1 Overview 2011 Human Touch Awards Call for Nominations Presented by Cancer Care Ontario to health care professionals, providers and volunteers in recognition of exemplary compassionate patient care Now in its fifth year, the Human Touch Awards recognize health care professionals, providers and volunteers in the cancer system who demonstrate exemplary compassionate patient care. All part-time and full-time health care professionals, providers and volunteers who provide direct patient care at either a regional cancer centre or as part of a Regional Cancer Program including home care, palliative care, public health, community hospital and primary care professionals are eligible to be nominated. For example, physicians, nurses, radiation therapists, pharmacists, social workers, dietitians, patient educators, psychologists, supportive care, palliative care providers, volunteers and clerical staff. Up to five awards will be distributed, with one designated as a volunteer award. Each recipient will be presented with an etched glass plaque to commemorate their achievement at the Human Touch Awards on Thursday, April 21, Selection Criteria Nominees must exemplify the attributes outlined in the following criteria. Health Care Professionals for: Volunteers for: Providing exemplary compassionate patient care with demonstrable real-life examples. *Note: including patient testimonial in nominating materials is strongly encouraged. Leading programs and initiatives to improve the patient experience, quality of life and knowledge of cancer and the cancer system. Supporting patients, families and caregivers by providing information about available treatments and services, helping them navigate the cancer system, and guiding decision making processes. Demonstrating leadership in his/her area of work by acting as a role model for others. Ensuring that patients have a smooth and effective transition from primary care to hospital care to community care, after care or palliative care. Helping to improve access to services, including responding to the diverse needs of patients. Demonstrating a commitment to care through exceptional volunteer work that enhances quality of life and/or access to care for cancer patients. Working to improve the patient experience at any stage in the cancer journey. 1

2 Exhibiting a passion for compassionate care that goes above and beyond average volunteer duties, such as coming up with creative solutions for improved care, resourcefulness and original contributions to care, examples of outstanding consideration and concern. Making a lasting impact on the lives of cancer patients and/or the health care professionals who treat them. Nomination Process Patients and patient s families, peers, regional vice presidents, supervisors and managers may nominate an individual. The nomination should include real-life examples that demonstrate the nominee s affective impact. It is strongly encouraged to include statements/testimonials/stories from patients. The nomination must be endorsed by both the regional vice president and the supervising manager. Submission deadline: Friday, March 4, Number of Awards Up to five awards will be distributed, with one designated as a volunteer award. Selection Criteria A regionally diverse and interdisciplinary panel will review the nominations and select the award recipients. The evaluation is based on the criteria outlined in the nomination instructions and will be determined by the strength of the submission. Award Presentation Successful recipients will be notified by Friday, March 25, 2011 and will be invited to Cancer Care Ontario s Annual General Meeting and Human Touch Awards ceremony on Thursday, April 21, 2011 where they will be publicly recognized and presented with their award. Cancer Care Ontario will provide travel and one night s accommodation for the award winners plus one guest. Cancer Care Ontario s Board Chair, Neil Stuart, President and CEO, Michael Sherar and Carol Sawka Vice-President, Clinical Programs and Quality Initiatives will present the awards. 2

Nomination Letter In a maximum of one to three typed pages, clearly articulate why the nominee merits a Human Touch Award while illustrating your points through real-life examples, patient stories, quotes and testimonials. Supporting documents such as notes/testimonials from patients, photos, articles/media coverage, etc. may be included in your nomination package. Please do not send originals as they will not be returned. Note: While long-service is commendable, awards will not be issued based on duration of service. Since winning, I have been asked to join the Oncology Nursing Program as a mentor.
