1 2014 Preparatory Nomination Form Background The Prudential - Davis Productivity Awards are made possible through the generosity of Prudential, as Anchor Sponsor, and the vision of the late J.E. Davis and A.D. Davis, co-founders of Winn-Dixie Stores Inc. and co-founders of Florida TaxWatch. Since 1989, the Davis Productivity Awards program has publically recognized and rewarded state employees and work units whose work significantly and measurably increases productivity and promotes innovation to improve the delivery of state services and save money for Florida taxpayers and businesses Winners Last year, 468 awards (including cash and plaques) were given to individuals and teams across the state for their innovations and productivity improvements worth $514 million in cost savings, cost avoidances, and increased revenue for state government Nomination Information The following pages include questions from this year s nomination form. Please use this document to gather and compose your submission before the nominations system opens on Sept. 2. The end of the form includes examples of previous winning nominations and what is not eligible for nomination. If you have any questions, please od&training@ucf.edu. The achievement falls clearly within an individual s standard job expectations. (i.e. an attorney for the state AG s office tried a case and won a settlement for taxpayers) More than half of the achievement was dependent on purchase of equipment, additional funding, a staffing increase, or assistance from non-nominees. The achievement was the result of implementing software/systems that is/are commonly used to handle similar issues (i.e. implementing a Sharepoint calendar for a group of employees).
