3 FY 2014 Comprehensive FY 2014 A) Final Amendments Final amendments started to be released on July 18, % of the Local Health Departments (LHDs) have signed their Final amendment % of the LHDs final amendments are pending LHDs action. 6-13% of the LHDs still are working on Amendment 4 B) Financial Status Report s (FSR) for Fiscal Year % were submitted by the due date. 17% were submitted within 30 days of the due date. C) Obligation Reports: Path: Grantee>Reporting>Progress Reports Available on September 13, Due date for submission to MDCH through the MI E-Grants system is September 23, Agencies should enter their best estimate of source of state funds for the remaining reporting period. 3

November 30, 2014: Due Date for Final FSRs for the WIC programs. December 15, 2014: Due Date for Final FSRs for remaining projects. E) Local Maintenance Of Effort (LMOE) Actual LMOE reports for FY 2014 are due March 31, Actual LMOE reports will be available to complete by going to Grantee>Reporting>Progress Reports and selecting Grant Program CO-2015 Comprehensive Agreement This is an attachment report in the Hearing Project. The LMOE form is available in your Documents list for attachment in the MI E- Grants system. 5

D) Application Submission Due Date Started releasing project applications on August 28, 2014 Application Submission Due Date: September 17, The Authorized Official should submit projects on the Grantee>Grant Application>Grant Application Preview screen. Comprehensive FY 2015 E) Local Maintenance of Effort Projected LMOE report will be available to complete as soon as you sign your agreement by going to Grantee>Reporting>Progress Reports. This is an attachment report in the Hearing Project similar to FY Report due date is November 1, F) First Amendment Planned Release date for Local Health Departments is October 28,

10 Federal Financial Assistance Report Schedule of Federal Financial Assistance A) The Federal Financial Assistance Report will be sent through the MI E-Grants system to each Local Health Departments Health Officer and Financial Officer in late October. FY 2014 Actual FY 2015 Projected B) The Adjusted Value of Federally Funded Vaccines report will be sent in late October through the MI E-Grants system. 10
