Reduces the financial risk to the clinic by coordinating case management activities that ensure appropriate treatment planning and potential patient referral. Provides individual patient counseling or education following Health Center protocols and coordinates patient s needs with the medical care provider. Assist in developing and maintain community interest, input and support. Provides health care programs, classes and screenings. Either in a supervisory capacity or directly in a complex situation, performs a variety of services including advising on social problems, arranging for discharge, postoperative care, and financial assistance to patients and their families, and alleviation of anxieties or fears concerning disabilities, illnesses, or an uncertain future. Performs marital and family counseling and group psychotherapy. May prepare a complete case history to provide information on a patient s personality and behavior as required for diagnosis and treatment. Master s degree in social work is required. The Licensed Clinical Social Worker has overall responsibilities for providing psychotherapy services including assessment and treatment plan. Certified Nurse Midwife Certified Nurse Midwife is responsible for perinatal care for patients in an office setting for scheduled office visits and in the hospital setting as indicated by the patient s condition. Practice will encompass all aspects of perinatal and gynecology care, co-ordination and communication with other providers such as dietitian, CPSP- LCSW and nursing staff.
