The FLW Lions Club was one of the first Lions Clubs formed on a military installation. Currently, our club is the only Lions Club that remains active on a military installation. The motto of Lions Clubs International is WE SERVE and the FLW Lions Club tries to live up to this motto. Lions Clubs International has 1.3 million members and is located in 208 countries and geographical areas. This is more countries and geographical areas than the United Nations. The FLW Lions Club is very active on the District and State level with four active Past District Governors (PDG) and several members serving on District and State level committees. On 30 January 2016, the vision, hope, and dream of our 25 Charter Members were realized as we joyously and successfully celebrated our FLWLC 50th Anniversary Program at the Pershing Community Center on Ft Leonard Wood. Mr. Simpson Is an outstanding member of the FLWLC for over 15 years with more than half of that time served in a very demanding and sustained attention to details of accountability in an annual by acclamation elected position as the club s treasurer.

3 US ARMY MANEUVER SUPPORT CENTER OF EXCELLENCE, FORT LEONARD WOOD, MO RETIREE APPRECIATION DAYS SEPTEMBER 8-9, 2017 PAUL K. CARLTON, JR. Lieutenant General (Retired) Lieutenant General (Dr.) Paul K. (PK) Carlton, Jr. entered the U.S. Air Force Academy in June He was a distinguished graduate of the U.S. Air Force academy and was commissioned in the U.S. Air Force on June 4, 1969 as a Second Lieutenant. Lt Gen Carlton attended the University of Colorado in Denver, Colorado, receiving his Doctor of Medicine degree in May 1973 and completed his General Surgery Residency at Wilford Hall Medical Center at Lackland AFB, Texas, in June 1978, followed by service as Staff Surgeon at RAF Lakenheath, England, from June 1978 to April Gen Carlton served as Chief of General Surgery at the U.S. Air Force Hospital at Luke AFB, Arizona, from April 1979 to May 1982, as Chairman of the Department of Surgery at the U.S. Air Force Regional Medical Center at Wiesbaden AB, West Germany, from May 1982 to August 1985, and as Commander of the U.S. Air Force Hospital at Torrejon AB, Spain, from August 1985 to May 1988, followed by service as Commander of the U.S. Air Force Medical Center at Scott AFB, Illinois, from May 1988 to August During General Carlton s time at Scott AFB, he deployed to Southwest Asia in support of Operation Desert Shield and Desert Storm as Commander of the 1702nd Air Refueling Wing Contingency Hospital from October 1990 to March Gen Carlton also served as the Director of Medical Services and Training with Headquarters Air Education and Training Command at Randolph AFB, Texas, as Commander of the 59th Medical Wing at Wilford Hall Medical Center at Lackland AFB, as Commander and Director of the Air Force Medical Operations Agency in the Office of the Surgeon General at Bolling AFB, Washington, D.C., and as the 17th Surgeon General of the Air Force until his retirement from the Air Force on September 23, General Carlton is a fellow and former Air Force governor of the American College of Surgeons. He was named consultant in general surgery to the Air Force Surgeon General in He conceptualized and implemented the first Air Force rapid-response surgical team in Europe the flying ambulance surgical trauma team. As a surgeon, he performed more than 4,000 operations as principal surgeon and 6,000 as first assistant. He has published extensively in medical literature. An active flier, General Carlton holds Federal Aviation Administration commercial, instrument, multi-engine, glider and instructor ratings. During his command of the 1702nd Air Refueling Wing Contingency Hospital, he completed 32 combat support missions and 140 combat flying hours in the C-21, C-130, KC-10 and KC-135. General Carlton was awarded the Airman s Medal for heroism by voluntarily risking his life at the Pentagon, Washington, D.C. on 11 September EDUCATION: 1969 Bachelor of science degree, U.S. Air Force Academy, Colorado Springs, Colo Doctor of medicine, University of Colorado, Denver ASSIGNMENTS: 1. FRIDAY, SEPTEMBER 8, 2017 RETIREE DINNER At 1800 hours Friday a no hosted social hour will be held prior to the Annual Retiree Dinner at the Pershing Community Center. The Retiree Council is sponsoring a delicious buffet dinner for your dining pleasure. The cost of the buffet is $24.50 per person and must be paid by August 25, Advance reservations with payment are required. Tables will not be reserved. A roster of paid patrons will be at the entrance for verification and issuance of tickets. The 399th Army Band will provide entertainment. No refunds will be allowed. RETIREE DINNER MENU Buttermilk Brined Fried Chicken, Beef Port Roast w/crispy Onion Straws, Rustic Red Skin Mashed Potatoes, Rice Pilaf, Vegetable Medley, Green Bean Almandine, Salad Bar, Freshly Baked Dinner Rolls, Water, Coffee, Ice Tea, and dessert. SATURDAY, SEPTEMBER 9, 2017 RETIREE DAY HEALTH AND BENEFITS FAIR The General Leonard Wood Army Community Hospital (GLWACH) will be providing many services to retirees and their families. Information on health issues, physical fitness, medication reconciliation, blood pressure checks, diabetes management, and much more will be available. GLWACH, Defense Finance and Accounting Service, Veterans Administration, Staff Judge Advocate, Army and Air Force Exchange Service, Commissary, Bruce C. Clarke Library, Family, Morale, Welfare, and Recreation, and many other local service organizations will be providing information and assistance to the Retirees and their spouses at Nutter Field House, Bldg Doors will open at 1000 hours after completion of briefings at Abrams Theater. SELF-CARE CARD You will have the opportunity to obtain a Self Care Card while attending this year s Retiree Appreciation Day Event. Breakfast is buffet style and is free to retirees and their spouses. Members of the American Legion will prepare the food. 6 RETIREE DAY ACTIVITIES Identification (ID) CARD FACILITY The ID Card Facility is located in Dillard Hall, Building 470, at Replacement Avenue, 2d Floor, Room Hours of Operation are 7:00 a.m. 11:30 a.m., 12:30-4:00 p.m., on Monday, Tuesday, Wednesday, and Friday and 7:00 a.m. 12:00 p.m. on Thursday. The Facility is closed on holidays. Appointments can be made by accessing the RAPIDS Appointment Scheduler at Customers without internet access may call the ID Card Facility for an appointment at (573) Services for walk-in customers are limited, but those walk-in customers with emergency needs will be accepted on a case-by-case basis during hours of operation. In honor of our nation s retirees, on Saturday, September 9, 2017, from 9:00 a.m. to 12:00 a.m., the ID Card Facility will be open for retiree customers. Customers must have two valid forms of ID, one must be a photo ID and be unexpired. For initial enrollment of dependents, the following documents are needed: original or certified marriage certificate, birth certificate, divorce decree, retirement orders/dd Form 214, and a SSN card for all dependents. JUDGE ADVOCATE GENERAL The Fort Leonard Wood Legal Assistance Office is open M, T, W, and F from 8-4, but closed from for lunch and the afternoon of the first Friday of every month. Will briefs explaining wills, living wills, and powers of attorney (both health care and financial) are held weekly on every Wednesday morning. Legal Assistance will offer to prepare and execute estate planning documents in one day after attending the will brief. Items and information necessary to complete a will in one visit: (1) military ID card (2) Names, addresses, and phone numbers for your agents, executors and guardians, if applicable (3) List of personal property to be distributed (4) addresses for all real property (5) Other specified arrangements. If you do not have all the required information, the personnel at legal assistance will ask that you take the questionnaire home for completion and return it to the office. Non-will related powers of attorney can be taken care of at any time during normal business hours. One-day will service will not be available at Nutter Field House on Retiree Appreciation Day, but legal assistance will be available.
