4 Programs United States Department of Agriculture 17 Agencies Rural Development is one of those agencies Within RD there are 3 agencies Rural Business and Cooperative Service Rural Housing Service Rural Utility Service RD delivers over 40 programs to rural communities All programs fall within the administrative oversight of one the three agencies listed above Missouri RD has 110 staff All staff are career with exception of State Director Missouri RD has 25 offices; State Office and 24 Field Offices

This allows for program delivery in urban areas.

12 Programs Responding to Healthcare Distance Learning and Telemedicine (DLT) Grant Most state and local government entities Non-profits For profit business Consortia of eligible entities Acquisition of eligible capital assets, such as: Audio, video and interactive video equipment Terminal and data terminal equipment Computer hardware, network components and software Inside wiring and similar infrastructure that further DLT services Acquisition of instructional programming that is a capital asset Acquisition of technical assistance and instruction for using eligible equipment $265,386 Grant - Bates County Memorial Hospital to expand upon an existing interactive telemedicine solution between Bates County Memorial Hospital, a 60-bed hospital serving rural communities in Bates County, and healthcare specialists with St. Luke Health System in Kansas City, MO. Bates County $286,813 Grant - Lester E. Cox Medical Centers to expand a telehealth network to connect six rural clinics, six ambulance stations, and three county health departments in Southwest Missouri. $122,524 Grant Compass Health, Inc. to deploy high-definition videoteleconferencing equipment at 18 rural health clinics.

13 Programs Responding to Healthcare Business Programs: Business and Industry Guaranteed Loans For-profit business Nonprofits and cooperatives Federally recognized Tribes Public bodies Eligible uses include but are not limited to: Business conversion, enlargement, repair, modernization, or development Purchase and development of land, easements, rights-of-way, buildings, or facilities Purchase of equipment, leasehold improvements, machinery, supplies, or inventory Debt refinancing when new jobs will be created and other conditions are met Business and industrial acquisitions when the loan will keep the business from closing and/or save or create jobs Example: $10,000,000 to Hermann Area District Hospital for debt restructuring. Example: $400,000 to Sedalia Health Group, L.L.C. to provide permanent working capital and restructure a short-term loan to upgrade an assisted living facility and assist with reopening facility during the stabilization period.

14 Programs Responding to Healthcare Rural Business Development Grant (RBDG) Towns Communities State agencies Authorities Nonprofit Corporations Institutions of Higher Education Federally-recognized Tribes Rural Cooperatives Eligible uses: Training and technical assistance, such as project planning, business counseling/training, market research, feasibility studies, professional/technical reports, or product/service improvements Acquisition or development of land, easements, or rights of way; construction, conversion, renovation, of buildings, plants, machinery, equipment, access streets and roads, parking areas, utilities Community economic development $61,899 to Missouri State Fair Foundation for kitchen equipment updates to allow the facility to become a certified kitchen and bring local foods from the farm to the plate.

15 Programs Responding to Healthcare Rural Economic Development Loans and Grants (REDLG) Electric and telephone cooperatives Eligible uses: Facilities and equipment for medical care to rural residents Community development assistance to nonprofits and public bodies (particularly job creation or enhancement) Facilities and equipment for education and training for rural residents to facilitate economic development $540,000 to White River Valley Electric Cooperative for Healthy Community, Inc., d/b/a Jordan Valley Community Health Center to renovate/expand a facility and purchase equipment to provide medical services in a persistent poverty area. $1,000,000 to White River Valley Electric Cooperative, Inc. to provide a loan to assist the Cox Medical Center Branson in the creation of a new Childcare Learning Center Facility.

16 Programs Responding to Healthcare Community Facility Direct and Guaranteed Loans and Grants Public bodies Community-based non-profit corporations Funds can be used to purchase, construct, and/or improve essential community facilities, purchase equipment and pay related project expenses: Health care facilities such as hospitals, medical clinics, dental clinics, nursing homes or assisted living facilities Community support services such as child care centers, community centers, fairgrounds or transitional housing Public safety services such as fire departments, police stations, prisons, police vehicles, fire trucks, public works vehicles or equipment Utility services such as telemedicine or distance learning equipment Local food systems such as community gardens, food pantries, community kitchens, food banks, food hubs or greenhouses $18,162,000 Loan to CMH Properties to purchase a Medical Office Building housing an Ambulatory Surgery Center, Endoscopy services, Outpatient Imaging Center, Women s Center, consolidated Information Systems and Physician's Offices. $62,600 Grant to Health Care Coalition of Lafayette County to purchase new telemedicine equipment for a Federally Qualified Health Care Clinic in Carrollton, MO. $987,750 Loan to Pathways Psychiatric Hospital, Inc. to make improvements and add an additional 14 beds to the current Royal Oaks Hospital located in Windsor, MO $94,100 Loan and $15,900 Grant to Steelville Ambulance District Assistance Corporation to purchase a new 2016 model ambulance and equipment.
