1 Good Samaritan Medical Center Community Benefits Plan 2014 This Addendum to the Community Benefits Plan 2014 is an addendum to the Community Benefits Plan approved by the Community Benefits Council on November 6, 2013 and subsequently approved by the Good Samaritan Medical Center Board of Directors on December 19, The revisions to the original Community Benefits Plan 2014 contained within this Addendum, shall become effective as of May 21, The Addendum is attached and made part of this document. 1
2 NOW THEREFORE, in consideration of the 2014 community benefits plan priority areas, Good Samaritan Medical Center, hereby agrees that the following constitutes revisions to terms and goals of the original plan. Priority 1 Obesity Prevention and Nutrition Education Data from the Population Health Improvement Report highlight the fact that obesity continues to be a major health issue in our service area. As such, obesity and nutrition remain a community benefits priority for Good Samaritan Medical Center. Obesity rates are alarmingly high everywhere, with all available data within the GSMC service area showing that over 25% of children in grades 1, 4, 7, and 10 qualify as overweight or obese. Rates among adults are no better, with most towns reporting between 20% and 25% of adults as obese. In Brockton, the rates are even higher, reaching approximately 30% of adults. GSMC will continue to work with community partners to reduce obesity rates by promoting nutrition education. The hospital will also work to effect obesity rates through educational seminars, support groups, and charitable giving. Support and provide technical assistance to the Brockton Public School membership to the Alliance for a Healthier Generation s Healthy Schools Program, and participate as a member of the Comprehensive School Health Advisory Group. Implement the Steward Farmers Market Voucher Program and track program outcomes. Collaborate with the UMass Extension Nutrition Education Program on the delivery of Expanded Food and Nutrition Education Program in high priority communities. Collaborate in Eastern Mass Healthy Food in Health Care Work Group meetings and participate as a Healthy Food Community Outreach subcommittee member. Collaborate with local YMCAs on Healthy Kids Day Campaign for nutrition education and fitness. Long Term Goals (modified as follows): Support community-based strategies to reduce obesity rates within the service area as identified in the Population Health Improvement Report and CHNA 22 Needs Assessment. Support the development and implementation of nutrition education programs designed to meet the needs of high priority populations. 2
3 Priority 2 Diabetes Prevention and Management Education Support educational and awareness programs offered at diverse venues within the hospital service area. Support Brockton Knocks Down Diabetes campaign by providing diabetes prevention and management education. Long Term Goals (modified as follows): Foster implementation of community-wide diabetes prevention and management education in the Greater Brockton area. Priority 3 Behavioral Health and Substance Abuse Care Coordination Behavioral health is essential to maintain well-being, a healthy family and interpersonal relationships, and the ability to live a full and productive life. People with untreated behavioral health disorders are at high risk for many unhealthy and unsafe behaviors, including alcohol or drug abuse, violent or self-destructive behavior, and suicide. A recent focus group, conducted by the hospital, found depression and access to mental health support services a top area of concern within the GSMC service area. Participants noted the interaction between issues such as chronic disease and poverty, with mental health sometimes acting as a cause and sometimes as a result. Given the prevalence of substance abuse and behavioral health issues among our population, Good Samaritan Medical Center is committed to continually increasing access to services for those requiring such assistance and level of care. Promote organizational collaborations by gathering and distributing information to key hospital departments and community organizations on resources available in our service area. Implement behavioral health navigator program to address the needs of mental health and substance abuse patients seen in the emergency room requiring psychiatric inpatient/outpatient care or substance abuse treatment. Track referrals to appropriate psychiatric and substance abuse treatment facilities for optimal patient care. Long Term Goals (modified as follows): Measure impact of Behavioral Health Navigator Program on emergency room length of stay for patients requiring psychiatric or substance abuse care. 3
4 Priority 4 Medical Legal Partnership Revise the focus of the MLP to align program with the needs of those most vulnerable to disruption of care due to lack of access to health insurance benefits. Coordinate training for appropriate staff. Hospital staff will identify and refer appropriate patient cases to the MLP. Priority 5 Cancer Education and Screening Provide a minimum of two community programs in the following areas: cancer screenings, cancer prevention education, cancer awareness campaigning, as well as other community campaigns as requested by community members such as the Delete Blood Cancer-DKMS (Bone Marrow Donor Registration Drive). Long Term Goals (modified as follows): Track results of screenings and make programmatic adjustments as needed. Priority 6 Cardiovascular and Stroke Education Programs Provide a minimum of four seminars to address cardiovascular disease. Offer a minimum of four stroke prevention seminars to community members. Offer a tobacco cessation program in the hospital and in the community as needed. Priority 7 Maternal-Child Services and Support Track number of pregnant women with addictions referred to community-based substance abuse treatment. Priority 9 4
5 Ambassador Outreach Events Deleted. Revised as Community Donations. Priority 12 Public Safety Initiatives To address the issue of public safety with our primary service area, Good Samaritan Medical Center will continue to collaborate with organizations that promote reduction in violence, and promote collaboration that creates positive relationships for community members, such as the Safe Corners Program. GSMC will further engage with the Brockton Interfaith Council (BIC) on addressing root causes of violence in our community. Furthermore, the hospitals will foster its relationship with the Brockton Police Department-Jail Diversion Program. The appropriate hospital staff will continue training on crisis intervention, with a focus on emergency personnel and the security department. GSMC will collaborate in appropriate emergency preparedness trainings. Collaborate with Safe Corners Advocates to connect youth in Brockton with services in the community, such as access to health care, social support programs and emergency services. Support victims of violence and their families by making referrals to domestic violence shelters and other community based programs as appropriate. Provide opportunities for hospital emergency and security personnel to receive ongoing training on emergency preparedness. Participate in the Brockton Police Department-Jail Diversion Program workgroup and promote the Crisis Intervention Team. Long Term Goals (Added): Periodically evaluate the hospital s public safety collaborations and make adjustments as needed. 21 5
6 Additional Priority Areas Priority 13 Financial Counselors / Enrollment Provide dedicated staff time to screening for health insurance, assistance submitting health insurance applications, personal follow up with patients via phone and mail, and interaction with other community organizations to advocate for health insurance coverage. Target Population: General Population Regions Served: Brockton, County-Plymouth, Other-Southeastern MA Health Indicator: Access to Health Care Sex: All Age Group: All Ethnic Group: All Language: All Statewide Priority: Address Unmet Health Needs of the Uninsured, Promoting Wellness of Vulnerable Populations, Reducing Health Disparity, Supporting Healthcare Reform Short Term Goals: Make financial counseling staff available for training to become Certified Application Counselors in accordance with new law and state regulations in order to continue to serve unmet needs in the community. Devote hospital staff time and resources for financial counseling, supporting health care reform by assisting uninsured patients and walk-in members of the community apply for public health insurance coverage. Long Term Goals: Ensure access to financial counseling for patients and community members in need seeking to apply for public health insurance in accordance to state and federal regulations. 6
7 Priority 14 Continuing Education for Health Professionals Through our accredited continuing medical education program, provide CME courses that offer contact hour credit in subject matter related to health priorities identified in the Population Health Improvement Report. All courses will be open to community providers. Furthermore, we will collaborate with industry partners to provide education on managing population health. Target Population: General Population Regions Served: Other-Southeastern MA Health Indicator: All Sex: All Age Group: All Ethnic Group: All Language: English Statewide Priority: Promoting Wellness of Vulnerable Populations, Reducing Health Disparity Short-term Goals: Offer a CME program at the hospital, providing CMEs to those that qualify. Engage medical professionals in development of course topics. Ensure CME course offerings include priority areas identified in PHIR. Long-term Goals: Provide opportunities for accredited continuing education for health professionals in the hospital service area. 7
8 Priority 15 Workforce Development Workforce development is vital function of well-established community hospitals. GSMC is committed to developing the skills of health professionals and those in training in our community and in local schools, as well as to our employees. GSMC provides a clinical setting to nursing students enrolled in outside organizations and internships for on-site training. GSMC staff work with students in jobshadowing and mentoring projects, which offer Continuing Nursing Education contact hours. GSMC maintains clinical affiliation agreements with nursing schools and paramedic schools. Maintain clinical affiliations with schools of nursing. Preserve clinical affiliations with paramedic schools. Provide clinical environment for student orientations. Long Term Goals: Sustain educational programs that further develop the local healthcare workforce. 8
