1 Quality By-Name List Action Collaborative Overview Zero: 2016 Background Communities cannot reach and sustain functional zero without a quality By-Name List (BNL) that is at the heart of their coordinated entry system. BNLs are key to developing actionable, real-time datasets that help communities reach the goals of ending veteran and chronic homelessness. A BNL is supported by crisis response teams and provides those teams the information they need to best target their engagement and housing navigation activities. Whether it lives in HMIS or Excel, the BNL requires frequent updates and strong oversight. Zero: 2016 is dedicated to supporting communities in building and refining quality BNLs and will offer Peer Learning calls from September 19 through February 2017 to encourage relationship building between communities in similar situations, offer solution-focused conversations with people who have successfully conquered BNL challenges, and provide a venue to share community successes. Our aim is that every community in this initiative has a quality By Name List for veterans experiencing homelessness and their chronically homeless population. That s our goal: Collectively achieve a score of 920! As a BNL Collaborative participant, we are all looking to you to 1) Reach a score of 10 (Quality BNL Threshold!), and 2) Support your fellow communities in reaching a score of 10. Currently, here s how we collectively score: 179/370 for veteran 231/550 for chronic 410/920 TOTAL. To reach our goal of 920, we need 510 points. BNL Collaborative Measures: 1. By Sept 2, 2016, 85% of BNL Collaborative communities have up-to-date scorecard scores. 2. By October 31, 2016, 30% of Veteran Quality BNL Collaborative communities and 30% of Chronic Quality BNL Collaborative communities improve their scorecard by at least one point. [11 Veteran BNL, 17 Chronic BNL] 3. By October 31, 2016, the BNL Collaborative will collectively improve aggregate scorecard points to be 60% of overall goal. 552/920 = 60% 4. By November 30, 2016, the BNL Collaborative will collectively improve aggregate scorecard points to be 75% of overall goal. 690/920 = 75% Faculty Faculty will be available to BNL Action Collaborative participants to offer support, brainstorm solutions and answer technical BNL questions. They will be present during introductory calls to emphasize the importance of BNLs and offer ongoing support through December 2016.

Teams should include people with the will to improve, the ability to generate and test new ideas and who have the energy and capacity to execute - to take action - on those ideas. Local leaders will be expected to participate on regular calls and engage with additional local leadership necessary to test change ideas between collaborative meetings. We will ask each participating community to identify or assign, at minimum, the following roles to individuals participating in the BNL Collaborative: 1. Lead project manager - This will likely be the identified lead organizer for Zero: On-the-Ground Expert - This will likely be the person in your community who is leading BNL calls/meetings and overseeing daily operations of the BNL. 3. Local Data Guru - This will likely be the identified Data Lead for Zero:2016. Further, local participation will vary by call topic. During office hours, any BNL Collaborative community can call in and discuss challenges and/or ask questions of the Zero: 2016 staff team. Reviewing outcomes and locking in gains.

5 Next Steps Confirm participation in the BNL Collaborative - Let your coach know you plan to participate. Participate in an initial call with your coach and a BNL Collaborative Team member to learn more and sign up for specific calls between September 19 - October 31st. Complete the BNL Scorecard - Establish your BNL improvement baseline! If nothing has changed since you last completed the BNL Scorecard, you do not need to complete a new BNL Scorecard; we will use the last scorecard as your current information.
