Photo Courtesy of Our International College of Dentists USA Section Convocation and Dinner Dance will take place on October 10, 2011 at the Mandalay Bay Resort and Casino, 3950 Las Vegas Boulevard South, Las Vegas, immediately prior to the ADA annual session. If you have not yet reserved a room at the Mandalay Bay Resort and Casino, please do so soon Space is limited. Refer to page 2 for housing and ADA annual meeting registration information through the ADA Housing Bureau, Experient. Our ICD Convocation and Induction Ceremony will begin at 1:00 p.m. on Monday, October 10, 2011 in the South Pacific Ballroom Sections E, G, H & I located in the North Convention Center of the Mandalay Bay Resort and Casino. Doors open at 12 noon. No tickets are issued for this impressive ceremony which is open to all Fellows, families and guests. Monday evening, our black tie optional program honoring our new inductees begins with a Social Hour (cash bar) at 6:30 p.m. in South Pacific Ballroom F located in the North Convention Center. Dinner begins at 7:30 p.m. in the South Pacific Ballroom Sections E, G, H & I located in the North Convention Center. Plan to stay after dinner to enjoy an evening of dancing and Fellowship. Space at our dinner dance is limited Please respond early. If you will be joining us, please mail or fax the Dinner Dance reservation card on Page 4 to ICD-USA by September 2, Your cancelled check/credit card statement will serve as your confirmation. No dinner dance tickets are mailed All tickets are held in the name of the purchaser and must be picked up on Monday, October 10, 2011 from 9:00 a.m.- 6:00 p.m. at the Islander Registration Desk located in the Bay View Court North Area of the North Convention Center, or from 6:30-7:30 p.m. in the South Pacific Ballroom Section F (during the Cocktail Reception). Your table number will appear on your ticket(s). Dinner tables seat ten (10). If you wish to sit with friends, please write each name and state (if state differs from yours) on the reservation card. Otherwise, seating is by state and District groups when possible. Our International College of Dentists 2011 USA Section Annual Meeting on October 9 and Convocation & Dinner Dance on October 10 will take place at the Mandalay Bay Resort and Casino, 3950 Las Vegas Boulevard South, Las Vegas, immediately prior to the ADA annual session. The Mandalay Bay Resort and Casino is connected to the Mandalay Bay Convention Center. The ICD Dinner Dance reservation card can be found on page 4. We strongly encourage you to secure your hotel reservation and/or ADA annual session registration through the ADA Housing Bureau, Experient, as soon as possible. Our block will be held until June 24, 2011 or until rooms are exhausted, whichever should occur first. (A deposit of $250 will be due for each hotel reservation, plus any applicable ADA registration fee.) To make ONLINE hotel reservations in our ICD Mandalay Bay Resort and Casino block and/or register for the ADA annual session please enter this link in your web browser: Planning to register more than one person? The option to add additional registrants is provided on the Registration Summary page at the end of the process. AVOID clicking the BACK BUTTON during the registration process. Doing so requires you to start the process over from the beginning. Type our Password: ICDLAS (all caps) in the Password box, then click Verify Password. Click on Register Now. Fill in required information in 1) Dentist Record Lookup, then click on Find Me. Verify/fill out your Profile including the question(s) below your address, then click Next. Verify Address Confirmation, then click Next. Under Registration Type, choose the appropriate category: RADA U.S. ADA Member or RG99 Housing Only. IMPORTANT: You may only choose the category RG99 Housing Only ($0 registration fee) if you plan to attend ONLY ICD functions, and do not wish to register for the ADA Annual Session or other ADA events. Click Next. Scroll down to verify Registration Summary, Profile and Badge and Financial Summary, click Next. In Room Search screen, choose arrival and departure dates, then click on Check Availability. ON THE TOP LINE OF THE FORM click on Mandalay Bay Resort and Casino. Click on Book Reservation and follow the prompts to finish providing the required information. IMPORTANT: If you plan to arrive outside the ICD window (October 7 10) you will see a red W by the Mandalay Bay Resort and Casino and you will be placed on a wait list for one or more nights, Experient will send you an updated confirmation when the room becomes available. If room block for one or more days chosen is CLOSED, you will see this symbol Ø and you will not be able to book the room. If this occurs please book a room at a nearby hotel (THEhotel at Mandalay Bay, Four Seasons and Luxor are the three closest hotels). To make HARD-COPY hotel reservations for Mandalay Bay and/or register for ADA activities utilizing a Hard Copy Reservation Form please go online using the following web address to download form: or call the USA Section office at to request a form. Our ICDLAS (all caps) code must be written in the top, right corner of Step 1 of the form. Forms submitted without this code will not be granted access to our Mandalay Bay Resort and Casino room block. Fax reservations to Experient at or , or mail to Experient/ADA Annual Session, P.O. Box 4088, Frederick, MD Please refer to ada.org/session for further information. Housing Hotel cancellations must be received in writing by Experient before 5:00pm Central Time on Tuesday, September 16, Cancellations received by this date will receive a full refund of the hotel deposit by Experient. Please allow 60 days for refund processing. Changes to reservations must be received in writing by Experient before 5:00pm Central Time on Friday, September 16. Failure to check-in to your hotel on the confirmed date of arrival will result in the forfeiture of the entire room deposit. As of Wednesday, September 21, 2011 any changes to hotel reservations must be made directly with the hotel. Hotel deposit refunds will not be issued after September 16. ADA Annual Session Registration Advance registration saves time and money, and ensures access to the largest number of ticketed courses. After September 16, all fees will increase and registration badge materials will not be mailed. To register for the Annual Session, please select the appropriate registration category and corresponding fee. You may also purchase tickets to courses, special events, or register friends and family. Again this year, all courses, both fee and non-fee offerings will be ticketed. To attend any course, you must present your ticket. All dental professionals, even those from the same practice, must register separately. Advance registration for the Annual Session closes at 5:00 pm Central Time on Friday, September 9, Registration, ticket and/or special event cancellations must be received in writing on or before September 9. Registration or ticket cancellations received by this date will receive a full refund, less a $10 processing fee per cancellation. No refunds will be given for registration cancellations received by Experient after September 9 for any reason. Please allow 60 days for refund processing. The ADA Opening General Session is on Monday, October 10th. First-class exhibits, unparalleled continuing education opportunities and a great tour program, available to registered attendees, are extremely popular. We will use our owned robes for the Monday, October 10, 2011 Convocation in Las Vegas, Nevada. The robes are being put up for sale to be picked up or shipped to you after the 2011 Convocation. Because of the interest in purchasing robes in the past, the Executive Committee has chosen to sell them at a set price of $100 per robe plus $15 shipping and handling (if applicable). You may also choose to pick up the robe or have someone pick up the robe for you after the 2011 Convocation at the Mandalay Bay Resort and Casino, South Pacific Ballroom located in the North Convention Center of the resort, 3950 Las Vegas Boulevard South, Las Vegas, Nevada. Our ICD Robes are described as a fine-grade black poplin with velveteen trim. They have three front panels lilac, traditional hue of the dental profession; gold and dark green, the official College colors. There are regulation black velvet bars on the sleeves, with a three inch gold band below the bars. We have a few hats available at no extra charge. The newer robes have brighter colors than the older robes, which have more subdued colors. Please see pictures for clarification. (Note: Robes weigh about 4 lbs and hats weigh about 1/2 lb.) If you would like to purchase a robe, please use the order form on page 4 and Past Pres eside dent s Dr s. mail form with payment to the USA Section office. June 24, 2011 Please see note on page two referencing the room block for the ICD during the ADA Annual Session in Las Vegas. Current Fellows - This is your notification! New Fellows will get mailings with additional information. Of course we would love to see you all there; it promises to be a wonderful few days in this exciting city talk about a lot to see and do. 2. June 15, 2011 All completed Candidate Information Forms with sponsor page signatures and letters of recommendation are due in the Section Office So if you have a terrific Candidate in the pipeline, this is your heads-up call. 3. October 10, 2011 The Annual ICD Convocation and Dinner Dance. Yes, this is a Monday; your calendars are fine. The ADA did a shift this year, and since we precede them, we shifted too. 4. November 10-13, 2011 ICD College at Large (CAL) Meeting in New Delhi, India. Frequently Asked Questions: 1. I know a District can have more than one Deputy; can a state have more than one Deputy? Yes, our current Bylaws allow for multiple Deputies in a single state. California and New York are examples of multiple Deputies. The request has to be made to the Board by the Regent. 2. If a Regent is unable to attend a state meeting but the Vice Regent can attend, can the Vice Regent be reimbursed for the travel? Yes, the Regent will have to make a special request for an exception to the Bylaws, to the Section Vice President. 3. I have heard about organizations supporting Lobbying Fees does the ICD do this? Absolutely not! We have posted on our website The International College Of Dentists, U.S.A. Section does NOT in any way use membership payments to provide funding or support for lobbying efforts. News from the College at Large Office: The College at Large has selected a New Secretary General; it is John V. (Jack) Hinterman, a U.S.A. Fellow. He has been a Fellow since 1978, and has been the Editor of the College during the past six years. His long-time experience in dental association leadership positions prepares him well for his new duties. Welcome to the new position, Jack. U.S.A. Section Foundation We are making a difference I feel extremely privileged to be your Foundation President. It is so rewarding to engage in the work with a focus on helping mankind. It is always a delight to see the results of the contributions we have made in locations like Kenya, Tanzania, Haiti and Cambodia. We could not have done any of this without your continued contributions and support. This year we are picking up the funding for the Section International Student Exchange and the Student Leadership Awards. Both are outstanding programs that support the growth and development of our next generation of future dental leaders. So, yes, We are making a difference and that is being noticed worldwide. 6 M E S S A G E F R O M T H E P R E S I D E N T BOARD OF REGENTS MEET IN OREGON The spring 2011 Board of Regents meeting was held at the Allison Inn and Spa in Newberg, Oregon May 11-14, Attending Fellows and guests enjoyed a wide variety of local venues, such as traveling by The Trolley for wine tasting with a luncheon at the Joel Palmer House restaurant, visiting a working llama ranch, lunch and tour at the world headquarters of A-dec dental equipment manufacturing facility, visiting the Evergreen Museum - home of the Howard Hughes Spruce Goose, and stopping by numerous art galleries and wineries. Over thirty committees and individuals presented reports to the Board of Regents representing the vitality and diverse activities which the USA Section Fellows are leading to make a difference. Some of the highlights include: Proposed a USA-based outreach project that would create a network of ICD Fellows to provide emergency dental care to families staying in one of 54 "Fisher Houses" during the time Wounded Warriors" are hospitalized to receive rehabilitative care. Voted to increase the number of Honorary Fellowship awards to four per year with one being from industry. Celebrated the agreement, adoption and documentation of a cooperative and mutually supportive legal governance relationship between the USA Section and the USA Section Foundation. During the first two days of meetings, two outstanding professional development courses were enjoyed by the Fellows, spouses and guests. Fellow Dr. Steve Beadnell, oral maxillofacial surgeon, presented "Home and Office Medical Emergencies". Dr. Fritz Fraunfelder, opthalmologist and retirement book author, presented "Retirement Rx", and then offered a copy of his evidence-based book to each Fellow/guest group in attendance. Overall the spring BOR meeting was productive, stimulating, enjoyable and rewarding. The USA Section is making a bigger difference through ongoing leadership by all Fellows. Jack W.
