Inside this issue: Nomination Form 2 Sixty CHAP Training Center and Field Instructors from around the state shared their knowledge and insight as we participated together in a week of learning activities. Elsie Dexter (a CHP-S/I from Selawik) inspired the group in her welcoming remarks by sharing three reflective questions that connected participants through the telling of their stories: 1)What kept you this long in the Health Aide field? 2) What kind of learning spiral are we leaving for the next generation? 3) Are we delivering positive energy and technology to help our village health professionals move forward? The spirit of the Convocation was highlighted through participants evaluation comments: awesome, healing, invigorating, wonderful opportunity to connect and share, and gain perspective. Through ten CME presentations and many interactive CHAP discussions, participants left with a variety of new ideas and tools. The nomination form has been revised and there are now more award categories. New Motivated CHA Recipient will receive a trophy for excellent service to their community, specifically awarded to a new CHA. Experienced CHA/P Recipient will receive a trophy for their hard work and devotion. Criteria is a minimum of 5 consecutive years served as a CHA/P. In Memoriam This award is given out only if a deceased CHA/P has been nominated. This award is to memorialize a CHA/P who gave outstanding service and patient care to their community. Outstanding Accomplishments in the Community Recipient will receive a certificate for accomplishments in their scope of work. For example preventative health care, 100% immunizations, emergency care, etc. 100% PEFs Recipient will receive a certificate for submitting 100% PEFs during a minimum of a 3-month time frame. 4 Volume 9, Issue 1 Page 4 Community Health Aide Association The Community Health Aide Association (CHAA) is making up for lost time. After a two year absence, we are meeting regularly. There have been 5 teleconferences so far with good attendance and we encourage participation, especially since the CHAA is all about supporting and advocating for CHA/Ps. One of the Association s main goals is that of networking with CHA/Ps from around the state. We are hoping for participation from health aides from all organizations. Two sub-committees were formed, the Safety Committee and the Burn-Out Committee. The Safety Committee is coming up with ways for the CHA/Ps to avoid going into unsafe situations. A letter will be sent out to address the village councils to urge that if there isn t a VPSO in the town, then a responsible individual will hopefully be assigned to go on emergency or after hour calls with the CHA/P. The Burn-Out and Retention Committee has developed a one-page document that can be laminated and posted in the clinics. It addresses What is, and What is Not an Emergency as well as the hours of operation of the clinic. There is a space for emergency contact information to be added. Community Health Aide Program Certification Board Newsletter Behavioral Health Aide Project Update 123 Behavioral Health Aide (BHA) positions have been established and 144 communities/villages are receiving village-based behavioral health services. Certification standards for the four levels of BHA provider have been drafted and the work group is drafting unified certification language that will include BHAs with CHAs and DHAs. BHAs have been receiving training/education through in-service training, conferences and workshops, 3-week Counselor Training Academies (non-ua based) and University of Alaska coursework. They can also be reached by phone at (907) Dental Health Aide Update ANTHC received a W. K. Kellogg Foundation grant to train Dental Health Aide Therapists (DHATs) to provide selected dental services for Alaska Natives living in remote and rural communities throughout the state. The first year of the program will be coordinated by the University of Washington School of Medicine s MEDEX Northwest, a program that trains physician assistants for the states of Wyoming, Washington, Alaska, Montana and Idaho. The first year of training will focus on oral disease prevention; courses will be taught by faculty from the UW s School of Dentistry. Alaska dentists will provide clinical rotation for the students. DHATs provide oral health prevention and other services to rural Alaskan communities under general supervision by dentists. Individuals selected for the program are residents from rural Alaskan communities. The training program is a two-year program with the first year in Anchorage and it starts on January 16, The deadline for applications was on November 20, This is an exciting career where you are based in your own home town, providing excellent service to your community. 5 Community Health Aide Program Certification Board Newsletter Below are the amendments to the Community Health Aide Program Certification Board Standards and Procedures, approved October 3, A full copy of the standards will be sent to each person certified by the board as soon as they are published. 1. References to the Alaska Community Health Aide/Practitioner Manual and Village Medicine Reference were updated to reflect 2006 edition. Amended Sec (5), as follows: (5) CHAM means the Alaska Community Health Aide/Practitioner Manual, 2006 as revised, or its successor if approved by this Board; Amended Sec (22), as follows: (22) [Repealed October 3, 2006] Amended Sec (c)(8)(D), as follows: (D) CHAM Medicine Handbook for medicine instructions; Amended Sec , as follows: Sec Transition. The Board shall use the CHA/P Curriculum, field work guidelines, and other materials and standards developed by committees of the Community Health Aide Directors Association, including the Academic Review Committee (ARC), Review and Approval Committee (RAC), (CHAM) Revision Committee, until such time as it can review and adopt such materials under this section. Amended Sec (a), as follows: (a) Development. The Board shall use IHS training and work guidelines, standardized materials accepted in the dental practice community, materials developed by DARC, applicable materials and standards developed by committees of the Community Health Aide Directors Association, including ARC, RAC, CHAM, and other appropriate resource material until fully integrated DHA program center and curriculum standards are developed and reviewed and approved by the Board. 2. Clarification of licensing and supervision requirements. Amended Sec (a)(9)(B), as follows: (B) as a community health aide, or community health practitioner only under the medical supervision of a licensed physician, who is familiar with the CHA/P program and CHAM and is employed by the federal government or employed by or under contract with a tribal health program operating a community health aide program in Alaska under the ISDEAA. This requirement does not preclude other physicians, dentists, and mid-level providers directing the day-to-day activities of a community health aide or community health practitioner under the direction of the physician providing medical supervision. Amended Sec by adding a footnote immediately at the end of the section title, as follows: The supervision (at what ever level is required) of a dental health aide who provides services for which a Medicaid claim will be made must be provided by a dentist. 3. References to the Indian Self-Determination and Education Assistance Act. There are a number of provisions in the Standards that make reference to the Indian Self-Determination and Education Assistance Act. Some provide the public law and statutory cite; none make reference to its many amendments. The amendment that follows simplifies references and make them all complete. Amended Sec by adding a new subsection (23), as follows: (23) ISDEAA means the Indian Self-Determination and Education Assistance Act, Pub. L , as amended, 25 U.S.C. 450 et seq. Amended Sec (6) and (7), as follows: (6) furnishes evidence satisfactory to the Board that at the time of consideration of the application the person is employed by the Indian Health Service or a tribe or tribal health program operating a community health aide program in Alaska under the ISDEAA (7) furnishes evidence satisfactory to the Board that the person will practice as a community health aide, community health practitioner or dental health aide only when employed by the Indian Health Service or a tribe or tribal health program operating a community health aide program in Alaska under the ISDEAA Certification Board Standards CHA I 25 CHA II 34 CHA III 49 CHA IV 46 CHP 239 Volume 9, Issue 1 Page 5 Amended Sec (b)(1), as follows: be employed by the Indian Health Service or a tribe or tribal health program operating a community health aide program in Alaska under the ISDEAA Amended Sec (b), as follows: (b) Employment. To be eligible to perform services under subsection (a), the person must be employed by the Indian Health Service or a tribe or tribal program operating a community health aide program in Alaska under the ISDEAA 4. Correct Error. Section (a) contains a cross-reference that is incorrect. It refers the reader to section (b) [Employment] for definitions related to supervision, which are actually found at (b) [Definitions of Levels of Supervision]. The following amendment corrected this error. Amended Sec (a), as follows: (a) Generally(a)Generally. The supervision of a dental health aide may be general, indirect or direct, as defined in section (b), provided that 5. Supervision During Training. Amended (a), as follows: (a) Minimum Requirements. A person who satisfies the requirements of subsection (b) may perform services of a certified dental health aide prior to being certified under this article to the extent the services are performed (1) as part of training required for certification; (2) as part of a required preceptorship under sections (b), (c)(1)(B) or (c)(2), (d), (c)(1)(D) or (c)(2), (d), (b), (b), (d), (3), or (2) while certification is pending after successful completion of all required training and preceptorship. 6. Standards for Amendments. This proposed amendment will address explicitly the authority of the Board to adopt amendments at the first meeting at which they are considered when the amendment is necessary to correct an error, does not have substantive effect, or is necessary to assure compliance with requirements to which CHAP is subject, in addition to emergency or exigent circumstances. Amended Sec , as follows: Sec Consideration at More Than One Meeting. The Board shall not take action on a proposed amendments to these Standards and Procedures at the first meeting at which the amendment was proposed unless it first determines that (1) an emergency or other exigent circumstances exists; (2) the amendment is necessary to correct an error in the Standards and Procedures; (3) the amendment does not have a substantive effect; or (4) the amendment is necessary to assure compliance with law or regulation to which tribal health programs are subject. Summary of Board Actions to Date The Community Health Aide Program Certification Board had held 43 meetings. 31 meetings were face-to-face and 12 were by teleconference. 7 Community Health Aide Program Certification Board Newsletter Alzheimer's Resource of Alaska The Alzheimer s Resource of Alaska was started by 2 sisters in 1984 after their mother was diagnosed with Alzheimer s. The agency offers information, referral services, family consultations, training, care coordination and in-home services. They teach families how to get a diagnosis for their family member, offer support groups, and set up community support services. In-home services include respite, chores, and consumer directed personal care attendant services. The group has a lending library and grant funds to purchase items and supplies for individuals 65+ years old with dementia that are not covered by other funding sources. Healthcare Hints- Kampu (herbal medicine) and acupuncture is covered by public health insurance. Judo-Orthopedics is a distinct paramedical profession of unique orthopedic techniques, developed over hundreds of years. Herbal remedies (that would be listed in US as a food supplement and usually not supervised by the FDA) would actually be classified as a Medicine in this country and regulated by the government. Also, the producers would be required to submit data to prove their efficiency and beneficial results. Even pets in this country can have specialized treatments. Veterinarians can provide acupuncture, aromatherapy, herbal therapy & dietary supplements specifically developed and beneficial for animals. Good Luck!! December 29, 1951 September 13, 2006 Martha Kayouktuk, CHA III We regret to inform you of the passing of Martha Kayouktuk, who served as a CHA III for NSHC and the Native Village of Diomede for the last 17 years. On many occasions Martha was the sole provider in a very isolated village. It is very difficult to get aero-medical support due to the runway being dependent on sea ice and helicopter support is marginal because of weather conditions. Martha s legacy extends much farther than many people realize. One of Martha s contributions to the Community Health Aide Program was her recommendation that Session I and Session II CHAs participate in ambulance runs for experience with emergency medicine prior to being on-call. This program was implemented and has resulted in much greater experience for CHA/Ps even from other areas in the state. Martha also contributed greatly to the overall health of the Native Village of Diomede. It is through her leadership and support that many of the residents of her village enjoy healthy lives today. Martha had resigned to spend more time with her family when she developed a terminal illness. Martha will be greatly missed by her community and her peers. April 2007 CHAP Forum The forum will take place April at the Hawthorn Suites in Anchorage, Alaska. Many new subjects will be brought up this year, such as controlled medication security and a member of the CHAA will do a presentation. Some great speakers are scheduled and we look forward to another great turnout by CHA/Ps. The Shining Star Award will also be given by the Chair of the Recognition & Support Committee, Herman Geffe, Sr. (Maniilaq). We are now entering the holiday zone. Next week will be Thanksgiving, followed a month later by Christmas, then New Year s, then Orthodox Christmas and New Year s. Holidays race by like blurred trees barely seen through the car window on my way to work. The time that used to be filled with family and grandkids now seems filled with overtime and last minute panic-shopping. I have had to pause in the frenzy, and practice taking in those deep relaxing breaths. What is all this racing around trying to do 3 things at once? Have I forgotten what the holidays were supposed to be about? It is time to do 2 things: time to be thankful, and time to take stock. The first is what Thanksgiving and Christmas (and Kwanzaa, and Chanukah, and others) are all about. I am thankful for my family, for my job, for food on the table and a roof with 4 walls surrounding the warmth of home. I have a reason to get up in the morning and go forward, and give thanks. I am grateful that this is so, despite the heavy impact that life has had on those I care about. It is also a season of New Year s, a renewal, a time to take stock of where I am and where I am going. My responses to that are still being formed. I keep thinking: I still have time. But I am a bit worried that it is not as much time as it used to be. I think of our health aide program from the same perspective: time to give thanks and time to take stock. I am thankful for the many health aides who arrive at their clinics to work and serve daily, Letter From the Chair Dorothy Hight, CHAPCB Chair whose heroism is hardly ever seen, but without which villages would suffer. And I give thanks for the resources devoted to our program. Monies have been given this year for various projects around the state. Some may think But it is not enough! The cup is halfempty. Of course it is not enough to do all that we could or should do. But it is more than we had yesterday. For that I am grateful. CHAP still draws the most caring and conscientious of people into the world of patient care, community consciousness, instruction and training, evaluation and management. Yes, the turnover is high at all levels, so we constantly are finding ourselves starting over again to develop the next group who will join us. And we hope they will stay. I am thankful for all who think that starting over is still a worthwhile endeavor; another clean slate on which to write the future of the program. And taking stock? New Year s is a renewal, a fresh start. Where is the health aide program at the end of 2006? What do we have going for us into the next year that is full of promise, potential, creativity, perseverance, and intelligence? The answers are as diverse and individual as each member of the program. How do you want your next year to go? It will be created by the thoughts and beliefs you have. If you believe it is going to be difficult, it will be. If you believe that we can make wonderful things happen, then we will. What sort of year will 2007 be for you and the program? Think it, believe it, and make it so!
