3 Policy Meta Maya Group (MMG) has an individual Quality Safety and Environmental Policy which all staff should be well aware of. Policies are available in Reception and on the website, or can be found in VBM. New employees are given a copy as reference.

5 Management Responsibilities Management will provide a safe workplace by: Providing training Providing personal protective equipment [PPE] Encouraging reporting of incidents and near misses Providing the required time and resources

6 Employee Responsibilities Employees are part of a team who apply safe work practices and common sense to all activities carried out as part of their work. Details of safety/health responsibilities are specified in Position Descriptions.

7 Employee Responsibilities Employees are required to: Obtain a thorough understanding of the OHS System including documentation, practices etc. Follow the procedures and practices as outlined in the OHS System Report incidents or near misses to the OHS management team

8 Risk Register Hazards and risks associated with MMG activities are listed on the Risk Register These hazards and risks apply to: Staff Safety, Staff Health and the Environment Controls are in place, in order to conduct our work safely If you identify any additional hazards or risks, please inform your Manager

9 High Risk Activities Staff Health/Safety Our five main areas of risk are: 1. Ergonomics and Manual Handling 2. Chemicals and Hazardous Substances 3. Atmospheric Contaminants 4. Heat Stress and Remote Work 5. Working at Heights

10 The DO s for Office Safety 1. Keep your work area tidy and organised 2. Ensure your workstation setup is comfortable for you 3. Take frequent breaks to ensure you do not suffer back/neck fatigue or eye strain 4. Refer to Officesmart exercise pamphlet

11 The Ergonomic Workstation

12 Manual Handling 1. Read and put into practices MMG s Safety Procedure Manual Handling 2. Complete Aveling s free online training on Ergonomics and Manual Handling 3. Staff will be trained in manual handling specific to their job requirements

13 Heat Stress and Remote Work Working Outdoors Staff may be required to work outdoors for a long period of time, sometimes in hot weather. If not controlled properly, this can lead to heat stress, potentially dehydration and in the worst case, death.

14 Preventing UV Damage Staff may be required to work outdoors for a long period of time, sometimes in hot weather. UV Damage to your body (e.g. skin and eyes) can occur if adequate protection is not used. To prevent UV Damage staff must: 1. Seek or provide shade 2. Wear field uniforms with sleeves rolled down 3. Wear a wide brimmed hat (Provided by MMG) 4. Wear safety sunglasses (Provided by MMG) 5. Wear SPF+30 sunscreen (Provided by MMG)

15 Preventing Heat Stress If not controlled properly, excessive exposure to heat can lead to heat stress, potentially dehydration. And in the worst case, death. Controls suggested for heat stress: 1. Installing portable shade; 2. Strategically-placed ventilation fans; 3. Scheduling activities to ensure less strenuous tasks are carried out in the hottest part of the day. 4. Use of PPE Including: long sleeves, full length trousers, sunglasses, sunscreen, wide brimmed hat, ice-vests or ice-scarves. 5. Keep well hydrated

17 Working at Heights What is Working at Heights? Working at height is a work situation where: A person has the potential to fall 2 metres or more; A person can gain access to within 2 meters of an exposed edge where there is a potential to fall 2 metres or more; A person could fall less than 2 meters, but the fall could result in significant incident.

18 Training Working at Heights (MMG-SE-P-035.G) External Training on Working at Heights to be provided to all Air Quality staff prior to starting field work. External training on Use of Elevated Work Platform is provided to all Air Quality staff as soon as possible after employment. This external training is also provided to other staff, if deemed necessary by the Section Manager.

19 Personal Protective Equipment (PPE) PPE Requirements All staff who will be conducting field work are provided with: Long sleeve shirt and long pants Safety boots Hard hat Wide brimmed hat Respirator Safety glasses The PPE highlighted on this page is compulsory PPE for all field work. The additional and specific PPE required for a job is highlighted in the HSE Plan, e.g. hard hat vs wide brimmed hat.

20 Additional Info Safety Glasses Must be used when using chemicals Also protects your eyes from dust Over-glasses can be provided if you wear prescription glasses Gloves Different types of gloves are available for different applications Refer to documented method

21 Environmental Impacts Environmental Controls These are included in the Risk Register Controls to be implemented in the field are captured by the Safety Plan

22 Safety and Environmental Planning for HSE Plan ( Safety Plan ) Purpose: To plan for the known hazards of a job. Must be conducted by the Team Leader prior to field work being conducted. Section Manager should review and authorise the HSE Plan prior to field work being conducted. All team members should review HSE Plan. Should be taken to site as a reference. JSA (MMG-SE-G-F-058.IS) Purpose: To identify any unforeseen hazards once on site. Completed EACH day that work is conducted. All team members to review and sign off on this form.

23 Safety and Environmental Planning Generic SWMS Purpose: To identify the hazards involved in particular jobs Controls listed on the SWMS must be implemented JSA (MMG-SE-G-F-058.IS) Purpose: To identify any unforeseen hazards once on site Completed EACH day that work is conducted All team members to review and sign off on this form

24 Incident Reporting All staff are required to complete a Safety Environment Incident Hazard Report Form (MMG-QA-G-F-105.G) immediately a safety/health incident or an impact to the environment occurs. If the Safety Environment Incident Hazard Report can not be completed immediately (e.g. off-site, urgently taken to hospital), the report must be completed within 24 hours.

25 Near Miss Reporting A near miss is something which MAY result in an adverse effect to staff or the environment if it is not corrected. Near misses must also be reported (MMG-QA-G-F- 105.G) as this allows us to take a proactive approach to keep staff safe.

26 Incident Investigations SEIRs are completed electronically and forwarded to the QSE Team. The urgency of the SEIR is assessed by the QSE Team. An investigation into the incident will be conducted, involving the relevant staff (e.g. Safety Representative, Division Manager, DODO, CEO). Safety Incidents will be tabled at the monthly Management Meeting.

27 Emergency Procedures Two Emergency Procedures which you should know: Canning Vale Location Chemical spills Fire Personal injury Etc.

28 MMG Practices 1. Log in Board: Allows us to identify who is in the building (for evacuation) Log in/log out system 2. First Aid Kits In each vehicle for site work Located in Kitchen area for Office based use Record any usage in books provided Checked and re-stocked regularly 3. Emergency Evacuation Drills Conducted for the Office on a regular basis Tests different high risk scenarios

29 General Housekeeping We want our premises to be a place which is: Clean Tidy Hygienic You can do your part by: Washing, drying and putting your dishes away when you have used them. Keeping the toilets clean and tidy Replacing things which are finished. Tidying as you go (particularly the floor and benches).

31 Right to Stop Work According to OHS legislation, all employees have the right to stop work if they feel that their safety or health is being threatened.

32 Complaints Your Division Manager is responsible for addressing any safety issues which you may have. The Safety Representative should also be informed of your safety concerns, and it is their role to work with you to have your concerns addressed. If you feel your concerns have not been addressed properly, please see the QSE Manager. If you still feel your concern has not been addressed properly, please see the CODO. If you still feel your concern has not been addressed properly, please see the CEO.

33 Traceability Complaints should not just be verbal. Any safety incidents or near misses need to be reported on MMG-QA-G-F-105.G Any suggestions for improvement to safety should be included on an OFI Opportunity for Improvement (MMG-QA-G-F-085.G).
