JOB PURPOSE To provide services in ENT and related specialties to Doncaster and Bassetlaw Hospitals NHS Foundation Trust (DBHNHSFT) and Rotherham Hospitals NHS Foundation Trust (RHNHSFT) in accordance with evidence based practice. This post will be based at Doncaster Royal Infirmary. Also to participate in clinical audit, teaching and maintenance of high standards of medical practice. Summary of duties:- 1. To contribute to the provision of ENT services, alongside rotational specialty trainees and established consultant staff. 2. Maintain and improve standards and quality of ENT services. 3. Participate in the on-call rota at Doncaster Royal Infirmary that covers the population of DBHNHSFT and RTHNHSFT. Out of hours cover is required as appropriate on an EWTD compliant 1 in 7 rolling rota, covering DBHNHSFT and RTHNHSFT. MAIN DUTIES/RESPONSIBILITIES The post holders will join the other 6 junior doctor grades in a partial shift system, working an average of 48 hours per week over a seven week period with internal cover. There is a daily ward round in the morning, conducted by the Specialist Registrar. The weekly rota is organised by Mr Raghavan who is the Clinical Tutor. Attendance at nominated teaching outpatient and theatre sessions is compulsory. There is a regular monthly commitment to Medical Audit and Clinical governance which is a mandatory part of the job description. There is an active Postgraduate Training Programme on a weekly tutorial basis organised by Mr Raghavan. In addition, formal education is arranged at the beginning of each 6 months period. Other teaching sessions are arranged throughout the post. The Department shares a 24 bedded adult ward with the ENT, General surgery and Ophthalmic departments. Adult patients undergoing elective surgery are admitted to the Theatre Admissions Unit prior to surgery and inpatients are then admitted to the ward. The ENT Department is well equipped; the Adult clinics are fully supported by Nurse practitioners, Audiologists and Speech Therapists. 4.1 Clinical Duties This is a permanent appointment is for a Trust Grade Doctor to work within the Department. The Trust Grade Doctor is responsible for the admission of patients to the ward and their continuing care and for handing on their care when they finish their shift. It is envisaged that the post holder will contribute to the delivery of service in the out-patient and operating theatre departments depending on previous experience and demonstration of competency. There are on average 40 medical outpatient clinics that run every week, 24 of these are Consultant run clinics, there are other specialised clinics with other disciplines such as Radiotherapy and Speech Therapy. There are on average 17 operating lists per week, the Trust Grade Doctor will be allocated to outpatient and theatre sessions equally with the other junior doctors. Although the post does not carry educational approval from the deanery, there will be opportunities for education and portfolio enhancement by completion of workplace based assessments in line with the Intercollegiate Surgical Curriculum Programme requirements. 4.2 Rota Duties The Trust Grade Doctor will work a partial shift system that will comply with the Heads of Agreement on Junior Doctors Hours December Date issued (February 2017) 3 of 21
4 The Trust Grade Doctor is also expected to provide daytime cover at the ENT Department at Rotherham District General Hospital as per the rota. 4.3 Job plan and framework Below is an example Job Plan is based on a one in seven rota covering both trusts (DBHNHSFT and RHNHSFT) 4.4 Salary The salary scale is at present per annum. It is subject to amendment from time to time in the light of national agreements. Education, Continuing Development and Validation You are expected to maintain your skills and up-date knowledge in accordance with Continuing Medical Education and Professional Development Guidance from the General Medical Council and the Royal Colleges to facilitate revalidation. 5 Audit, Clinical Governance and Research You will be expected to actively participate and contribute to the audit and governance programme of the Anaesthetic Unit, the Surgical Care Group and the Trust. You are reminded of your duty under the Health & Safety at Work Act 1974 to take reasonable care to avoid injury to yourself and others; to officially report all incidents, accidents and hazards using the Critical Incident Reporting Procedure; to use safety equipment provided for your protection at all times and to co-operate with management in meeting statutory requirements. Maintaining confidentiality of information related to individual patients or members of staff is a very important aspect of your work within the Trust. Failure to maintain confidentiality of such information may constitute a serious disciplinary offence. Staff should also bear in mind the importance of sharing essential information with carers and others, with the consent of each patient. There will also be circumstances where critical risk information will need to be shared with partner agencies, subject to guidance and advice available from your manager. 9 9. THE TRUST Who we are and what we do In 2004, Doncaster and Bassetlaw Hospitals became one of the first 10 NHS trusts in the country to be awarded foundation trust status. So we have more freedom to act than a traditional NHS trust, although we are still very closely regulated and must comply with the same strict quality measures as nonfoundation trusts. As well as being an acute foundation trust with one of the busiest emergency services in the country, we are also an associate teaching hospital of Sheffield University, an associate college of Sheffield Hallam University, and have strong links with Health Education England and our local Clinical Commissioning Groups in Doncaster and Bassetlaw. We are fully licensed by Monitor and fully registered (i.e. without conditions) by the Care Quality Commission (CQC) to provide the following regulated activities and healthcare services: Treatment of disease, disorder or injury Nursing care Surgical procedures Maternity and midwifery services Diagnostic and screening procedures Family planning Termination of pregnancies Transport services, triage and medical advice provided remotely Assessment or medical treatment for persons detained under the Mental Health Act 1983 We provide the full range of district general hospital services, some community services, including family planning and audiology, and some specialist tertiary services including vascular surgery. 10 We serve a population of more than 420,000 across South Yorkshire, North Nottinghamshire and the surrounding areas and we run three hospitals: Doncaster Royal Infirmary (DRI) DRI is a large acute hospital with over 500 beds, a 24-hour Emergency Department (ED), and trauma unit status. In addition to the full range of district general hospital care it also provides some specialist services including vascular surgery. It has inpatient, day case and outpatient facilities. Bassetlaw District General Hospital (BDGH) in Worksop BDGH is an acute hospital with over 170 beds, a 24-hour Emergency Department (ED) and the full range of district general hospital services including a breast care unit and renal dialysis. It has inpatient, day case and outpatient facilities. Montagu Hospital in Mexborough Montagu is a small non-acute hospital with over 50 inpatient beds for people who need further rehabilitation before they can be discharged. There is a nurse-led Minor Injuries Unit, open 9am-9pm. It also has a day surgery unit, renal dialysis, a chronic pain management unit and a wide range of outpatient clinics. Montagu is the site of our Rehabilitation Centre, Clinical Simulation Centre and the base for the Abdominal Aortic Aneurysm screening programme. We are also registered to provide outpatient and other health services at Retford Hospital, including clinical therapies and medical imaging. Our site at the Chequer Road Clinic in Doncaster town centre offers audiology and breast screening services. We also provide some services in community settings across South Yorkshire and Bassetlaw. The rehabilitation beds we used to have at Tickhill Road Hospital in Doncaster transferred to Montagu Hospital in August 2012 however we still provide outpatient care of older people there. Our achievements 2013/14 During 2013/14 there have been a number of important developments at the Trust, and we have made some significant improvements to the quality and sustainability of our services. This was the first full year of our strategic direction Looking Forward to our Future and its vision, mission, values and strategic themes have been central to our work throughout. 12 Provide the safest, most effective care possible In 2013/14 we improved our performance in a number of key quality indicators, including: 42% reduction in patient waiting over four hours in A&E (comparing winter Quarters) 36% year on year reduction in Clostridium Difficile 18% reduction in serious falls 9% reduction in patient readmissions 5% reduction in cardiac arrests It is of course very important that we get things right first time, but also that we respond effectively when things do go wrong. We saw a 19% reduction in new complaints year on year and there was a 45% reduction in the number of outstanding complaints. We also found new ways of assessing patient experience through the Friends and Family Test and other surveys. We are implementing a new policy to ensure that we learn lessons from complaints, concerns, comments and compliments when they are received. We have reduced mortality following improvements to our emergency pathways and the introduction of seven day working in many of our services at DRI and Bassetlaw Hospital, providing for earlier senior decision making and service availability to implement those decisions. As a result, our Hospital Standardised Mortality Ratio (HSMR) has reduced from 106 to 102 as at December The Trust s seven day working programme has also been recognised in national publications. Despite on-going work in this area, pressure ulcers remained at a high level. We are implementing a new strategy to address this and expect to be able to report improvements in 2014/15. The CQC conducted an unannounced inspection of all our sites, which found us fully compliant with all inspected care standards. We ended the year with a green rating for governance from our regulator, Monitor. Control and reduce the cost of healthcare We delivered our financial plan for the year, including the delivery of a Date issued (February 2017) 12 of 21
13 planned surplus of 3.5m (net of year-end adjustments), cost savings of 17.0m and the investment of 20.1m in capital assets, such as buildings and equipment. Delivering our planned level of surplus is very important as this funding is retained by the Foundation Trust to be invested in further developments to improve patient care in future years. Our financial health is also assessed by Monitor, the independent regulator for NHS Foundation Trusts, with a particular focus on our continuity of services risk ratings (with rating from 1-4, with 4 being the lowest risk). Throughout the year we have maintained at least a strong 3 rating, rising to a continuity of services risk rating of 4 at the end of the year. More detail on the ratings and their calculation is included later in this report. Develop responsibly, delivering the right services with the right staff The Board has focussed on the issue of safe staffing levels throughout the year. We have invested 7m in additional staff and beds and, following the introduction of the use of the Association of UK University Hospitals (AUKUH) acuity and dependency tool, supported the allocation of funding for 30 new nursing posts in 2014/15. 85% of the indicators that changed within the staff survey showed improvement. We completed work on a new strategy for People and Organisational Development, to underpin this thread of our strategic direction. We attracted significant inward investment from Health Education England to support our Nurse Practitioners programme. Day surgery is a reliable, efficient and effective treatment method and we improved our day surgery rate from 81% to 84%. It is important that we protect staff and patients from avoidable infections and for the second year running the Trust was in the top five in the NHS for flu immunisation of its staff. We consulted staff, external partners and other organisations to interpret what each of our four Strategic Themes should mean to our workforce. Focus on innovation and improvement We agreed a new Research and Development Strategy, which is being monitored regularly by the Board and has been externally endorsed by the National Institute for Health Research. We opened the new Fred and Ann Green Rehabilitation Centre at Montagu, a new Day Surgery Unit at Doncaster and the ATC at Bassetlaw. The Rehabilitation Centre supports an innovative multidisciplinary rehabilitation pathway that has reduced length of stay for patients by 32% since the Centre opened. We successfully bid for and delivered a range of maternity improvements at both Bassetlaw and Doncaster. We have also established a new frailty assessment pathway, delivered through the new Stirling Ward at DRI in collaboration with local partners. The Board approved the 18m ihospital Programme strategic outline business case. This sets out our ambition to use information technology to better support clinical decision making and release time to care. Detailed business cases were approved for Patient Administration, Emergency Department, Maternity Services and Telecommunications systems upgrade and replacement. Against the background of having a clear strategy and commitment from the Board, we successfully bid for Department of Health Technology Funds, which resulted in awards totalling 5.5M. The funds will support and accelerate our programme of innovation for improvement. Trust staff won a number of national awards including a Midwife of the Year Award, a Royal Pharmaceutical Society Award, a Gold Training Journal award for the Best Organisational Development Programme, and a Nursing Times Award for Excellence in Supporting Staff Health and Wellbeing. 15 10. THE SERVICE The trust clinical services are arranged into 6 care groups. Each care group has a senior management team of a Care Group Director, General Manager and Head of Nursing. This innovate structure builds management responsibility across patient pathways allowing for improved clinical outcomes and increased cross specialty working. These posts will be based in the Surgical Care Group. The Surgical Care Group incorporates the specialties of Theatres, Anesthetics, GI/General Surgery, Endoscopy, Ophthalmology and ENT. Doncaster Royal Infirmary is the largest site and provides the base for all emergency surgery and major elective surgery. Bassetlaw Hospital has a single surgery ward with in-patient beds and provides mainly elective services. The majority of emergency admissions are transferred to Doncaster for surgery. There are Day Surgery Units on all three sites along with Endoscopy. As part of the changes in service delivery, it is expected that Bassetlaw Hospital will be developed along the lines of an elective treatment centre for surgery, performing day case and elective surgery of intermediate complexity. Complex elective surgery and most emergency surgery will be delivered at DRI. There are plans for the development of an Assessment and Treatment Centre (ATC) at Bassetlaw, and some surgical cases will be assessed and managed on this unit, along with acute medical patients. Bassetlaw Hospital There are four wards at Bassetlaw District Hospital designated for General Surgery/Trauma and Orthopaedics/Gynaecology/A & E with a total of 70 beds. Plus a day case unit of 5 spaces. 32 General Surgical beds/gynaecological beds 5 Day Surgical beds 38 Orthopaedic beds Out-Patient clinics are presently carried out at the District General Hospital and also at Retford Hospital - Retford being a small market town eight miles from Worksop. Doncaster Royal Infirmary There is a Surgical Assessment Unit and Ward with 19 beds, 2 GI wards (54 beds in total), and dedicated Vascular, Urology and ENT wards. 17 11.0 OTHER INFORMATION Medical examination The successful candidate may be required to undergo a medical examination prior to appointment. Registration Full registration with the General Medical Council is essential. Health and safety Doncaster and Bassetlaw Hospitals NHS Foundation Trust recognises its duties under the Health and Safety at Work Act 1974 to ensure, as far as is reasonably practicable, the health, safety and welfare at work of all its employees and in addition the business of the Trust shall be conducted so as to ensure that patients, their relatives, contractors, voluntary workers, visitors and members of the public having access to Trust premises and facilities are not exposed to risk to their health and safety. All medical staff employed by Doncaster and Bassetlaw Hospitals NHS Foundation Trust will be expected to comply with Trust Health and Safety Policies. Safeguarding Children, Young People and Vulnerable Adults All staff and volunteers working within the Trust have a duty to be aware of their own and the organisation s roles and responsibilities for safeguarding and protecting children and young people, and vulnerable adults. If your normal duties are directly or indirectly concerned with patient care you must ensure you receive sufficient training, information and supervision on the measures required to prevent and control risks of infection. Local policies The post holder will work at the Doncaster Royal Infirmary, Bassetlaw and Montagu Hospitals. Therefore, he/she is expected to be aware of the local policies and procedures and to take note of the standing orders and financial instructions of the Trust. 18 12.0 OTHER FACILITIES Library and postgraduate facilities The post Graduate centre has recently been rebuilt providing state-of-the-art facilities. There is an active Postgraduate Centre at the Doncaster Royal Infirmary with Teaching Programmes for all grades of staff. There are regular weekly Seminars where Consultants and Junior Staff present cases to hospital colleagues and General Practitioners, and monthly evening meetings at Montagu, to which guest speakers are invited. There are purpose-built professional Libraries adjacent to the Postgraduate Centre under the charge of a full-time Librarian. There are over 15,000 volumes in stock and access is available by a joint scheme with other Libraries and with the National Lending Library. The Library subscribes to some 150 journals. At Bassetlaw District General Hospital there is a Post Graduate Medical Education Centre with associated library facilities. Librarian support is present and a wide range of journals and publications are available GENERAL INFORMATION ABOUT THE AREA Doncaster The Metropolitan District of Doncaster covers an area of approximately 200 square miles and the catchment area for the local PCT is around 300,000 people. The major industries in the area historically have been coal mining and engineering, but the decline in these activities has seen significant diversification and introduction of new industries. Communication and transport are ideal with ready access to the motorway network in all directions and London is only 90 minutes away by Inter City train.the airport provides easy access to European destinations. Doncaster boasts a first-class shopping centre and the market held on Tuesday, Friday and Saturday is famous throughout the North. Meadow Hall shopping centre is only 30 minutes away - one of Europe's largest indoor shopping malls. Leisure and recreation facilities are well provided for, especially at the Dome - one of the largest leisure facilities in Britain including swimming pool, ice rink and sports hall. There is also a plan to extend facilities with a dry ski slope. The building of a man-made lake and new sports stadium nearby has enhanced these facilities. Date issued (February 2017) 18 of 21
19 Doncaster is also proud of its Grade 1 Race Course which has recently been completely refurbished. It is the home of the world s oldest classic race - The St Leger and on non-race days the Race Course is used as an Exhibition Venue where the Doncaster Motor Show is staged every year. The local rugby club has benefitted from a new clubhouse and stands and there are several good golf courses close by. Worksop Worksop is situated in the northern part of Nottinghamshire and comprises a mainly rural area of 250 sq miles. Its population of 106,000, 10% of the county total, is focused on Worksop (population 39,500) on the western border and Retford (population 21,000), a market town in the centre of the district, with population clusters such as Harworth and Bircotes in the north near the A1, at Carlton-in-Lindrick and Langold along the A60, and at Tuxford and East Markham down the A1 along the southern border. Bassetlaw Hospital is situated in Worksop, and has traditionally also provided the hospital of choice for local residents of North Derbyshire and South Yorkshire. Therefore the total catchment population for the Hospital is around 170,000. Retford Hospital is the headquarters for Community Services and many community staff are based at local clinics and GP practices in the district. Doncaster and Bassetlaw Hospitals NHS Foundation Trust provides services on five hospital sites for 410,000 people living in Bassetlaw, North Nottinghamshire and Doncaster, South Yorkshire, and employs 6,000 staff. The Trust s income for the financial year 2012 was over 330 million. Annually, more than 160,000 patients attend the Accident and Emergency and Minor Injuries Departments. Every year 122,000 patients attend new outpatient appointments, and we treat more than 48,000 in-patients and day cases. Managerially the Trust is organised into Clinical Care Groups, led by Clinical Directors, which operate with considerable autonomy across all hospital sites. The Trust is developing integrated clinical services across the hospital sites, and consultants are encouraged to offer specialist interest services on two or more hospital sites. 20 14.0 THE SURGICAL CARE GROUP The Surgical Care Group incorporates upper and lower gastro-intestinal surgery, ENT, Ophthalmology, OMFS and Orthodontics, Anaesthetics, Critical Care, Recovery, Theatres, Day Surgery Services and Pre-operative Assessment. The Care Group Director is Miss A Durham Hall, who is supported by five Assistant Care Group Directors and Ms Karen McAlpine is the General Manager of the Care Group. The Assistant Care Group Director for ENT services is Mr. A Aftab and the Business manager for the service is Miss Laura Freeman. Doncaster Royal Infirmary has 119 beds in general surgery on 4 surgical wards. Two of these wards are dedicated to GI surgery. In addition there is a 23 bed Critical Care Unit. Day Case beds are available on all Trust sites providing surgical services and a new Day Surgery Unit opened at the DRI in Summer The 8 general operating theatres are located on the lower ground floor of the East Ward Block. At Bassetlaw Hospital there are beds on a general surgery and gynaecology unit and a 6 bed ICU/HDU facility. The CSU has introduced a new rota which will see all consultants work across the two main sites. General surgical emergencies are located at DRI site. There is a surgical oncall rota at BGH site to cover in-house and minor surgical emergencies.
