possible care. Do you speak Spanish or another language other than English? We have Spanish-speaking nurses and translators on call. TTY/TDD services are available, too. If you d prefer not to talk about your health concern over the phone, the AudioHealth Library might be for you. These helpful prerecorded messages cover more than 300 health topics in English and Spanish. Just call the 24/7 NurseLine number and choose the AudioHealth Library option. 2 Nine months. Many questions. Future Moms can help any time, any day Having a healthy baby is every mom s goal. And it starts with a healthy pregnancy. You want to make the right choices and take care of yourself so you can reach that goal. But it s not always easy to do it alone. That s why there s Future Moms. It s a program that can answer your questions, help you make good choices and follow your health care provider s plan of care. And it can help you have a safe delivery and a healthy child. Sign up as soon as you know you re pregnant. Just call us toll free at One of our registered nurses will help you get started. You ll get: A toll-free number you can use to talk to a nurse coach any time, any day, about your pregnancy. A nurse may also call you from time to time to see how you re doing. A book that shows changes you can expect for you and your baby during the next nine months. A screening to check your health risk for depression or early delivery. Other useful tools to help you, your doctor and your Future Moms nurse keep track of your pregnancy and help you make healthier choices. Free phone calls with pharmacists, nutritionists and other specialists, if needed. A booklet with tips to help keep you and your new baby safe and well. 3 MyHealth Advantage Helping you stay healthy and save money We go through medical histories, pharmacy claims and doctor visits, and then connect the dots to find ways to help you avoid health problems, stay healthy or save money. If we find something you could do to improve your health, you ll get a MyHealth Note in the mail. MyHealth Notes have information on: Tips to save money. We ll tell you when you can save money on your prescription drugs and other health care services. For example, if you switch from a brand-name drug to a generic one, you often pay less. Prescription drugs. If we find that you have prescriptions for two drugs that you shouldn t take at the same time, we ll let your doctor know. And, if it s time for you to get a refill, we ll remind you. Checkups, tests and exams. If it s time for you to have an exam or test, we ll remind you to make an appointment. Recent claims and prescriptions. This easy-to-read summary of your recent medical and pharmacy claims is great information to share with your doctors. Coming soon to a mailbox near you! ConditionCare is a program that gives you resources and tools to help you take care of certain health conditions. When you enroll in ConditionCare, you get: 24/7 toll-free access to a nurse coach who can answer questions about your condition. A health screening and follow-up calls to help you reach personal health goals. Educational guides, newsletters, tips and tools on how to take care of your health. Which health conditions does ConditionCare address? The program aims to help members with: Asthma (pediatric or adult). Chronic Obstructive Pulmonary Disease (COPD). Coronary Artery Disease (CAD). Diabetes, types 1 and 2 (pediatric or adult). Heart Failure (HF). Can my spouse and family members join? If they re covered by your health plan and have any of the conditions above, they can join. Call us and we ll check if they can join. How much does it cost to join ConditionCare? It s free because ConditionCare is already part of your health plan. 6 How do I sign up? Call us toll-free at We ll verify your identity, talk about your condition and ask if you want to sign up for the program. We might also call you fi rst to find out if you or your eligible family members want to join. What happens once I sign up for ConditionCare? Our ConditionCare staff will ask you some questions about your health. We ll use your answers to help you reach your health goals and follow your doctor s care plan for you. You can take this health assessment at a time that s best for you. How is my doctor involved in ConditionCare? ConditionCare doesn t replace your doctor. Instead, our nurses work with your doctor to help you follow your care plan. That s why we make sure your doctor knows if you participate in ConditionCare. How long will I be in the program? You may stay in ConditionCare as long as you keep your health plan and while the program is offered. You won t have to sign up each year to participate. And you may stop taking part in the program any time. A ConditionCare nurse might call you from time to time to find out if you have questions and to update your care plan. What about protecting my privacy and health information? To protect your privacy, we first verify your identity by asking you to tell us your date of birth or address before speaking with you about your health. ConditionCare also follows all state and federal laws protecting your privacy and health information. Your conversations with ConditionCare nurses, your doctor and other health professionals are private. We ll only use the information you give us to help you follow your customized care plan.
