1 Mental Health by Design in Schools - Award Application Instructions Thank you for your interest in the Mental Health by Design Schools Award. Please review the goals, guidelines, selection criteria, and timeline before submitting an application. Applications are due January 23, 2017 by 11:59 PM. GOALS MHxD is an extension of Active Design in Schools (ADS), an award program that aims to support infrastructure enhancements in schools that promote physical activity and healthy living among students and members of the school community. For MHxD, we will select 15 high schools to receive grants of $10,000 to support built environment projects that have a demonstrated link to improved mental health outcomes. The MHxD award is administered by the Fund for Public Health in New York City in collaboration with the New York City Department of Health and Mental Hygiene and ThriveNYC. The Active Design Toolkit for Schools highlights several projects, such as the installation of murals and gardens, which provide emotional wellness benefits like improved mood and decreased stress and anxiety. Projects such as these may qualify for the MHxD grant. Other examples of acceptable MHxD proposals include projects that: - Facilitate mindfulness: Meditation room, yoga room, mindfulness walking path, mediation/relaxation garden, outdoor classroom - Promote safety and perceptions of safety: Improvements to metal detector waiting areas or safe corner/space for LGBTQ students - Increase students connections to one another and social networks: Buddy post or new or more open, inclusive public spaces - Improve access to and use of creative spaces: Upgrade art or music rooms or increase natural light and reduce excess noise - Encourage help-seeking behavior and reduce the stigma associated with mental illness: Public art/mural or upgrade library MHxD also aims to decrease mental health stigma, make mental health resources more accessible, and empower student leaders. In order to achieve this, and assist schools in maximizing their funds, MHxD grants come with two additional wraparound supports: 1. MHxD will partner with the social impact design agency, Hyperakt, to train and provide technical assistance to the grant winners throughout the duration of their projects. Hyperakt will work with 2 student leaders from each school via The Student Development Lab, an interactive 3-class engagement that will train the students in design strategy, social impact storytelling, and communications. The classes will take place outside of school at the Hyperakt office and students will be provided metrocards for their travel within the city along with other incentives. Each pair of students will create a brand, messaging and social media campaign that will highlight and promote their school s project and overall mission. 2. MHxD will fund two Architectural Consultants to guide and support each school in refining their projects, selecting vendors, managing the projects, and ensuring the enhancement funding is fully used. Each Architectural Consultant will also communicate with Hyperakt and the Student Development Lab to keep student teams aligned with their project s timeline and ensure they are engaged from planning to implementation. 1
2 KEY ROLES AND RESPONSIBILITIES MHxD support team responsibilities You and your students will receive: - Two on-site visits and phone technical assistance with an Architectural Consultant - Via the Hykperakt Student Development Lab: 1 webinar for Project Leads in February 2017 to provide guidance on student selection to the Student Development Lab and 3 classes in design strategy, social impact storytelling, and communications for your selected 2- member student team at the Hyperakt design studio - A personalized, student-led communications strategy to message and market your project - Assistance from the DOHMH MHxD core team Project Lead 1 responsibilities: - Recruit students to inform project design and implementation - Keep students engaged- Because this grant provides the wraparound support of an Architectural Consultant, the Project Lead will spend more time engaging students and less on the details of the project itself (e.g., selecting vendors and monitoring implementation). - Support the students elected to the Student Development Lab (SDL) at Hyperakt: 1) Debrief with them after their SDL classes and assist them with associated action items. 2) Ensure they are connecting what they learn to the project and wider student body involved in the project - Be primary point of contact for the Architectural Consultant assigned to your school, Hyperakt and DOHMH technical assistance team - The leads, students, Architectural Consultant, and DOHMH will meet together once in February to review goals and develop a project plan and time commitments. Project Lead 2 responsibilities: - Support Lead 1 when needed or requested - Be secondary point of contact for the Architectural Consultant assigned to your school, Hyperakt and DOHMH technical assistance team. Student input and participation is the cornerstone of MHxD. It is expected that a core group of students will be involved in the design and implementation of your school s project. In addition to identifying two student MHxD leaders, we encourage each school to find creative ways to seek input from the broader student body, such as through surveys or social media. Student leader role (2 students): - Attend 3 Student Development Lab off-site classes at Hyperakt - Serve as liaison to student body for project ideas via Hyperakt and DOHMH technical assistance team - Attend regular team meetings with project leads 2

SAVING YOUR APPLICATION: You must click "SUBMIT APPLICATION" or your work will be lost. If you need to stop and come back at a later time, you may make edits as long as you are completing the application on the same computer or device. We recommend using the following process to fill out the application: Copy/paste the questions in a separate document Take your time to answer these questions in the separate document Open the Survey Monkey and copy/paste your answers from the separate document into this online application Complete this online application and upload photos of existing spaces Click "Submit Application" to SAVE and you may return to same computer to edit questions until the close of the application period 3
4 SELECTION CRITERIA All NYC DOE high schools are eligible to apply and will be informed about the grant opportunity. All complete applications will be reviewed by a panel using a uniform grading scale. Proposals will be evaluated on the extent to which they: 1. Have a demonstrated link to improved mental health outcomes. 2. Increase or expand programming that supports the mental health and emotional wellness of students. All projects must include specific mental health programming in addition to the infrastructure enhancement. For example, proposals for meditation rooms must also include a plan for meditation classes, how often they will occur, who will teach them, how the school will sustain them, how classes will be used to promote mental health, and if they will be used as an alternative to disciplinary action. 3. Include plans for involving students throughout the process, from planning to implementation. The plan must also describe the process by which the school will select 2 students to the Hyperakt Student Development Lab and support their ongoing participation on the project. We will prioritize projects from schools that: Illustrate high need and dedication to improving health equity in low-income neighborhoods Demonstrate the capacity to implement the project Demonstrate the capacity to maintain the project and associated programming Have established linkages and support from school groups or community organizations ADDITIONAL INFORMATION Awardees will be encouraged to conduct school engagement activities such as surveys, brainstorming and visioning sessions with students, staff, parents and the community to finalize budgets and project plans. Upon completion of a group s project, groups will also be required to submit a simple final report, photos, and receipts for expenses associated with the project. In addition to funding, awardees will receive technical assistance from the NYC Department of Health and Mental Hygiene s Healthy Living by Design Unit and the Division of Mental Hygiene. Please note that we do not fund: salaries, consultant fees, speaker honorariums, rent, utilities, web hosting and development, 501(c)(3) incorporation fees, luxury items, audio/visual equipment, subscriptions, back-to-school items, or parties. Right to Reject Proposals The Fund for Public Health in New York City may reject any or all proposals received and may ask for further clarification or documentation. Submitted information that does not respond to all items in this RFP may be excluded from further consideration and alternative information packages may not be considered. The Fund for Public Health in New York City may decline to review an application in the event the respondent submits a response after the submission deadline and/or any disparity is found during the evaluation process. 15
16 B. Costs The respondent will be solely responsible for any costs incurred in preparing, delivering, or presenting responses to this RFP. Respondents will not be reimbursed for any costs incurred in preparing proposals. C. Fulfillment of Requirements By submitting an information package, the respondent acknowledges that the respondent has read and understands this RFP and is capable of fulfilling all requirements. D.
