2 Overview Building on our successful Institute of Healthcare Improvement Accelerated Patient Safety Programme, the South West Patient Safety Collaborative and South West Academic Health Science Network are launching the Patient Safety Launch Pad. This is a nine-month programme that aims to support health and social care professionals to improve patient safety and quality across the South West region. Aims At the conclusion of this programme, participants will be able to: 1. In partnership with peers, launch a plan to improve safety and quality at a system and team level 2. Describe the skills, theory and practical tools critical to developing a successful patient safety and quality improvement project 3. Participate in an ongoing patient safety community of practice at a local level 4. Use different diagnostic tools and measures to determine the safety of a system Course Outline The five days of dedicated training will focus on the following key areas to help participants develop their own quality and safety improvement plans. These include: Day 1 System of Improvement: understanding the critical foundations that lead to sustainable improvement in patient safety and quality. Day 2 Measurement for Improvement: provide a comprehensive overview of measurement for improvement including the use of run charts and the Framework for Patient Safety. Day 3 Building a Safe Culture: provide a comprehensive overview of the critical elements that comprise a safe and effective culture. This will include how to measure your team or systems safety culture using the Safety, Communication, Operational risk, Resilience and Reliability, and Engagement (SCORE) safety culture measurement tool. Day 4 Teamwork and Human Factors: this will provide you with the effective skills and capabilities to effectively engage key stakeholders in successfully implementing your patient safety system. Day 5 Adoption and Spread: an opportunity to develop implementation plans that can lead to successful adoption and spread. Faculty The programme will be delivered by an experienced faculty of patient safety and quality improvement experts from across the South West and United Kingdom. The programme will also be working in close conjunction with the Q community, offering mentoring opportunities to Q members who are invested in improving patient safety. 1

3 Additional Programme Benefits Alongside five days of dedicated training, the programme will include access to the following benefits: 1. The Life QI (quality improvement) system to effectively monitor and manage QI plans. Life QI is an online platform that allows you to build and manage your improvement project whilst gaining access to a national community of over 3000 users 2. Support in assessing and understanding the safety culture in your team and organisation 3. Ongoing mentoring and coaching throughout the nine-month programme 4. A local Community of Practice dedicated to supporting participants to develop their QI projects 5. Support in spread and adoption of best practice in patient safety and QI Course Outline Systems and Measurement 15 and 16 May Seymour Suite, Mercure Southgate, Exeter Day 1 - Starting the Journey 08:00-09:00 Programme Registration (Reception) Refreshments and pastries will be available. 09:00-09:30 Introduction to the South West Patient Safety Collaborative (Jono Broad & William Lilley) An explanation of the full programme, including how participants can connect with past and future participants and the ongoing support that is available. 09:30-09:45 A Local Improvement Story During this session, participants will be introduced to colleagues who have previously undertaken the Patient Safety Officer Programme. 09:45-10:15 Frameworks for Patient Safety (Helen Smith & Jono Broad) Upon completion of this session, participants will be able to explain the components of patient safety that affect the environment of work and how they relate to each other. 10:15-10:35 Refreshment Break (Reception) 10:35-12:00 Model for Improvement (Joanne Watson) Upon completion of this session, participants will be able to describe and apply the Model for Improvement methodology using a fun and interactive problem solving simulation. 12:00-13:00 Lunch (Restaurant) 13:00-14:00 Simulating Improvement (William Lilley) This session will be a continuation of the previous. 14:00-15:10 Designing and Developing Aims (Joanne Watson) During this session, participants will be provided with time and space to explore quality improvements that they can address using improvement methodology. 2

4 15:10-15:30 Refreshment Break (Reception) 15:30-16:00 Introduction to the Life QI System (William Lilley & Jason Williams) This session will provide an introduction to the Life QI software to design and build improvement projects. It will also provide access to a growing national community of over 3,000 improvers. This will require the use of participant s laptops. 16:00-16:30 Closing Remarks (All Faculty) 17:30 Networking Opportunity at the Mercure Southgate Day 2 - Measurement for Improvement 08:00-08:30 Refreshments including Breakfast Pastries (Reception) 08:30-09:00 Review Plans for the Day 09:00-09:45 Measurement for Improvement (Helen Smith & Richard Blackwell) During this session, participants will be introduced to the foundations and principles of Measurement for Improvement, including a fun and interactive simulation. 09:45-10:00 Refreshment Break (Reception) 10:00-12:00 Introduction to Run Charts and SPC Charts (Richard Blackwell) During this session, participants will learn how to review and analyse improvement data through the use of run charts and statistical process control (SPC) charts. They will also learn how this type of analysis can inform the development of further tests of change and contribute to the success of the improvement project as a whole. This will be completed through use of the Life QI system and will require the use of individual s laptops. 12:00-13:00 Lunch (Restaurant) 13:00-15:00 Introduction to Driver Diagrams (Joanne Watson & William Lilley) Upon completion of this session, participants will be able to describe a driver diagram and visually depict their theories of the changes required to create the outcome of interest through use of the Life QI system. 15:00-15:15 Refreshment Break (Reception) 15:15-16:30 Collaborate to Improve (Jo Redgrave) Led by our Collaboration Coach, this session will allow participants time to work on their driver diagrams and project plans. This will include support in ensuring that implementation ideas will lead to successful spread and adoption. 3

5 Faculty and Speaker Biographies Richard Blackwell, Information Analysis Manager, SW AHSN Richard joined the SW AHSN from his position as Operational Performance Manager at the Royal Devon and Exeter NHS Foundation Trust. Richard heads the data analysis work which currently includes analysis of current data in order to understand the current system and analysing variation trends across the whole healthcare system in Devon, Cornwall and Somerset so that the SW AHSN network members can contextualise their successes and challenges. Chloe Brind, Events Manager, SW AHSN Chloe organises all types of events for the SW AHSN, including; Hackathons, Conferences, Workshops and Exhibitions. She is also involved in organising national events for the AHSN Network and the Patient Safety Collaboratives. She graduated from the University of Glamorgan with a 2.1 BA (Hons) in Photography. Prior to joining the SW AHSN, Chloe worked in the Events Industry as an Operations Manager. She has worked on several high profile events such as Formula 1, Victoria s Secret Fashion Show, Wimbledon, and has thus developed strong communication and organisational skills. This knowledge has been utilised when arranging complex events & meetings within the SW AHSN, for members and for external organisations. You can follow Chloe on Twitter Jonathan Broad, Patient Leader - Lay Advisor Jonathan (Jono) Broad is a well-known patient leader in the South West. Jono has worked in the voluntary (third sector) in both social housing and healthcare for more than 30 years, with experience in leadership, strategy, service improvement and service design. Jono firmly believes that the only way to get the best outcomes for patients is to place them at the centre of both their own patient journey and service design. Whether you call it patient centred care, co-production or any number of quality improvement terms, if the patient is not involved in a full and meaningful way then we will fail both the patients and the system that we as a nation have come to love. There are many ways to engage patients in improvement methodology but the first one has to be to simply invite them in and listen, learn and then lead. Jono would be delighted to help you get the most out of your quality improvement by helping you get the most out of your patient engagement. You can follow Jono on 4

6 Matt Hill BMedSci, BMBS, MRCPCH, FRCA, the, MSc Consultant Anaesthetist at Plymouth Hospitals NHS Trust (PHNT), Regional Patient Safety Lead (Safety Culture) for the SWAHSN and Honorary Fellow at Plymouth University Peninsula Schools of Medicine and Dentistry. Matt has been a Consultant at Derriford since He is lead for the safety culture work at PHNT, and for the Patient Safety Culture work stream run by the SW AHSN across the acute hospitals, mental health, nursing homes and primary care in the South West and beyond. He is a Clinical Lead for the NHSI National Safety Culture Group and a member of the NHSI Leadership Culture Community. He is a member of the founding cohort of the Health Foundation Q Initiative. He has run and mentored QI projects and regularly teaches QI methodology to staff from all backgrounds. He is an Honorary Fellow at Plymouth University Peninsula Schools of Medicine and Dentistry where he is the lead for the Patient Safety and Quality Improvement module of the MSc in Simulation and Patient Safety, and also teaches on the MSc Healthcare Management, Leadership and Innovation. He is also faculty for the MSc Applied Heath Services Research at Exeter University. You can follow Laura on Twitter Laura Hitt, Corporate Services and Communications Co-ordinator, SW AHSN Laura co-ordinates a range of regional events for the SW AHSN and the network members, often in partnership with NHS acute trusts in the South West. She also co-ordinates the organisations use of various communication platforms including Social Media, newsletters and website management. She dedicates a large amount of time to maintaining the Information Governance and IT requirements to remain compliant with NHS standards. Previous experience includes co-ordinating and managing events within prestigious venues in the South West, teaching and coaching swimming lessons and working in a GP practice. Dr Bettina Kluettgens, Director of Patient Safety, SW AHSN Bettina trained as a pharmacist in Germany and has worked in different healthcare sectors in England for the last 16 years. Bettina completed a PhD in clinical pharmacy at the University of Bath and then worked for Lloydspharmacy/Celesio in business development and operational management where she worked with primary and secondary care healthcare partners, academics and patients to test new ways of providing complex care to patients with cystic fibrosis. Bettina then joined the NHS at the Royal Devon and Exeter NHS Foundation Trust where she was Deputy Chief Pharmacist and Medication Safety Officer, working with colleagues inside and outside the hospital to improve medication safety. Bettina then joined the SW AHSN in September

7 William Lilley, Programme Manager- Patient Safety, SW AHSN William is currently the Patient Safety Lead for the SW AHSN and manages the South West Patient Safety Collaborative. Prior to joining SW AHSN, William worked at Bromford, an award winning social enterprise that provides housing and support to over 60,000 residents in Central England. William worked in partnership with the in-house Bromford Innovation Lab and Intelligence Teams, leading on the implementation of a wellbeing programme that sought to transform how the organisation met the complex health and social needs of its resident population. William led on the design of a range of improvement projects including; an Accident and Emergency based youth worker pilot which aimed to reduce frequent attendances by addressing non-clinical issues, a home based palliative care service and a renal care social support programme. In 2013 William was selected to undertake the Clore Social Fellowship, a prestigious programme focused on supporting aspiring leaders in the social sector. You can follow William on Jo Pendray, Improvement Lead Patient Safety, SW AHSN Jo joined the South West Academic Health Science Network in February 2016 and now works within the South West Patient Safety Collaborative as an Improvement Lead for Patient Safety. Jo specialises in safety culture work and has presented nationally on the safety culture work in the South West. Jo has a BA (Hons) in Social Psychology and spent her early career working as a Human Resources specialist in the public sector; since then she has had a diverse career, including roles in quality management in the education sector and facilities and admininstration management in the charity sector. You can follow Jo on Twitter Jo Redgrave, HR Consultant and Executive Coach, Sevenstones HR Jo Redgrave is a highly experienced HR Consultant and a Fellow of the Chartered Institute of Personnel and Development (FCIPD) as well as a qualified Executive Coach (ILM level 7), together with over 20 years of experience working with senior managers and leaders across the commercial, charity and public sectors. Her main area of expertise is working with leaders on managing and implementing improvement and change. Jo is highly skilled and competent at using a range of coaching methods and techniques and incorporates a coaching approach into all aspects of her HR consultancy work. Jo has worked with leaders across a number of organisations to design and deliver a range of Organisational Design (OD) based projects. Jo applies a positive approach to her coaching work and this facilitates those she works with to develop their own understanding, awareness and insights as well as taking responsibility for applying their learning and change as an outcome. Jo, herself, has a highly professional approach with her own strong clear personal values, high personal integrity and an ethical approach to her work. 6

8 Helen Smith, Medical Director, Devon Partnership Trust Dr Helen Smith is the Medical Director and Consultant Forensic Psychiatrist at the Devon Partnership NHS Trust. She is the clinical lead for the Quality Improvement Academy in the Trust and in the South of England Improving Safety in Mental Health Collaborative. Helens first consultant post was within the Forensic Services in the West Midlands, before moving to Devon in She has a Master s degree in Criminology from the University of Cambridge (1996). Helen completed the Patient Safety Officer training at the Institute for Health Improvement in Boston USA (2008) and has a Post Graduate Certificate in Patient Safety and Clinical Risk Management for the University of Leeds (2010). You can follow Helen on Joanne Watson, Deputy Medical Director, Torbay and South Devon NHS Foundation Trust Joanne Watson is Deputy Medical Director at Torbay and South Devon NHS Foundation Trust. She has opportunities/responsibilities to deliver care fit for the 21 st Century across health and social care with the vision to create systems which support people to live healthy, productive lives with services centred around what matters to you. Her clinical expertise is in diabetes and long-term conditions management alongside expertise in quality improvement methodology. Joanne was a Consultant Endocrinologist at Musgrove Park Hospital, Taunton from , moving to Torbay in September Her quality improvement skills were built from her Health Foundation/Institute for Healthcare Improvement Quality Improvement Fellowship in A year spent sharing an office with Don Berwick and Maureen Bisognano is life changing and since returning to the NHS, person-centred care (for patients, carers and staff) has been a driving force in her work with a wide variety of local to national roles. Jason Williams, Director, Seedata Jason is a Director at Seedata, the software company that developed Life QI - the health and social care quality improvement platform. He is responsible for both the UK and international rollout of the platform so works closely with healthcare organisations to help them introduce, embed and make the most of Life QI to support their improvement work. 7
