Every single medical and hospital staff member contributes to a culture of safety by supporting each other at all times, valuing teamwork and placing patient safety above all else. The intent is to always provide a safe, quiet, comfortable and clean healing environment for every patient served. Good communication between physicians, hospital staff and patients is essential. To ensure a culture of safety at Swedish/ Edmonds, directors and managers, including those with nonclinical backgrounds, are now rounding with patients up to one hour a day to hear firsthand what s working well and what opportunities there are to enhance patient care and communication. In the coming months, physicians will begin receiving training using the Swedish Safety Bundle. The Safety Bundle asks physicians to make a personal commitment to providing safe care by using the following safety behaviors and their associated error prevention tools: Culture of safety training sessions will be offered to explain each error prevention tool and how they enhance communication and safety. All medical staff will be expected to understand and use these tools in order to ensure a culture of safety at the hospital. Additional information will be provided about training sessions for physicians in the near future. Please make a personal commitment to patient safety by receiving this training. 2 New Advance Care Planning Consult Service Few patients with multiple admissions and chronic lifethreatening illness have advancecare planning documents available for end-of-life decision making during hospitalization. Physicians are often sought out to help with this process. While committed to helping their patients make good decisions, providers are not compensated for the significant amount of time it takes to make meaningful end-of-life decisions. Thanks to a grant from the Swedish Foundation, Swedish/Edmonds has developed a new and free advance directive planning consult service aimed at broadening the use and acceptance of advance directives by current and potential patients. This service helps guide your patients through the completion of advance directive planning materials in groups or individually. The below table provides a few examples of challenges patients may experience and how the Advance Care Planning Consult Service can help. To make a referral to the Swedish/ Edmonds Advance Care Planning Consult Service, please call End of Life Decision Making Challenges I am not a doctor and have no idea what are the right decisions to make. My doctor knows what is best for me. My attorney gave me standard advance directive legal forms. They were so easy to just read and sign. Consult Service Provides Education and personal support by volunteers with healthcare backgrounds who can guide the creation of advance directive materials that can be reviewed with their physician. A unique 6 Step Guide is reviewed with each patient in order to stimulate values clarification and identify personal wishes about possible end-of-life scenarios. Yes, I have all my documents completed and they are in my safety deposit box. I didn t even know that I was her surrogate for health-care decisions. I wonder what she really wants to have happen. Follow up contact from volunteer to ensure patients reviewed their wishes with their physician Specific instructions about how to make sure their documents are accessible should hospitalization occur unexpectedly Assists patients in depositing forms where they will be accessible when needed Assists patients in deciding how to select the right surrogate for them Surrogates attend the consult along with the client and actively demonstrate their understanding of the person s wishes I never got around to completing them all. As I ve entered credits for 2012, I noticed that there are some physicians who have not yet registered. If you have not registered, it is not possible for me to enter CME credits you earned. Registering is easy and only takes a couple of minutes. Go to Click For Health Professionals. Then click CME, and then click My CME Profile. Enter your last name and the last four digits of your Social Security number, then check I am a new user. Complete the form to complete the registration process. You should receive a confirmation . Those who have registered can get their CME transcript online. Go to click For Health Professionals, then CME, and then click CME Transcript. Enter your last name, the last four digits of your Social Security number and then the beginning and end date of the report you would like. 5 New Process for Vendor Credentialing Vendors visiting Swedish/Edmonds or any location within Swedish are now required to use a new system for credentialing. Last month, Swedish began using the Reptrax Vendor Credentialing System. All vendors who enter any location within Swedish, including Swedish/ Edmonds, must be a member of Reptrax. Vendors are required to sign in every time they visit Swedish/ Edmonds and they should have a badge that s only good for 24 hours and includes their photo. No other identification such as the vendor's company badge will be acceptable to enter hospitals within the Swedish system. We ve been in need of a process to verify the credentials of vendors who visit our hospital, says Mark Nunes, director of Facilities and Safety at Swedish/Edmonds. Reptrax helps to ensure that vendors visiting Swedish/Edmonds have proper vaccine inoculations and background checks. This is a major patient safety issue and Joint Commission requirement. All vendors have been notified regarding the new credentialing system and they may apply for a Reptrax membership by going online to Once a member of Reptrax, a vendor will have access to login and print their badge at any one of three kiosks located in the hospital. One kiosk is located in the OR next to room 314; another is located at the Volunteers' desk at the hospital entry on the third floor. A third kiosk will be installed on the first floor at the entry of the hospital near Admitting as soon as a data line is "dropped". Because Reptrax is a new system, all clinical staff members are asked to be on alert for vendors in the hospital without a Reptrax badge. If you see a vendor without a Reptrax badge, please inform them that they must go to one of the kiosks and sign in or they must leave the hospital. If a vendor declines to use a kiosk or leave the hospital, please contact hospital security at extension It may take a few weeks or possibly months for all vendors to obtain Reptrax membership, but it is critical that this new process is implemented for the safety of Swedish/Edmonds patients. EKG Reminder Physicians entering an order at Swedish/Edmonds for an asap or stat EKG in Epic, please remember to tell the nurse or unit secretary to page for the EKG so the order is not missed.
