2 Not So SMART Objectives 40 Latina/Brazilian women will receive breast screening at a local qualified health care institution. Create, promote and disseminate culturally/ linguistically appropriate breast health educational materials and program through brochures, fliers, mailed information to clients on agency s data base and agencies webpage. Information and materials created will be in Spanish, English and Portuguese. To reach at least 600 south Asian females living in the target counties of Middlesex, Ocean, Somerset and Cumberland by reaching out to at least 4 places of worship, grocery stores and an equal number of beauty salons for identifying and recruiting screening-eligible subjects to the project.
3 Not So SMART Objectives X Medical Center will educate 100 women from our X community, focusing on the women who meet the demographics of our target groups. Provide transportation assistance to at least 275 mammogram screening-eligible women. Ensure that a minimum of 60%, or 216 participants, comply with first-year ICP goals (inclusive of scheduled mammogram screening) and remain in WWC for a minimum of one year.
4 Not So SMART Objectives Provide targeted group education and one-to-one education to at least 300 Asian Indian women at 12 educational workshops at the X Temple in X during the grant year. Hold 2 workshops for 150 male members of the X Temple presented by a male physician. Connect with extremely low-income Latina women ages of X County about the importance of breast health, inclusive of breast self-awareness, current scientific evidence regarding the mammography guidelines, and the importance of a primary care home.
5 Good Abstract Example Utilizing the Dr. Jan Astin Mobile Digital Mammography Van, will provide access to mammograms and educational programs for women in Atlantic, Cape May, Cumberland, Gloucester, Salem, Monmouth, and Ocean counties who are uninsured; have never had a mammogram; are from high-risk, diverse populations (such as undocumented immigrants); and/or are without financial means to pay for transportation. Outreach activities will include scheduling the mobile van in areas where transportation or financial issues are obstacles, attending community events to educate women about breast self-awareness and early detection, advertising in English and Spanish newspapers, and partnering with faith-based organizations to coordinate educational lectures. At least 1,600 women will be educated and screened, which includes free mammograms for 500 women.
6 Good Abstract Example Provides a culturally competent and familial approach to increasing awareness, knowledge, and screening behaviors in low-income, minority women in Burlington and Camden counties. By delivering messages in a personal and heartfelt manner, this program addresses barriers impeding mammography screening such as uninsured, ethnic minorities, women 65+ and those who do not get regular screenings. Patient navigators refer uninsured women for breast cancer screenings and help navigate each patient from referral to screening. Also targets insured women to encourage utilization of insurance coverage for screenings. At least 500 women will receive education and 125 women are expected to receive a mammogram.
7 Good Salaries Justification example Part-time Project Coordinator, X, will help set-up the 15 educational sessions, collect and analyze pre- and postprogram surveys, send reminder cards, and make follow-up calls to participants who have not ever had a mammogram or have not had one in the past two years. She will also track participants who receive their mammograms and complete progress reports. She will spend 10 hours per week on the program, at a total cost of $13,000 ($25 per hour). Nurse Educator, X, will assist in presenting the educational sessions with the Project Director, X. She will spend an average of 15 hours per month working on the program at a total cost of $7,488 ($40 per hour).
8 Salaries Justification example The Breast Health Outreach Coordinator is an hourly, part-time health educator who manages all aspects of X. The position is responsible for conducting community outreach, identifying target audiences, conducting education sessions, performing telephone and reminder card interventions, assisting with mammogram scheduling, and collecting, monitoring and reporting data and outcomes. This position is essential to the program. This position description is great, but it would have been useful to include how much they make per hour, how many hours a week, etc.
9 Salaries Justification example Community Outreach Health Workers: Will provide outreach to clients at Latino churches, flea markets, Laundromats, connect Latino female s clients with the clinics, resources and support systems, help patients screening appointments, identify and access financial services to pay for their health care needs, transportation issues, etc. Program Manager: Will assist clients, develop and maintain a tracking system on our data base to capture client s personal information, services provided, events or workshop attended, etc. Assure all clients intakes and mammogram are complete on a timely matter and accurately recorded. The position descriptions are good, but it would have been useful to include how much they make per hour, how many hours a week, etc. Also, with 2 Community Health Outreach Workers, it would be useful to talk about why you need both and what makes the positions different.
10 Salaries Justification example 60% full-time equivalent of Care Coordinator to function as primary coordinator of Care Team functions and ICP compliance, inclusive of breast cancer screenings = $20, % FTE of Nurse Care Manager to oversee care coordination and to support much of the program design = $17, % FTE of Outreach Coordinator to support outreach and enrollment strategies = $16, % FTE of Financial Counselor to support patient navigator component = $9,600. There is not enough information provided here as to why all of these roles are needed, how they are different, etc. Also should include detail on how many hours, how much per hour, etc. for each role.
11 Salaries Justification example Nurse Practitioner with minimum 5 yrs. clinical experience will be hired to provide breast health education, clinical breast exams and prescriptions for recruited participant. She will work for 20 hours a week and paid at the rate of $40 dollars per hours. Patient Navigator will be a bilingual individual, part of the community with good knowledge about the cultural beliefs, possessing good communication skills and passionate about serving the community. The person will be paid $25 per hour for a total of 25 hours of service per week. This is an incomplete justification. There is no justification provided for several of the positions, which would be essential for understanding why all of these positions are needed.

13 Good Supplies Justification Appointment reminder cards are sent to patients a week in advance of their scheduled appointment to serve as a reminder. The capabilities brochures provide an overview of the X Program including, who should have an annual screening mammogram, how to prepare for the exam, and what to expect during the exam. The brochure also provides information about the importance of early detection and breast self-awareness. Letterhead and envelopes are used to send patients a reminder that it is time to schedule their yearly mammogram. We also use these supplies to send written results to referring physicians and patients. All Komen items are less than $1 per item; number of items to purchase for $1 was tallied and divided by the total number of items ordered. Komen Breast Self Awareness Cards - 1,700 cards at $0.33 each Komen Facts for Life sheets cards ordered at $0.15 each Komen Awareness bookmarks bookmarks ordered at $0.06 each

15 Good Supplies Justification 750 Komen folders at a cost of $1.25 each ($938). Because only whole dollar estimates are accepted by the online grants system, 938 folders are listed above (at $1 each) to get the true cost. Light, healthy refreshments for 25 small group sessions ($28 x 25 sessions = $700). Seven brunch/lunch education sessions targeting 350 women ($7 per woman x 350 women = $2,450). Each event will be decorated with a variety of pink items including balloons, tablecloths, and other items to encourage greater participation and create meaningful memories that can increase overall compliance ($290).
