ASPAN position statements are a great resource. Current/Pos_Stmt_11_Social_Media.pdf. It is such a relief to vent our frustrations, but think twice about doing it on any social media. The most you can post about your work day is I had a great day!, absolutely no particulars. Any detail could be connected to a patient or another staff member. Do not share/display photos or yourself or co-workers engaged in activities which may reflect negatively on you or your companions. Remember: employers, schools, lawyers all have access to it. Timelines can be seen on it to determine if you were using a cell phone during work time for socializing. Separate the professional from the personal and if you question it, don t post it. Now on a positive note: because we all use & love the links of social media, we need someone to create a Facebook page for ISPAN! We can post events, polls and CEU offerings at your institutions. 3 Happy Fall Y all! ASPAN REGION 2 DIRECTOR Page 3 Smells like the fair & festivals outside! Autumn is definitley my favorite time of the year, and I would have to say my our entire unit seems a bit more festive! South Mississippi in that late summer season gets pretty hot and humid so we welcome autumn with open arms. We even bust out our fashionable scarves and long sleeves when the temp drops to 60 degrees. I must say its not only the weather, but we love football and there s plenty to love in Mississippi! Extraordinary things are going on in healthcare, Perianesthesia nursing and in ASPAN! ASPAN President, Jacque Crosson, has shared with us valuable information on professionalism in past several issues of Breathline. She has discussed the value of listening and mentoring, both are vital in a thriving perianesthesia setting. Truly one of the most valuable skills we can develop is listening and communicating well. I read a quote on Pinterest the other day and I d like to share it with you, because it really resonated with me. It says this, The biggest communication problem is when we do not listen to understand and only listen to reply. When I think about it I can so relate to this statement. Being an effective listener and improving my communication skill is a personal quest for me. This goes for dialogue between me and colleagues, my patients, my friends, my children, my spouse. I hope you ll take time to evaluate your listening manners A Approach is everything. Imagine waking up in a warm bed, cold room. Your eyes are sleepy, and you really don t want to get out of bed. A million tasks, problems, demands, events, and opportunities await us every day. So we climb out of bed, coffee, shower, dress and out the door we go. Our approach to our work makes all the difference, our perspective of our work. The way we act and interact with others makes a difference. Professionalism and attitude bring energy that can certainly rub off on others and contribute to healthy work environment. Also, I think its important in our profession that we guard against discontentment that feeling of dissatifaction with work or in some cases fear. Consider ways to guard yourself. Encourage someone else! Find a tangible ways to actively encourage others in your unit. Leave a thank you note or give a word of encouragement to a colleage. Be thankful! Thank a mentor! Think back to that special person that mentored you along your journey and go out of your way to acknowlege the difference they made in your professional life. Strive to increase knowledge and education, knowledge empowers us! I d like to close with words of our beloved Robin Williams from one of my all-time favorite movies, Dead Poets Society. But if you listen real close, you can hear them whisper their legacy to you. Go on, lean in. Listen, you hear it? Carpe Hear it? Carpe, carpe diem, seize the day boys, make your lives extraordinary! 4 Page 4 VP/President Elect Hello, Greetings from Sioux City, Iowa. I am Holly Meis, VP/President Elect of ISPAN, and I would like to thank the ISPAN board for providing the funds for me to attend the Leadership Conference in Nashville, Tennessee last month. What a wonderful opportunity to converse with other nurses throughout the country and meet some of the ASPAN board members. I tried to attend a variety of topics and especially enjoyed the session titled Social Media Sparking Interest. Some interesting discussions took place, which prompted me to display the ASPAN position statement about social media in my nurses lounge at work. The suggestion was made for all nurses to ask their co-workers if their patients were having a safe experience when you observed them using a hand held digital device at the bedside. At Mercy Medical Center in Sioux City, construction is underway in remodeling our new state of the art intensive care unit. This will eventually be a 20 individual bed unit with the projected completion date sometime in the Fall of In my own department in the Phase 1 PACU, we have been using the STOP BANG screening tool for Obstructive Sleep Apnea the last year and a half. I presented our data in a power point to our anesthesia group last month and pointed out how our STOP BANG scores can impact the patient's experience in the PACU. What a wonderful Fall ISPAN Conference it was in Carroll, Iowa last weekend with such a unique opportunity to hear Nancy Strzyzewski, a national ASPAN speaker, talk about Perianesthesia Nursing for Special Populations. Are you registered to vote or have you already submitted your absentee ballot or voted early at your local County Auditor s office? Know when and where you can vote. You can check your local County Auditor s office website for information regarding early voting by mail (Absentee Ballet Request), or early voting in Person (Satellite Voting). The National Council of State Legislatures at is another source. According to some estimates, one in every 45 voters is a nurse. Election project.org states in general election turnout in presidential election years has hovered between 50 and 60 % and in midterm election years such as 2014, it has remained at about 40%. The midterm elections will decide wheather Demorcrats or Republicans control the U.S. Senate and The balance of power in the House of Representatives. In Iowa we are one of the few nationwide who will determine this outcome. Iowans have been bombarded with an onslaught of more than 30,000 television commercials Since the June primarys. $13.8 million has been spent on the Braley-Ernst race between June 4 and first 10 days of September. According to political analysts the Iowa Senate race is the most heated in the nation. Live TV debates have been held for the U.S. Senate in Des Moines, Burlington and Davenport by Democrat U.S. Rep Bruce Braley and Republican state Sen. Joni Ernst. 9 Awakening News USING EVIDENCE IN PRACTICE Evidence In Practice By Donna Dolezal RN, MSN, CAPA, CPAN Page 9 What is the big deal about evidence based practice? Why is using evidence based decision making important? Let s go back to 1497 when it took months for ships to sail around the Cape of Good Hope on the way to the East Indies to gather spices and other goods to return to Europe. In 1497 Vasco de Gama left for the first sailing trip to India. During the trip 100 of the 160 men on the trip died from scurvy- or lack of vitamin C. It wasn t until 1601 when Captain James Lancaster in England suspected vitamin C may be the cause of scurvy. He researched this by sending lemon juice on one of four ships leaving England for the East Indies. Crews on the ship were given 3 teaspoons of lemon juice each day. When they arrived in the East Indies, half way, 110 of the 278 sailors had died on the ships without lemon juice. On the ship with lemon juice zero died. These results were reported to the British government but were not implemented on the British naval ships. In 1747, 146 years later, Dr. Lind ran the same study with the similar results. It was 46 more years, now 192 years since Captain James Lancaster sent the first ships with lemon juice, until this was implemented into practice for the British Navy. (Berwick, 2003) Can you imagine if, in healthcare today, something known to prevent 50% deaths was not implemented for over 100 years or even 10 years? Most would be appalled- both healthcare workers and the general public. So why don t we all implement and use evidence based practices? Using and implementing research that is already published continues to meet barriers. Barriers to using evidence based practice are reported by Wilkinson (2011) to include insufficient skill, knowledge or time, lack of support from supervisors, and lack of autonomy. Often the emphasis is on doing rather reflecting and a culture of busyness can create demands on time for both staff and managers. Gifford (2014) reported nurses often lack awareness of relevant research, or are isolated from knowledgeable colleagues to discuss research findings. Decisions are often based on budgetary and financial issues, local competition or strategic fit rather than research evidence. The Iowa Model of Evidence Based Practice (Titler, 2001) focuses on using current literature in clinical practice. It provides a step by step process. Triggers for using evidence can be problem or knowledge focused. Attendance at a conference or a journal article may identify a practice or process that your clinical area is not currently using. Problem focused triggers may have identified a clinical problem or a question. Continued on the next page

10 Awakening News Page 10. Narrowing the focus and not trying to solve all of the problems prevents the project from becoming overwhelming. One example from our facility is we identified increased length of stay in PACU for patients with undiagnosed sleep apnea as a problem. The focus was narrowed to how to identify patients with undiagnosed sleep apnea first before we could tackle how to decrease the length of stay.. If the trigger is a priority for the clinical area, the next steps are forming a team, gathering relevant literature and critiquing the literature for use in practice. Although gathering literature can be daunting, it has become. much easier with search engines on the internet. Looking for practice guidelines and reviews (such as the Cochrane review) are good starting points. After gathering relevant literature, the next step is to determine if there is sufficient evidence to pilot a change. If the change is successful it can be adopted into practice. Implementing the change into practice can be the real challenge. Titler and Everett (2001) published Translating Research into Practice which addresses the barriers associated with implementing evidence based practice in the clinical setting. The translational research model addressed 5 items: 1. Characteristics of innovation-narrow or wide? Cost/benefit? Policy or standard need to be written? 2. Communication process can be challenging. Is enough or do users need to be contacted individually. What committees or groups need to be involved? How will education occur- broadcast, small group, on line education? 3. Scope of users. Does this include nurses only from one unit, the whole facility, physicians, or nursing assistants? Does it involve information technology programming? 4. Social system- culture of unit(s) making change, support from administration, informal unit leaders support 5. Adoption of innovation- time and resources needed to implement and audit and/or monitor. Evidence based practices or processes may be initiated by individuals within the unit or by a department or unit outside of the unit. No matter where the trigger came from, implementation of evidence based practice is important. Staying current with our practice by reading journals, attending conferences and asking questions are important to give the best care possible to patients. Trust the process; keep it simple, focusing on each step and most importantly, be patient. Remember, it took the British navy 192 years to include lemon juice in sailors diets. Berwick, D. 11 2014 ISPAN Annual Fall Conference The Fall Conference took place the weekend of October 3-4, 2014 at St. Anthony Regional Hospital in Carroll, Iowa. The business meeting was held Friday evening after dinner at a charming local winery. All the committees were heard from, and the main topic for the evening was reviewing and approving ISPAN policies. The Conference took place Saturday. Featured speaker was Nancy Strzyzewski MSN, RN, CPAN, CAPA, ASPAN Director for Education. Entitled Perianesthesia Nursing for Special Populations, it encompassed ASPAN Vision and Benefits; Adverse Events in Ambulatory Surgery; Common Comorbidities and Anesthesia; Growing Pains: What s New in the World of Pediatrics?; Pharmacology of Adolescents and Adults; and Resilience in Middle Years. Diane Lange, BSN, RN, CPAN was elected Treasurer. We still need a candidate for Vice President/President Elect!! Names were drawn for a scholarship to next year s Fall Conference and a lovely bracelet donated by Beth Brown. Contributions of personal hygiene items were collected for the local Domestic Violence organization. Information tables were present for ASPAN/ISPAN and ABPANC. An added attraction was a poster presentation. Topics were Bedside Handoff; Certification; Endtidal CO2; IV Sticks and Decreased Pain Modalities; Pledgets for Cataract Surgery; Pre-op Warming; Serotonin Syndrome; and STOP BANG for Obstructive Sleep Apnea. 13 HANDOVER PRACTICE IN THE SURGICAL ARENA Bedside handover has a strong presence in nursing today. Information transfer at the bedside involving the patient and their loved ones in their care feels like the ideal pathway. The question is can this practice take place in the ambulatory setting? When thinking about this, certain scenarios come up as red flags, such as procedural flow, turnover, and throughput. Can bedside handover truly affect the procedural patient s experience and procedural efficiency? The answer is yes. Evidence supports the practice of bedside handover as a means to improve patient safety, patient satisfaction, and nurse satisfaction. AORN quotes that Joint Commission estimates that 80% of serious medical errors are associated with miscommunication during patient transfers (Seifert, 2012). Although somewhat challenging to implement in the beginning, the reward is worth the effort. The planning phase requires team involvement on the pre-procedural and intra-procedural sides. Planning how this event will occur and what content will ultimately be exchanged is critically important for implementation success. Testing the change is the next step toward implementation. Picking a smaller group of folks to test this with in the beginning gives a better picture of potential bumps in the road leading to full team implementation. Creating a safe environment for handover to take place is essential. Allowing for introduction of the newly arriving team member and offering a simple managing-up statement such as, you will be in great hands with Susie, RN. This practice helps instill confidence in the next phase of care for the patient and their loved ones. This again is critically important given the quick pace nature of procedural medicine. Calming patient and family anxieties are crucial in this setting, and taking the opportunity to decrease anxiety will positively impact the patient and family experience. A continual drive for performance improvement is essential in procedural nursing. We have a small amount of time to make a big impact, and bedside handover is one simple practice that offers a lot of bang for the buck. Simply put, our patients deserve it. If you doing a good job 95% of the time that s about 200 failures a day, and the consequences of these can be serious: anything form inappropriate treatment to delay of treatment to extended lengths of stay and increased healthcare cost. MEMBERSHIP MILESTONES Sandy Hughes RN, CAPA, works in STAR, the pre-admission arena at Unity Point St. Luke's Hospital. Even though she doesn't see her patients in person she makes a difference, taking her time, making sure they understand everything. She will work through her lunch or stay a little late to accommodate her patient's schedule. She is a wonderful team member and has worked very hard to ensure the admission navigation process is thorough and complete. For this she has been nominated by her co-workers and awarded the 2014 Smulekoff Kristin McVay RN, MSN, MHA has worked as the PeriAnesthesia Manager at St. Luke s Hospital in Cedar Rapids where she implemented the pre-surgical RN navigation and bedside handover in the Perianesthesia units. She has a system-based certification in Improvement work and finished her two masters degrees 4 years ago. 16 ABPANC SEEKS NOMINATIONS FOR PRESIDENT-ELECT ABPANC is accepting nominations for its President-Elect position. The term of office is one year, followed by a two year term as President and one year term as Immediate Past President. The purpose of the position is to prepare the individual to serve as President and provide leadership to the component. Requirements: 1. ASPAN/ISPAN Membership. 2. Willingness to serve. 3. Ability to commit to time requirements of service. Specific Job Duties and Responsibilities: 1. Perform the duties of the President in the absence or incapacity of the President. 2. Represent the President and component as requested or required. Perform other such duties as may be delegated by the President or the Board of Directors. 3. Attends the President s Luncheon at the National Conference. 4. Serve on the Board of Directors as a voting member. 5. Observe and assist the President in preparation for assuming duties and responsibilities of that office. Serve as co-chair the Education Committee. 7. 19 The Leadership Development Institute held in Nashville, TN this past September, was my first national ASPAN sponsored event. It's theme, Igniting Professionalism: Excellence in Practice, Leadership and Collaboration, resonated with me. I hoped to gain from the experience and insight of some of ASPAN's most notable members. The tone for the weekend was set by the first presentation, Mastering Teambuilding which featured the five regional directors. After dividing us into groups, we were tasked with creating the "world's greatest bedpan." This was a project we could all get behind. : ) Armed with construction paper, tape, straws and rubber bands, we set to work. In order to create our masterpiece, our group had to come together in short order. Communication was key to avoid chaos with such large group of relative strangers and with our time constraint. All team members participated either through sharing of ideas or with construction. The best possible outcome would be a result of a truly engaged team. Each of us brought our own sensibilities, experience and creativity to the group. What we practiced and accomplished has obvious applications to our day to day work as perianesthesia nurses. We collaborate with patients, families, and the various members of the healthcare team to produce the best possible outcomes for our patients. I hope to utilize what I learned with my participation in ISPAN as well. ISPAN's benefits come from the combined sensibilities, experience and creativity of its members.
