2 Frankston Mornington Peninsula Primary Care Partnership Strategic Plan Preamble In 2013, the Frankston Mornington Peninsula Primary Care Partnership (FMPPCP) made the strategic decision to integrate its planning processes with those of its collaborators to develop a single primary health care planning framework for the Frankston Mornington Peninsula catchment. In partnership with local government, Peninsula Health, the Frankston Mornington Peninsula Medicare local (FMPML), the FMP PCP developed the Peninsula Model of Primary Health Planning ( the Peninsula Model see Appendix 1). The Peninsula Model provides not just a shared framework for collaborative planning and service development, but a platform for enhanced service delivery with a focus on shared priorities identified through a rigorous population health planning process. The FMPPCP is uniquely placed to share the leadership and focus its resources on auctioning the Peninsula Model priorities, which have been developed in alignment with PCP guiding principles of: Tackling health inequalities Person and family centred Evidence based and informed Cross-sector partnership Accountable governance Wellness focus Sustainability. The development of the Peninsula Model and its priorities was informed by inter-sectoral and community consultation, and its implementation is guided by Alliances with ongoing consumer representation and engagement. The Peninsula Model strategic management is governed by the Peninsula Model Executive Group (PMEG) comprising the membership of the previous FMPPCP Strategic Directions Committee and the Executive group of the FMPML. PMEG is chaired by the Chair of the PCP. Separate governance arrangements remain in place for each organisation, including a PCP Sub-Committee to oversight PCP-specific business and strategy. The PCP remains auspiced by Peninsula Health. FMPPCP Strategic Plan
3 The Strategic Plan Goals: are maximise health and wellbeing outcomes promote health equity avoid unnecessary hospital presentations and admissions. In summary, the FMPPCP through the Peninsula Model aims to improve client and patient experiences of the health and community care system by facilitating a more integrated joined up system with a focus on better health outcomes, particularly for populations at greatest risk and of greatest need. Priority issues The priority issues of the Peninsula Model are: Ageing Well Child Health Aboriginal Health Mental Health Chronic Disease Prevention and Better Health e-health Work on each priority facilitated and directed by an Alliance of stakeholders and consumers. The FMP PCP provides secretariat and project support to the Alliances and their working groups. Details of each Alliance are available on the Peninsula Model website and in Appendix 2: Summary of Alliance Workplans. FMPPCP Strategic Plan

5 STRATEGIC INTENT STATEMENTS FRANKSTON MORNINGTON PRIMARY CARE PARTNERSHIP STRATEGIC PLAN (Year 1-2) Strategy Goals: maximise health and wellbeing outcomes promote health equity avoid unnecessary hospital presentations and admissions Target Populations are Aboriginal and Torres Strait Islanders, Vulnerable children and families, Older people, people with complex needs. FMPPCP is also taking a place based approach and targeting the small areas of Frankston North, Hastings and Rosebud West.
