3 Effective Date: 12/01/13 Page EMT - 2 EMERGENCY CONTROL CENTER (ECC) Purpose The ECC will be used to provide a facility for decision-making and expeditious use of resources and personnel to resolve the emergency condition as quickly as possible, in support of the Incident Commander. When an extensive emergency exists that involves a large part of the plant or which could affect the immediate community, the ECC will be activated. The ECC will be equipped with telephones and/or radio communications, as necessary, to expedite the transfer of information concerning the emergency. The ECC will have the Site s Emergency Action Plan and Emergency Checklists and other plans available, as necessary, for the emergency situation. Location Primary: Secondary: Conference Room in the North Wing of the Administrative Building BASF Port Arthur Admin 1 st Floor Conf. Rms. Responsibility For Activating The Emergency Manager or his alternate shall be responsible for activating the ECC. All Emergency Management Team Representatives (or their alternates) will convene upon activation of the ECC.
4 Effective Date: 12/01/13 Page EMT - 3 EMERGENCY SITE ADMINISTRATIVE CENTER In the event that the APT/A Director of Manufacturing or AP Sr. V-President of Global Technology declares a Corporate Crisis, the site will establish the Administrative Center in the Site Manager s Conference Room. The Administrative Center will track activity and developments at the site and report these to the Crisis Operations Center in Florham Park per the BASF Corporation Crisis Management Plan, via the following forms: Incident Report: ASAP (who, what, when, where) Situation Report: every two (2) hours or as directed Media Contact Report: per occurrence Government Agency or Regulatory Contact Report: per occurrence Casualty Report: ASAP These forms are located in Appendix C.
5 Effective Date: 12/01/13 Page EMT - 4 EMERGENCY MANAGEMENT TEAM (EMT) ORGANIZATION This group will assume emergency responsibilities when it becomes necessary to activate the EAP. The EAP can be activated by: The gas release or spill being a Level 1, 2 or 3 A major fire or explosion. An incident involving multiple injuries. The Emergency Management Team may use three (3) different organizational structures to handle any and all Beaumont site emergencies. Organization I Declared by Incident Commander Emergency that may or may not have off-site impact to surrounding neighbors and can be mitigated by one or all ER Team(s) within the normal work schedule with no additional resources required. Organization II Declared by Emergency Manager Emergency that may or may not have off-site impact to surrounding neighbors and can be mitigated by one or all ER Team(s) which would require the need for additional resources and time to complete the emergency response outside of the normal work schedule. Organization III Declared by Director of Manufacturing or Sr. VP Emergency that may or may not have off-site impact to surrounding neighbors and can be mitigated by one or all ER Team(s) which would require the need for additional resources and time to complete the emergency response outside of the normal work schedule. 8 Effective Date: 12/01/13 Page EMT - 7 INCIDENT COMMANDER Roles (See Page EMT 13 and 14) Safety Officer The Safety Officer is responsible for assuring the health and safety of all emergency personnel by coordinating monitoring and assessing the operating strategy, mode of attack and incident tactical priorities. The Safety Officer has the authority to temporarily bypass the chain of command when it is necessary to correct unsafe acts immediately, such as removing all personnel from areas of imminent danger. Identify hazardous situations associated with the incident Communicate as necessary with the Incident Commander & Operations Chief Monitor and assess hazardous situations and make safety related recommendations to the IC Develop measures for assuring personnel safety Review and provide input into incident action plans as necessary Exercise authority to stop and prevent unsafe acts as necessary Investigate accidents that have occurred within incident areas Continually evaluate the situation Maintain awareness of the emergency response Strategy and Tactics being employed on the emergency scene. Conduct safety briefing and instruct response personnel on hazards and safety procedures Identify hazardous situations associated with the incident and develop incident specific safety plan for approval by the Incident Commander Ensure that only those resources required for Tactical operations, including personnel and equipment, are within the incident area Ensure personnel accountability is in place for those deployed to the emergency site; including the responders used for Rescue and Backup Exercise emergency authority to remedy, stop, or prevent unsafe acts Participate in planning meetings Develop listing of and ensure compliance with OSHA and other safety requirements Identify and ensure compliance with individual personnel training requirements as applied to safety regulatory issues Review decontamination procedures for safety and effectiveness Determine level of Personal Protective Equipment required for personnel at the incident Identify required on-site Personal Protective Equipment and clothing Investigate accidents that have occurred within incident areas Request industrial hygiene monitoring as may be required for the protection of personnel at incident. Report results to IC Ensure that buddy systems standby crews and backup systems are provided for hazardous entry situations Ensure appropriate public health sanitation standards are met Maintain an incident log
9 Effective Date: 12/01/13 Page EMT - 8 Operations Chief The Operations Chief is responsible for the management of all tactical operations directly applicable to the primary mission. The Operations Chief activates and supervises organizational elements in accordance with the action plan and directs its execution. Also, directs the preparation of unit operational plans, establishes tactical priorities, requests or releases resources, modifies the action plan as necessary, and reports changes to the Incident Commander. The following are additional duties: Obtain a briefing from the IC Continually evaluate the situation. Maintain direct involvement of the emergency response Strategy and Tactics being employed on the emergency scene through the Tactical Unit, Branch, Division and Group Leaders Activates, organizes and manages the necessary Operations Sections units (Fire, HazMat, Rescue, Medical, etc.) Brief and assign operations personnel in accordance with the action plan Notify Staging and Logistics of names and locations of Operations personnel Ensure that only those resources required for Tactical operations, including personnel and equipment, are within the incident area Coordinates emergency countermeasures with the process control activities by working closely with the Planning Assigns emergency response personnel and supervises incident operations. Establish and maintain personnel accountability of those deployed to the emergency site; including the responders used for Rescue and Backup Reassign initial response personnel as appropriate Evaluate effectiveness of current actions and request additional resources as needed. Operates within the Tactical Priorities set by command Report information about special activities, events, and occurrences to the Incident Commander. Updates IC periodically on situation and progress Coordinate with key personnel during any transition phase for the orderly transfer of resources and operations activities Identify resources not needed for operations during demobilization Maintain an incident log

All team members may or may not be needed, depending on the nature and magnitude of the incident. Some functions may be combined. 14 Effective Date: 12/01/13 Page EMT - 9 EMERGENCY MANAGER CHECK SHEET EMERGENCY CONDITION (Page 1 of 2) Inform the Incident Commander that the Emergency Communications Center is in place. Relay message to all sectors. Maintain contact with the Incident Commander. Ascertain the nature and extent of the emergency. Notify Communications, if necessary, to initiate Emergency Management Team notification Check on status of personnel - headcount and possible injuries. Notify appropriate off-site BASF contacts when necessary. May also be used to activate the Crisis Management Team. Evaluate impact of emergency on other plant facilities and direct actions, as deemed necessary. Activate family notification procedures, if necessary. Approve news releases or direct approval for news releases for Media appropriately. Evaluate impact on community and direct actions, as deemed necessary. 10. Ensure media and LEPC are contacted appropriately. 11. Have government agencies been notified as required? (NRC, TCEQ, LEPC, OSHA, TRC) 12. Initiate Emergency Follow-Up Checklist.
15 Effective Date: 12/01/13 Page EMT - 10 EMERGENCY MANAGER CHECK SHEET EMERGENCY FOLLOW-UP (Page 2 of 2) Evaluate extent of damage and approve plans for repairs that are needed. Form Incident Investigation Team. Appoint Team Leader. Participate in the investigation and critique of the emergency for corrective actions. Determine status of personnel affected by the emergency. Update the appropriate off-site management. Review the impact of the emergency with Production Planning. Approve plans for start-up of plants that were affected by the emergency. Have department head review and document the following: A. Impact of emergency on plant(s) B. Personnel affected by emergency C. Effectiveness of Emergency Action Plan D. Recommendations for improvement in EAP. E. Recommendations for eliminating the recurrence of the emergency situation 9. Review community impact and document. A. Injuries and/or illness B. Property damage C. Effectiveness of Community Alert D. Needs as related to the incident E. Other concerns as related to the incident 10. Conduct debriefing on the functioning of the ECC, IC and the EAP 11. Check with Communication / Media or Human Resources on communications with community leaders, media, and others as appropriate.
16 Effective Date: 12/01/13 Page EMT - 11 SITE EMERGENCY ADMINISTRATIVE COORDINATOR RESPONSIBILITIES Responsibility In the event that the APT/A Director of Manufacturing or AP Sr. VP of Global Technology declares a Corporate Crisis, communicate with the BASF Crisis Ops Center (COC). The Manufacturing Support Team Leader is designated as the Emergency Administrative. Actions Provide COC with the name and telephone number of the Emergency Manager. Submit Standard Reports at scheduled intervals: Incident Report: ASAP (who, what, when, where) Situation Report: every two (2) hours or as directed Media Contact Report: per occurrence Government Agency or Regulatory Contact Report: per occurrence Casualty Report: ASAP Receive information from COC via standard forms. Transfer all crisis related phone calls to the Crisis Phone Center in Mt. Olive ( )
17 Effective Date: 12/01/13 Page EMT - 12 INCIDENT COMMANDER RESPONSIBILITIES Responsibility Actions A. The Incident Commander is designated by the EAP as a qualified BASF employee that has received required HAZWOPER training to assume the role and responsibilities for Incident Command. Beaumont TRACER team members may also be established as Incident Commander. B. Directs all activities in the immediate vicinity of the emergency. C. Directs the on-site emergency responders, contacted ER, Emergency Response team, and supervisor of the affected area (if not the Incident Commander) are under the direction of the Incident Commander. A. Establish command post to direct and coordinate all actions within the affected area. B. Designate a safety officer who is knowledgeable in the operations being implemented at the emergency response site. Safety officer s responsible for identifying and evaluating hazards with respect to operations for the emergency in progress. C. Relay information to Emergency Manager in respect to support needed and information for the media or regulatory agencies. D. Work with the Emergency Manager and his team. E. Direct off-site emergency response services (medical, fire, mutual aid, contractors, etc.).
18 Effective Date: 12/01/13 Page EMT - 13 INCIDENT COMMANDER CHECK LIST This checklist is intended as an aid for the Field IC and not requirement for all Emergencies. The Field IC should only check off what is appropriate for the type and Level of Emergency Response needed. 1. Determine the problem and chemicals involved. 2. Set up Command Post 3. Select Officers (Safety Officer min.) i. Gather information on chemicals involved, MSDS, ERG book, NIOSH book ii. Complete Emergency Response Plan 1. Proper PPE 2. Physical Hazards 3. Communications 4. Select Operations Chief if deemed necessary (incident requires multiple ER Teams or is a Level III incident) i. Contact Emergency Response (Andrew Garcia) ii. Contact EHS Representative on Call 5. Select a person as a scribe. 6. Contact Emergency Manger (Site Manger or Designee) 7. Select radio channel (I/C) and communicate to rest of team. 8. Select radio channel for appropriate Teams needed to mitigate the emergency. 9. Select a person to be Logistics if possible. 10. Make sure all personnel are alerted and accounted for on site. (5222 enacted and alarms sounded) 11. Restrict or barricade affected area. (contact security for assistance) 12. Select someone to call Industrial/Commercial neighbors (if alarm is sounded) and regulatory agencies as needed. 13. Determine the impact on the site as well as off site. 14. Determine if any outside assistance will be needed or any special equipment needed. 15. Determine what monitoring will be needed and what meters will be needed. 16. Determine course of action to take care of the problem (plan) and path to get to problem. 17. Check qualifications of all responders for the task they are assigned to carry out. 18. Rescue/Medical Team i. Confined Space and/or High Angle ii. Notified Hospital and/or Company Doctor iii. If needed set up Triage location for injured personnel (Logistics to coordinate)
19 Effective Date: 12/01/13 Page EMT - 14 INCIDENT COMMANDER CHECK LIST 19. Fire Brigade i. Determine Offensive or Defensive response needed ii. Additional resources needed 20. HAZMAT Response Team: i. Entry team (min. of two people) ii. Back up team (min. of two people) iii. Decon team (same or one level lower PPE required) 21. Emergency Response Plan must be establish before sending anyone in Hot zone. 22. Ops Chief should go over the Plan with teams and make sure everyone understands their jobs. 23. Safety should go over all the hazards and safety guidelines with the teams. 24. Ops chief to suit up teams in proper PPE at this point. 25. Safety or some one should keep tract of: i. Who is on air and time they went on air. ii. Who enters hot zone and time they entered / exited. 26. Logistics should have extra SCBA bottles and drinking water set up at this time. 27. Team to establish Hot and Warm zones by monitoring. 28. Ops Chief to have Decon Team set up before entry. 29. If needed have someone in proper PPE monitor battery limits of site (Upwind for baseline & Downwind). 30. OK for ER Team to enter Hot Zone to mitigate the problem. 31. Decon of all entry members and equipment completed before leaving warm zone. 32. Monitoring to be done to determine no hazards still exists (can be done by backup team). 33. Account for all response members. 34. End Emergency Response and sound all clear signal. 35. If needed send entry team members as well as any other possible exposed persons to Dr. Hamby s office for chemical exposure concerns. 36. Determine Reportable Quantity (RQ) of chemical and if needed notify agencies. 37. Debrief all team members. 38. Conduct a critique of the response with all members and identify deficiencies and corrective actions. Document meeting. 39. Have all response equipment re-serviced, inspected and returned to proper locations
20 Effective Date: 12/01/13 Page EMT - 15 HEALTH & SAFETY COORDINATOR RESPONSIBILITIES Responsibility Assist Incident Commander with hazards and safe operations associated with the type of Emergency Response. May act as Safety Officer or other rolls associated with I/C structure, Coordinate activities related to injured personnel. Coordinate Health and Safety needs between EMT structure and Field I/C structure to ensure the safety of all personnel and surrounding community has been addressed. Actions A. Identify and evaluate hazards with respect to safe operations for emergency in progress. B. Assist in determining appropriate level of PPE for responders. C. Coordinate / respond to Industrial Hygiene needs of the Incident Commander. D. Work directly with Emergency Manger under organization II or III of the EMT structure. E. Coordinate provisions for evacuation of injured personnel to the appropriate medical facilities. F. Communicate with doctors and hospitals to determine condition of injured. G. Compile a list of injured and/or casualties.

Could serve as Operations Chief or other rolls associated with I/C structure. Actions A. Work directly with Incident Commander B. Assist Operations Chief with duties (if not assigned this position) C. Assist in directing actions of on-site Fire Brigade. D. Assist in directing actions of on-site Rescue team. E. Assist in directing actions of on-site HAZMAT team F. Determine the need for assistance from Sabine Neches Chief's Association. G. Coordinate outside resources (S.N.C.A., vendors, and/or contractors) for emergency response.

Serve as communication link for the Emergency Manager, as directed. B. Notify families of injuries/casualties. C. Notify the family minister in case of casualties. D. Notify Corporate Public Relations. E. Assign personnel to switchboard to help Security personnel during the emergency F. Dispatch community contact teams as needed: People Exposure: Team should have information on the material that could have caused the exposure. Property Damage: Team should have the ability to recognize and access property damage in general terms. G. Assure that the STAN messaging system is updated as needed with the current information that may impact and/or concern the surrounding community. H. 23 Effective Date: 12/01/13 Page EMT - 18 MEDIA COORDINATOR RESPONSIBILITIES Responsibility Coordinate all communications with media. Arrange media training for Emergency Management Team members on a frequency to maintain competency. Will work with the Communications as needed. Actions A. Establish a media briefing area. B. Arrange for Media Attendant. C. Access information to give to media and update as necessary. D. Provide initial press release statements, approved, within 20 minutes of event, and updated every half hour. Ensure responses and statements are positive. E. Provide personal protective equipment for news media personnel admitted to the plant, and provide escorts. F. Brief escorts, if necessary, on handling the media, their safety, and the divulging of information that could be misconstrued and damage BASF s image. G. Assist the Communications to insure that the STAN messaging system is updated as needed with the current information that may impact and/or concern the surrounding community. H. All communications to the outside or distribution of information inside the plant site must be cleared for release through the Emergency Manager. (NOTE: FAMILIES SHOULD BE NOTIFIED FIRST and NEWS MEDIA HELD PENDING VERIFICATION OF NOTIFICATION.) I. After approval by the Emergency Manager, release names of casualties and/or injured to local media.
24 Effective Date: 12/01/13 Page EMT - 19 HISTORIAN COORDINATOR RESPONSIBILITIES Responsibility Maintain a log of all communication, events, etc. during the emergency. Emergency Action A. Maintain a running account of events and actions initiated as they occur in an emergency log. B. Keep the Emergency Manager and other s informed on pending action items and check off items as completed on the log.
25 Effective Date: 12/01/13 Page EMT - 20 ECOLOGY COORDINATOR RESPONSIBILITIES Responsibility Coordinates activities related to ecology. Actions A. Advise Emergency Manager of ecological/ toxicological impact. B. Respond to ecological needs of Incident Commander. C. Contact appropriate regulatory agencies, if necessary. D. Track weather conditions. E. Coordinate activities related to proper disposal of wastes associated with the emergency. F. Coordinate activities for Contractor(s) that would assist the Beaumont site with air monitoring, contaminated water, and contaminated soil excavation and disposal associated with the emergency response and remediation conditions.

Advise Emergency Manager of cost associated emergency response. B. Respond to financial needs of Incident Commander. C. Contact appropriate department and corporate personnel, as necessary. D. Work with Engineering / Planning concerning remediation activities.

Meal (food / drinks) ii. Additional supplies (insect spray, lighting, tools, etc.) iii. Internal communication needs (radios, office supplies, etc.). C. Contact appropriate departments and corporate personnel, as necessary. D. Work with Maintenance / Planning concerning emergency and remediation activities. E. Work with Operations concerning needs of unaffected areas. i. Help maintain production of non-effected process areas ii. Coordinate supply of raw materials iii. Coordinate shipping activities
