3 3 CHURCH POLICY STATEMENT: Children and/or teenagers participating in church activities that involve traveling extensively, traveling outside of the greater metropolitan area, or being away from home overnight are to be provided with Parent Permission Form that must be signed and returned to the leader of the sponsoring class or church organization before the activity. The use of the Parental Permission Form represents a good-faith effort on the part of the church to keep parents fully informed and to provide the proper care for the children/ and or teenagers. I hereby grant permission to participate in the Youth Retreat. I have read the accompanying information about the activity. In the event of an emergency I may be reached at the numbers above. I further grant permission to the Director of the activity (or authorized designee) to dispense to my child any prescribed medication he/she is currently taking. I understand that I will be notified in case of serious injury or illness. However in the event that I cannot be reached, I hereby give permission for my child named above to be medically treated by a physician or medical facility as appropriate. I also release and any other church employee whether volunteer or compensated from liability for administering medication or seeking medical assistance for my child if needed. I understand that serious acts of misbehavior on my part may result in my immediate dismissal from the Tour and return home at the expense of my parents/guardians. 4 RETREAT RULES 4 I will participate in the youth Retreat program as planned. This means I am required to attend all workshops, services and activities. I will not leave the Retreat grounds at any time! A youth leader must know where I am at all times. I will exercise Christian etiquette at all times! I will dress appropriately at all times. Girls - No halter-tops, spaghetti straps, low-cut, or midriff blouses! No short shorts, tight pants or bikini swimsuits! Boys No muscle shirts, no underwear showing, and no Speedos or bikini swimsuits. No girls in the guys rooms and no guys in the girls rooms. NO EXCEPTIONS!! I will not bring CD or tape players to the worship services or workshops. No cell phones allowed! I will clean up after myself and respect the property of the Great Wolf Lodge. I will retire to my room at the designated time for devotions and be in bed at Lights Out. I will cooperate in every way possible to make this youth Retreat a benefit to others and myself. Any failure on my part to comply with these and other rules that will be discussed at the Retreat site, is just and fit cause for my dismissal from the Retreat grounds! RETREAT OBJECTIVES 1. Learning to live, serve, worship, and play in ways that honor God. 2. Enlarging my personal knowledge of life, God, Christ and the Bible. 3. Applying Christian truths and virtues to my life for victorious living. 4. Discovering and developing talents in worship and service. 5. Finding God and God s plan for my personal life. 5 NECESSITIES AND EXPECTATIONS 5 Please mark all your personal items. Bring your bible! 1. You will need appropriate pajamas, swimming suits and towels, bug spray, sunscreen, shower shoes and toiletries (i.e., deodorant, soap, powder, lotion, toothpaste, toothbrushes etc.). 2. Swimming suits are expected to be appropriate for a Christian Retreat atmosphere. No Speedos or bikini swimsuits. NO EXCEPTIONS!!! 3. Shorts must be of appropriate lengths and pants/jeans should not be tight. 4. No low-cut blouses are acceptable. 5. Please bring a pair of blue jeans to wear with your Retreat souvenir t-shirt on Saturday. 6. Electronic Devices (CD players, IPods, appropriate music, etc) are allowed on the bus and in the rooms with permission from the dorm leader, but not in the sessions at all. No cell phones or pagers. Main contact number is on The Life Permission Form. In addition, parents will receive a list of at least five numbers that they can use to reach their child if necessary. 7. No guys in the girls rooms, and no girls in the guys rooms. NO EXCEPTIONS!!! 8. If you have a medical card, please travel with it, as well as any medications that you are required to take. A medical form MUST be filled out and signed by your parent, so we can be aware of any pertinent medical conditions. 9. Bring a minimum of $20 to $30 of personal spending money. The Retreat agenda and rules for the weekend will be given to you upon arrival at Great Wolf Lodge and you will be expected to adhere to all rules set forth by Great Wolf Lodge or your parents/guardians may have to make arrangements to pick you up early from the Retreat center. Behavior that endangers others or disrupts the learning environment in anyway will not be tolerated!
