2 CHECKLIST FOR MA, MS, AND MFA AWARD APPLICANTS The following materials must be received in the Department of Art and Design, Office of Graduate Studies via the Panthera application system by: Priority University award deadline: December 10 for University Fellowships* Art and Design award deadline = January 25 Art and Design general application deadline (without award consideration) = February 1 *Additional forms are reuired and may be found online at: Department of Art and Design MA/MFA Award Application Check list: Award Application Form and supporting materials for Teaching Assistantships, Project Assistantships and Layton Fellowships, if applicable. (upload in Additional Resources area, select document type: Miscellaneous Document in the Panthera application) Note: Please complete the information in this form and save it as a single PDF. If you do not intend to apply for all options included in this form, Layton, Teaching or Project Assistantships, you may delete the section(s) before saving the form as a PDF. Digital Portfolio: Twenty images (max.) of recent work, upload using the SlideRoom feature in the Panthera application. 5 TEACHING ASSISTANTSHIP APPLICATION New Applicants: The position for which you are applying reuires candidates to pass a criminal background check (CBC) as a condition of employment. This includes a review of any pending charges or convictions.
