2 I. Purpose Employers who will respond to releases or threats of releases of hazardous chemicals must have a written plan prepared according to OSHA Standard 29 CFR (Q). The purpose of this document is to guide an employer through the preparation of a written plan. The attached fill-in-the-blank plan is not intended to be a complete representation of OSHA's requirements for emergency response. The employer will be required to develop the procedures and policies necessary for a specific location. ELB and Associates is available to assist with the development of these policies and procedures. II. Contents At a minimum, the following components must be included in an emergency response plan: A. Pre-emergency planning with outside parties (e.g. Highway Patrol, Paramedics, HazMat Units) B. Personnel roles, lines of authority, training, communication (e.g. Chief-in-command, delegation sequence, communication interferences) C. Emergency recognition and prevention (e.g. Principles of first reaction) D. Safe distances, places of refuge (e.g. Location, safety, distance, upstream vs. downstream) E. Site security and control (e.g. Strikes, storms, sabotage) F. Evacuation routes and procedures (e.g. Posted, updated, completed) G. Decontamination (e.g. Clean zone vs. Hot Zone, PPE, Emergency Vehicles) H. Emergency medical treatment and first aid (e.g. Availability, adequacy, accessibility) I. Critique of response and follow-up (e.g. Deficiencies, changes, frequency) J. PPE and emergency equipment (e.g. Types, accessibility, fit) K. Emergency Response (e.g. State Plan, Local Plan, SARA Title III) III. Personal Protective Equipment & Emergency Equipment Type Quantity Storage Location Inspected By Note: All emergency personal protective equipment and emergency equipment will be inspected monthly and a written record maintained. XIII. Procedures for Handling Emergency Response How an emergency situation is handled often is the key to preventing uncertain consequences. The senior emergency response official responding to an emergency should become the individual in charge of a site-specific Incident Command System (ICS). All emergency responders and their communications should be coordinated and controlled through the individual in charge of the ICS assisted by the senior official present for each employer. The ICS Commander must be capable of the following: Identify all hazardous substances or conditions present. Implement appropriate emergency operations as required. Assure employees exposed to inhalation hazard are wearing positive pressure SCBA. Limit quantity of individuals at emergency site and assure that the buddy system is implemented for responders. Maintain back-up rescue personnel and medical equipment to be readily accessible. Designate safety official. When necessary, implement appropriate decontamination procedures. A. Skilled Support Personnel At times, employers will find it necessary to immobilize certain skilled support personnel. These individuals may or may not be the employer's employees. If potentially exposed, these individuals must be given an initial briefing at the site prior to participation in any emergency response. This briefing should include instruction in the wearing of appropriate PPE, what chemical hazards are involved at the site and what duties are to be performed. 8 Contractor Name Address Telephone # Contact Service B. Specialist Employees In some occasions, employers may find it necessary to call upon specialist employees. Specialist employees are employees, who in the course of their regular job duties, work with and are trained in the hazards of specific hazardous substances, and who will be called upon to provide technical advice or assistance at a hazardous substance release incident to the individual in charge. Specialist employees must receive the proper training in handling the hazards or potential hazards, or demonstrate competency in the area of this specialization annually. The safety and health of these individuals must be assured by the employer. Training Employee training is a critical element to implementing a successful emergency response. The various roles offered by each designated individual play a significant part to the overall response by an organization. Training should be based on the duties and functions to be performed by each responder of an emergency response organization. The skill and knowledge levels required for all new responders, those hired after March 6, 1990, should be conveyed to them through training before they are permitted to take part in actual emergency operations on an incident. 9 Employees who participate, or are expected to participate, in emergency response, should be given training as follows: A. First Responder Awareness Level Individuals likely to witness or discover a hazardous substance release and who will initiate an emergency response sequence by notifying the proper authorities of the release. They notify authorities of the release. These individuals must demonstrate, at a minimum, competency in the following areas: 1. What hazardous materials are and their associated risks with them in an incident. 2. Potential outcomes in an emergency with hazardous materials. 3. Ability to recognize hazardous materials in an emergency. 4. Understand the role of the first responder and DOT Emergency Response Guidebook. 5. Ability to recognize need for additional resources and to notify communication center. B. First Responder Operations Level Individuals who respond to releases or potential releases of hazardous substances as part of the initial response to the site for the purpose of protecting nearly persons, property, or the environment from the effects of the release. These individuals serve defensive roles without actually trying to stop the release. Main function is to contain the release from a safe distance, keep it from spreading, and prevent exposure. Minimum 8 hours of training and/or sufficient experience. These individuals must demonstrate, at a minimum, competency in the following areas: 1. Basic hazard and risk assessment techniques. 2. Select and use PPE. 3. Basic hazardous material terms. 4. Basic central, containment, confinement operations. 5. Basic decontamination procedures. 6. Relevant Standard Operating Procedures (SOPS) and termination procedures. C. Hazardous Materials Technical Individuals who respond to releases on potential releases for the purpose of stopping the release. These individuals approach the point of release in order to plug, patch or stop the release of a hazardous substance. Minimum 24 hours of training. These individuals must demonstrate, at a minimum, competency in the following areas: 1. Implement emergency response plan of the employer. 2. Use field survey equipment to classify, identify, verify known and unknown hazardous material. 3. Function within the ICS. 4. Select and use specialized PPE. 5. Understand hazard and risk assessment techniques. 6. Advanced control, containment, confinement operations. 7. Understand decontamination procedures. 8. Understand termination procedures. 9. Understand basic chemical and toxicological terms and behavior. LC-1010 Rev. 06/16 Page 9
10 D. Hazardous Materials Specialist Individual who respond with and provide support to hazardous materials technicians. Requires a specific knowledge of the various substances involved with an incident. May require acting as a site liaison with regulations. Minimum 24 hours of training. These individuals must demonstrate, at a minimum, competency in the following areas: 1. Implement local emergency response plan. 2. Understand classification, identification and verification of known and unknown chemicals by advanced instruments. 3. State emergency response plan. 4. Select and use specialized chemical PPE. 5. Understand in-depth hazard and risk techniques. 6. Perform special control containment and confinement operations. 7. Determine and implement decontamination procedures. 8. Develop a site safety and health plan. 9. Understand a site safety and health plan. 10. Understand chemical, radiological, toxicological terms and behavior. E. On Scene Incident Commander Individual who assumes control of the incident. Minimum 24 hours of training. This individual must demonstrate, at a minimum, competency in the following areas: 1. Know and implement employer's incident command system. 2. Implement employer's emergency response plan. 3. Know and understand hazards of employees working in chemical PPE. 4. Know how to implement local emergency response plan. 5. Know of State emergency response plan and Federal Regional Response Team. 6. Know and understand decontamination procedures. XV. Refresher Training Required annually, and should be of sufficient content and duration to maintain competencies (approximately 8 hours). Training records should be retained by employer, including basis used to demonstrate competency. XVI. Medical Surveillance Members of response team should receive a baseline physical exam which is retained on file. In the case whereby employees exhibit signs or symptoms of exposure from a response activity involving a hazardous substance(s), additional surveillance and medical consultation is required. XVII. Post-Emergency Response Upon completion of the emergency response, if it is determined that it is necessary to remove hazardous substances, health hazards, and materials contaminated with them from the site of the incident, the employer must provide: Proper Employee Training Proper Equipment LC-1010 Rev. 06/16 Page 10
