2 PCP in 2013/14: DH identified a need for Paediatric Allied Health Services in Gippsland The catchment required a referral pathway to be able to refer children to the most appropriate service for their needs As a result of the promotion of the Human Service Directory during this project CWG had an increased in the number of Paediatric and Allied Health services listed on HSD A Gippsland Paediatric Health Interest Group was formed and has held an annual forum for the past 2 years with a 3 rd forum planned for A Gippsland Physio Network has been developed as a result of this project The Paediatric Pathways continue to be promoted at meetings and forums across CWG. ereferral projects - Liz Meggetto S2S S2S is being widely used in Gippsland Central West Gippsland is one of the catchment areas with highest use in Victoria in ereferral In 2013/14, 47,000 referrals were sent and received in S2S in Central West Gippsland 27 organizations and 202 services with over 2000 practitioners registered in CWG including Gippsland Wide services Argus/S2S An LCHS GP liaison officer has been identified to continue to drive this project in Latrobe CWGPCP will be working with Baw Baw organisations to implement this work in the near future ecare Planning - Liz Meggetto Linda Rowley is the project worker for this project The 1 st phase of the project was rolled out in Baw Baw in 2013 with the second phase being rolled out in Latrobe in 2014 Designed for electronic care planning, through the S2S platform, to increase interagency communication about complex clients care needs There are currently 9 partner agencies signed up to the MoU ecare plan is a living electronic document information can be viewed instantly about a clients care, goals and current services. Duplication is reduced as clients only need to have 1 care plan. State funded project Protocols have been developed in both Baw Baw and Latrobe including the used of Shared Consent Forms As of 15 th September, live care plans are being developed for clients
3 Over 50 staff have been trained in ecare Planning and an evaluation has been planned for November Assessment Alliance - Liz Meggetto DH funded initiative The goal was to create a suite of best practice guidelines for collaborative assessment across numerous health services. This would provide a consistent assessment practice, streamed lined processes with clients and elimination of duplicated effort Collaborative Assessment Best Practice framework developed Ensures all different organisations can do collaborative assessment in line with the best practice principals Project working group established in 2013 Piloted guide in Bass Coast and launched in September 2014 Available to view on CWGPCP website 6 Best Practice principals were developed Person Centered Practice, Informed decision making, Interagency collaboration, Trust and respect, Continuous learning and quality assurance practices Next step is to approach the Baw Baw and Latrobe Assessment Alliances to identify areas where they may be able to enhance collaboration in collaborative assessment Dementia Falls Prevention - Liz Meggetto Identified by DH as a priority area for Gippsland Scoping analysis completed to see what was needed 2 Dementia Falls Prevention Pathways developed (overview pathway and a complex pathway) A forum, a workshop, a GP information session and 2 video conferences were held Evaluation sessions were completed indicating: Increased awareness and knowledge about falls prevention for people with dementia, Increased awareness of local services, Increased knowledge of dementia related falls prevention statistics and interventions, increased awareness of the benefits of physical activity for people with dementia, increased knowledge of how to develop and monitor and modify individual exercise programs, increased insight of barriers that consumers experience and how to overcome these barriers to effectively engage in falls prevention activities and an increase in knowledge and confidence to engage consumers and carers in falls prevention activities A comprehensive evaluation will be completed in November 2014 and will
4 be submitted to DH as part of the final report. Food Security - Julie Hocking Food Security Project has been renamed to be called Food For all Baw Baw (FFaBB) Focusing on increasing peoples opportunities to access fresh and healthy food Point of Sale surveys were conducted at the Longwarry local hotel, newsagent, post office, grocery. It was found several families did not consume fruit and vegetables at all. Interventions will include looking at simple messages in places where people will see them, 2&5 message, live lighter campaign materials and messages, footpath stickers, school breakfast program evaluation, vegie man, achievement program signup, healthy oils campaign and information 4 Healthy Together Latrobe Update Latrobe City Council Health Literacy - Liz Meggetto The Gippsland Guide to Becoming a Health Literate Organization is in draft format and has been circulated to our member agencies for feedback The guide is based on the 10 attributes to becoming a health literate organisation CWGPCP is hoping to engage a champion in each organisation to drive Health Literacy activities internally Guide developed to support a consistent approach to Health Literacy and Health Literacy initiatives in organisations across Gippsland Alistair Edgar provided an overview of Healthy together Latrobe (HTL). Main focuses are to increase physical activity, increase fruit and vegetable consumption, decrease smoking and decrease alcohol consumption Working across targeted settings of childcare centres, schools, kindergartens, workplaces and communities. Working at a whole of population level Funded until June 2015 Key highlights include: Achievement program within schools and workplaces 90% of early childhood services have signed up to the Achievement Program 18 workplaces currently registered for the Achievement Program Estimated reach of the Achievement Program in Latrobe is currently 14,000 people
5 5 Future direction for CWG PCP in 2014/15 6 Department of Health - Policy Directions Currently planning to implement FoodCents Implementing Heart Foundation working groups and other healthy living programs Latrobe City is working on developing a community profile utilising a lot of existing data sets which Latrobe City collects Food systems data currently being entered into GIS Achievement program is underway in many of Latrobe s large employers including power generators, LRH, LCHS and Latrobe City Council. Think on your feet social marketing campaign developed to decrease sedentary behaviour in the workplace CWGPCP Future Directions Expand Argus/S2S ereferral into Baw Baw and increase sustainability in Latrobe Continue to support agencies to build sustainability of Interagency ecare Planning Conduct a medium term evaluation on the outcomes of the Dementia Falls Prevention Project (June 2015) in partnership with Curtin University Continue to strengthen the partnerships and sustainability of the Food Security Coalition in Baw Baw Begin the implementation of the Gippsland Guide to Becoming a Health Literate Organisation within our member agencies Partner with Monash University to conduct a long term evaluation of the outcomes of our Health Literacy work ( ) Conduct further academic investigations into the correlations between the Built Environment and the health of people living in Baw Baw, in partnership with Curtin University Develop quality improvement action plans for CWG based on the results of the Assessment of Chronic Illness Care (ACIC) Continue to develop Care Pathways in partnership with member agencies for specific chronic conditions (Mental Health is the next condition to be targeted) Investigate opportunities to partner with the Gippsland Primary Health Network once the successful organisation has been announced CHIP guidelines are due to be released and will be circulated to all organisations DH in collaboration with GML are investigating a care pathway website and have put together a discussion paper to scope how this work will be progressed Vulnerable children projects currently taking place included: the taskforce 1000 project, Streamlining antenatal services project (from antenatal-4
6 7 Discussion with DH and partners 8 Meeting closed noon years), Pathway to Good Health project. For more information about these projects, contact Brooke Campbell at DH. DH are reviewing all IHP catchment plans across Gippsland. General themes emerging from this included: need to strengthen evaluation design and indicating how agencies are tracking against expected progress on each initiative. Sustainability and scalability of initiatives was also identified as an area for consideration. Revised IHP plans due back to the department at the end of October Victorian Healthy Indicators Project Tony Triado Central Office DH has been leading this project. 22 indicators will be trialled and potentially condensed. A web based portal has been developed to load data to enable benchmarking with the aim for this to be available in December There will be training provided to agencies on how to use the indicators and how to load the data onto the web based portal. A working group will be overseeing the trial. A competitive tender process will be held for the Primary Health Network with no changes made to the Gippsland boundaries. It is expected that the successful tenders will be announced in the final 3 months of this financial year. PCP Health Promotion reporting requirements have recently been provided to the PCP s with the reporting deadline for PCP IHP being extended to 30 th November. DH encourage PCP s to continue to progress implementation of their IHP and Strategic plans. General discussions between all members present was held.
