1 Organizational Overview TRANSFORMATIONAL LEADERSHIP OO4 The administrative and nursing organizational chart(s). Administrative Organization Chart o Link to Exhibit OO4.1 Medical Center Organizational Chart Nursing Organizational Chart o Link to Exhibit OO4.2 Nursing Organizational Chart Structural Relationships Advocate BroMenn Medical Center (ABMC) is a not-for-profit, acute care 221 bed, Level II Trauma Center located in Normal, Illinois. Medical services cover a wide variety of acute care, outpatient, rehabilitative, and preventative health care needs. ABMC s organizational structure is organized to enable the Chief Nursing Executive (CNE) to influence decisions and organizational change throughout the organization. Laurie Round, MS, BSN, RN, NEA-BC, of Patient Services and CNE, works in strong collaboration with the Executive Leadership Team. The CNE is one of six s who report directly to the President, Colleen Kannaday, MS, FACHE, within the organization. Laurie has a matrix reporting relationship to Susan Campbell, DNP, RN, NE-BC, Advocate Health Care s (AHC) Senior and Chief Nursing Officer. Laurie direct reports are s who are responsible for Inpatient Medical/Surgical, Nursing Practice, Behavioral Health, Critical Care Services, Perioperative Services, Quality Management and Pharmacy Services at ABMC. Laurie has Full-Time Equivalents (FTEs) within her area of responsibility. Laurie s area of responsibility includes the Mother Baby Unit, Pediatrics/Outpatient Infusion, Acute Rehabilitation Unit, Intensive Care Unit, Cardiovascular Care Unit, Central Telemetry, Emergency Department, Emergency Medical Services, Medical Oncology Specialty Unit, 6 West Surgical Unit, Progressive Care Unit, Palliative Care, Mental Health Unit, Perioperative Services, Central Services, Quality Resource Management, Regulatory Compliance, Infection Control, Care Management, Social Work, Nursing Operations, Clinical Administration, Nursing Resource Pool, Nursing Administration, Diabetes Education, Wound/Ostomy, Magnet Program, Pharmacy and Therapy Services. Risk Management, Patient Safety, and Clinical Informatics have a matrix reporting relationship with Laurie and direct reporting relationships within the AHC system structure. Clinical Education, Radiology Nursing and Wound Healing Nursing have a matrix reporting relationship to the of Nursing Practice, who reports to the CNE. OO4 Advocate BroMenn Medical Center 1
2 Cardiovascular Services has a matrix reporting relationship with Laurie and a direct reporting relationship to the of Operations. Laurie provides clinical leadership to all areas of inpatient and outpatient nursing practice at ABMC. She is responsible for organizing, planning and directing clinical operations in accordance with system and medical center strategic and tactical plans, missions and values. Laurie ensures strategic direction, to deliver high quality, state of the art patient care. Laurie is responsible for maintaining efficient and effective use of human resources, financial resources related to clinical operations and overseeing AHC Key Result Areas which are the areas of focus for the organization. Operational Relationships Operational relationships that allow the CNE to influence organizational decisions and change include: Executive Leadership Team meetings, chaired by the President and attended by the of Medical Management, of Operations, Vice President of Business Development, of Development in the Office of Charitable Giving, of Human Resources, and the Vice President of Finance Medical Executive Committee meetings Governing Council meetings Nursing leadership (Board) meetings Nursing Practice Council meetings To ensure Laurie has the information needed to oversee nursing practice throughout the organization and influence change, she regularly leads or participates in meetings and workgroups within the AHC system, medical center, and community. With her noteworthy leadership abilities, Laurie has enabled positive changes for the entire medical center. She has done so by being a supportive leader and effective change agent for nursing. Her success in improving patient safety is based on the strong partnerships with the medical staff, medical center associates and community members and partners. As CNE, Laurie leads or serves as a member of the following teams and committees: Hospital Governing Council The council meets every other month. Laurie represents nursing on this council as a non-voting member. Executive Leadership Team (ET) meets weekly to discuss operational and strategic progress. The meeting is led by ABMC President, Colleen Kannaday. Decisions are made during these meetings that provide direction to the organization. ET plans and discusses safety, risk, quality and clinical reports and approves and reviews financial budgets, targets and performance. The team determines direction of hospital-wide culture and makes decisions related to community initiatives and activities. Organizational changes influenced by the CNE, as an ET member, include additional OO4 Advocate BroMenn Medical Center 2
3 FTE s to place pharmacists in the Emergency Department to complete the medication reconciliation process for all admitted patients, the establishment of a nursing resource office to build a flexible workforce, a nursing salary compression response based on market data and analysis, and using the voice of nurses and nurse leaders to identify and obtain needed resources. Chief Nurse Executive Council (CNEC) is an AHC Committee comprised of all AHC Nursing s and CNE s. The council makes system-wide decisions that impact nursing and develops the strategic direction for nursing. For example, the council agreed to evaluate the role and scope of advance practice nurses (APNs) and ensure each hospital was fully utilizing the education, training and scope of the APN role as allowed by law. Laurie, with the support of the ET, worked with the Medical Staff Credentialing Committee to change the ABMC Medical Staff Bylaws to allow APN s to practice in the clinical setting at ABMC. In August of 2014, in a collaborative agreement with McLean County Neurology, Victoria Steinkoenig, APN was granted privileges as an Inpatient Neurology APN, working collegially with physicians, to diagnose and treat patients with acute and chronic illness, and to prescribe medications and treatment. Medical Executive Committee The committee consists of the chairs of each Medical Staff Department. Laurie represents nursing on this committee. Medical Staff Committees - Laurie is the ET representative at the Department of Obstetrics, Pediatrics, Emergency Services, and Pharmacy and Therapeutics Committees. Nursing Meeting- Laurie meets weekly with the nursing directors and the of Pharmacy to: Ensure a collaborative approach to providing one single standard of practice for all areas where nursing is practiced Support the Professional Practice Model and the care delivery model Update on organizational and system-wide initiatives supporting standardization of best practices throughout AHC nursing Collaboratively lead and integrate nursing professional practice to meet ABMC nursing strategic initiatives Clinical Leadership Meeting Laurie serves as chairperson. This meeting is attended by managers, directors, Advanced Practice Nurses, Infection Preventionists, Regulatory Compliance, Risk Management, Patient Safety, Pharmacy, Laboratory, Radiology, and Hospitality/Nutrition Services. The team meets monthly to: Evaluate progress toward reaching nursing and organizational goals and objectives Provide consistent communication of information impacting nursing practice and to share best practices Discuss changes in healthcare, both internal and external to facilitate innovative ideas and to manage change proactively OO4 Advocate BroMenn Medical Center 3
4 The following reports are reviewed as part of the agenda: National Database of Nursing Quality Indicators (NDNQI) Quality and Safety reports Patient Satisfaction and Associate Engagement Quality Management Oversight Council and Patient Safety Committees - Laurie is the co-chair and voting member with James Nevin, Jr, MD, CPE, FACEP, Vice President of Medical Management. Patient Safety Committee Clinical disciplines meet monthly to collaborate, prioritize and implement the safety initiatives of the medical center based on safety event reporting and cause analysis data. The committee provides support and resources to proactively reduce risks to patient safety and avoid system failures. The primary focus is the achievement of AHC s patient safety metrics, included in the Balanced Scorecard and the Safety & Quality Close report. The medical center safety metrics are reviewed on a monthly basis to trend progress toward goal achievement. All metrics are reported and aligned to the Quality Management Oversight Council. Quality Management Oversight Council (QMOC) - QMOC coordinates required monitoring, clinical process improvement, patient safety, and quality reporting activities in alignment with the AHC system mission, vision, philosophy and goals. The purposes of the QMOC are to develop, innovate, implement and support best health outcomes and best patient experience across the medical center by: 1. Hardwiring best practices 2. Identifying and responding to shared departmental issues and opportunities 3. Creating efficiencies and cost-savings 4. Ensuring the appropriate oversight of results Strategic Capital Committee - Laurie is a member of the medical center s Strategic Capital Committee. The committee is responsible for prioritizing allocations for capital equipment based on annual allowances. The committee meets quarterly to discuss needs brought forth by various service lines, the nursing division and facilities. The Capital Committee is comprised of select executive leadership and is chaired by the of Finance. ABMC has invested in significant capital projects to update equipment, expand services and improve the infrastructure of the facility which support nursing and care delivery. In a healthcare environment with competing priorities, the CNE ensures that nursing capital needs are taken into consideration for the provision of patient care. Nursing Practice Council Laurie is a member of the Nursing Practice Council. The Council meets monthly and is comprised of clinical nurses, who represent their units, and nurses in supportive roles. The Council is co-chaired by the of Nursing OO4 Advocate BroMenn Medical Center 4
5 Practice and a clinical nurse. The monthly agenda consists of reports on nursing initiatives, shared governance projects, quality measures, and provides a forum for nursing to bring forward their recommendations for new initiatives, projects and concerns. The CNE Advisory Group Laurie established this group to provide her with valuable feedback. The group is comprised of front-line nursing associates who were chosen from past Nursing Excellence Award recipients. The group represents diverse areas of nursing practice and meets quarterly with Laurie to discuss issues relevant to ABMC nursing. The CNE Advisory Group does not replace Shared Governance or CNE rounding on clinical units, but instead provides additional devoted time to hear from clinical nursing associates regarding their perspective of the work environment and nursing practice. Building relationships with clinical nursing associates in an informal setting affords Laurie the opportunity to gain insight into the clinical milieu, resolve issues, and provide support and mentorship to nurses who are daily at the point of care delivery. Meeting with nurses from varied areas of the organization encourages collaboration and communication among the nursing associates. Candid and transparent communication creates a safe venue for ideas and advisement, as well as fostering a workplace where nurses feel respected and valued. Topics of discussion vary based upon feedback from the group members. Advocate BroMenn Annual Nursing Summit Laurie is the organizer and leader of this educational and inspirational event for nurses at ABMC to gather and make collaborative decisions related to organizational nursing direction. During the 2015 Summit, the importance of the Shared Governance structure was reinforced and ongoing learning and preparations related the Magnet journey were discussed.
