Our Global Program is an essential driving force for achieving this mission. Komen has sustained a strong commitment to supporting projects and programs that will identify and deliver cures for breast cancer. This commitment has resulted in important progress that has contributed to major advances in breast cancer over the past 30 years. We began with a single grant for $28,000 in With increasing investments over time, now totaling nearly $2.2 billion, Komen is the largest source of non-profit funds dedicated to the fight against breast cancer in the world. At the global level, Komen has supported breast cancer research, education and community programs in more than 30 countries. Through its dedicated global engagement and community grant-making strategies, Susan G. Komen has had significant impact in dispelling the pervasive stigma surrounding breast cancer and addressing socio-cultural barriers that impede early detection and access to care around the world. For more information about Susan G. Komen, breast health or breast cancer visit CATERPILLAR GRANTS: PROMOTING ACTION AROUND THE BREAST CANCER CONTINUUM OF CARE In Latin America, Susan G. Komen and the Caterpillar Foundation are partnering to improve detection of breast cancer and save lives by increasing awareness, strengthening services, and removing barriers to access early screening, treatment and care in program communities. During the first cycle of the partnership, Susan G. Komen commissioned a report of the burden of breast cancer in Sao Paulo City and achievements, opportunities and most promising areas that Komen, Sao Paulo local government authorities, local foundations and Non-Governmental Organizations (NGOs) can invest in and support in order to improve the breast health care in Sao Paulo. The assessment concluded that Brazil in general and Sao Paulo City specifically have made remarkable progress in addressing breast cancer, but that challenges and gaps remain. The assessment team identified cross-cutting constraints across the breast health care system components which were translated into several recommendations. Based on the assessment, Komen has chosen to address specific constraints in the third cycle of the Caterpillar Grants. 3 Caterpillar Grants seek to create opportunities for the development and continuity of projects / programs aimed at increasing awareness, promoting early detection, improving access to services and ultimately improving the lives of those at risk and touched by breast cancer. Projects selected must meet one of the four activity areas and will be considered based on impact, viability, capacity, collaboration and sustainability. I. Mass Media Public Awareness Campaign Public awareness and community involvement activities in breast cancer through a variety of mechanisms that address potential geographic and socioeconomic inequities in service delivery access in specific areas or subpopulations of Sao Paulo. Under this priority area, applicants will propose the development and implementation of public awareness campaigns that will elevate breast cancer knowledge in the community, demystify fears and stigmas surrounding screening, and provide information on available resources. II. Training for Medical Providers Training of health care providers on standard breast health care guidelines by level of care, protocols and referral processes. Issues of quality related to breast health services most often exists in the primary levels of the healthcare system. General Doctors, family physicians, and gynecologists often lack the necessary skills to detect breast cancer early and to accelerate the patient s access to the required oncology services. Projects in this category may include but are not limited to: Training of general doctors / family physicians / gynecologists with no previous training on breast cancer. Quality assurance training for radiologists and radiology technicians responsible for taking and reviewing radiological breast images. Training on advanced techniques and innovative approaches to breast cancer screening, diagnosis and treatment. III. Training for Community Members and Educators Breast cancer education and awareness is core to Komen s missions and key to Brazil where breast cancer is the leading cause of cancer mortality in women. One third of all breast cancer cases occur in the state of Sao Paulo. Continuous education is essential to driving women to seek preventive services at earlier stages. Projects in this category may include, but are not limited to: Training of community health promoters and educators. Community-based breast-self awareness sessions aimed at educating low-income and marginalized groups. IV. Reducing Barriers in Access to Screening and Treatment People in all walks of life often face access barriers to affordable and high-quality breast health services. While the barriers are abundant, projects in this category must focus on three key areas identified as having a significant impact in a woman s decision and ability to access breast health services: Improvement of services related to the navigation of patients around the continuum of care. Solutions targeted at improving challenges surrounding the patient referral system. Communicating patient rights related to breast health and providing guidance to community members on the availability and access of services through the Ministry of Health, SUS, and/or the Family Health Program. 4 KEY DATES Pre-Application Due December 18, 2013, by 5 p.m., CST Pre-Application Decision January 20, 2014 Full Application Due March 7, 2014, by 5 p.m., CST Award Notification March 31, 2014 FUNDING INFORMATION Applicants may request up to a total of $150,000 (combined direct and indirect costs) for one year of funding with the possibility of renewal at Komen s discretion. Budget Guidelines: Personnel salaries should not exceed 35% of the total budget and included personnel must provide a 10% minimal level of effort Indirect costs cannot exceed 20% of total direct costs (including any indirect costs paid through subcontracts or consortia) Direct costs include, but are not limited to: salaries for project personnel, consultants, project supplies, publications, travel, printing and administrative support. Indirect costs include, but are not limited to: office supplies, mailings, telephone and other communications, utilities, equipment rental and legal support. Travel costs are allowed Publication costs and meeting-related poster printing costs are allowed Funds may NOT be used for: Tuition, scholarships or fellowships are not allowed Visa costs are not allowed Medical or scientific research is not allowed Equipment purchases are not allowed Construction or renovation of facilities is not allowed Political campaigns or lobbying is not allowed Clinical services or patient care costs are not allowed Fundraising costs and events are not allowed Professional membership dues are not allowed Applicant organizations may serve as the lead organization in only one application, but may collaborate with other groups. Komen grants do not fund individuals or for-profit organizations. DESIGNATED RECIPIENTS Grants will be awarded to the lead organization designated in the grant application. Grant Agreements will be executed between Susan G. Komen and the lead organization. The lead organization will serve as the administrator of the grant funds and will hold responsibility for the disbursement of the funds to other participating organizations, management of the budget, and submission of all required documents and reports. It is expected that the lead organization will enter into subcontracts with any other participating organizations, and assurances that these contractual agreements have been executed will be required prior to funding. 5 ELIGIBILITY Applicants must conform to the following eligibility criteria to be considered for funding. Eligibility requirements must be met at the time of full application submission. Lead organization must be a nonprofit institution or organization working in Sao Paulo, Brazil and must include legal proof of status in the pre-application. Past Komen grantees are eligible to reapply if prior grants were completed in good standing. Applicants must represent non-governmental organizations that address breast cancer issues in some capacitycivil organizations, universities, clinics or hospitals located in or providing care to women in Brazil. Other participating organizations may be nonprofit organizations or industry partners, however, industry partners may not be reimbursed for any fee or profit. All organizations and industry partners must agree to adhere to the terms of the contract with Susan G. Komen. REVIEW PROCESS Susan G. Komen utilizes a multi-step approach to application and review that requires submission of a pre-application and submission of a full application upon invitation only. Pre-applications are first administratively reviewed for eligibility, adherence to formatting requirements, and responsiveness to the grant priority specified in this RFA. Applications that do not meet eligibility, formatting or responsiveness requirements will be administratively withdrawn and will not be reviewed or scored. Each qualified pre-application is then reviewed by a panel of three reviewers with appropriate expertise. The reviewers assess the strengths and weaknesses of each application based on the defined review criteria described below. Only applicants with pre-applications deemed most meritorious and aligned with Komen s global mission and the objectives of the Komen/Caterpillar partnership will be invited to submit full applications. Applicants will be notified of pre-application review decisions via . Once notifications are sent, applicants will be granted access to reviewer comments. Applicants invited to submit a full application will be notified. Pre-application Review Criteria The applicant must address the following criteria in the pre-application narrative: Purpose of the Project Project Implementation Project Impact Describe how the proposed project addresses one of the priority areas as described in the RFA. Provide a detailed summary of the proposed project and explain how it is integrated within the specific priority area. Include the priority area and specific activities related to the proposed project. Provide a brief summary of the collaborations established with other institutions to implement the proposed project. Describe the anticipated impact of the proposed project. Why is the proposed project important to the breast cancer patient, survivor, co-survivor community? How will the applicant organization assess the proposed project s impact? Is the proposed project an innovative approach? How? Caterpillar Grants Yr3, Request for Applications Page 5 of 8

6 PRE-APPLICATION SUBMISSION INSTRUCTIONS All pre-applications must be submitted in accordance to the requirements and instructions of this Request for Applications (RFA). All application materials must be in English and must be submitted via to No paper applications will be accepted. Pre-Applications must be completed by 5pm CST (U.S.) on Monday, December 18, Pre-Application Sections The following information is required to submit a complete pre-application. 1. GENERAL INFORMATION Provide the organization s name and contact information. Provide the title of the proposed project. The title is limited to no more than 81 characters in length (including spaces). Do not use abbreviations or all capital letters. Include in this section the proposed start and end date of the project. Include the amount of funding requested for the proposed project. Provide the name and contact information for the Project Director. A signature from the Authorized Signing Official is required in this section. The Authorized Signing Official is a legal representative authorized to sign documents or agreements on behalf of the applicant organization. Grant Priority Area: Select the priority area for the proposed project. Partnering Organization(s): Provide the name and contact information for the institution(s) partnering with the applicant organization to implement the proposed project. 2. PURPOSE OF THE PROJECT (400 word maximum) Describe how the proposed project addresses the priority area selected and provide the following information: a) Why is the project necessary / important? b) Provide evidence to describe the risk / need within the community. c) Explain how the services provided will not be duplicative of the efforts of other organizations in the community. d) Describe the target audience impacted by the proposed project. 3. PROJECT IMPLEMENTATION (600 word maximum) Provide a detailed summary of the proposed project and explain how it is integrated within the specific priority area. Include specific activities related to the proposed project and provide a brief summary of the collaborations established with other institutions to implement the proposed project. Answer the following questions: a) Why is the applicant organization qualified to implement this project? b) What does the applicant organization expect to achieve? c) What is the overall strategy for the proposed project? PROJECT IMPACT (300 word maximum) Describe the anticipated impact of the proposed project and include the following information: a) Why is the proposed project important to breast cancer and the community? (reflect on the impact to breast cancer patients, survivors, co-survivors) b) How will the applicant organization assess the proposed project s impact? 5. PERSONNEL Provide a list of all personnel involved in the proposed project and describe their role. 6. BUDGET Provide a summary in U.S. dollars ($) of the anticipated expenses for each category. 7. SUPPORTING DOCUMENTS The following supporting documents must be submitted with the pre-application must be in PDF format: Proof of NGO Status Letters of Recommendation (maximum of 3) Proof of collaboration with proposed partners (if available will be required for the full application) CV/Resumes for all personnel 8. OFFICIAL AUTHORIZATION Provide the name, title and signature of the Project Director and Authorized official. After reviewing the information and confirming that all fields are completed, save your document in PDF format and submit it via to catgrant@komen.org. Pre-Application Template The Pre-Application Template will be provided to interested applicants via . The Pre-Application Narrative (Sections II -IV) is limited to 3 pages. Applicants may exceed the recommended page length for a given section as described below provided that the total narrative is no more than 3 pages, including figures and tables. Cited Publications and Pre- Application Supporting Documents are not included in this page number limit. Document Format Please follow the formatting requirements below. Applications not adhering to these format requirements will be administratively withdrawn prior to review. Must be in PDF file format. Font Size: 12 point or larger. Font Type: Arial or Times New Roman. Page Size: No larger than 8.5 inches x 11.0 inches (21.59 cm x cm). Recommended lengths for each narrative section of the application are provided. 8 Pre-Application Supporting Documents The following documentation is required to support the pre-application: Proof of NGO Status Applicants are required to provide legal proof of NGO status. Letters of Recommendation Letters of Recommendation are encouraged but not required. Applicants are limited to a maximum of 3 letters of recommendation. Proof of Collaboration with Proposed Partners Applicants submitting a proposal with a partner must provide a statement from the partner(s) indicating that they are indeed collaborating to implement the proposed project. This is a mandatory requirement for the full application. Applicants can include the proof of collaboration in the pre-application if available. Resumes/CVs for Project Personnel Resumes/CVs are required for all project personnel who will require a salary. Resumes/CVs must be no more than 4 pages each. The Pre-Application supporting documents are not included in the Pre-Application Narrative 3-page limit. Pre-Application Submission Checklist Review and check-off the check boxes in the Pre-Application Submission Checklist provided on the last page of the Pre- Application Template to ensure that all tasks that have been completed and reviewed. PRE-APPLICATION SUBMISSION DEADLINE Applications must be submitted by 5pm CST (U.S.) on Monday, December 16, Applicants are strongly encouraged to complete, review, and submit their applications with sufficient time to allow for technical difficulties, varying time zones, human error, loss of power/internet, sickness, travel, etc. Extensions to the application submission deadline will not be granted, with the rare exception made for severe extenuating circumstances. ADMINISTRATIVE REVIEW Applicants must conform to the stated grant priority areas and follow the pre-application submission instructions, including page limitations and format guidelines, such as the prescribed font size. Failure to adhere to these instructions will result in applications being administratively withdrawn from consideration prior to peer review, without appeal. FULL APPLICATION SUBMISSION Only applicants with pre-applications deemed most meritorious and aligned with Komen s mission will be invited to submit full applications.
