4 Awards and Recognition The CREST Awards are presented during the Awards Banquet at the FCA International Leadership Council. First place in each category receives the CREST Award with the remaining nine finalists receiving certificates of international recognition for placing among the top ten safest companies. One entry is selected by the Chairman of FCA to receive the CREST Chairman s Award. This Award recognizes the contractor who has taken his or her current program to new heights. Submitting an Entry Chairman s Award winner Tim Wies from T.J. Wies Contracting, Inc. (located in St. Louis) poses with FCA Chairman Mike Cassidy of Long Painting (located in Kent, WA) and Randy Bender, Chairman of the FCA Safety Committee (located in Bettendorf, IA). All entries must be completed and submitted entirely by March 3, 2017 and must include: n Completed CREST Awards online registration, which may be accessed at n A minimum of three (3) and a maximum of six (6) photos/ images that represent the company s safety compliance program (digital format only). n Company logo identification image (digital format only). Note: Submission materials and information will not be returned. 4

5 Categories It is necessary to designate entries by one trade category. Companies performing under more than one of the following categories must submit based on the category in which a majority of its work is performed during the award year. In evaluation, emphasis is given to the following categories based on annual hours and total recordable cases. An award winner is chosen on the basis of injury and health frequency rate data, the company s insurance experience modifier, safety statement and safety program information. Architectural Glass & Metal Drywall Finishing Flooring Industrial Coating Painting & Wallcovering Sign & Display Each category is divided up by the following hours worked and a winner will be selected for each of the following ranges: 1 25,000 hours 25,001 50,000 hours 50, ,000 hours 100, ,000 hours More than 150,000 hours FCA reserves the right to reject any submission. 5

7 Determining Safety Excellence The FCA CREST Awards program is designed to objectively evaluate contractors based on key leading and lagging measures. These are metrics associated with efforts that cause excellent safety performance and the resulting numbers that match the performance (i.e. low injury/illness rates). Companies are required to submit basic information. Formulas have been created to standardize values for these measures. For the evaluation of safety effort, the following data is collected: n Year(s) the company has had a written safety program in place n Safety training orientation for all new employees n Employment of a full-time safety representative FCA CREST Awards address these rates as follows: n Fatalities The fatality measure is a critical factor. No company with a fatality within the calendar year will be considered for an award. n EMR Trend All entries must implement less than 1.0 minimum requirements for Experience Modification Rates (EMR) to qualify. If an entrant does not meet these minimum requirements, he or she may submit a letter explaining why the contractor s EMR is over 1.0. n Ongoing safety training n Number of employees who have current OSHA 10 or 30 training n Safety program impact on the company n Injury/Illness Rates Days Away, Restrictions and Transfers (DART) Recordable Incident Rate (RIR) Lost Work Day Case Rate Severity Rate n Recordable Incident Rate (RIR) Trend Change over a five-year period. The winner in each category is selected based on the overall point value from the items above. The company in each category with the highest score will receive the FCA CREST Award. This process eliminates subjectivity in identifying which company has the best safety program. FCA CREST Award judges merely review the data and calculations. Note: A detailed explanation of the measured data is available on FCA s website, All entrants may be required to submit a copy of their OSHA 300A form. Additionally, all entries may be evaluated in light of their OSHA history. 7

8 Publicity and Ownership It is essential that images not be restricted against publication in connection with the FCA CREST Awards and any publicity thereof. It is the entrant s responsibility to make sure all photographs or images are cleared for release for use by FCA and its publications, neither of which assume responsibility for copyrights or photographic fees. This also includes appropriate releases from owners. The photographer s credit line is given when provided. n FCA gives wide publicity to the award-winning entries. n Publicity is also sought for non-winning entries in international and local trade publications looking for specific project examples. Additionally, all entries may be included in FCA publications and the website. By participating in this program, the entrant agrees not to bring claim against the jury, FCA or its members. FCA reserves the privilege of retaining possession of all entries, including submissions for reproduction and subsequent exhibits. 8

9 Image Requirements n Entries must include a minimum of three (3) and maximum of six (6) images that clearly show finishing safety elements. n High resolution, 300 dpi photos; uncompressed file size of at least 17MB. n The selection of images must be comprehensive enough to professionally depict the company s safety practices on the project and/or during the company safety training program. Company Logo Requirements All entries must be accompanied with one company logo image. The image must be in a digital format such as jpeg, png, eps or ai files. It will be used to identify the company on the evening of the Awards Banquet during a special presentation of all CREST entries. 9
