1 We are a reinforced concrete subcontractor specialist with over 15 years of experience, who are continually looking for ways to improve the safety and quality on all our sites. Attached are some of the initiatives TVC has taken on.

2 1. Super Green Infrared Boiler Powered by solar panels In our yard, we have tried to be as energy efficient as possible by using an infra-red boiler, solar panels and a wood burner. 2. Wood Burning Off cut timber from waste produced on Site

3 3. In House training Room Going forward, we now have the capacity to give all our operatives in-house training. We have our own in-house venue and facilities for training. As of 2015, all our operatives will receive CITB 1 day site safety awareness training, delivered from our in-house venue.

4 4. In House demonstration Room Where we can hold regular training sessions as required, and we can give our operatives familiarisation training on the equipment we use regularly. In partnership with Peri Ltd.

5 5. All lights are on PSV sensors no switches & Recycling bins throughout the office 6. Drug & Alcohol Testing ALL SITE MANAGERS WILL BE ABLE TO LOCATE A COPY OF THE DRUGS AND ALCOHOL POLICY, IF YOU WSH TO VIEW IT. WE ARE DOING A 5% RATIO RANDOM DRUG & ALCOHOL TESTING ON A QUARTERLEY BASIS. ALCOHOL ALLOWS FOR A READING OF.08% DUE TO NATURAL SALIVA AND FOOD PRODUCTS THAT MAY CONTAIN TRACES.

6 7. Plastic Ply In February 2013 TVC conducted research to find out whether a plastic sheet (made from recycled plastic) could be a viable alternative to Plywood in formwork applications. Although the name had not been finalised, the Laboratory who produced it identified it as P2 (their second edition), TVC called it Green Form and its name on site (nick-name) was Plastic Ply. Whereas Plywood and timber have been used for many years and lots is known about its behaviour and characteristics, P2 was developed in a laboratory and is a new product so the research conducted on site was essential to create P3. P2 was tested at two different sites, in various temperatures and by different people. A lot of the findings were positive, especially on the finish it created but it also found a lot of faults which will hopefully be rectified by P3.

7 8. Green Vest/Box In back up to our site management team, we have introduced Green Vest monitors, whose role is to help our over-stretched site managers to keep a clean and tidy site. HSE Monitors are onsite for your benefit. Please listen to their advice and follow their instructions. A HSE monitor (Green Vest) will ensure that PPE and First Aid supplies are stored in one place, that stock levels are managed correctly and that the site is in a clean and tidy state. Feel free to report any unsafe working area or unsafe work practise to your HSE monitor. Health, Safety & Environment is everyone s responsibility. Any queries or concerns must be report to your HSE monitor or the HSE Manager Paul Harte The Green Box must only be used to store the items listed on it. Under no circumstances must it be used to store tools/nails etc. Access to the Green Box must be via the site supervisor or HSE monitor. Banksmen must ensure all items are secure before lifting. If you notice and damage/theft to the box or its contents, it must be reported to the site manager immediately.

8 9. LED Safety Signs TVC are implementing their own LED signs for sites. We hope that this message will become more effective than posters and hard back signage for areas with a particularly high risk Safe Start Every year, in January, every TVC employee and labour only sub-contractors at the time are required to attend a ½ day course. The course covers 4 main topics that have been issues in the previous year and general site safety awareness. The idea of this is that we learn from our mistakes, errors of judgement and near misses so that they do not occur again. The course is a joint effort put together and presented by our external safety consultants South Wales Safety Consultancy and ourselves. The course also gives all the men an opportunity to voice any concerns, ideas or suggestions they may have.

9 11. Site Safety Notice Boards We have Health, Safety & Environmental Notice Boards on all sites, which are updated as needed by the Site Manager or the Green Vest Operatives on site. These include posters on personal and work safety, and they also include posters reminding the site operatives of the importance of Environmental safety. Also on the notice boards is any contact details and procedures for accidents and near misses. TVC also produce an annual safety poster, with content that we hope will hit home with the operatives onsite, as their safety is paramount.

10 12. Site Folder System 1-17 Above is an example of the folder system that we use on every site. The folders are prepared in the office before being sent out to site. 11 13. Health & Well-being/Fit for Work TVC are bringing in occupational health and safety screening for all our sites, not just for the rail works. Health Screening; What does the Occupational Health Surveillance Assessment cover? Eyesight to Occupational Health Standard plus colour blindness & peripheral vision Hearing Test using the very latest mobile Siemens equipment Lung Function Test using a digital spirometer Skin Surveillance using a BIA Skin monitor Hand Arm Vibration Syndrome Surveillance Musculo-skeletal Assessment Blood Pressure Urinalysis for Protein, Glucose and Blood (optional) Review of medication Occupational Health Questionnaire How do the assessments work? The assessments will be conducted within a private room at your premises by a trained Occupational Health Practitioner. The assessment will last 20 minutes The employee will be advised of their results during the assessment and a report will be sent to the organisations H&S Manager within 14 days 14. In-house Face Fitting A qualitative fit test is a pass/fail test that relies on the employee's response to a test agent. The OSHA protocols include saccharin, isoamyl acetate (banana oil), Bitrex and irritant smoke. For a saccharin or Bitrex test, an administrator challenges a subject wearing a respirator with a test aerosol. The subject dons the respirator and a fit test hood. The test aerosol (saccharin or Bitrex) is sprayed inside the hood while the subject performs prescribed exercises. If the subject can taste the test agent, the respirator fails the test and another respirator must be tested. Prior to conducting the test, the administrator must determine if the subject can detect the test agent. If the subject can t detect the test agent, another one that can be detected must be used. The fit test procedure requires about 15 to 20 minutes. At TVC, we have trained up our own men to be able to carry out the face fitting tests. This ensures that we can meet the demand of operatives that need testing so that everyone is working in the most possible safe manner. 15. Safety Harness Training Harness training, is a power point presentation touching on working at height, how to fit a harness and emergency rescue. TVC have our own in-house trainer who is qualified to carry out this training. This again means we can meet the demands of our workforce to ensure everyone is adequately trained.

12 16. Onsite Fall Arrest Training 17. Supervisor s Hand Book Early 2015, our aim is to produce a supervisor s hand book. Not only to cover safety, quality and production but to explain to our supervisors what is expected of them, and their roles and responsibilities. It will also cover our expectations on how they communicate with the clients (the Main Contractor). It also covers how we expect the aesthetics of our sites to be, such as TVC branded combisafe edge protection, safety signs with TVC logo, hi-vis vests with company logos etc. Our aim as a company is to be more professional going forward.

13 18. ISO s 9001, & TVC are currently working towards the above standards for environment, quality and health & safety subjects. The ISO standards have further evidenced TVC s commitment to providing quality work, on time and safely to the highest standards. Our aim for completion was December 2015, which we were on target for, and managed to complete it in August 2015.
