At the time, the topic of health care reform was red hot around the nation, and still is today. Increasingly, accountability is making inroads into the U.S. health care system in an attempt to curb a reimbursement structure that seems to have spiraled out of control, now pushing the government s Medicare program to the brink. Medicare is broke. Health care is the largest expenditure in the national budget, and it keeps growing. A major fundamental shift has to take place in the U.S. health care system, said Radke. The health care system in America has become too complex and costly to continue business as usual, experts say. Today, an emerging trend is starting to take shape across the nation to improve care, reduce costs, and cut waste. It s called the Accountable Care Organization. Are these new entities now being rolled out selectively in communities the new paradigm for health care delivery in America? We felt like we wanted to drive the bus rather than just ride in it. So we took the lead in an effort to try to control our own future. And I think we will do well, she said. Radke has taken on a dual role that is likely to shine the national spotlight on her group and its ability to cut costs and deliver high quality patient care to a key segment of the population traditional Medicare fee for service beneficiaries and be financially rewarded for the achievement. But first some history: The 278 member physician group she represents provides care across all medical specialties and covers the entire county. BPN sees itself as the back office for a doctor, performing administrative functions for the practice. We help them stay independent because they typically cannot afford to hire more administrative staff, said Radke. She added, In Florida, there are other groups like ours that provide back office assistance to help their physicians. I respect the hospital partners. But a lot of doctors are having to make a decision: Do I stay in business or do I become acquired? We offer an alternative solution for them. For a fee of $300 a year, a physician can become a BPN member. We have been extremely successful as far as our financial position, she said. If you are a new doctor joining BPN, for the first 12 months you pay $300. After that, we have waived the fee in the follow up year since I have been the CEO. We have also waived the annual membership dues for the last three years. BPN works with several Medicare Advantage companies that offer bonuses to the physicians who meet certain quality measures and other benchmarks to keep patients healthy. County to host sea turtle obstacle course Brevard County is sponsoring an obstacle course called It s Tough to be a Turtle, from 10:30 a.m. to 1:30 p.m. on Saturday, Jan. 26, at Riverwalk: A Family Park, 5355 U.S. Highway 1 in Rockledge. The program is being held in conjunction with the Space Coast Birding and Wildlife Festival. Sea turtles are sighted on Brevard beaches each summer, but they only spend a tiny fraction of their life out of the water. What do they do the rest of the time they are in the ocean? 3 Comedian Brian Regan to perform at King Center in Melbourne on Feb. 16 Live Nation will welcome Brian Regan to The King Center for the Performing Arts at 8 p.m. on Saturday, Feb. 16. The King Center is on the Melbourne campus of Brevard Community College. Critics and peers alike agree that Regan has distinguished himself as one of the premier comedians in the country. He fills theaters nationwide with fervent fans that span generations. Late last year, Regan s highly anticipated second album, All By Myself, was released on CD available only through his website. The live concert recording was originally released as a digital download in December 2010, again only available through his website. Releasing two critically acclaimed hour Comedy Central specials and DVD s in as many years 2008 s The Epitome of Hyperbole, and 2007 s Brian Regan Standing Up has set a standard of excellence. The Epitome of Hyperbole has also been seen on CMT. His non stop theater tour has visited more than 80 cities each year since 2005 and continues through In January 2012, Regan broke the record for the most consecutive shows by a comedian at Abravanel Hall in Salt Lake City, with 10 sold out performances. More than 26,000 tickets were sold. In March 2010, he performed five sold out shows at Abravanel Hall breaking the previous record of four shows set by Jerry Seinfeld in The King Center will host a Picnic on the Patio for this performance. These two organizations play a role in making sure abused and neglected children are placed in safe, permanent homes, where they can become the people they are destined to be. One of the ways to support our GAL children is through annual Superhero Run. More than 400 runners participated in last year s event, making it a huge success. 4 Eastern Florida State College selected as BCC s new name as it adds 4 year degrees Brevard Community College has a new name to reflect its expanding mission: Eastern Florida State College. The name was approved by the Board of Trustees in a move that was required because BCC will start offering four year degrees and become a state college next year. Initially, the college will offer a bachelor of applied science degree in organizational management with two concentrations general management and health care management in August The college will also remain firmly committed to its community college roots, continuing to offer more than 100 two year associate degrees and certificate programs as its primary mission. The name now goes to the Florida Legislature and Gov. Rick Scott for final approval during the spring 2013 legislative session. It is expected to become official July 1, This is a historic day for the College and our community, said Alan Landman, chairman of the Board of Trustees. BCC President Dr. Jim Richey called the decision a milestone that best serves the long term interests of the college and the Space Coast. I m excited about our future because Eastern Florida State College is more than a new name, he said. It represents the new opportunities we can offer our students and our community. We re now exceptionally well positioned to do that through the addition of four year degrees. The renaming process began in August when BCC officials began reaching into the community for ideas and to gather perceptions about the college that could be used to help attract students, build business partnerships and enhance academic programs. About 200 people participated through written surveys, focus groups and the BCC s Facebook page. They represented key stakeholders including Brevard business and community leaders, elected officials, and BCC faculty, staff, students, prospective students and alumni. The people surveyed were asked to serve as representatives of their group, organization or constituency to broaden the outreach. More than 70 names were suggested and fewer than 10 recommendations based on the research were given to Richey for final consideration. The research revealed several key factors that led to choosing Eastern Florida State College, Richey told trustees. Focus groups showed prospective students wanted a name that went beyond Brevard s borders, and spoke to the larger world in which they live and the global economy that awaits them. Furthermore, many business and civic leaders wanted a name that elevated the college on a regional level, and could help the BCC advance its role in creating jobs and spurring economic development and growth. Those two factors stood out strongly, said Richey. Both groups were looking at new horizons for the college and wanted us to do the same. They were enthusiastic about our future. In another area, Richey said the BCC s new positioning statement Explore. Achieve. Succeed. honors the community s space heritage and also provides an aspirational message to students. The college will use the slogan in its marketing and advertising efforts to attract students and build business partnerships. The statement is the promise we make that enrolling at Eastern Florida State College includes the opportunity to learn about yourself, reach your goals and have a successful life, said Richey. It s a great balance of our proud tradition and the unlimited possibilities of the future. The school s new four year degrees will be highly attractive to Brevard students for reasons that include cost, with tuition at Eastern Florida State College lower than at four year state and private universities. Furthermore, the four year degrees will: l Allow Brevard students to stay close to their homes and families, making it easier for those who are working their way through college to continue their education. l Provide people already in the work force with the chance to learn new skills without leaving the region. l Allow students to benefit from the small classroom size and personalized instruction that have long been hallmarks at BCC. Under the new name, tuition for two year associate degree courses will continue to be set by the state at the community college level. Tuition for four year baccalaureate degree courses will be set at the state college rate, not the state university rate. Students who earn their two year degrees at BCC will continue to be automatically accepted into the University of Central Florida though the DirectConnect program. They will also be eligible for entry into Florida Tech and other four year universities. BCC is the latest community college in Florida to change its name. Some 22 other institutions in the 28 member Florida College System have also taken the step in recent years when they started offering four year degrees. The name changes are fueled by a 2008 law passed by the Florida Legislature that allowed community colleges to start offering four year degrees after studies showed Florida needing more workers with at least a bachelor s degree to meet the future demands of employers. It s the second time BCC has changed its name since it opened in 1960 as Brevard Junior College. It was renamed Brevard Community College in 1970 when the state created the community college system. Spanish Boot Camp classes planned for professionals The Viera Regional Community Center, 2300 Judge Fran Jamieson Way, is offering Spanish Boot Camp classes. The classes are designed to bring law enforcement and health care professionals to a level 1 proficiency in speaking, reading, writing, and understanding Spanish. Classes for law enforcement professionals begin on Tuesday, Jan. 8, and are from 6 to 8:30 p.m. They are held every Tuesday for six weeks. Classes for health care professionals begin on Thursday, Jan. 10, and are from 6 to 8:30 p.m. They are held every Thursday for six weeks. Spanish Boot Camp is for students ages 18 and older. The fee is $80 for the six week session. All activities are supervised by Brevard County Parks and Recreation staff at the Viera Regional Community Center. 5 Holmes named 2012 McCartney National Defense Space Award recipient to be honored on Jan. 8 CAPE CANAVERAL The National Space Club of Florida has named Air Force Maj. Kenneth Holmes as its 2012 Forrest S. McCartney National Defense Space Award recipient. He will be recognized at the Jan. 8 luncheon meeting at 11:30 a.m. at the Radisson at the Port in Cape Canaveral. Brig. Gen. Anthony Cotton, commander of the 45th Space Wing, will give an update on the 45th Space Wing and Eastern Range at the meeting. The NSC is proud to recognize Maj. Holmes for his contribution to the defense of our nation through his leadership and technical expertise of space capabilities, said NSC Chairman Jim McCarthy. He is the perfect choice for the first award renamed after General McCartney. McCarthy, along with Cotton, will present the award to Holmes at the luncheon. The award, previously known as the Florida National Defense Space Award, was renamed the Forrest S. McCartney National Defense Space Award in memory of Air Force Lt. Gen. McCartney, who served with exemplary distinction as the fourth director of the Kennedy Space Center. McCartney died on July 17, Holmes provides and integrates space capabilities to support in garrison and deployed Special Operations Forces worldwide. He is responsible for coordinating Friendly Force Tracking (FFT) and Personnel Recovery (PR) support for global situational awareness of the Air Force Special Operations Command (AFSOC), and key allied and coalition Special Operations Forces (SOF) assets. He also leads planning and execution of formal initial assignment and pre deployment training for 14 space Subject Matter Experts (SMEs) across the MAJCOM. In 2012, as the chief of space training for AFSOC, Holmes revised the command s space instruction and training plan, enabling full certification of space personnel, and provided subject matter expertise for combat support to SOF. He took his expertise on a temporary duty (TDY) assignment to Osan Air Base in South Korea to support Exercise KEY RESOLVE as the Space SME, where he led the combat plans division and SOF Special Technical Operations (STO) integration. This resulted in the development of key tactics, techniques, and procedures (TTP) utilizing SOF against adversary communications determined critical to mission success. He also aided Combined Joint Special Operations Air Component analysis of inoperable tracking devices and coordinated resolution ensuring continuous tracking of a special operations medical team. As the FFT lead for Emerald Warrior 2012, he coordinated and satisfied all special operations, conventional and interagency tracking requirements in the U.S. Special Operation Command s premier exercise. Holmes leadership and technical expertise culminated with a 140 plus day deployment to Bagram Air Base in Afghanistan, where he was the STO chief and space OIC for a Joint Task Force (JTF). During this period he led JTF integration of STO and space capabilities supporting more than 19,400 task force personnel during 384 direct action missions. He targeted 1,272 hours of enemy communications significantly disrupting adversary lines of communication resulting in 1,471 enemy captured or killed, including 166 high value individuals. Holmes was also the task force lead for limited distribution operations, where he coordinated deconfliction with national reporting agencies during eight kinetic operations minimizing the signature of our elite forces. The now named Forrest S. McCartney National Defense Space Award was created in 2005 by the National Space Club Florida Committee to recognize significant achievements and contributions made by DOD personnel while on duty in Florida. 7 Author Ryan Pearson says five super powers available to teens; these heroic abilities aren t exclusive to comics Cynical adults may sneer when they say, Youth is wasted on the young. But young world traveler Ryan Pearson sees a more positive message in George Bernard Shaw s often repeated quote. I see it as meaning that youth is an opportunity to seize direction, enlightenment, significance and to expand one s powers, says Pearson, author of Green Hope from The Element Series, ( about a teenager blessed with wealth and fame who discovers he has the added responsibility of super powers. It s sad that so many teens get sidetracked by trying to fit in with a crowd, or worrying that they don t measure up somehow. Pearson says all teens have super powers they just need to recognize them: l Your inner mutant : Many teens like to make a big deal out of not caring what others think about them, precisely because they care about what everyone thinks of them. No one else in the world is quite like you. Explore your interests and find what you love whether or not it s what other teens love. You ll get a head start on developing valuable skills. l Punisher fitness training: You don t have to be built like the renowned vigilante from the Marvel universe, but you ll look your best and feel your best if you establish a good exercise routine now. Not only will working out give you a nice physique, it s a good way to reduce stress and it even gives you a natural high thanks to the release of endorphins, chemicals that make your brain happy. l Batman s first rule in fighting: Despite the fact that it would make his crime fighting much easier, the Caped Crusader absolutely refuses to use guns. That s because a deranged criminal with a gun shot and killed Bruce Wayne s parents when he was a child. The result is that his fighting methods are more moral and creative, and he always knows what to do when a quick decision is needed. Getting into the habit of making your own decisions based on your values and your understanding of right or wrong, instead of following the crowd, will help make even the hardest choices easier. l Cultivate your spidey senses: Teens are naturally impatient, impulsive and impetuous. Slow down! Take your time on the road, in relationships, during confrontations and when contemplating big decisions. Part of why Spider Man is so fast is that time slows for him during tense situations. Likewise, teens who can slow down emotionally driven decisions and better understand their consequences, much like a spidey sense, will make wiser ones. l Know your kryptonite : Some kids just seem to have it all: academic excellence, athletic accomplishments, popularity, and a clear complexion to boot. But everyone has their limits, like Superman s kryptonite. Knowing your limits and learning how to worked around them, or strengthen them, is a lifelong challenge for everyone. Brevard Ocean Rescue unveils new lifeguard towers to provide better protection To boost public safety and provide better protection for its staff against the elements, Brevard County Ocean Rescue unveiled two new lifeguard towers on Dec. 12. The two new towers are set up at the Cocoa Beach Pier and at Shepherd s Park. Ocean Rescue Chief Jeff Scabarozi believes the new towers increase visibility for both lifeguards and visitors to Space Coast beaches. Cocoa Beach is one of the busiest stretches of coastline that we protect, said Scabarozi. We want our guests to swim near lifeguards and with these new towers, it is much easier to locate lifeguards on the crowded beach. Visibility for our lifeguards has also been improved. I was overjoyed, said Gordon. I could only think of my residents. I went into nursing due to my love for people. What I love the most about my job is when a resident smiles and says thank you. 10 Candlelighters of Brevard a key support link to families whose children have cancer no state or federal funding; $10 ticket for chance to win Hyundai By Ken Datzman INDIAN HARBOUR BEACH Roughly 13,000 children are diagnosed with cancer each year. And when the diagnosis is made, it is devastating for the child, the parents and the families. Cancer is the leading cause of death by disease among U.S. children between infancy and age 15, according to the National Cancer Institute. However, while the incidence of invasive cancer in children has increased slightly over the past 30 years, mortality rates have declined by more than 50 percent for many childhood cancers. Both Carrie Wam, executive director of Candlelighters of Brevard Inc., and Adis Perez, director of that nonprofit organization s family services, have seen firsthand the advances in treatment efficiency. My son is a 20 year cancer survivor and Carrie s daughter is a 22 year survivor, said Perez, adding that their experience dealing with childhood cancer drew both of them to Candlelighters. That is the reason we are here. I started as a Candlelighter mom and so did Carrie. At that time, we were in the same situation that local families face today when their child is diagnosed with cancer, added Perez. She and Wam have dedicated their professional nonprofit careers to helping families cope with what they both went through when their children were diagnosed with cancer. Local families whose children have cancer must travel outside the county to receive treatment, which involves time and other costs. There are no pediatric oncology services available in Brevard, said Wam, who has led Candlelighters of Brevard for the past 20 years. These families have to travel to Orlando, Jacksonville, Tampa, and Miami, for instance. That creates a lot of additional expenses for them. As part of its mission, Candlelighters of Brevard provides financial help and emotional support to these families. More than 80 percent of them don t receive any other form of financial assistance, said Wam. An independent entity, their organization serves 16 to 18 newly diagnosed childhood cancer patients each year. And we stay with them through their whole process. Leukemia patients, for example, are typically treated for three years. We follow up as well, said Wam. Leukemia (blood cell cancer) and cancers of the brain and central nervous system account for more than half of all new childhood cancer cases, says the American Cancer Society. Since its inception, Candlelighters of Brevard has provided services and resources for more than 250 area families, said Perez. We have 54 active families, meaning that their sons or daughters are at some point in their treatment and are using our services. For more than 20 years, Candlelighters of Brevard has been going about its mission quietly in the community, while at the same time trying to raise awareness of its importance as a nonprofit. We do not receive any state or federal funding, said Wam. We host three major fund raisers throughout the year, and the rest of our operating money comes from private donations, corporate donations, memorials, and thrift store sales. One area business that has stood behind Candlelighters for years is Brevard Hyundai Dealers. To help raise funds for the organization this year, it has donated a new 2013 Hyundai Veloster. Candlelighters of Brevard is selling tickets for chances to win the vehicle in a drawing. Tickets are $10 each or 10 for $50. They are available for purchase at area Hyundai dealers or the Candlelighters Thrift Store at 1875 A South Patrick Drive. Wam said 6,000 tickets have been printed. The odds are pretty good. She said the late Bruce Nelson Sr., who brought the Hyundai franchise to the county, was a huge supporter of Candlelighters and his children are carrying on that tradition today because they know how much he loved Candlelighters. Bruce Nelson Jr. operates the Hyundai franchise today. The drawing for the new Hyundai will be held on Saturday, Feb. 9, as part of the Candlelighters of Brevard s annual Have a Heart Valentine s Extravaganza at the Eau Gallie Yacht Club in Indian Harbour Beach. These dinner plates and other pieces are made in England, and are showcase quality. Wam said the community in general has been very generous to Candlelighters of Brevard. But it has taken us 20 years to finally receive some recognition, in that people trust Candlelighters and believe in our mission, in what we do and how we do it. Over the years, Candlelighters has tweaked its programs and services to better meet the needs of area families, said Wam. We tailor our programs to fit their needs. Candlelighters of Brevard is overseen by a volunteer board of directors. The community leaders include Paul Phelps, the organization s president, and Nancy Dyer, its secretary. 11 BSO to host its annual Symphony for the Palate at the Eau Gallie Yacht Club a wine paired dinner and auction; EGYC to introduce Executive Chef Cook By Ken Datzman The Brevard Symphony Orchestra has built a successful fund raising event around one of the oldest beverages in the world wine. And wine s popularity among U.S. consumers has only grown over the past two decades. The industry marked its 18th year of consumption growth in 2011, according to the Beverage Industry Group. The annual BSO benefit is one of the best attended social events in the region. The program pairs great wines with gourmet food, in a country club setting. The event also includes a premium wine auction. It s usually a well attended event, said businessman Harry Brandon, who is a member of the BSO board of directors, an organization in its 59th season. This is an interesting program that brings together people who love good wine and good food and good friendship. They have a great time every year. I think it s going to be one of our best years for Symphony of the Palate. The seventh annual gathering is set for 6 p.m. on Saturday, Jan. 12, at the Eau Gallie Yacht Club in Indian Harbour Beach. The event will get under way with a champagne reception, said Darcia Jones Francey, the chairwoman for Symphony of the Palate. This is a sponsorship only benefit. Lance has extensive training in his field. Cook is a 1996 graduate of the highly regarded Culinary Institute of America. The Hyde Park, N.Y., institution is one of the leading training grounds for chefs and hospitality professionals. His resume includes having worked at the Four Seasons Hotel in Boston, the Wilson Country Club in North Carolina, and the Stonebridge Golf and Country Club in Georgia. Cook holds the Certified Executive Chef designation, a program offered through the American Culinary Federation. He is also a Certified Food and Beverage Executive, through the America Hotel and Lodging Institute, among holding other industry credentials. The wines served with the dinner will be selected by Pettigrew and local wine enthusiasts. We did this last year and it was very successful, said Brandon, who runs Brandon Properties in Brevard with his wife Wendy. We have six or seven couples who have been invited to handle this part of the program. These are people in the community who are knowledgeable about wine and food pairings. It s going to be a fun event. They will be selecting wines from Petty s Meat Market based on the menu that will be featured at Symphony of the Palate. The food is always excellent at this event, and is carefully selected, said Pantello. A highlight of the evening for Symphony of the Palate is the live wine auction. It s a very lucrative part of the event for the BSO, said Delisle. She added, Jackie (Pantello) has been very helpful because of her contacts in the community and her knowledge of wines. 13 Second annual charity bike event scheduled here Feb. 24; to feature six time Tour de France winner The Infinity Racing Organization Inc., a cycling association focused on supporting the local community, will host its second annual Gran Fondo Brevard on Sunday, Feb. 24. The Fondo, a special format charity bike ride event, will give riders the opportunity to compete in a timed course throughout Brevard County beginning at 7:30 a.m. starting from Viera High School. The ride has four routes, (Gran Fondo), 74.2 (Medio Fondo), 55 (Piccolo Fondo) and 20 (Mini Fondo) miles, which includes one of the longest, most challenging courses in the state, and the most scenic and diverse roads in the Southeast region. The bike course will challenge the stamina of riders of all skill levels, among them six time Tour de France and former national champion Kevin Livingston. Along with the cycling event, this year s program will feature a 5K Gran Fundo Walk/Run that will go around Viera High School and back. Following both events will be a party to include local food, retail, and service vendors. There will be several athletic and health testing activities available geared toward improving the health and well being of the attendees. All proceeds of Gran Fondo Brevard will benefit the KLD Youth Foundation. The nonprofit organization provides outside of school programs to address the growing problem of unsupervised youths in Brevard County. Financial support for KLD comes from state funds, grants, and private donations. KLD designs individual plans for each child to holistically develop his or her body, mind and spirit. The key to KLD s success is the personal, individualized attention given to each particular challenge of each student. Steve Bland has successfully completed an intensive 10 week session of specialized training at the FBI National Academy in Quantico, Va. Bland becomes the ninth member of the Palm Bay Police Department to graduate from the elite training course that is only offered to the world s top law enforcement officers by invitation only. Bland was part of the 251st session of the academy. As an FBI National Academy graduate, he enters into a select group made up of less than one percent of the nation s law enforcement officers. They were handpicked by their departments and, along with about 250 other officers, completed the 10 week course, which began Sept. 30 at the FBI training facility. The course included instruction in law, behavioral science, forensic science, understanding terrorism/terrorist mindsets, leadership development, communication, and health and fitness. I m honored to have been a part of this program and proud to have represented this department, said Bland. This type of focused training is so beneficial and will help us raise the level of professionalism in everything we do here and I m thankful to have been a part of it. Bland is a 16 year veteran of the department. During his tenure he has served in patrol, investigations, as a field training officer, in youth services and special investigations. He is currently assigned as a lieutenant supervising the Special Victims Unit, Youth Services, Major Case Unit, Special Investigations Unit, Crime Scene Unit, and also serves as watch commander and SWAT commander. 19 Accountable Care Organization Continued from page 1 Radke felt her organization was ready to take the next step on the road to improving the quality of care member physicians deliver to patients. In January, her organization formed a separate company Medical Practitioners for Affordable Care. MPAC, of which Radke is the executive director, an Accountable Care Organization. It was formed in order to have a launching pad to apply to become a Medicare Shared Savings ACO, said Radke, who came to her position at BPN after working on the insurance side of the health care industry, and for large health maintenance organizations for more than 20 years. The concept of an ACO was first discussed in 2006 by Elliot Fisher, director of the Center for Health Policy Research at Dartmouth Medical School, during a meeting of the Medicare Payment Advisory Committee, as experts sought ways to slow the costs of Medicare. Thousands of baby boomers (people born between the years 1946 and 1964) become eligible for Medicare every day and the trend will continue years into the future. Because of its potential cost savings and focus on quality, the ACO idea started gaining support within the industry as a step toward health care reform. The government began receiving its initial round of applications for the ACO Shared Savings Program in January The first ACOs were launched in April. ACOs are entities formed by groups of doctors and other health care providers that have agreed to work together to coordinate care for people with Medicare. The goal is to lower costs and improve care for patients, while rewarding practitioners for reaching certain quality standards. Participation in an ACO is purely voluntary for providers. The Medicare Shared Savings Program, and other initiatives related to ACOs, is made possible by the 2010 Affordable Care Act. The government estimates that federal savings from this initiative could be up to $940 million in the first four years. At the end of the year, if there is a savings that you have achieved, after hitting certain benchmarks, you share in the savings with the government. So there really is no downside to the program, she said. The higher the quality of care providers deliver, the more shared savings the providers may keep. If ACOs save money by getting beneficiaries the right care at the right time for example, by improving access to primary care so that patients can avoid a trip to the emergency room the ACO can share in those savings with Medicare. MPAC made application in March hoping to be become part of the Medicare Shared Savings Program. We had to file a 1,100 page application explaining in great detail how we were going to meet 33 quality measures, including how we could achieve cost savings without rationing care. The quality measures relate to care coordination and patient safety, appropriate use of preventive health services, improved care for at risk populations, and the patient and caregiver experience of care. In June, MPAC received confirmation that it had been approved to be one of the first 154 ACOs in the U.S., said Radke, whose organization has a three year contract with Medicare and is one of 10 ACOs in Florida. This is huge. We are excited to be a part of it. I am passionate about it. I just think we are gong to be able to really change the landscape of health care in Brevard County, as it pertains to the senior population we will be taking care of. We feel like we can save money through this program, she said. As more providers make application to be part of the program, Radke expects there will be roughly 300 ACOs in the U.S. in January. There is one more round that will go live in January of 2013 and then the process closes for a year. These ACOs across the nation have entered into agreement with the Centers for Medicare & Medicaid Services, taking responsibility for the quality of care they provide to people with Medicare coverage in return for the opportunity to share in savings realized through high quality, well coordinated care. This is not just about keeping patients in our ACO, said Radke. We are going to extend our communication to physicians who are not in our ACO. We are in partnership with everybody in the local health care community. We re working with Wuesthoff and with Health First. We want to send a very loud message that we are not exclusive, and that is very important. The MPAC physicians had to upgrade their infrastructure including information technology systems to participate in the program. Physician groups in ACOs need a robust electronic health record system that is capable of advanced reporting, including disease registries and patient population case management. All of the 152 MPAC physicians in the program are converting to electronic medical record technology, said Radke. Our physicians have wholeheartedly embraced the transition. It s quite an undertaking to shift from paper to the electronic mode. I think it shows the level of commitment and dedication that our physicians have for this program. About 75 million Americans have more than one chronic disease, requiring coordination among multiple specialists and therapies, which can increase the potential for miscommunication, misdiagnosis, potentially conflicting interventions, and dangerous drug reactions, according to a report by the Institute of Medicine. Health professionals and patients frequently lack relevant and useful information at the point of care where decisions are made. Electronic health records and mobile technologies offer significant potential to capture and share health data better. Patients should be encouraged to use tools such as personal health information portals to actively engage in their care, said Radke. We have given our patients access to their own medical information. They can log in and see their medical history. If they are traveling, get sick, and end up in the emergency room, they can give their coded card to the ER doctor and the doctor will be able view their medical history and see what medicines they are taking. That medical history is electronic. The patient can go anywhere in the U.S., log in, and see it. The ACO initiative will also test the effectiveness of several innovative payment models and how they can help experienced organizations provide better care for beneficiaries, work in coordination with private payers, and reduce Medicare cost growth in general. These payment models will allow groups that are successful in achieving better health care and lower cost growth to move away from a payment system based mainly on volume under the current fee for service model, toward one where the ACO is paid based on the value of care it provides. If you look at where there is a lot of waste in the system, it s the duplication of testing, said Radke, adding, We now have a system that allows doctors to talk to each other. Each doctor in the ACO will have a complete medical record snapshot of the patient. The U.S. Department of Health and Human Services says better coordinated care can reduce unnecessary duplication of services, reduce preventable medical errors, and lower costs. According to a recent article from the Kaiser Family Foundation, Most hospitals and doctors get paid more by delivering more, not necessarily better, care. ACOs will reward providers for holding down costs and meeting certain quality measures, such as reducing hospital readmissions or emergency room visits. It continues, In many ways, ACOs aim to replicate the much touted models of care at the Mayo Clinic in Rochester, Minn., and the Geisinger Health System in Pennsylvania, where hospitals and doctors coordinate their efforts within the same organization. Inefficiencies, an overwhelming amount of data, and other economic and quality barriers hinder the progress of improving health care, according to the Institute of Medicine. About 30 percent of health spending in 2009 roughly $750 billion was wasted on unnecessary services, excessive administrative costs, fraud, and other problems, says the Institute of Medicine. Better use of data is a critical element for a continuously improving health system, according to that report. People with Medicare have received information from the government about what an ACO could mean for them, and will receive additional information from their provider. Right now, we have about 15,000 patients who are assigned to our ACO. We are notifying them, letting them know their doctor is in the ACO. Nothing changes for them. That s the main message, she said. Large organizations around the nation including medical schools from Harvard, Johns Hopkins, and the University of Michigan have contacted Radke. They want to include us in their work groups because we are one of the 154 ACOs they are looking to mine data from. It s very rewarding to be recognized for some of these accomplishments. These are organizations that really set the bar for health care. She added, But I only did the ACO application. The doctors are the ones doing the work.
