3 The Almost Home Kids model is present in any place there is a need without regard to geographical boundaries. All key community and policy stakeholders are fully committed to the AHK model and recognize it as the premiere standard of care. Dear Friends; Thank you for making 2017 another memorable and remarkable year for Almost Home Kids. Your support has allowed us to stay true to our Mission and forge ahead with our Vision - it has empowered our clinical staff to provide the excellence in care that has become synonymous with Almost Home Kids, and driven our leadership team to take a significant step towards realizing the goal of serving more children with medical complexities closer to their family and in their own community. Following many months of planning and partnering with OSF Healthcare Children s Hospital of Illinois we officially broke ground for the brand new Almost Home Kids in Peoria. Scheduled to open in early September of 2018, the 12-bed home will offer children with medical complexities and their families in Central and Southern Illinois the same community based programs that are available at our current locations in Chicago and Naperville. As always, we are eternally grateful to the Board, President's Council and Medical Advisory Committee members for their leadership and guidance, to the event Committees for their expertise and passion, and to the numerous volunteers who help make our house a home. We are excited to look forward to 2018 as a year of growth and new possibilities as the changing landscape of healthcare necessitates innovative programs like ours, while remaining grounded and focused on providing the best possible care right here at home. 4 The Almost Home Kids Model AHK is licensed by the Illinois Department of Health, under the Alternative Healthcare Delivery Act as a Children s Community Health Care Center 501(c)(3). It is the only community-based facility in a home-like setting in Illinois dedicated exclusively to the care of children with complex medical conditions. The organization is poised to share its expertise with other institutions therefore increasing the capabilities and capacity of the region to care for children with complicated health needs. How We Do It Medically Complex Defined The Children's Hospital Association (CHA) uses 3M to define Children with Medical Complexities: 5b...Significant lifelong chronic disease 6...Significant chronic disease in multiple organ systems 7...Dominant chronic disease in 3 or more organ systems 8...Dominant and metastatic malignancies 9...Catastrophic condition status Eligibility Requirements Ages from Birth to 21 Children who are Medically Complex Children who are Technology Assisted Adrianna with Cecilia, AHK RN, during Halloween Transitional Care A bridge from hospital to home AHK provides a 120-day Transitional Care Program for children who are ready to be discharged from a hospital but for whom preparations at home need to be made. In a comforting environment, children receive 24-hour medical and nursing support from skilled pediatric nurses. AHK trains families on how to both care for their child with medical complexities and how to prepare their home. Respite Care The Respite Care Program at AHK offers children a comfortable place to stay with access to medical and nursing support while their parents/ caregivers tend to personal or urgent family issues. The Respite Transportation Program provides round trip transportation for children from their home to an AHK facility. Community Support In addition to training and care, AHK provides community resources and activities that become a support system the family can depend on for years to come. Services 24/7 care by Pediatric Advance Life Support (PALS) and Ventilator Certified Registered Nurses On-site Pediatric Medical Director Speech Therapy Occupational Therapy Physical Therapy Development Therapy Nutrition Consultation Weekly Care Conferences Discharge Coordination Meetings Individual Educational Coordination Fully Accessible Playground (Naperville) Beautifully Landscaped Grounds with Wheelchair Accessible Paths (Naperville) On-site Respite Emergency Respite Respite Transportation Services Case Management Parent Advocacy Assistance Care Coordination Medical Team On-site Pediatric Nurse Practitioner On-site Social Worker Medical Education Program Sleep Study Telemedicine and Telerounding Program On-site Certified Child Life Specialist Dental Consults Community of Support From navigating the healthcare system to providing emotional support, Almost Home Kids introduces families to a continuum of care that is community based.
5 The Future of Collaboration Telemedicine Telemedicine is the use of medical information exchanged from one site to another via electronic communications to improve a patient s health status. Almost Home Kids is partnering with the Lurie Children s Telemedicine program to provide real time consultation services for our children at our Chicago and Naperville locations. Our goal is to work with multiple subspecialty providers who will provide their assessment on varying conditions via telemedicine in order to minimize transport of the child outside of AHK and expedite care. With the Telemedicine program, we can also utilize the technology to connect with families and have them be an active participant in our clinical rounds with our multidisciplinary team when they are unable to be physically present. The equipment needed for this program is already in place at both sites, and Telemed session for Lurie Children's Hospital clinical staff training is well on its way to making this cutting edge program a reality reducing costs and helping to improve the health and wellbeing of children with medical complexities at AHK and in ultimately at home and in their community. The Future of Healthcare Clinically Integrated Network The mission of the CIN is to improve the health of the pediatric population by providing comprehensive, coordinated, and family-centered care in an efficient and effective manner. They strive to involve families, promote excellence, demonstrate respect for independent thinking, collaborate, and ensure accountability and transparency. At AHK we have worked closely with Lurie Children s to provide the best solutions in the continuum of care of children with medical complexities. For example the use of a care coordination note, embedded within the Electronic Medical Record at Almost Home Kids, is accessible to specialists utilizing the child s medical record within the same healthcare system. This allows for easy updates to the child s record, and ensures coordination of care across the multidisciplinary team. Transitional care frees up NICU/PICU beds for higher acuity patients possibly reducing the need to increase bed capacity, and provides a practical and responsive approach to evolving payment models and cost constraints. 6 Our Financial Summary Our fiscal year 2017 runs from September 1, 2016 to August 31, During this time, AHK s Program Expenses totaled $6.5 million, which includes investments toward expansion initiatives. Our expenses are up 7.1 % over last year and the increase is attributed to clinical programming costs. We continued to enhance the capabilities for our clinical team and increased efficiency in care coordination by adding traveling nurses that alternate to Chicago or Naperville based on daily census needs. After receiving $4.1 million fee for service revenue from the State of Illinois, third party insurance companies and cash payments, AHK turned to philanthropy to close the gap for our medical services. With the generosity of 1,331 donors, AHK s fundraising contributions for the year tallied $1,229,615. The organization remains focused on fundraising efficiency and is committed to providing critical healthcare services for children, regardless of the family s ability to pay. The inherent shortfall resulting from actual cost of clinical care versus reimbursement rates is omnipresent. AHK raises unrestricted dollars to fund the operating shortfalls and provide Respite Care services to ensure that families and caregivers receive a much needed break from 24-7 caregiving. 7 Our Thanks Josie with her dad Milo with AHK Child Life Specialist Kari Your Support Makes an Impact There were 315 Respite Care days at Almost Home Kids this year, giving families a much needed break from 24/7 caregiving. The majority of our respite care funding comes from individuals like you, corporations and foundations, stepping up to provide this critical service for our families. Nearly 50% of our annual fundraising goal comes from event proceeds. This year AHK hosted or was the beneficiary of 13 fundraising events raising more than $562,000 for greatest needs at Almost Home Kids! The Keith Veselik, MD Medical Education Program at AHK, has provided Residents and Medical students from 9 universities and 4 health systems a unique and impactful educational experience for more than 420 current and future physicians. We recently formalized an agreement with Northwestern University Feinberg School of Medicine to host a 2 week elective rotation for their 4th year medical students!
