3 Introduction To provide the best possible healthcare for the people we serve, it is essential that GP Federations attract and retain staff who are appropriately qualified, professional in the service they deliver, happy and productive in their work and committed to lifelong learning and developing in their role. This information pack is designed to provide you with a brief overview of the role of GP Federations and Practice Based Pharmacists, the terms and conditions of employment within GP Federations and information which should assist you in the completion of your application form. Thank you for your interest in applying for a position within GP Federations and we look forward to receiving your application. Introduction Northern Ireland GP Federations Evidence from both the Royal College of GPs and the Kings Fund points to the fact that it is vital that GP federations, are uniquely placed at the centre of the primary care arena, and that they can pave the way for greatly improving quality standards and accessibility to care. GP Federations not only provide better outcomes for patients but will also result in significant and sustainable efficiencies for the service. As well as this priority objective, Federations will act as a catalyst for better outcomes in the other major strategic objectives of our service namely; GMS PLUS/SHIFT LEFT/TYC/Donaldson/GP recruitment/federation alignment to trusts and other key stakeholders/out of hours/7 day working/potential gains/improved efficiency/better patient outcomes/sustainability/reform. In Northern Ireland the creation of the federation model has been both led and funded by GPs themselves. Key Components of the Northern Ireland GP Federation Model An average size of a GP federation is circa 100,000 patients with 20 practices. Their boundaries are in line with the current boundaries for Integrated Care Partnerships. Each Federation has been established as a Community Interest Company Limited by Guarantee in the not for profit sector. 3

4 Currently there are 17 Federations incorporated covering 1.8 million of a patient population. Northern Ireland is the only part of the United Kingdom that has a unified model of Federations governed by a unified Members Agreement covering its entire population. Governance arrangements are as outlined in Fig 1. Figure 1 Indicative Corporate Structure: GP Federations NI GP Practices Appoint one GP partner from each practice to represent the Partners as a Member of the company and act as a Director Members Committee The Member s Committee represents the Practices and acts as a senior decision making body for the organisation. Member s Committee may appoint other Directors to take key roles in the organisation Appointed Directors Board of Directors The Board of Directors takes executive responsibility for managing the organisation Within this organisational model the GP practice is sovereign with the number of Member Directors being one per practice. Member Directors have formed a Members Committee in which the major governance authority of the organisation in vested. The Members Committee have the authority to appoint Directors in line with the rules of the Members Agreement. It is these Directors that form the Board of Directors who are charged within the delegated authority of the members to take responsibility for the managing of their own Federation. 4

5 The Federations The Federations exist in the following locations: Belfast: North, South, East, West. South East: North Down, Down, Lisburn, Ards Western: Derry, South West Southern: Armagh & Dungannon, Craigavon, Newry & District Northern: Causeway, East Antrim, Mid Ulster, Antrim/Ballymena See Appendix A for member practices. The Federation Support Unit (FSU) The Belfast and South East Federations have already established and incorporated a FSU. The other Federations are currently in the process of setting up similar FSUs in their areas. The FSU is a community interest company and has the following organisational attributes: CIC Company in the Not for Profit Sector. The company is owned on a share basis by each of its constituent federation membership (companies); this eliminates the necessity for individual named members. Any surplus created by the FSU will be reinvested in the shareholders federations for the good of the community they serve locally or by agreement can be used for wider projects of greater scope. The creation of the company is governed by a shareholders agreement giving exclusive rights to the shareholders. The shareholders agreement has sufficient flexibility to allow if required for an extension of membership. The FSU will have a recognised management structure employed by the FSU. The FSU has been designed to provide federation members with excellent, affordable support initially in the provision arena. Some examples of a FSU functions include: central management expertise and specialists, planning, accounting, contracting, communication both internal and external and human resources. It is envisaged that the FSU will outsource as many of these services as possible. The core purpose therefore of the FSU is to ensure that clinicians are Free to focus on ensuring they provide the best clinical outcomes for their patients while improving the quality of care patients receive 5

6 Practice Based Pharmacists in GP Federations The Health Minister has approved a Business Case for Practice Based Pharmacists (PBPs) which details HSCB proposals to commission PBPs in GP Practices to promote safer, more rational and cost effective prescribing and to deliver better health and wellbeing outcomes for patients. GP Federations will employ PBPs to deliver this service. This is consistent with Department of Health policy and will assist in implementing extant strategies such as Making it Better through Pharmacy in the Community Strategy and the Medicines Optimisation Quality Framework. The Business Case also takes account of workforce development implications for the wider pharmacy workforce and will contribute to addressing the workforce challenges being experienced in provision of General Medical Services (GMS) at present. The role of the Lead Practice Based Pharmacist and the Practice Based Pharmacist is outlined in the accompanying job descriptions but in summary the objectives are: To improve safety and reduce level of errors and waste by managing prescribing systems, carrying out medication reviews and medicines reconciliation associate with patient discharge To release GP time spent on prescribing activities to increase overall GP capacity and improve patient outcomes To improve prescribing quality through implementation of the NI Formulary To ensure consistency of prescribing To reduce prescribing costs Two waves of recruitment have already taken place. On completion of this recruitment exercise it is hoped that a Practice Based Pharmacist will be placed in all GP Federation member practices. Please note: Waiting lists compiled from previous Practice Based Pharmacist recruitment are not affected and will continue to be used for the stated period of time. The term and conditions for these posts can be found in Appendix B. 6

7 Recruitment Process Process Dates Open applications 28 February 2017 Close applications 13 March 2017 Anticipated Shortlisting NB Eligibility will be assessed at the point of shortlisting Week commencing 20 March 2017 Interviews 3 rd 14 th April 2017 Following the interview: You will be notified that: (1) you have been successful at interview and will be made a job offer for a named GP Federation area OR (2) you have been successful at interview and are on the waiting list pending an offer being made OR (3) you have been unsuccessful at interview; A response to a job offer is normally required within 3 days; References will be sought for all successful candidates. Please note this occurs in advance of a job offer being made. Referees will be given one week to respond. References not received within this time will delay your final outcome notification being sent;as each Federation obtains a complete picture of all of the PBPs who have accepted an offer, PBPs will then be allocated to a GP practice(s). In making this allocation the Federation will, as far as possible, take into account details such as geographical location, previous working arrangements with GP practices in the Federation area, preferred hours of working etc. Any preferences or supplementary information that has been provided will be passed to the Federation for consideration. This process can take a number of weeks. Pre-employment checks May 2017 Possible Start Date Determined according to individual GP Federation needs. Please note: it will not be possible for candidates to take up a post until all criteria specified within the Personnel Specification have been met Pre-employment checks The following pre-employment checks will be carried out prior to appointing someone to a post: References All appointments are subject to two satisfactory references being received. Please be specific when providing addresses/contact details for your referees. One of your 7

8 references must have knowledge of your present work/or most recent employment & be in a supervisory/managerial capacity. Please note that we always seek a reference from your last employer, where applicable. Professional Registration/Qualification Checks/Verification of Identity The Recruitment Team will carry out checks to confirm professional registration and any qualifications which are listed as essential in the Personnel Specification. You will also be required to produce original documents to verify your identity, one of which must be photographic identification. Proof of qualifications and/or professional registration will be required if an offer of employment is made if you are unable to provide this within a reasonable time period (10 working days from conditional offer of appointment) the offer may be withdrawn. If successful, staff will be required to produce documentary evidence that they are legally entitled to live and work in the UK eg Passport/travel documents. Failure to do so will result in non-appointment. Pre-Employment Health Assessment Successful candidates will be required to undertake a pre-employment health assessment, which in the first instance will take the form of a questionnaire. Access NI Checks As part of the Recruitment and Selection process it may be necessary to carry out an Enhanced Disclosure Check through Access NI before any appointment to this post can be confirmed. Tips for completing / submitting your application form All sections of the application form must be completed in full. Please note shortlisting will be carried out on the basis of the criteria set out in the Personnel Specification, using solely the information you provide on the application form. Therefore you must ensure that your application form clearly indicates how you meet the criteria stated in the Personnel Specification, as failure to do so may result in you not being shortlisted. 8

9 For example, be specific about dates of employment; qualification subjects and levels (including any sub-parts); and number, expiry date and nature of professional registration (including part/branch of the register as appropriate). A relevant postgraduate pharmacy qualification is considered to be a Certificate, Diploma, MSc, PhD or equivalent in subject area relevant to the role. Experience of working in general practice should involve working in the GP practice rather than provision of advice/recommendations when dispensing prescriptions in community pharmacy or liaising with GPs/practice staff regarding the interface between primary and secondary care Repeat information (if applicable) across questions do not presume that if you have mentioned something in one question it crosses across all questions. Each criterion is marked separately. Please note that essential and where relevant, desirable criteria may be used at shortlisting. Applicants should therefore make it clear on their application form whether or not they meet the desirable criteria, as per the Personnel Specification. Failure to do so may result in you not being shortlisted. Preference for Location and Hours of Work In order to assist with offers of employment, a preference sheet is provided to allow you to state your preferences with respect to the area in which you work and contracted hours of working. Please complete this form and bring it when you attend for interview. Please remember that offers of employment will be based on the information that you provide and therefore you should indicate any preferences that you would be prepared to consider. For example if you state one preferred Federation and full time working hours only, then you will not be offered a part time post in that Federation even if one becomes available. It should be noted that successful candidates may be expected to work with more than one GP practice. Waiting List Following interviews a waiting list will be compiled for future same or similar vacancies arising. The waiting list will normally be held for 12 months. Please note it is your responsibility to ensure that the contact details you have provided are maintained. Further details on how the waiting list will operate can be found in Appendix C. Consider attending a training course on competency based interview technique details of courses can be found << Spend time reviewing the NHS Healthcare Leadership Model as this forms the basis for assessment Listen carefully to the question asked and ensure you address all parts Consider using an answer structure i.e. Situation, Behaviour, Outcome (SBO) to answer questions. This ensures good flow and the panel members can easily follow the content. However, be aware that the panel is likely to be using a marking structure and the majority of marks may be allocated to the aspect of behaviour. Use job or education based examples as far as possible to respond to the criteria. Candidates should think carefully in advance about choosing a relevant example that addresses the competency and best demonstrates their ability in this area Give a good level of detail within the answer, not only explaining what you did but also how you did it and why you did it in a particular way Always talk in the first person i.e. I led on... or I achieved Ensure to elaborate on acronyms (at least once) for clarity of understanding 11

23 Appendix B: GP Federation Terms and Conditions GP Federations offer a competitive remuneration package and terms and conditions of employment. Our employment practices and policies adhere to all relevant employment legislation and we are committed to promoting diversity and equality of opportunity in employment for our staff. Place of Employment The place of employment will be determined by the GP Federation. Where possible, the Federation will consider aspects such as previous working with an individual GP practice(s) if relevant, geographical location etc. but final placement of pharmacists within GP practices must meet the business need of the GP Federation. It should be noted that successful candidates may be expected to work with more than one GP practice. Working Hours / Pattern Working pattern (distribution of working hours across the working week) will be determined by the GP Federation and must meet the business need of the GP Federation. Continuous Service Previous employment with any other employer is relevant only for superannuation purposes. Salary Practice Based Pharmacist Entry level with no experience of working as a practice pharmacist and no Independent Prescribing qualification Entry level with experience of working as a practice pharmacist and no Independent Prescribing qualification Entry level with experience of working as a practice pharmacist and an Independent Prescribing qualification Salary 33k 34k 35k Any further pay progression will be dependent on meeting additional agreed skills as required by Federation 23

24 Lead Practice Based Pharmacist Salary Entry level 43k Entry level with significant experience as practice pharmacist i.e. >5 years 45k Any further pay progression will be dependent on meeting additional agreed skills as required by Federation Annual Holidays The holiday year runs from 1 April to 31 March. If you are in the service of the Federation on 1 April in any year, entitlement to annual leave with pay in that leave year will be 25 days in addition to statutory and bank holidays as specified below (annual leave will be allocated on a pro rata basis for part time staff). Statutory Holidays You will receive the following Statutory Holidays with pay: 1. St Patrick s Day 2. Easter Monday 3. Easter Tuesday 4. 1st Monday in May 5. Last Monday in May 6. 12th July 7. Last Monday in August 8. Christmas Day 9. Boxing Day 10. Sick Leave If you are entitled to minimum statutory sick pay (SSP). This will be paid by the employer at the rates specified from time to time by the Department of Health and Social Services. GP Federations may consider an Occupational Sick Pay Scheme. 24

25 Maternity Leave and Pay Statutory Maternity Leave (SML) and Statutory Maternity Pay (SMP) is in place for those who are eligible. Pension This post will be superannuable from the date of commencement of employment. Date of Commencement The specific date of commencement will be determined by the period of notice required by your previous employer and compliance with all criteria specified within the Personnel Specification Mileage Claims Costs associated with necessary business travel will be reimbursed. Members of staff will be reimbursed for miles travelled in the performance of their duties which are in excess of the home to agreed work base return journey. Indemnity GP Federations require staff to have current indemnity insurance that covers all the tasks that are required to be undertaken. An allowance of up to a maximum of 500 is available to offset the cost of adding extensions to the principal insurance cover to allow delivery of the Lead Practice Based Pharmacist and Practice Based Pharmacist job description. This allowance is available on submission of valid insurance certification. Annual Appraisal All staff will participate in an appraisal process with their line manager. This is an opportunity to review their personal contribution over the previous year and plan for the year ahead, both in terms of their own development needs and the practical objectives that they should be working towards. 25

26 Appendix C: Waiting List Principles All successful candidates will be ranked in order based on their performance at interview. Offers from the waiting list will then be made as and when posts become available in rank order but also taking account of individual location and working hours preferences which must match the vacant post. The following rules will be applied: Applicants will only be offered posts which they have indicated on their preference sheet that they are interested in. Formal offers of posts within GP Federation Areas will be issued by to the address given at the point of application. Offer s will require a response by the deadline indicated. If the applicant does not contact the Recruitment Team by the deadline then the Recruitment Team will move to the next person on the waiting list. In such instances the applicant will retain their rank order and will be contacted again when another suitable vacancy arises. If the Recruitment Team is unable to reach the applicant at the numbers given or through their account and they have been unable to leave a message on their phone or mobile, then the Recruitment Team will proceed to the next person on the waiting list. As above the applicant will retain their rank order and will be contacted again when another vacancy arises. If an applicant refuses to accept a permanent post for an area they have indicated on their preference sheet, they will normally be removed from the waiting list unless there is a valid reason provided for their non-acceptance. Once an applicant has been offered and accepted a permanent post they will be removed from the waiting list.
