5 INTRODUCTION Forty-four years ago, two innovative thinkers at the Hospital Association of Southern California established National Health Foundation as a way to extend the impact the association could have on improving healthcare. Since then, NHF has been working closely with stakeholders on a variety of health issues. However, just as the field of public health has evolved over the years, so has National Health Foundation. Today our approach is more at the community level, while retaining that connection to hospitals. Within the community, our focus is on people having the opportunity to make choices that allow them to live a long, healthy life, regardless of their income, education, or ethnic background. The quality of the air and water, of schools and businesses, grocers and markets, parks and outdoor space, bicycle lanes, sidewalks, and more all play a role in a person s health. National Health Foundation s mission is to improve the health of underresourced communities by taking action on these social determinants of health. To do that, we are working within communities, we are developing young leaders, we are changing the physical and social landscape of some of the most under-resourced neighborhoods. We empower, we facilitate, we lead, and we lift up those around us. National Health Foundation is never working alone. We believe in leveraging the work done by other groups and not recreating the wheel. We make connections between existing resources and build sustainability into our programs so that we are not the sole champions of an effort. We want change to exist long after we have moved on. Using these strategies, we have created and deployed critical programs that are changing: the way neighborhoods can access healthy foods; where individuals can safely exercise without using their financial resources; the level of expectations teen mothers set for themselves, their academics and their futures; and individuals chances of staying healthy by connecting them to a home. We have seen progress in people, in communities, and at the systems level and this report gives you a summary of that change. 3
6 It is becoming common knowledge that where you live can shape your health and impact how long you will live.
7 LETTER TO THE COMMUNITY When National Health Foundation opened its doors in the 1970 s no one was talking about social determinants of health or upstream solutions or the root causes of illness like they are today. But more and more, that is exactly where the public health sector is moving. Those academic terms simply mean that health is shaped by so much more than seeing your doctor; health is something that starts in our families, in our schools and workplaces, in our playgrounds and parks, and in the air we breathe and the water we drink. It is becoming common knowledge that where you live can shape your health and impact how long you will live. A person can only be as healthy as their community enables them to be and a neighborhood should not be hazardous to your health! Too often, these low-income and under-resourced communities are also communities of color. This is not right. This is not fair. National Health Foundation is working to change that. National Health Foundation is answering the call to action, tackling things like: where you can buy healthy food to improve your experience with diabetes; the availability of fresh drinking water on your high school campus; and whether you have a place to call home. It is these seemingly simple things that keep you healthy. National Health Foundation wants everyone to have a chance at a healthy life and so we are tackling those seemingly simple things that are really intersections of politics, institutional racism, corporate interests and cultural norms. We are meeting people where they are, working with them to understand how their health is affected by their homes, jobs and schools, and creating solutions that lift everyone up. In this annual impact report, you will see that work in action and, hopefully, the empathy that drives us. Because at the end of the day, what really matters are the people. 9 FOOD ACCESS We Believe Food is Health Nutritious food is crucial for a healthy life free from disease. Accessing healthy foods can be a challenge for resource -strapped families or for those who live in neighborhoods without grocers. Can you imagine if your grocery store did not sell produce? Or if your school did not have clean, fresh drinking water throughout the campus? Our Response National Health Foundation is changing food landscapes in high schools and larger communities through a process of community engagement and empowerment. We facilitate teams of high school youth in researching barriers to healthy eating in their schools and neighborhoods. Then we support those teams to carry out solutions that make healthy food more available for everyone. These programs currently focus on neighborhoods in South Los Angeles. OUTPUT SINCE THE PROGRAMS BEGAN Students worked on increasing food access in their community community members provided input on this program community members participated in 180 nutrition and food education demonstrations hours of training, research and engagement received by students advocacy meetings hosted by youth with council members, neighborhood councils, classmates, faculty and the community 3,000 20% South Los Angeles residents now have access to fresh produce choices through two local markets makeovers. more students at one South LA high school are now participating in the school lunch program because of a redesigned lunchroom. high school students now have access to fresh water thanks to the installation of one hydration station. high school students now have access to healthy snacks throughout the day because of a student-developed snack sharing system. 100% Food waste at a South LA high school was significantly reduced with a student-developed snack-sharing program s that is now being considered for county-wide adoption. of youth Community Health Liaisons working on food access have graduated from high school and have gone onto college. 7

11 BUILT ENVIRONMENT We Believe The Built Environment is Health The built environment includes all of the physical parts of where we live, shop, work and play. The built environment influences an individual s level of physical activity, what kind of food gets purchased in that community, and so much more. These resources are directly tied to an individual s ability to make healthy decisions. Our Response National Health Foundation is changing the built environment of under-resourced communities through a process of community engagement and empowerment. National Health Foundation leads teams of high school youth that are making it easier and safer for residents of South Los Angeles to get outside and get active. These young people have awareness of what changes are needed and the motivation to see those changes through so that they and their neighbors can exercise, commute, shop and live in a safe community that supports healthy decisions. OUTPUT SINCE THE PROGRAMS BEGAN students worked to improve the built environment in their community hours of training, research and engagement received by students Walk This Way signs installed along Central Avenue covering 27 landmarks in a 4 mile radius. hours of free physical activity classes annually provided through local partners. According to research, South LA residents are now more likely to take a walking trip because of the 11 Walk This Way signs posted in the 4-mile radius of the Central Avenue corridor % people are more physically active because of new, free physical activity classes, 41% of whom are consistently returning to class. corner stores have improved capability to maintain their healthy food offerings to the community because of technical assistance provided by youth. of youth Community Health Liaisons working on food access have graduated from high school and have gone onto college. 9

13 EDUCATION We Believe Education is Health Research has shown that academic success is an excellent indicator for a teen s overall well-being and similarly a primary predictor and determinant of health in adulthood. During adolescence, reducing the prevalence of health-risk behaviors such as teenage pregnancy and early parenthood has shown to increase school achievement and academic success as well as promote healthy behaviors for a lifetime of wellness. OUTPUT programmatic tracts for: pregnant and parenting teens, young women at risk for teen pregnancy, and young men at risk for teen parenthood are available at six high schools in South Los Angeles students served in 2016; 1,050 to date since 2007 peer group educational sessions provided in 2017; more than 800 hours since 2007 items were donated to these teen moms to prepare them for parenthood through a partnership with Baby to Baby Our Response National Health Foundation partners with Los Angeles Unified School District (LAUSD) and select South Los Angeles high schools to implement school-based programs aimed at reducing teen pregnancy rates and increasing high school graduation and completion, thereby positively impacting student health. 92% 96% of participants have remained in school, graduated and/or obtained a GED six months after completing the program. of participants successfully avoided teen pregnancy or parenthood six months after program completion. Birth control utilization increased by 35% among participants. 97% 98% of participants expressed a positive change in behavior, such as improving their eating and exercise habits. of participants agreed or strongly agreed that they have the ability to seek out services in their community. 11

15 HOUSING We Believe Housing is Health Now more than ever before we have research indicating that housing is one of the most efficient ways to improve health outcomes. Housing often determines who has access to transit, grocery stores that sell fresh produce, jobs that pay living wages, safe parks, and good schools. A wide range of current and future outcomes are all inextricably tied to housing. Our Response After piloting the city s first recuperative care program nearly 10 years ago in partnership with other community based organizations, National Health Foundation has embraced its role in addressing homelessness by expanding on its recuperative care program to more effectively address this root cause of poor health. Today we meet people where they are on their path to a home and we use our established partnerships to help make that connection to housing. OUTPUT of referrals from hospitals were accepted unduplicated individuals have been safely discharged from the hospital to Pathway Recuperative Care days of service were provided to those individuals unduplicated individuals have been safely discharged from the hospital to Recuperative Care since the inception of the program in 2010 individuals moved from Recuperative Care to Bridge Housing Individuals stayed in Pathway for an average of nine days % individuals (67% of program participants) now have a place to call home thanks to housing placement through the recuperative care program. individuals coming through the recuperative care program were placed in permanent or permanent supportive housing. individuals were entered (or confirmed) in the Coordinated Entry System and now have access to county housing services. of recuperative care clients received an individualized care plan and a connection to a medical home. Of the 262 Dignity Health clients referred to Pathway in 2016, only 4% were readmitted to the hospital within 30 days. Since launching in 2010, $34 million has been saved through costs avoided to hospitals and healthcare systems. 13
