1 DRAFT Michigan Association of Local Public Health Nurse Administrators Forum NURSE ADMINISTRATORS FORUM Meeting Minutes Tuesday, March 15, :30 am - 12:30 pm Location: MPHI, Okemos, MI 1. Call to Order: The meeting was called to order by Andrea Tabor at 9:35 am. 2. Administrative Items A. Approval of the Agenda: Motion by Lisa Hahn, support by Nicole Greenway to accept the agenda as distributed. Motion approved. B. Approval of the Minutes: Motion to approve the February 9, 2016 minutes by Lisa Hahn, support by Michele Ford. Motion approved. C. Treasurer s Report: Ann Hepfer reviewed the report. Motion to accept the Treasurer s report for February 2016 made by Michele Thorne, support by Anne Young. D. Chair s Report: Andrea Tabor ed her report prior to the meeting. Accreditation Commission Meeting They didn t have a quorum at last meeting so couldn t do any official business. MDHHS has sent a letter to Genesee County letting them know they can postpone their on-site visit. Meeting with MDHHS: Exec. Comm. met with Sue Moran, Orlando Todd and Mickele Robinson last week. Main purpose was discussed the letter that MALPH had sent to the Governor and leadership at the State agencies in response to Flint s situation focusing on lack of resources available to LHDs and need to improve communication between State agencies and local health departments. MALPH Exec. Committee members felt the meeting went well. The result is that there will have regular interagency meetings with mid-level personnel to discuss issues and what could be improved. DHD #2 Denise Bryan former Wurtsmith Air Force Base problems with perfluorochemicals (PFCs) issues in the water (Clark Marsh). In 2012 the LHD sent out a Do Not Eat the Fish notice. In Dec 2015 Department of Defense and DEQ tested the ground water for PFCs. By Feb 12, 2016 the DEQ had results but did not share them with the LHD. A letter was sent to Nick Lyon from DHD #2 regarding this issue. The LHD was assured there was no health threat. On Feb. 23 the LHD received a phone call from the State Toxicologist that things have changed and The purpose of the Nurse Administrators Forum is to enhance the health of Michigan citizens, based on our knowledge of Public Health Nursing practice, by providing effective leadership, collaboration, legislative outreach and professional mentoring for Public Health Nurse Administrators.

2 there was a pending action item. Water results were acceptable but over the long term there may be a health threat especially if fish was consumed. A toxicologist had informed one resident to seek an alternative water source. Knowing you can t tell one and no others - the LHD wanted to send out a Health Advisory. State officials wanted the LHD to wait to release the Health Advisory. Letters were sent on Feb. 22 & Feb. 26 th stating there was an issue with the water and not to consume the fish. The LHD (District #2) was not aware of either letter. The LHD sent a Health Advisory on Feb. 26 and Sanitarians went door-to-door to educate residents. Denise reached out to Orlando on the beginning of March to let him know that the LHD was out of the loop for the communication to residents and the State agencies were working in a vacuum. Sue Moran has since communicated with Denise and acknowledged that the State should have done better. Much discussion on the need to determine if this (PFC levels in water) is an issue in other communities. EH Forum will continue discussing at their upcoming meeting. Lobbyist - Budget hearings continues pretty uneventful. House perspective - took out language regarding revising mental health services in Michigan (had included giving money and oversight to Health Plans) and need to determine how much funding can be sent to Detroit Public Schools. $80 million tax credit for some insurance companies was included in the budget (to decrease insurance) some want to use this for Detroit Public Schools. Flint water situation discussion continues- Speaker of the House has said he won t approve any additional supplemental funding for this which spurred some controversy. Public Sector Consultants are currently doing an audit within MDHHS. Governor announced that there is going to be an internal audit within MDHHS lead by the Attorney General. It s unusually quiet largely due to the issues that are going on. It was mentioned when DHS and MDCH were merging it created challenges on who was responsible for what so increased challenges with communication. Also mentioned that the philosophy changed to run government like a business which you can t. It boils down to leadership. Flint Water Legislative Public Hearings start tomorrow discussion on what MALPH may contribute. Meghan agreed to attend. Executive Director Report The Executive Committee will be meeting with U of M next month to talk about mission and vision of U of M s School of Public Health. Bills that are being watched: Hooker Bill (immunizations) Meghan was told that they are not going to see a Committee meeting (good news). Several Democrats that came up with 6-7 bills to combat the Hooker bills (which also probably won t see a committee either). Vending Machine inspections and licensure there is a bill that would eliminate the inspections another bill came out to have MDARD do the inspections. EH Forum is responding to the impact(s). Vision screening bills HB 5307, 5535, 5356 concerns as this mandates parents take the child to go an optometrist. Copays, deductibles, transportation and provider capacity are some of the barriers. MDHHS Healthy Homes Section - Carin Speidel/Karen Lishinski, gave presentation on Healthy Homes (see attached). For children with Elevated Blood Lead Level for 5 or above (capillary or venous) mailings are going out State-wide recently to educate residents on the program. LHDs may be getting calls regarding this mailing. State is following up with children who are 5 or over. It was suggested that the letter that is being sent should include the LHDs contact information. Flint situation offering an environmental investigation for all cases with children 5 or more. EBL side partnering with them to make the necessary fixes. Also doing inspections on homes where there isn t a EBL child but water has tested high. WIC Caseload reductions/funding Birth rates have decreased so number of women/children eligible for the program has decreased. Some has to do with policy changes (ex. DHS increased food stamps benefit which decreased the need for food). Health Officers are going to ask Stan Bien if a group could be formed to look at this issue. Stay tuned. 2

3 Agriculture and Rural Development Budget enhancements have been proposed to Governor lab needs funding ($3 mil) and to implement Food Safety Training (1.2 mil) to farmers. Dogs on patios legislation Oakland County has been doing some dogs on patio by variance. Environmental Quality Carrie gave updates, which included: new Septage Waste person (Adam Rozenthal); EPA is going to be doing a thorough adult of the drinking water programs the first week of April that includes 13 counties (will probably need files sent to Lansing); Auditor General released an audit report on Drinking Water Program about two weeks ago. There was a finding for non-community water program stating that DEQ didn t assure that LHDS did everything they needed to do by required timelines (will be contacting some LHDs on this). Health and Human Services Orlando and Mickele attending. Wayne County had a mercury issue resident tried to make gold from mercury as a result there were sick individuals. Feeling positive about the 1017 budget. Agreed that the Leadership meeting with MALPH went well last Friday. Discussed the need for improved communication. E. Correspondence: none 4. Old Business A. Higgins Lake (May 2-3) Speaker Decision: Anne Young stated that she is working with a speaker on work-life balance for the afternoon session. Anne has contacted Lisa Letts to present on the topic of performance management for the morning session. A registration form is to be sent soon. 5. Speakers A. Rachel Schumann and Taggert Doll, Child & Adolescent Unit, MDHHS HB 5307, 5535, 5356 include proposal to change PA 451 of 1976 Revised School Code related to Vision screening and follow-up. Mandating that schools complete school age vision screening by January 1 of each year. They added several grades for screening. Schools are to be notified, but no provision to notify LHDs. A provision is added to allow parents to opt out of the screening. All concerns with this proposal were reviewed and acknowledged. MDHHS is continuing to identify concerns with this proposal and has provided feedback and suggestions for improvement. Some positive parts of the proposal include increased communication between MDHHS and MDE, and requiring follow-up of children that fail screening. Janine plans to see Rep. Price on 3-18 and will discuss our concerns. This proposal is currently being considered in the House Education Committee. B. Nancy Peeler, MDHHS, Manager, Early Childhood Section Nancy described the process for case management of children with an elevated blood lead level (dating from April 2014) that move out of Flint. MDHHS will contact the LHD (Health Officer) either in Michigan or other states, to determine ability to provide follow-up. Case management is being offered for all present and former residents of Flint that had an elevated lead level. Repeat testing is strongly encouraged. Nancy described the availability for Medicaid reimbursement for two case management visits for any child with an elevated lead level of 5/dL or greater (recent Medicaid change). Michelle Thorne has billed Blue Cross and gotten paid for case management visits. 3

4 JoAnn Hoganson will share the Insight EMR note that Kent has developed for Lead Case Management. 4. Old Business, continued B. Strategic Planning Action Plan Review: #3, Strengthen our communication strategies to raise awareness about public health issues in Michigan. Andrea Tabor and JoAnn Hoganson plan to attend the Day at the Capitol event. C. Bylaws Proposal: Michelle Thorne reviewed proposed changes to the Bylaws. All changes are highlighted on distributed document. Proposed changes were reviewed. Lisa Hahn moved and Dranoel Knox supported that Bylaws should state licensed nurse instead of registered nurse. Motion passed without objection. Lisa Hahn moved, with Jane Nickert supporting the adoption of the Bylaws as amended. Motion passed without objection. Michelle will make the approved changed and Lynn McDaniels will upload them to the Dropbox. E. Evidenced Based Practice: Lisa Hahn described a document that she has developed to assist with training staff on Social Determinants and Health Disparities. She will it out to the members. A copy is also attached. Michelle Thorne stated that she has added several folders to the Dropbox related to EB search engines. F. Secretary Vacancy: Jane Nickert has volunteered to take meeting minutes until the fall election and Deidre Reed has volunteered to manage the listserve messages. 6. New Business: A. New Members/Leadership Pins: Welcome to new members Ruth Manier from Dickinson-Iron and Sheila Wilson of Muskegon. 7. Reports A. NAF Committee Reports 1. Awards: Michele Thorne requests a member volunteer to take her place on this committee. 2. Bylaws: Michelle Klein has volunteered to work with Toni LaRocco on this committee. 3. Data: Reminder that Michele Thorne is requesting changes to the MDHHS Record Retention Guidelines. 4. Education & Orientation: No report. 5. Nominating- No report at present, but several Board positions will need to be filled at the next election. 6. Strategic Planning: Action Plan #4 will be reviewed at the April meeting. Kathy Haskins requested clarification about the item in Action Plan #3 related to development of an info document about NAF. There is no document developed at this time. B. NAF Representative Reports: 1. COMON: Lisa Hahn states that the next meeting is March 17,

5 2. MPHA: Janine Chittendon reported on the Board Meeting: Janine was unable to attend. Sarah Bryant listened in. Board Retreat is set for Saturday, March 19, 2016 in Lansing. Plan to discuss response to Flint water Crisis among other items. PHN Section: Janine also reported on this meeting. Program Committee Annual Conference is April 22 (Earth Day), It will be held at Kent County Health Department. Registration fees: $50 members, $60 non-members, and $25 for pre-licensure students. Brochure was sent to NAF electronically. Letters were issued to the House Health Policy Committee members in support of current non-medical waiver process and urge a NO for HB 5126 and 5127 as well as to memberships to contact their legislators. Discussed HB 5307 which is now tie-barred to HBs 5355 and Discussed the development of an annual PHN award. C. MALPH Committee Reports: 1. Governance- This committee is on hiatus and Michele Thorne recommends that we remove his from the agenda until further notice. 2. Premier Conference- Janine Chittenden reports that the 2016 Conference: October at the Kalamazoo Radisson. Theme is Public Health: Leading Prevention across Michigan. The committee is working on sponsorships through insurance and medical companies, schools, etc. RFP s for sessions are due by April1!! Please encourage your staff. Keynote topics- Flint water update, Ohio s algae bloom, opioid addiction, and Michigan State Police on marijuana. D. MDHHS Committee Reports: 1. CSHCS Advisory Committee: Janine Chittenden that there has not been a recent meeting. 2. Family Planning: Andrea Tabor reports that the meeting for February was cancelled due to weather. 4. MACI Sue Schryber reports the following: Barbara Wolicki (INE Program at MDHHS) will be retiring in early April. SE Field Rep position still vacant. Waiver update: Schools: Nov 2014 vs Nov 2015: waiver rate decreased by 40% (8000 less waivers Nov 2015 than Nov 2014). Religious waivers decreased from 4406->1954, Philosophical waivers decreased from 13,510 -> 8442, medical waivers decreased from > 828. Childcares/preschools: waiver rate dropped from 4% to 2% (Oct 2014 vs Oct 2015), 1640 fewer waivers in Oct 2015 than 2014 (overall decrease of 46%). Proposed Legislation: HB 5126/5127: Bob Swanson heard that they may not proceed to discussion/hearings. HB revises the PHC Section Upholds and strengthens our waiver education rules. HB revised the School Code to reflect the proposed changes to

6 HB 5363: Revises the School Code and requires the school to notify each pupil enrolled in the school the immunization status of pupils enrolled in Kindergarten and grades 2, 5, or 7. o Notification of the number of children enrolled with an immunization waiver. o Post all immunization information on the schools website and main reception area. Proposed ideas to increase immunization rates: o MDHHS plans to do quarterly pediatric recall (6-18 month olds) in hopes of increasing pediatric immunization rates. o Soon providers will be able to generate AFIX reports in MCIR for their practice themselves Flu Season: predominantly H1N1 strain vs H3N2 except for adults >65 years where they are equal. 5. MIHP- No report 6. MPHA PHN Section Academic Practice Partnership: No report 7. Body Art- No report. E. Free Forum (Open Discussion): Discussion about the STI Billing Toolkit webinar. Discussion of provision of services to refugees (billable to Medicaid if client is already enrolled). Janine Chittendon queried if any other LHDs have been successful in transferring info out of EZLink for CSHCS. Several LHDs are still in process. Macomb identified that KLNA is now charging additional funds to convert the data. Other LHDs will continue to work this process and discuss at the April meeting.
