4 4 WELCOME LETTER Welcome to the 2016 UK OPAT Initiative National Conference. We are delighted to welcome you to this two day conference on Outpatient Parenteral Antimicrobial Therapy (OPAT). The UK OPAT initiative is a multi-stakeholder project that supports, guides and encourages the establishment of standardised OPAT services throughout the UK wherever the clinical need may exist. This meeting is bringing together nearly 300 OPAT experts and we hope that you take the opportunity to share expertise, discuss success stories and learn about upcoming challenges in the OPAT setting. We are delighted to welcome a broad mix of stakeholders from many centres across the UK who are looking to establish, strengthen or extend OPAT services. The conference has been organised with a number of key objectives in mind that include: Defining new pathways for early discharge and admission avoidance for patients with infection Commencing discussions around the establishment of a nursing network for OPAT The value of OPAT in complex conditions BSAC is very grateful to the range of companies present for their support of the conference which enables us to maintain registration fees at a minimum. 14 14 SPEAKER BIOGRAPHY Sharon Bamber Clinical Scientist (Microbiology & Infection Control) OPAT Service Manager, Wirral University Teaching Hospital NHS Foundation Trust, Merseyside Sharon has over 25 years experience working in the field of medical microbiology within the NHS, the majority of which has been spent working within clinical laboratories in the North West of England. Her current role utilises the clinical knowledge and managerial skills of a Clinical Scientist to enable the development of a new service incorporating best practice in infection control and antimicrobial stewardship. 15 SPEAKER BIOGRAPHY 15 Eoghan de Barra Consultant in Infectious Diseases, Imperial College Healthcare NHS Trust, London Eoghan de Barra received his medical degree in Dublin and proceeded to gain specialist training in Infectious Diseases and General Medicine in Ireland and South Africa. In 2011 he took a post as research fellow and lecturer in the department of Tropical Medicine and International Health at the Royal College of Surgeons in Ireland, Dublin and conducted malaria vaccine research work in Dublin and University of Oxford. In parallel, he worked as a consultant physician in the department of Infectious Diseases at Beaumont Hospital, Dublin, Ireland s national neurosurgical centre. In 2014 he moved to London to work at Imperial College Healthcare NHS Trust as a Consultant in Infectious Diseases, where he is the clinical lead for the OPAT programme. 16 16 SPEAKER BIOGRAPHY Penelope Bryant Infectious diseases Paediatrician, Royal Children s Hospital, Melbourne, Australia A/Prof Penelope Bryant is an infectious diseases paediatrician and researcher who trained in Australia and the UK and who now works at the Royal Children s Hospital in Melbourne. She is passionate about getting children home from hospital and leads the biggest paediatric acute home care service in Australasia. Her publications provide evidence for the use and misuse of IV antibiotics in hospital and at home and her research aims to discover how we can improve our provision of antibiotics for children. 17 SPEAKER BIOGRAPHY 17 John Cunniffe Consultant in Medical Microbiology, Wirral University Teaching Hospital NHS Foundation Trust, Merseyside John Cunniffe has worked at Wirral University Teaching Hospital since During this period he has served as head of department and clinical lead for the joint laboratory project. He is currently Infection Control doctor for the organisation, and is actively engaged in antimicrobial stewardship and sepsis management initiatives. He has been involved in the Wirral OPAT project from the outset, sits on the steering group, and supports the weekly MDT review. 18 18 SPEAKER BIOGRAPHY Chris Oyewole Durojaiye Consultant Physician in Infectious Diseases and Tropical Medicine, Department of Infection and Tropical Medicine, Royal Hallamshire Hospital, Sheffield Teaching Hospitals NHS Foundation Trust, Sheffield Dr. Chris Oyewole Durojaiye is a Consultant Physician in Infectious Diseases and Tropical Medicine at Royal Hallamshire Hospital, Sheffield, United Kingdom. He is also a Honorary Senior Clinical Lecturer at the University of Sheffield. Dr Durojaiye completed his specialty training in infectious diseases and medical microbiology in Cardiff. His special interests include outpatient parenteral antimicrobial therapy (OPAT), viral hepatitis and tropical infections. 19 SPEAKER BIOGRAPHY 19 Mark Gilchrist Consultant Pharmacist in Infectious Diseases at Imperial College Healthcare NHS Trust, London Mark Gilchrist is Consultant Pharmacist in Infectious Diseases at Imperial College Healthcare NHS Trust, and Honorary Senior Lecturer at Imperial College, London. He is immediate past chair of the UK Clinical Pharmacy Association Pharmacy Infection Network (UKCPA PIN), co-chair of the British Society for Antimicrobial Chemotherapy (BSAC) UK Outpatient Parenteral Antimicrobial Therapy (OPAT) Initiative and a spokesman for the Royal Pharmaceutical Society on antimicrobials. Mark trained in Scotland before moving to London to concentrate on a career within infection. He has particular interests in antimicrobial stewardship strategies including OPAT, tuberculosis and critical care. Postgraduate education includes an MSc and non-medical independent prescribing status. He has delivered many lectures and workshops on antimicrobial stewardship both at national and international level and has published around his areas of interest. He was awarded Faculty Fellow of the Royal Pharmaceutical Society in 2013, elected onto BSAC council in 2014 and was a member of the working group that reviewed and developed the 2015 Public Health England Start Smart then focus stewardship toolkit. 20 20 SPEAKER BIOGRAPHY Achyut Guleri Consultant Clinical Microbiologist & Clinical Director, Pathology Services, Blackpool Teaching Hospitals NHS Foundation Trust, Hon. Senior Clinical Lecturer, University of Central Lancashire Dr Achyut Guleri is a Consultant Clinical Microbiologist and Clinical Director of Laboratory Medicine at Blackpool Teaching Hospitals NHS Foundation Trust, and Honorary Senior Clinical Lecturer at the University of Central Lancashire. He is also Lead Microbiologist for Lancashire Cardiac Centre, and strategic clinical lead for community/home IV therapy service (OPAT). His special interests include clinical mycology; patient safety; healthcare associated infections and antibiotic stewardship; quality, innovation, productivity and prevention (QIPP) and service development; transformational projects; and antibiotic resistance. Dr Guleri studied at the Medical School, University of Rajasthan in India. He remained in the faculty of the Department of Medical Microbiology at SMS Medical School & Hospital, Jaipur, India for 9 years before accepting a position as Head of Clinical Mycology, Dammam Central Hospital for Eastern Province, Saudi Arabia. 21 SPEAKER BIOGRAPHY 21 Elizabeth Hart Consultant in Infectious Diseases and Acute Medicine, Department of Infectious Diseases, Nottingham University Hospitals, Nottingham Liz Hart is a Consultant in Acute medicine and Infectious Diseases at Nottingham University Hospitals NHS Trust. She has worked within the OPAT (Outpatient Parenteral Antibiotic Therapy) Department since its early days. She uses her wide ranging activities to increase awareness of the OPAT service. This includes increasing the numbers of acute medical patients that are eligible for OPAT with the aim of appropriately managing the patient with multiple co-morbidities safely in the outpatient setting. 22 22 SPEAKER BIOGRAPHY Carolyn Hemsley Microbiology and Infectious Disease Consultant, Department of Infection, Guy s and St Thomas NHS Foundation Trust, London Carolyn Hemsley is Clinical Lead and Consultant in Infectious Diseases and Microbiology at Guy s and St Thomas NHS Foundation Trust in London. She studied at Oxford University, completed her specialist training in Oxford and London and completed a PhD on Pneumococcal Virulence factors at Tufts University, Boston in collaboration with Imperial College, London. Appoint as a consultant at GSTT in 2007, she took over as Clinical Lead for Microbiology and Infectious Diseases in In her time as a consultant she has developed a bone infection service at GSTT and in 2009 set up an OPAT service which she continues to lead. Her clinical practice also involves management of infection in the non-hiv immunocompromised supporting Guy cancer centre. She has been involved with BSAC OPAT initiative since inception, PI for OVIVA at GSTT and is a member of UK Clinical Research Infection Group (UKCRIG). 23 SPEAKER BIOGRAPHY 23 Tim Hills Lead Pharmacist, Antimicrobials and Infection Control at Nottingham University Hospitals NHS Trust (NUH), Nottingham Tim Hills is Lead Pharmacist, Antimicrobials and Infection Control at Nottingham University Hospitals NHS Trust (NUH). He joined NUH in 1999 and has held a number of posts before becoming the antimicrobial pharmacist in Tim is an active member of the UK Clinical Pharmacist s Association Pharmacy Infection Network and was lead author of The introduction and impact of an HTML based antibiotic guidelines website the winning submission for the inaugural UKCPA Antimicrobial Management Award in He chairs the East Midlands Antimicrobial Pharmacist s Group. His interests include OPAT where he is part of the Nottingham OPAT team and BSAC OPAT Standards committee and the treatment and prevention of Clostridium difficile associated diarrhoea. 24 24 SPEAKER BIOGRAPHY Philip Howard Consultant Pharmacist in Antimicrobials, Leeds Teaching Hospitals NHS Trust, AMR Project Lead at NHS-Improvement, Honorary Senior Lecturer at Leeds University, Leeds Philip Howard is a spokesman for the Royal Pharmaceutical Society on Antimicrobials, and for the charity Antibiotic-Action. He was a member of the Dept of Health ARHAI Antimicrobial Stewardship Group that produced the original Start Smart then Focus Antimicrobial Stewardship Guidelines, and the HPA Primary Care Unit which helped produce the RCGP TARGET guidance on Antimicrobial Stewardship in Primary Care. His research interests include Antimicrobial Stewardship, and led the first global hospital Antimicrobial Stewardship survey. He has been involved in AMS education and training across the world. In 2012, he won the GHP/GSK/UKCPA Clinical Leadership Award. 25 SPEAKER BIOGRAPHY 25 Andrew Jackson IV Nurse Consultant and Director IVTEAM, The Rotherham NHS Foundation Trust Andrew has held a senior IV nurse role since Andrew splits his time between clinical, educational, research and leadership activities. However, the vast majority of his time is occupied with clinical work and leadership activities. In addition, Andrew is the creator and director of IVTEAM.com. Achievements include: Introduction of a hospital-wide vascular access team. Development of an integrated hospital and community IV service. Development of a peripheral IV site monitoring tool the VIP score. Development of peripheral IV bundle. Development of PICC passport. Qualified independent prescriber. Organisational CLABSI rate of 0.3 per thousand catheter days. 26 26 SPEAKER BIOGRAPHY Dilip Nathwani OBE President, BSAC Consultant Physician in Infectious Diseases and Honorary Professor of Infection at Ninewells Hospital & Medical School, Dundee Professor Dilip Nathwani serves as National Specialty Adviser for Infectious Diseases to the Scottish Government Health Department, Recent Chair of the European Study Group on Antibiotic Policies and is President of the British Society for Antimicrobial Chemotherapy (BSAC). He trained at Cambridge University medical school before completing his paediatric training in London followed by his higher specialist training in paediatric infectious diseases and immunology at St Mary s Hospital London, Great Ormond Street Hospital London and Newcastle Children s Hospital. He has also worked within the field of paediatric HIV and TB in Pietermaritzburg, South Africa. His specific areas of interest are OPAT and antibiotic stewardship.
