3 Welcome! You may already know what it takes to be healthier. But knowing what to do and doing it are two different things. It s the doing it part that can be hard. We all struggle with that. But if you are willing to do 4 simple steps to earn 4 healthy points, your rewards will be great! You ll be on track to better health. 4 How It Works Just follow these 4 easy steps: 1. Make an appointment for a checkup with the doctor. Your doctor s name and phone number are on your Anthem member ID card. 2. Bring the Healthy Returns 4 Healthy Choices Care Plan (see inside back page) with you to the doctor. Ask your doctor to review the Care Plan with you. Your doctor will check off the best choices for you to complete to earn points. Have your doctor fax a copy of your completed Care Plan to Anthem. The fax number is listed on the Care Plan.

5 3. Earn a total of 4 points. You can earn points in many ways. The points you can earn for each choice are listed on the next page and on the Care Plan. You don t need to report your points earned to Anthem. We will track your points for you. Use your care plan to keep track of the points you earn. When you have earned at least 4 points, you can submit the form to get your gift. 4. Once you ve earned 4 points, choose your Healthy Returns gift! Mark your choice on the tear-out mailer included in this booklet. Be sure to complete the mailer. You must do this to receive your gift. Detach the mailer, fold it in half, and tape it closed so that the Anthem address shows on the outside. Drop it in the mail. The postage is already paid. You should receive your reward within 4 weeks.

6 Getting Started Ready to enroll? This is all you need to do: Begin by making an appointment to see your doctor. (Parents of children enrolling in the program must make an appointment with their child s doctor.) Bring the attached Healthy Returns 4 Healthy Choices Care Plan (see inside back page) to your visit. How to earn points: To get your free gift, you must earn a total of at least 4 points. You earn points when you: See your doctor for a checkup and tests. (up to 2 points) Visit your doctor. Ask your doctor to complete the Care Plan for you or your child. Your doctor will make sure that you or your child is enrolled in the program by faxing the Care Plan to Anthem. (1 point) Get the tests or screenings that your doctor says you need. Your doctor will let us know that you have received these. (1 point) Attend a health education class. (2 points) Call Anthem at (800) Members with hearing loss or speech impairment may call the TTY line at (800) We will enroll you in a local health education class that meets your needs. We will help you find classes on diabetes, heart care, managing your weight, and how to quit smoking. If you are a parent of an enrolled child, you may need to attend the class as well. We will confirm that you have completed the class so that you can earn your points.

7 Call MedCall to learn more about making healthy choices. (1 point) Phone MedCall, the 24-hour nurse help line. The number is (866) Members with hearing loss or speech impairment may call the TTY line at (800) Tell the nurse on the phone: I m an Anthem member enrolled in Healthy Returns 4 Healthy Choices. I m calling to learn about MedCall services I can use. The MedCall nurse will talk to you about MedCall services. The nurse will connect you to an audiotape you can listen to. MedCall has audiotapes on diabetes, heart care, eating right, watching your weight, or quitting smoking. MedCall will let Anthem know that you have called, so you can earn your point. Call Anthem and request a Quit Kit to stop smoking. (1 point) If you smoke and are ready to quit, call us at (800) Members with hearing loss or speech impairment may call the TTY line at (800) Ask for a Quit Kit. It s the first step to a smoke-free you! Anthem will confirm your request for a Quit Kit, so you can earn your point. Complete the chosen action items on your Care Plan. You should have a total of at least 4 earned points. The next step is to choose your gift! See the next page to make your choice.

After you attend 4 meetings and follow the instructions sent with the vouchers, Anthem will send you 4 more meeting vouchers.
