We re very excited and looking forward to a great week! The schedule includes great activities such as a dance, beach time, lighthouse tour, marsh ecology, marsh trawl, Savannah ghost tour, and riverboat dinner cruise! Junior Camp is June 20 th -24 th at Burton 4-H Center on Tybee Island in Savannah, GA. ALL YELLOW FORMS (2) DUE ON OR BEFORE MAY 9, 2016 to the Fayette County Extension Office 1. Georgia 4-H Code of Conduct Form Please go over this document with your child. You and your child must sign this form. 2. Medical Information and Release Form (2 sided) This form MUST be completed regardless if your child has any specific allergies or prescribed medicine. Please list any prescribed medications and instructions for each of them. You MUST list any over-the-counter medication with dosage that he/she may take as needed on the back side of this form. Acetaminophen, Ibuprofen, Pepto-Bismol, and Neosporin are common medicines available at camp, so if you wish for your child to be given these medications as needed you must include that on the form. No physical is needed. Please make sure to sign BOTH sides of the form. If there is any additional information that you want us to know about your camper, please provide it to us in writing. 2 2016 Junior Camp at Burton 4-H Center Information Sheet Please keep this information until after camp is over 1. DEPARTURE FOR CAMP: Details are still being finalized, but we typically carpool with neighboring counties on a bus to camp. Please check your for details as we get closer to camp. 2. BUS RIDE: The 4-H ers are only allowed to bring 1 suitcase (no larger than 50 pounds and not to exceed 62 inches when you total length + width + height), pillow and a backpack. They can bring small snacks but ONLY WATER to drink. The sticky drinks get spilled and that messes up the buses. Since it is a long ride, they can bring head phones for the ride, but we are not responsible for them. 3. MONDAY and FRIDAY LUNCH: The 4-H ers on the bus need to bring money for a fast food lunch on the way to Burton and for lunch on the way home. Please do NOT pack it in your suitcase! Please make sure you child knows to save money for lunch on Friday. It might be a good idea to seal that money in a separate, labeled envelope. Your child is still responsible for keeping up with this and all of their spending money. 4. PACKING: See packing list for items to bring to camp, and what NOT to bring to camp. Please be sure to label everything with camper s name and Fayette County 4-H. 5. CODE OF CONDUCT: All 4-H ers will be expected to observe the rules and regulations of the camp. Please thoroughly read the Georgia Code of Conduct before you and the camper sign it. All 4-H ers are cautioned not to bring or purchase items (such as water pistols or balloons, shaving cream, fireworks, etc.) that could cause disciplinary problems. Cases of misconduct may result in the 4-H er being dismissed from camp and sent home at the parent/guardian s expenses. As an adult signing the code of conduct, you also share in your child s conduct and their consequences. 6. SPENDING MONEY: 4-H ers may bring a small amount of money for souvenirs and snacks from the canteen at the 4-H Center. Note on the tentative schedule there are shopping opportunities on Tybee Island and on River Street in Savannah. Remember your child is responsible for their own money. Please do not send large amounts. Visit to view items available for purchase at the camp canteen. 7. MEDICATION: Please make sure to turn in all medication (along with full written instructions) to the 4-H office personnel when you check in to leave for camp. Please make sure to complete both sides of the medical release form. The back side of the form allows you to list all of the over-the-counter medications that your camper is allowed to take while at camp. No camper will be permitted to have or use any medications that are not listed on this form while at camp. Please send medicines with your child in a Ziploc bag with the label, instructions, and dosage taped to the inside of the bag. These will be collected the morning of departure. Friday morning all medications will be packed in your child s suitcase. Please notify in writing on medical form if this is an issue. 8. DIETARY RESTRICTIONS: Please make sure to notify the office of any dietary restrictions or substitutions that need to be made for your camper. Turn over

3 9. CELL PHONES and EMERGENCY NUMBERS: Junior campers may bring cell phones to camp. During those times that the Junior Camp schedule legitimately allows campers to return to their cabins/dorms/rooms, campers may use their cell phones; but, the phones should remain in cabins/dorms/rooms during the day s activities and must be put away at lights out. Cell phones may be confiscated and returned at the end of the week if their possession or use is determined to be disruptive to camp activities. If you have an emergency and you need to get in touch with your camper, please contact the Fayette County Extension Office at x7 from 8:00 am 5:00 pm or Burton 4-H Center (912) CAMP PHOTOS: We have a 4-H photo site which we will post photos from camp. We will send an out when photos are posted. The site address is: select pictures & videos and the password is fayetteco4h 11. CAMP SAFETY: For your child s safety, no camper will be allowed to leave camp without: A. A signed letter of permission from the parents or guardians B. Proper checkout as arranged through the County 4-H Agent. 12. ARRIVAL HOME: TBD

4 2016 Junior Camp at Burton 4-H Center Packing List Campers are restricted to only one suitcase (pillow and sleeping bag may be strapped to outside.) Waterproof sunscreen! Comfortable clothes for 5 days that can get sandy and dirty (must meet school dress code) More formal attire for the dinner cruise and dance (ladies: skirt or appropriate sun dress, men: collared shirt) Linens (for extra-long twin mattresses) and or sleeping bag Pillow Alarm Clock One-piece swim suit or tankini that meet in the middle (all campers will wear 4-H Center swim shirt while at beach) Plastic bag for dirty & wet clothes and large ziplock bags Towels, washcloth, and beach towel Insect repellant Sunglasses/hat Raincoat/poncho- it rains often at the beach! Spending money for souvenirs, change for drink machine Waterproof watch Toiletry items (toothbrush, toothpaste, soap, shampoo, deodorant, shower shoes etc.) Shower shoes (may be flip-flops, but they cannot leave the cabin. Flip-flops cannot be worn to beach!) Sport Sandals (must have a heel strap. Wear these to the beach!) Comfortable Shoes (Bring at least 2 pairs one that can get wet and muddy for the marsh. Rain boots work well) The right shoes make all the difference! Please bring appropriate shoes Optional Items: Cell Phones are allowed at Junior Camp, but are brought at your own risk and should be used only at night in cabins Disposable camera (bring expensive cameras at your own risk!) Day pack Water bottle Pen, paper, and stamps to write letters to family and friends * Please write your child s name & Fayette County on EVERYTHING, so nothing gets left behind * Clothing guidelines: 1. No clothing should be worn that violates the Code of Conduct. 2. Keep these clothing guidelines in mind: halter tops, backless shirts, one shoulder tops, shirts with spaghetti straps, tube tops, extremely tight clothing, visible midriffs, revealing pants, or clothing that advertises alcohol or tobacco products are NOT allowed. Tank tops straps need to be wide enough to touch the shoulders or at least 2 inches wide. There should also be no visible cleavage or visible undergarments (boys and girls). 3. Camp is rough on clothes bring only clothes that are okay to get wet or muddy. 4. Bring extra socks and underwear you always need them! Turn over

5 What NOT to bring: FLIP FLOPS are NOT allowed at 4-H Summer Camp for safety purposes Expensive cameras, radios, video games, CD players, MP3 players, portable DVD players, electronic games, other electronic devices, shaving cream, aerosol cans, weapons of any kind (knives, matches, lighters, scissors, water guns, water balloons, fireworks) or anything else that would not be allowed at school. o If any of these items are found in the possession of a 4-H er, the article(s) will be confiscated and will not be returned to the owner until we return to Fayette County after camp. Multiple offenses could constitute grounds for the camper to be dismissed from camp at the parent/guardian s expense. o Please do not bring any expensive items to camp. Cameras, nice watches, jewelry, and other valuable items will travel at the owner s risk. The Fayette County Extension Office and Burton 4-H Center are not responsible for lost or stolen items. (For those that wish to take photos, we suggest disposable cameras) 4-H Statement: 4-H ers are to leave radios (boom boxes and walkmans), computers, televisions, and video games at home unless authorized by and used under direct supervision of the member s leader. 4-H ers should be discouraged from bringing cell phones. No programs are to be disturbed or interrupted with personal electronic equipment. 4-H ers are not allowed to bring skates, roller blades, bicycles, laser lights or other personal items that would make safety an issue. Safety and damage/loss of expensive items are the concern.

6 2016 Junior Camp at Burton 4-H Center Rules & Regulations The following will be covered during the camper orientation held upon arrival. Acquaint your campers with these rules and regulations before your trip and it will help make a smooth week at camp. Listen to counselors and adults. A raised hand indicates that it is time to listen. Shoes, wet or dry, must be worn at all times. The only exception is the volleyball court. Please, no flip flops outside of the dorms Do not leave money in the cabin. No running or climbing on the A-Frame roof. No students allowed on dock, trail, or marsh boardwalk without an adult. No food or drink in dormitories. Rinse off the mud and sand outside at the hoses. Wet shoes can stay outside. Quiet hours are from 10:00 p.m. until 7:00 a.m. Collect only empty shells. Shells are to be rinsed off outside, not in the sink. Do not collect live sand dollar sea urchins. Campers are responsible for cleaning their dormitories before departure. Cleanup is supervised by the adults and agents in that dorm. No camper is to leave the Camp property without an adult. Put trash and recyclables in appropriate containers. The Burton 4-H Center wants to keep your child safe and our grounds maintained. Your assistance with these rules is greatly appreciated.

10 GEORGIA 4 H CODE OF CONDUCT 4 H ers Name: County Address: Phone School: Grade: Year: BEHAVIOR STANDARDS The Georgia 4 H Code of Conduct is valid for one year and applies to all activities coordinated through Georgia 4 H. 4 H ers are expected to attend all sessions as part of a planned program exhibiting positive character and behavior including (but not limited to) trustworthiness, responsibility, respectfulness, caring, citizenship and fairness. 4 H ers are expected to be responsive to the reasonable requests of leaders and respectful of the needs for their personal safety and the safety of others. 4 H ers should dress appropriately, use appropriate language and respect the rights of others. 4 H ers may not behave recklessly or in a manner which prohibits others from participating in the program in the manner attended. 4 H ers may have access to technology at UGA/CES offices and facilities. Technology use is for educational purposes. 4 H ers may not access inappropriate websites or materials. Realizing these guidelines are not all inclusive the University of Georgia Extension staff and volunteers reserves the right to make adjustments to these policies. CONSEQUENCES OF MISBEHAVIOR 4 H ers and adults who observe a breach in the Code of Conduct must report the misbehavior to the appropriate leader. The leader will complete an incident report and determine the next steps regarding the incident. If 4 H ers are found participating in actions listed below, law enforcement or other legal authorities may be notified and may lead the review and consequences related to the incident. In these incidents, 4 H ers may be removed from the event and suspended or expelled from future 4 H participation. These behaviors may include, but are not restricted to: Possession or use of illegal drugs Possession or use of a weapon Assault or harassment Inappropriate sexual behavior If the 4 H er is found participating in the actions listed below, 4 H leaders may be notified and may lead the review and consequences related to the behavior. 4 H ers misbehaving will have the opportunity to explain their actions to leaders in charge of the activity and may request a review board. The person coordinating the event may also convene a review board for the purposes of determining what has occurred and what disciplinary action should be taken. A review board will consist of one Extension faculty or staff member, two volunteers and three 4 H members. The Extension faculty member coordinating the event will serve as chairperson. In some cases, incidents are deemed serious and may be referred to law enforcement or other legal authorities. If the 4 H er receives consequences from the leader or through the review process, his/her parents/guardians may be notified; the 4 H er may be sent home at the parents expense and may be suspended from participation in 4 H events. Suspensions may be up to one year. If a 4 H er wishes to appeal the decision of the review board, the 4 H er must appeal in writing through the County Extension office. Appeals must be filed within 10 days of notification of the disciplinary action. The appeal is sent to the Program Development Coordinator of the 4 H member and the State 4 H Leader for ruling by the State 4 H Leader Following any disciplinary review, the person coordinating the activity will provide written notification to the appropriate parties including but not limited to the 4 H er, his/her parent/guardian and his/her county Extension faculty member. Breaking curfew or disturbing the peace Unexcused absences from the activities or premise of an event event Unauthorized use of vehicles during the event Reckless or inappropriate behavior Use of foul or offensive language Possession or use of alcohol or tobacco Breach of the 4 H Code of Ethics Remaining in the presence of those who are breaking the 4 H Code of Conduct Theft, misuse or abuse of public or personal property Possession of fireworks PARENT/GUARDIAN & 4 H er AGREEMENTS Release Waiver of Liability and Covenant Not to Sue I have read the Georgia 4 H Code of Conduct and agree to participate fully in all aspects of program activities.
