The quaint town is lined with beautiful trees, Victorian and Colonial style homes and independent shops. Our charming center features a grand dining room for fine dining experiences, including kosher and vegetarian diet menu selections, and offers private suites for post-acute rehabilitation stays. Services available also include long-term care, certified wound care, orthopedic rehabilitation, cardio-pulmonary rehabilitation, respiratory therapy, comfort care, and respite stays. Our dedicated team helps residents and family members feel at home while receiving individually-focused healthcare. From nursing care and nutritional support for recovery, to the activities of daily living, we focus on an innovative, interdisciplinary approach. Our services include diabetes management, ostomy care, respiratory therapy, tracheostomy care, IV management, enteral nutrition, and in-house diagnostics. rehabilitation Whether recovering from a stroke, fall, or any other health condition that would benefit from rehabilitation, our therapy department provides a variety of services for our community. Working to gain a clear understanding of personal goals, we are committed to returning our residents to the highest possible level of function and independence. Utilizing state-of-the-art programming and advanced modality services, our team has the education and experience to help residents meet goals through physical, occupational and speech services. We also offer cardiac and pulmonary programs. respite Our team manages a respite program that provides caregivers time-off from caring for a loved one at home. In the event that care is needed for a long weekend or for a few weeks, family members can feel secure knowing their loved one is provided with a safe, home-like healthcare setting where both short- and long-term residents receive clinical monitoring by an educated, skilled staff whose goal is to meet needs quickly.

5 wound care We offer a comprehensive wound care program led by a Certified Wound Care Physician. Our advanced treatments include negative pressure wound therapy, post-surgical management and in-house debridement. Our certified team recognizes the specific needs of residents admitted for wound care treatment. health & wellness We embrace the concept of socialization for every resident and believe the healing process and long-term health stability relies on Whole Person Care. From live music events, fine dining experiences and community field trips, to pet therapy, outdoor gardening and religious services, including observance of Jewish traditions and kosher meal options, our staff supplements wellness by delivering opportunities for residents to maintain a fulfilled and pleasurable stay. From one-on-one interaction to group participation, our energetic team strives to utilize tailored activities to meet the individualized needs of our residents. Our compassion is expressed both by how care is delivered by our staff, and how we organize and operate our building and programs. comfort care Our comfort care program, which includes both palliative and hospice care, provides for individual resident wishes. Our goal is to comfort the resident and help them transition through this phase in their life. Our facility s nursing team also offers support to the families and loved ones of a resident going through a Graceful End of Life Transition. It is our staff s intention to be mindful of when visitors are present and to anticipate their needs.
