2 Measles POD Exercise Play During this exercise it will be important to remember and comply with the following statements: It is important NOT to jump ahead in the exercise scenario. In other words don t focus on what might happen, focus on the Common Operating Picture that is presented during the exercise to make informed decisions. In any exercise a number of assumptions and artificialities may be necessary to complete play in the time allotted. During this exercise, the following apply: 1. The scenario is plausible, and events occur as they are presented. 2. There is no hidden agenda, nor any trick questions. Exercise Basics: 1. This exercise contains a total of 19 scenario and inject pages. 2. The exercise is self-paced and it should take approximately three hours to complete. However, based on the time spent on each scenario and inject the exercise may be completed in under three hours or it may take much longer than three hours to complete the exercise. 3. There is a scheduled 15 minute break approximately half way through the exercise. 4. All videos embedded within this exercise can be enlarged to FULL SCREEN by simply clicking on the full screen icon in the lower right hand corner of the video. 5. Thoroughly review all scenarios and injects as they are presented during the exercise. At the end of the exercise you will need to complete an evaluation (Hotwash) on how your team performed during the exercise. The Hotwash will consist of your team providing comments on three areas in which it performed well (Strengths) and three areas for your consideration to improve or modify existing plans (Areas for Improvement) to enhance your team s preparedness, response and / or recovery capabilities. Found below are Hotwash examples: Strengths During the exercise our team consulted our disaster plans and procedures and found them to be complete. Areas for Improvement During the exercise our team consulted our disaster plans and procedures and found portions of the plans and procedures that need to be revised. 3 Measles POD Scenario # 001 Image of Church Rally Day 1 Saturday May 7 9:53AM During the first full weekend of May churches from across the region come together to host the first annual "Season of Hope Youth Rally" in Palm Coast. Over the next two days, rally participants will take part in a wide variety of engaging activities. Day 2 Sunday May 8 5:00PM At the conclusion of the rally everyone gathers for one last time to participate in the rally s Closing Ceremony. Following the Closing Ceremony, rally organizers meet to evaluate the two days of fun and fellowship. 4 Measles POD Scenario # 002 Image of Press Release Day 3 Monday May 9 9:17AM IMMEDIATE RELEASE MEASLES CASE IN INTERNATIONAL TRAVELER CONFIRMED IN DUVAL COUNTY ~ Residents encouraged to remember vaccination is the best protection ~ TALLAHASSEE - The Florida Department of Health has confirmed measles in an adult international traveler who attended a faith based conference in Jacksonville, Florida, at the Osborn Convention Center. Most of the traveler s time was spent in Duval County; however, the traveler also spent time in Alachua, Flagler and Marion Counties visiting local churches during the infectious period of April 30 thru May 4, The traveler was hospitalized on May 5 and released on May 6 and on May 7 returned to his country of origin via the Jacksonville International Airport. The department continues to work closely with health care professionals and organizations in an effort to maintain its current level of readiness to identify cases and respond to any diagnosed cases of measles in Florida. The department is also working with the Centers for Disease Control and Prevention as well as the organizer of the faith based conference to notify all conference attendees as well as all establishments the traveler visited while infectious to identify potentially exposed individuals. 10 Minutes Situation Manual Tabletop Exercise 4 Disaster Resistant Communities Group

5 Measles POD Inject # 003 Image of Hospital Day 4 Tuesday May 10 11:17AM Over the past five hours numerous individuals have been arriving at area hospitals, urgent care centers and primary care providers with "flu-like" symptoms (high fever, chills, fatigue and persistent coughs). The majority of these cases, which include otherwise healthy young adults and children, exhibit signs and symptoms more serious than the common flu. Epidemiology personnel at the DOH-Flagler have noticed Red Alerts within the Electronic Surveillance System for the Early Notification of Community-based Epidemics (ESSENCE). The causes for the Red Alerts are the elevated numbers of Influenza Like Illness (ILI) symptoms being reported by Florida Hospital Flagler. Based on information from the Florida Department of Health (FDOH) regarding Measles, Florida Hospital Flagler collects blood samples from several patients then ships the samples to the FDOH Bureau of Public Health Laboratories (Public Health Lab) to be analyzed. Based on the current information will the DOH-Flagler activate any of the components of its Emergency Operations Plan (EOP)? If the above answer is yes. Identify the components of the EOP that will be activated and briefly describe the actions that will be implemented at this time. Yes No If the above answer is no. 7 Measles POD Inject # 005 Image of ICS Day 5 Wednesday May 11 8:29AM Over the past 24 hours, hospitals and clinics have continued to experience an increase in patients presenting with severe flu-like symptoms. Two physicians at Florida Hospital Flagler have noticed an increasing number of patients presenting with flat red spots on their faces and necks and come to the conclusion that there may be an outbreak of Measles in Flagler County. Following protocols the hospital contacts the DOH-Flagler. The blood samples shipped by the hospital to the FDOH Public Health Lab have tested positive for Measles and the DOH- Flagler has been notified. Based on the current information and with guidance from the FDOH the decision has been made to fully activate the DOH-Flagler s EOP including its Strategic National Stockpile (SNS) and Mass Prophylaxis (POD Plan) Annexes. Will the DOH-Flagler utilize the Incident Command System to effectively and efficiently manage the response to this public health emergency? If the above answer is yes. Identify the individuals by name and title that will be responsible for fulfilling the various ICS roles that will be staffed for this incident. (Example: ICS Position: Incident Commander / Responsible Individual: John Harper Environmental Health Coordinator) Yes No If the above answer is no. 8 Measles POD Inject # 006 Image of Partners The DOH-Flagler fully understands that it will need the support of various government agencies (Community Partners) to successfully respond to the current public health emergency. Identify each of the Community Partners in which the DOH-Flagler plans to rely upon in the days to come. In identifying each partner, list point of contact (POC) information (name and phone number) for the partner, then list the support that the DOH-Flagler expects to receive from the identified partner. (Example: Community Partner: County Sheriff s Office / POC: Thomas Highsmith / Support: Provide site security for the POD once it has been opened.) Is there a network of community and faith based organizations (Community Partners) within Flagler County that can be called upon to provide volunteers to assist with POD operations? If the above answer is yes. Briefly describe how the DOH-Flagler will access this network and utilize the volunteers that will be made available from the network s community and faith based organizations. 9 Measles POD Inject # 007 Image of Volunteers The DOH-Flagler s Community Partners are ready to step-up and assist during this public health emergency. However most indicated that their paid and / or volunteer staff members do not have the training to assist with response activities such as helping to open and manage a POD. Explain in detail how the DOH-Flagler will provide just in time training for individuals that would like to assist but have no training. This includes DOH-Flagler staff members that do not have the necessary training. (Note: Access a set of Point of Dispensing (POD) Operations - Just In Time Training videos.) When paid and volunteer staff members from Community Partners assist the DOH-Flagler during a public health emergency are they covered by the DOH-Flagler s: Liability Insurance? Yes No Workers Compensation Insurance? 10 Measles POD Inject # 008 Image of IAP Day 5 Wednesday May 11 1:32PM Based on current information and the DOH-Flagler s POD Plan develop an Incident Action Plan (IAP) that focuses on INITIAL RESPONSE ACTIVITIES associated with opening and staffing PODs. Ensure that all objectives are completed using SMART methodology. (Note: Review the list of some of the Initial Response Activities Operational Concerns that need to be addressed in developing this IAP.) In preparing the IAP list the objectives, identify any resources that will be needed to undertake each listed objective and prepare a timeline for completing each objective. (Example: Objective: Call the POC for the location to be used as a POD to ensure they are available and to make arrangements to use the facility. / Required Resources: POD contact list. / Timeline: 15 Minutes.) 20 Minutes Situation Manual Tabletop Exercise 10 Disaster Resistant Communities Group

11 Measles POD Inject # 009 Image of School Has the DOH-Flagler entered into written agreements with the owners / operators of the locations that have been identified as POD host sites for the use of their facilities? (Note: Access an example of a POD Facility Usage Agreement.) If the above answer is yes. When were the agreements last reviewed and / or up-dated? Yes No If the above answer is no. 12 Measles POD Inject # 010 Image of POD Staff Day 5 Wednesday May 11 3:17PM Based on CDC guidelines, the DOH-Flagler will need to distribute pharmaceuticals to the county s at risk population as quickly as possible. What will be the operational hours of the POD? (Example: 8:00AM through 8:00PM Remember POD staff will need to arrive at least 30 minutes before the POD opens and stay at least 30 minutes after the POD closes.) (Note: The pharmaceuticals that will be administered at the POD will be the Measles Vaccine for all individuals and Immune Globulin for individuals that have been exposed. Both have to be administered via injection. Based on this fact processing individuals through the POD will be time consuming.) How many staffing shifts per day will be required to maintain the POD? One Two Three Four Based on the planned POD Staffing Pattern, the at risk population and the number of shifts per day, how many staff members will be required to adequately operate the POD on a daily basis? 15 Minutes Situation Manual Tabletop Exercise 12 Disaster Resistant Communities Group

Identify and briefly describe the key messages in which the DOH-Flagler s PIO will focus in an effort to keep the public informed regarding the current public health emergency. As the hashtag #FlaglerMeasles begins to trend on Twitter and Facebook and rumors of a high volume of ill individuals presenting at Florida Hospital Flagler s Emergency Department will the DOH-Flagler s PIO take any actions? If the above answer is yes. Describe in detail the actions that will be taken at this point. Yes No If the above answer is no. Explain why no action will be taken based on the current information. (Note: For future reference you may want to access the Just In Time Disaster Training Library for additional information on Social Media During Disasters and Text Messages During Emergencies. Other than the pharmaceuticals which are being delivered to the DOH-Flagler are there additional equipment or consumable supplies that will be needed to operate the POD? If the above answer is yes. Develop a list of additional material resources that will be needed to establish and manage the POD. (Note: List each item and its quantity.) 15 Measles POD Inject # 013 Image of IAP Day 6 Thursday May 12 8:07AM As the DOH-Flagler moves forward with its plan to open, staff and operate a POD there is a need to develop an Incident Action Plan (IAP) focusing solely on POD OPERATIONS. In preparing the IAP list the objectives, identify any resources that will be needed to undertake each listed objective and prepare a timeline for completing each objective. (Example: Objective: Transport available equipment and consumable supplies to the POD host location. / Required Resources: Truck. / Timeline: 2 Hours.) (Note: Review the list of some of the POD Operations Operational Concerns that need to be addressed in developing this IAP.) 20 Minutes Situation Manual Tabletop Exercise 15 Disaster Resistant Communities Group

16 Measles POD Inject # 014 Image of POD Floor Plan Day 6 Thursday May 12 8:27AM The team assigned to set-up the POD has just arrived at the school that will be used to house the POD. Is there a process in place which the team assigned to set-up the POD will utilize to preinspect the school prior to utilizing the facility to house a POD? (Note: Access an example of a Facility Survey that can be utilized to conduct such an inspection.) Yes No If the above answer is yes. Briefly explain the pre-inspection process. If the above answer is no. Briefly explain why there is not such a process. 18 Measles POD Scenario # 016 Image of POD Operations Day 6 Thursday May 12 12:34PM The POD set-up team was able to fully prepare the school to use as the POD. It is early in the afternoon on the first day of POD operations. The staff and management team for the POD are in place and have participated in the daily POD operations briefing. The local television station s meteorologist has forecast heavy rain for the next two days. As POD operations continue at a steady rate, will the DOH-Flagler monitor area residents for possible adverse reactions associated with the pharmaceuticals being administered? If the above answer is yes. Briefly describe how this will be accomplished. Yes No Identify the agency(s) that will be notified if there are adverse reactions to the pharmaceuticals. If the above answer is no. 20 Measles POD Inject # 018 Image of Heavy Rain Day 7 Friday May 13 9:53AM As expected, there are numerous minor glitches that have arisen since the POD opened, however POD staff and management are successfully working through each of these challenges. Due to the large volume of residents being processed through the POD and the fact that heavy rains continue to inundate the region, the floors of the school remain very wet. None of the POD staff have noticed the condition of the wet floors so no one has posted Slippery Floor signage. Suddenly someone cries out that an elderly woman has slipped on the wet floor and is in severe pain and cannot get up. Briefly explain the actions that will need to be taken. Will the POD management team and other key stakeholders need to be notified of this incident? Yes No Will an accident report need to be completed by the POD management team? 21 Measles POD Inject # 019 Image of Downsizing Day 8 Saturday May 14 4:47PM It is the late afternoon of the third day of POD operations and the activity level has been dropping off since noon. By early evening the DOH-Flagler receives a report from the POD that the number of residents that were administered the Measles vaccine during the past four hours was negligible. The decision is made to transition all of the dispensing operations back to the DOH-Flagler office and discontinue POD operations. Based on current known conditions and the DOH-Flagler s POD plan develop an Incident Action Plan (IAP) that focuses on activities associated with DEMOBILZING POD OPERATIONS. In preparing the IAP list the objectives, identify any resources that will be needed to undertake each listed objective and prepare a timeline for completing each objective. (Note: Review the list of some of the Demobilizing POD Operations Operational Concerns that need to be addressed in developing this IAP.)
