2 Funding our Community Health Needs Agenda Grants ( The Health Resources and Services Administration (HRSA) State Office of Rural Health (SORH) National Institutes of Health (NIH). NIH is one of the world's foremost medical research centers. Agency for Healthcare Research and Quality (AHRQ) Program Opportunities United States Department of Agriculture (USDA) USDA Rural Economic Development Loan Grant (REDLG) REDLG Case Studies USDA Community Facilities Loan/Grant Program (CF) CF Case Studies

4 Funding our Community Health Needs Solving Priority Health Issues Behavioral Health Vision Hospital recognizes that there is a desperate need for a Behavioral Health Patient Care Program in our community. Our Inpatient and Outpatient programs will focus on: behavioral health services related to mental illness addictions or intellectual developmental disabilities management and coordination of patient healthcare The Adult/Senior Care Services are designed to serve both adult and geriatric patients.

5 Funding our Community Health Needs Solving Priority Health Issues Behavioral Health Project s Goals and Expected Outcomes Focus will be on the expansion of a comprehensive community Behavioral Health Patient Care Program: Inpatient Clinic Outpatient Clinic Crisis Medical Stabilization Planning and developing co-location or integration of behavioral health services with primary care. The proposal includes a ten (10) Bed Inpatient Adult/Geri Psych unit and Intensive Out-Patient Care facility. Plans include expanding our ongoing partnership with Laurelwood to provide oversight of our program and utilizing tele-psychiatry to provide the day-to-day mental health services of our Inpatient Program.

6 Funding our Community Health Needs Solving Priority Health Issues Additional Primary Care Providers and Resources Vision Hospital recognizes that additional primary care physicians are needed in the County to provide appropriate access points to care. Hospital Strategy Hospital is working to add new primary services in 2018 and beyond through a partnership with College of Medicine to provide 3 rd and 4 th year residents. With this partnership, along with a partnership with the College of Nursing, the hospital will be able to enhance the stability of primary care by encouraging patients to remain within the community and assure greater availability of access points. Non-physician office employees will be added to work with physicians and care for patients.

7 Funding our Community Health Needs Solving Priority Health Issues Cancer Treatment Options Vision The Hospital recognizes that Cancer is the second leading cause of death in the United States. Over half a million people die each year in the U.S. due to cancer. Some cancers may be prevented through screening and early detection. Hospital Strategy Hospital currently offers programs that provide screening and education for early cancer detection. Hospital has hired a Board Certified Oncologist and has opened an infusion center. Hospital will be adding a cancer specific diagnostic tool. Hospital has a goal of updating and enhancing the infusion clinic to improve patient care, access and increase home-town services. Non-physician employees will be needed to care for patients and perform diagnostic tests and treatments.

8 Funding our Community Health Needs Solving Priority Health Issues Cardiology Care Vision Coffee Regional Medical Center s Cath Lab and Cardiac Rehabilitation Program will be available in Douglas for our patients. Hospital Strategy A new Cardiac Catheterization Laboratory offers complete diagnostic cardiac services. It features state-of-the-art equipment to detect the presence of blockages or other evidence of heart disease to help determine needed treatment. Cardiac rehabilitation is the process by which individuals are restored through exercise to their optimal physical, medical and psychological status after a cardiac event. Highly trained physicians are needed to further expand cardiology services, as are cardiology technicians and caregivers. All construction projects will be competitively bid.

10 Funding Program with the USDA Rural Economic Development Loan Grant What does this program do? The Rural Economic Development Loan and Grant (REDLG) programs provide funding to rural projects through local utility organizations. Under the REDLG loan program, the USDA provides zero interest loans to local utilities which they, in turn, pass through to local businesses (ultimate recipient Hospital/Clinic) for projects that will create and retain employment in rural areas.

To receive funding under the REDLG program (which will be forwarded to selected eligible projects), an entity must be described as: Nonprofit utilities that are eligible to receive assistance from the Rural Development Electric or Telecommunication Programs

12 Funding Program with the USDA Rural Economic Development Loan Grant What is an eligible area? Intermediaries may use Rural Economic Development Loan & Grant (REDLG) funds to lend money for projects in rural areas or towns with a population of 50,000 or less. How much funding is available to intermediaries? Up to $2 million in loans may be requested Zero Percent Interest Up to 10 years to repay

The Rural Economic Community Facilities Loan/Grant (CF) programs provide funding to rural projects direct by the USDA State office. Under the CF Loan/Grant program, the USDA provides low interest loans to local community for projects that will create and retain employment in rural areas.

May use Rural Economic Community Facilities Loan/Grant (CF) funds to lend money for projects in rural areas or towns with a population of 50,000 or less. How much funding is available to intermediaries? Up to $3 million in loans may be requested form the State Office (more from Federal Office-Washington) Low Interest Rates Up to 7-40 years to repay

16 Funding Case Studies Organization Chatuge Regional Hospital, a sister location of Union General Hospital, is located in the beautiful North Georgia Mountains on Highway 76 North in Hiawassee, Georgia. Vision Chatuge Regional Hospital recognizes that there is a desperate need for a Behavioral Health Patient Care Program in our community. Project s Goals and Expected Outcomes Focus will be on the expansion of a comprehensive community Behavioral Health Patient Care Program: Inpatient Clinic (10 Bed Adult/Geri Psych Unit) Exterior and interior renovations Total Project Cost $3M funded by the USDA REDLG $1.75M

Vision Emanuel Medical Center recognizes that there is a desperate need for a Behavioral Health Patient Care Program in our community. Project s Goals and Expected Outcomes Focus was to add a 10 Bed Adult Geri Psych Unit for the community Behavioral Health Patient Care Program: Inpatient Clinic (10 Bed Adult/Geri Psych Unit) Exterior and interior renovations Total Project Cost $1.8M funded by the USDA CF $1.45M
