Rehab can significantly impact 5 stars for QM for both Short and long stay residents by not just preventing functional decline or addressing changes in condition but also in reducing the risk of re-hospitalization for skilled patients, ensuring safe transitions to the community, caring for residents with behaviors, caregiver training and more. We utilize a unique program, Treat and Teach, where short episodes of rehab are tailored for long term residents with a conservative approach to keep them at their highest level instead of waiting for a decline. I am proposing to engage my audience in group activities to think outside the box and utilize rehab to their fullest. My session aims to bring out creative clinical thinking on how therapy can assist them achieve the 5 stars they want! 5 List three session take-aways: 1) What Quality Measures should I be looking at in my SNF and how do they impact my star rating? 2) Collaborative approach towards QM- how can therapy address residents before they decline to allow for maximizing of functional outcomes?
