4 Educational Content The Texas College of Emergency Physicians Annual Meeting 2009 is designed to meet the CME needs of healthcare professionals that routinely deliver emergency medical services to the citizens of Texas. Discussion of topics of interest to emergency medical service providers will be presented. These include subjects related to direct patient care and patient advocacy as presented in traditional didactic sessions and panel discussions that encourage audience participation. Faculty: Rick Robinson, MD FACEP 8:15 9:15 am Current Issues in Emergency Medicine Faculty: Nick Jouriles, MD FACEP. Angela Gardner, MD FACEP Emergency Medicine (EM) is currently at the forefront of Healthcare Policy Reform. High profile challenges such as universal healthcare, tort reform, increasing Emergency Department visits in the face of declining resources, and the National Report Card on EM are but a few of the issues we are involved with as a specialty. The presenters will outline several important efforts underway where EM is leading the charge. 9:15 9:45 am Texas Emergency Medicine Review Faculty: Angela Straface, MD FACEP Texas Emergency Medicine (EM) continues to push for improved access to care throughout the state. TCEP remains a strong proponent of tort reform which is responsible for unprecedented numbers of new Texas medical license applications. The presenter will review the many issues TCEP is currently advocating for along with those we oppose that may harm recent progress within the state. 9:45 10:30 am What the AMA Means to Emergency Medicine and Our Patients Faculty: James Rohack, MD The American Medical Association (AMA) is actively involved in healthcare reform efforts. As the largest medical society in the United States the AMA is able to influence discussions in this area. The keynote speaker will review strategic components of the AMA s position on healthcare policy. 10:30 10:45 am Break Visit the Exhibitors 10:45 11:30 am Healthcare Policy Town Hall Meeting Faculty: James Rohack, MD FACEP, Angela Gardner, MD FACEP, Nick Jouriles, MD FACEP, Arlo Weltge, MD FACEP. Angela Straface, MD FACEP Healthcare continues to be one of the major issues of concern to Americans. Calls for reform to the current system echo from coast to coast. The panel will provide insight into the issues expected to be of vital importance to Emergency Medicine as healthcare reform attempts unfold this year. 11:30 1:00 Lunch Texas Emergency Bulletin Panel Faculty: John Haynes, MD FACEP The Emergency Medicine (EM) literature provides ample opportunity to reflect on individual and group practice standards. The panel will review select EM articles and debate the pros and cons of results contained in each. The audience is encouraged to participate in the debate.
6 Friday, April 3rd - Track 1 1:00 2:00 pm The Management of CHF in the ED: Are We Drowning in Evidence? Faculty: Robert Suter, DO MHA FACEP Congestive Heart Failure (CHF) patients frequently present to the Emergency Department (ED). Traditional workups and therapies versus new modalities have been widely debated in recent years. The speaker will review diagnostic, pharmacologic, and ventilatory strategies used in treating this patient population. 2:00 3:00 pm Post-Resuscitation Care Faculty: Jay Kovar, MD FACEP The initial assessment, evaluation, and interventions which occur at the point of medical contact have significant impact upon outcomes of morbidity and mortality. Emergency providers must know how to recognize and implement processes to maximize survivability and reduce complications and their associated costs. The speaker will review these points in detail as they apply to post-resuscitative care. 3:00 3:15 pm Break Visit the Exhibitors 3:15 4:15 pm Medical Errors Faculty: David Robinson, MD FACEP Medical errors and efforts to minimize them are gaining attention through various news media. Recognition that errors occur is the first step in establishing a program to address frequency and monitor reversal outcomes. The presenter will provide an overview of the problems associated with medical errors and offer solutions to arriving at effective reduction control efforts. 4:15 5:00 pm Hot Topics in Tox Faculty: Larissa Velez, MD FACEP The average Emergency Physician addresses complaints of known or suspected toxicolological emergencies frequently. Prompt recognition and treatment of these conditions provides for best outcomes. The speaker will review key concepts regarding trends in toxicology as they relate to Emergency Medicine. Competition 1:30 4:30 pm Resident Research Poster Presentation Faculty: Samuel Luber, MD Residents from the various Emergency Medicine training programs in Texas will present research posters. The most notable research project(s) will be determined based on the individual Resident s presentation of his/her poster and subsequent answers to questions from Faculty in attendance.
7 Friday, April 3rd - Track 2 1:00 2:00 pm Customer Service Pearls Faculty: David Arai, MD FACEP Providing quality healthcare to ensure best outcomes is integral to the mission of every Emergency Medicine (EM) training program. The importance of patient satisfaction is not always given the same emphasis during the residency years. It is a critical component to group contract longevity and therefore becomes important to the individual provider once he or she joins the EM workforce. The speaker will provide insight into the importance of customer service in this setting. 2:00 3:00 pm EMTALA: New and Improved Faculty: David Wright EMTALA continues to loom large over Emergency Medicine. This landmark legislation drives the engines of our Emergency Departments whether we like it or not. The speaker will review recent regulatory changes along with common compliance pitfalls. Keys to a less stressful coexistence between CMS and Hospital Administrators will be provided. 3:00 3:15 pm Break Visit the Exhibitors 3:15 4:15 pm Boarding Solutions Faculty: Shkelzen Hoxhaj, MD FACEP, James Williams, DO FACEP Boarding of admitted patients in the Emergency Department (ED) continues to be one of the major issues impacting quality of care in the hospital setting. Failure to promptly transition patients from the ED into the hospital proper contributes to increased morbidity and decreased patient satisfaction. Novel approaches to decompressing the ED have been met with opposition from the inpatient side. The speakers will address this topic and review controversies surrounding the solutions enacted at select institutions. 4:15 5:00 pm Flip This ED Lessons in Transition to a New Home Faculty: Keith Stone, MD FACEP Many Emergency Physicians will experience the transition from an existing Emergency Department (ED) to a new or remodeled one. Whereas most will welcome the changes promised by the new physical plant and equipment few may realize the potential problems often encountered with systems and personnel. The speaker will review the case of an ED that transitioned from 24 beds caring for 57,000 patient visits per year to a new facility with 44 beds that witnessed an immediate 17% increase in volume. Complex issues including the mistake of incorporating old processes in a new facility and the importance of alliances and teamwork in making the transition a success will be reviewed.
8 Saturday, April 4th - Track 1 8:00 9:00 am Disaster Management: The Art of Triage Faculty: Ira Nemeth, MD FACEP Disaster Medicine has emerged as an important subspecialty within Emergency Medicine (EM) in recent years. A number of natural disasters as well as the terrorist attacks of 911 have solidified the need for highly skilled healthcare workers as integral members of regional disaster coordination efforts. Triage is one the most important skill sets required in disaster management. 9:00 10:00 am Give Me a Break: Pediatric Orthopedic Injuries Faculty: Antonio Muniz, MD FACEP FAAP Emergency Physicians routinely encounter children with orthopedic injuries. Knowledge of the unique features of pediatric orthopedics will assist in the proper management of these injuries. The speaker will review common pediatric orthopedic injuries with emphasis on recognition and management. 10:00 10:15 am Break Visit the Exhibitors 10:15 11:45 am Workforce Issues in United States Emergency Departments Faculty: Carlos Camargo, MD FACEP The Emergency Medicine (EM) workforce needs have changed significantly over the last 20 years. Several studies have concluded a serious shortage of personnel available to keep pace with increasing Emergency Department (ED) volumes. The speaker will review the current state of the EM workforce and offer insight into immediate future needs within the United States (US). 11:45 1:30 pm Lunch TCEP Business Session 1:30 2:30 pm Neuro-Ophthalmologic Diagnoses in the Emergency Department Faculty: Kathleen Delaney, MD FACEP Emergency Physicians are often faced with evaluation and treatment of neuro-ophthalmologic conditions. Knowledge of the neuroanatomy associated with these conditions will lead to improved care and higher yield consultations when planning follow up. Common visual disturbances and their management will be addressed by the speaker. 2:30 3:30 pm Challenges in Psychiatric Care in Texas Emergency Departments Faculty: Robert Greenberg, MD FACEP Texas like most other states has seen a drastic decline in psychiatric services available to patients with acute decompensated mental illnesses. These patients often consume significant Emergency Department resources when they present for stabilization. The legal considerations that accompany management of these patients should be understood in detail by those providing their care. The speaker will engage in open discussion regarding the
9 Saturday, April 4th - Track 1 - Continued challenges of emergency psychiatric care of patients in Texas with an emphasis on the legal challenges unique to the state. 3:30 3:45 pm Break 3:45 4:45 pm Law Enforcement Medicine Faculty: Jeffrey Metzger, MD FACEP The interface between law enforcement and medicine is growing. Police officers use many devices that can have medical implications. The field of tactical medicine has developed significantly in the last two decades. Opportunities exist to work with law enforcement agencies in their attempt to improve their service to the community through educational programs. 4:45 5:45 pm Sepsis Review Faculty: Christopher Colvin, MD FACEP Since the landmark study was published in the New England Journal of Medicine, Early Goal Directed Therapy (EGDT) has rapidly become an emerging standard of care for treatment of septic shock. Timely recognition of sepsis coupled with aggressive resuscitation is promoted for this patient population. The speaker will review management strategies and limitations of therapy along with current best practices as defined in the 2008 Surviving Sepsis Guidelines. Local research in this area will also be discussed. Competition 8:00 11:45 am Clinical Pathological Case (CPC) Competition Faculty: Joanne Oakes, MD FACEP Residents and Faculty from the various Texas Emergency Medicine training programs will participate. Residents will present interesting cases. Faculty will discuss the cases and work through the differential diagnoses suggested. Residents will then summarize the case informing the audience of the actual diagnosis and outcome. Best Presenter (Resident) and Best Discussant (Faculty) will be selected by those in attendance. 1:30 4:45 pm Emergency Medicine Jeopardy Faculty: Antonio Muniz, MD FACEP FAAP Residents and Faculty from the various Texas Emergency Medicine training programs will participate in a competition to determine the best group fund of medical knowledge.
10 Sunday, April 5th - Track 1 Saturday, April 4th - Workshop 8:00 5:45 pm Ultrasound Faculty: Greg Press, MD RDMS Jason Gukhool, MD Dave Spear, MD FACEP Ultrasound is a valuable tool that many Emergency Physicians use in diagnostic and procedural applications. This TCEP Ultrasound Workshop will focus on (1) procedural applications and (2) out-of-hospital applications. Ultrasound is commonly used to guide procedures such as central and peripheral intravenous access, paracentesis, thoracentesis, pericardiocentesis, abscess drainage, foreign body localization, and lumbar puncture. Emergency Physicians are leading the way in utilizing ultrasound in out-of-hospital settings such as disaster and mass casualty events, military and space applications, and ground and air prehospital use. The didactic sessions will be followed by a hands-on training workshop using live models. 8:00 9:00 am Applied Capnography Faculty: Raymond Fowler, MD FACEP Waveform capnography is one of the most critical skills healthcare personnel need to master. A thorough understanding of the physiology of carbon dioxide transport is required to provide for management of high acuity patients. This skill has been validated in both the pre-hospital and Emergency Department settings as contributing to best outcomes for patients requiring ventilation assistance. The speaker will review capnography and illustrate its application in this setting. 9:00 10:00 am Pediatric Gastroentiritides Faculty: Ashley Kumar, MD Outbreaks of pediatric gastroenteritis are common in Texas. Patients often present to the Emergency Department dehydrated. Emergency Physicians must be able to differentiate between bacterial and viral gastroenteritides in these children. Knowledge of those pathogens involved with current outbreaks is essential to providing best outcomes. 10: am Marco on LLSA Faculty: Marco Coppola, DO FACEP The Life-Long Learning Self-Assessment (LLSA) has become an annual requirement for all Board Certified Emergency Physicians. This two hour session will summarize the pertinent concepts of the twenty articles from which the 2009 LLSA examination is derived. American Board of Emergency Medicine recommendations for test taking techniques will be offered. Audience participation will be encouraged.
