1 58th Street Community Services Ministries (Ephesians 4:11-13) The Rev. Arnett E. Thornton, Pastor P R O G R A M D E S C R I P T I O N Introduction/Purpose:, 58th Street Community Services Ministries, serves the neighborhood residents of Hough, Central, Fairfax and surrounding areas, as well as members of the church in the following ways. Its purpose is to plan, promote, coordinate and executive programs in the aforementioned areas, aimed at improving the living conditions of the Wade/Park/Central/Hough communities. This is done through the obtaining of resources and funds to share where feasible; and for the purpose of developing new approaches and methods where necessary. Additionally, it is the purpose of this program ministry to address health, wellness and nutrition issues, particularly as relates to prevention measures and screenings in the African American community. In this wise, the program places emphasis on preventive health measures such as screenings for hypertension, diabetes and cancer. News, information and other health resources are provided through a partnership with the Hough Norwood (NEON) health center located at 8300 Hough Avenue in Cleveland. The Second Chance Project Other program services include providing housing assistance, job training and placement and educational assistance to participants in one of our other programs called, the Second Chance project. This community re-entry program is now in its fourth year and addresses the needs of the formerly incarcerated through job readiness assistance, transitioning housing referral and general resource and information of community services geared to this population. This program was funded by Neighborhood Connections of the Cleveland Foundation. Partnerships have included the Downtown Citizen's Circle; North Star Re-entry Program (Oriana House and Jobs Partnership of Cleveland at Mt. Zion Congregational Church in the University Circle area.
2 Page 2 (58th Street Community Service Ministries) Hot Meals Program 58th Street Community Services Ministries also sponsors a hot meals program. This program provides hot meals to those in need on the last two Wednesdays of each month from 4:00 6:00 p.m. This program is coordinated by Mrs. Louise Alexander with assistance by Mrs. Cecelia Barber. The meals are served at, and have been since January, And was funded by the Emergency Assistance Plan Division of Catholic Charities. Life Net Ministry: A new component ministry is being offeredwill also provide if needed, life skills coaching and counseling for those individuals who feel 'stuck in a rut.' LifeNet offers personal development workshops and discussions. Program Planning & Development: This ministry will provide training to those individuals and/or groups interested in organizational planning and resourcing. It will assist with grantwriting training, including how to establish and outline organizational bylaws and needs assessment surveys. Funding is currently being sought to help organizations and groups in the church who desire to implement projects, but don't have the funds to do so. Through an application process, small grant funds can be secured through this ministry in order to do so. G O A L S & O B J E C T I V E S To achieve certain educational and charitable objectives within the community through securing and improving healthcare by providing justice and equal opportunity for all citizens by providing social services and counseling; To provide housing assistance, job training and placement, educational assistance and development of respect for self and others; To network with other non-profit and governmental agencies, and to apply for funds through grants and gifts from various funding sources; along with technical assistance for planning and development.
3 D E L I V E R A B L E S The effectiveness of each of these programs can be assessed by developing measures to determine if the above-mentioned goals are achieved. For example, for the Second Chance project, each participant will have a plan of action called the Individual Re-integration Plan, which is based on his or her personal goals. After initial screening, intake and recruitment the IRP can be reviewed. It will include a 'survival kit' of things for the participant to do in order to get his/her life back on track. Additionally, the plan is reviewed at each visit and will be used to gauge the participant's progress and success in achiving personal goals. The number of participants that agree to attend and participate in the program will serve as a measurement of the program's success. The decrease of recidivism by the increased number of participants is also a measure of success. It is intended that by the end of the project, the participant will be in a position to fully re-unite with their families and re-integrate into the community. Success of the project will also be measured by the decrease of drug and alcohol use, attainment of gainful employment and/or enrollment in educational pursuits such as continuing education or vocational training. 5 Patricia DeLeon Melton Bio Rev. Patricia DeLeon-Melton is an ordained Methodist minister in the Christian tradition, and has been for the past thirty plus years. In that time, she has also worked in the field of human/social services as a certified chemical dependency counselor and as a case manager for the Center for Families & Children in Cleveland, OH. She is the Program Director of a community services ministry called, 58th Street Community Services Ministries. This program was funded to implement a community re-entry project called, the Second Chance project, and addresses public health disparities in the African American community. It also assists churches and community groups with capacity building projects. Early in 2013, Rev. Melton became a member of the American Psychological Association (APA) and the Psychology Club at Argosy University. Also, in that year she became a consultant for a local social service agency, called the Hough Services Provider Network, as part of Catholic Charities, Diocese of Cleveland. This program works in partnership with the Department of Children & Family Services (DCFS) that addresses the needs of children in the foster care system who are aging out. Rev. Melton is currently a candidate for a degree in Clinical Psychology & Behavioral Health Sciences from Argosy University. It is her goal to pursue a career in specialized work for families and youth in crisis, particularly those affected by substance abuse and alcoholism. January, 2014
