4 Know Your Numbers Campaign June 1 July 31 In order to qualify for the 2018 HealthyMe rate, you and your spouse/domestic partner must each complete and submit an individual biometric screening and confidential online PHA before July 31, The HealthyMe rate allows colleagues to receive: $600 reduction on your 2018 annual medical plan contributions if you participate and are enrolled in a Marsh & McLennan Companies medical plan*; OR if you and your spouse/domestic partner are both enrolled in a Marsh & McLennan Companies medical plan*, and only one of you completes the requirements (this is known as a Blended rate ). $1,200 reduction on your 2018 annual medical plan contributions if you AND your spouse/domestic partner both participate and are both enrolled in a Marsh & McLennan Companies medical plan*. *Excludes Hawaii HMSA (Hawaii HMO and Hawaii PPP). 3

5 Know Your Numbers Campaign June 1 July 31 THREE WAYS TO COMPLETE YOUR BIOMETRIC SCREENING Go to Wellbeing At Work and under Resources select Healthyroads to schedule your appointment. Your spouse/domestic partner will need to register separately for their own appointment. Sign up for an onsite location screening, Visit a Quest Patient Service Center (PSC), or See your health care provider and submit your completed Healthyroads Biometric Screening Health Care Provider Form to Healthyroads. COMPLETE AND SUBMIT AN ONLINE PERSONAL HEALTH ASSESSMENT You and your spouse/domestic partner must each complete and submit a confidential online PHA by July 31,

8 Biometric Screenings Scheduling a Biometric Screening on the Healthyroads TM Website Once you have registered on the Healthyroads TM website, obtain your biometric screening one of three ways on the Healthyroads TM home page: Click Schedule your Biometric Screening. 1. At an onsite location screening. 2. At a PSC. Click Biometric Screening Health Care Provider Form 3. If you are completing your biometric screening with your health care provider. 7

9 Biometric Screenings Scheduling a Biometric Screening From the Healthyroads home page, access the Quest Diagnostics Health & Wellness appointment system: Click Schedule your Biometric Screening. Click Register Now and enter MMC in the Registration Key. Enter your Employee ID and date of birth. Your spouse/domestic partner will enter your Employee ID plus S and their own date of birth to register. You will need to establish a new username and password for this year. Enter a username and password, and click Continue. 8

10 Biometric Screenings Scheduling a Biometric Screening Follow instructions until you arrive at Schedule a Screening. You can either select At A Patient Service Center (PSC) or At An Onsite Event for a screening appointment. A confirmation of your screening appointment date, time and location will appear. You can add an Outlook appointment to your calendar. You will receive an confirmation of your screening appointment details. Note: Your spouse/domestic partner will need to register separately and schedule an appointment under their own username. 9

11 Biometric Screenings Screenings with a Health Care Provider Complete a biometric screening with your health care provider using the Healthyroads Biometric Screening Health Care Provider Form (accessible via the Healthyroads home page or Wellbeing At Work under Resources). (mmc-providerforms@ashn.com) or fax ( ) completed forms to Healthyroads to receive completion credit. 10

12 Personal Health Assessment (PHA)

13 Personal Health Assessment Free confidential online PHA provides a detailed picture of your health status. Includes questions relating to wellness and lifestyle health factors, as well as behavioral health factors and skills. Receive an instant overview of your overall health status, including a personalized action plan identifying potential risk factors. The Healthyroads Lifestyle Coaching Program is available to colleagues and spouse/domestic partners that meet the plan s eligibility criteria to work on identified health risks. All wellness resources are private and confidential: HIPAA policy laws prohibit the Company from receiving specific employee data. Marsh & McLennan Companies only receives aggregated data reports that are not personally identifiable. 12

15 HealthyMe Rate The HealthyMe rate allows colleagues to receive: $600 reduction on your 2018 annual medical plan contributions if you participate and are enrolled in a Marsh & McLennan Companies medical plan*; OR if you and your spouse/domestic partner are both enrolled in a Marsh & McLennan Companies medical plan, and only one of you completes the requirements (this is known as a Blended rate ). $1,200 reduction on your 2018 annual medical plan contributions if you AND your spouse/domestic partner both participate and are both enrolled in a Marsh & McLennan Companies medical plan*. Biometric screening numbers that are completed and submitted between August 1, 2016 and July 31, 2017 can be used for the 2018 HealthyMe rate. In addition, you MUST complete and submit your annual online PHA between January 1, 2017 and July 31, 2017, in order to receive the 2018 HealthyMe rate effective January 1, 2018.** *Excludes Hawaii HMSA (Hawaii HMO and Hawaii PPP). **Healthyroads Biometric Screening Health Care Provider Forms must be submitted via fax or to Healthyroads no later than July 31,

17 HealthyMe Rate Know Your Numbers completion indicator will be loaded to the benefit s enrollment system and the HealthyMe rate will be automatically reflected during annual enrollment. Benefits-eligible new hires and their covered spouses/domestic partners hired during the period of June 1, 2017 December 31, 2017, and colleagues on an approved Leave of Absence from June 1, 2017 July 31, 2017, will be automatically eligible to receive a Marsh & McLennan Companies medical plan* HealthyMe rate in *Excludes Hawaii HMSA (Hawaii HMO and Hawaii PPP). 16
