3 In 2014, the Mission and Ministry Fund of Catholic Health s awarded more than $8.6 million to 40 groups and organizations. These grants support the planning, development, and implementation of initiatives to promote healthy communities and prevent violence across the country and around the world. Catholic Health s founding congregations conceived of the Mission and Ministry Fund as a way for the organization to live out its commitment to creating healthy communities. Since its inception in 1996, the fund has awarded more than $55 million to 409 programs across the globe, with nearly $10 million specifically allocated to violence prevention programs. Testifying to CHI s innovative spirit and continued dedication to building healthy and safe communities, this year s grants together constitute the largest award in the fund s history. Many of the grants address priority community health needs identified by new Community Health Needs Assessments, which the Patient Protection and Affordable Care Act requires tax-exempt hospitals to conduct every three years. Some of the most critical needs identified by the assessments include effective care transitions for vulnerable patients; and needs for more robust mental and behavioral health care and services. Mission and Ministry Fund grants support innovative projects designed to improve the health of a community. Catholic Health s participating congregations, affiliated facilities and strategic partner organizations are welcome to submit applications. Supported initiatives meet identified needs, invite collaboration with community partners, and are replicable in other communities. For more information, visit

4 DOMESTIC PROJECT GRANTS ALEGENT CREIGHTON HEALTH Omaha, Nebraska Telepsychiatry for Rural Communities $455,251 over two years In the least populated parts of the country, mental illness is particularly difficult to treat. Limited resources and low mental health literacy often make it necessary for rural patients to travel long distances to access care, or to go without care. In Community Health Needs Assessments from communities supported by Alegent Creighton Health (ACH), mental health issues were cited as top concerns, particularly in areas with vulnerable and underserved rural populations. ACH plans to work with local public health departments, community mental health agencies, individual hospitals, and local community leaders to bring an innovative telepsychiatry program to rural populations in Nebraska and Iowa. ACH has already successfully implemented telepsychiatry programs in Columbus and Omaha, NE. The new program will focus specifically on the health and well-being of rural individuals and families who deal with mental illness and substance abuse. The primary goals of the project are to expand the number of direct care resources available to rural patients and to provide rural primary care providers with increased consultation services. ACH plans to use measures outlined by the American Telemedicine Association to measure progress. Sheree Keely CENTURA HEALTH AT HOME Denver, Colorado Healthy Living Colorado $1,320,554 over three years The future of health care in the United States is not certain, but Centura Health At Home (CHAH) has for years been developing programs that blend new technology with increased patient involvement and self-management. Centura seeks to address growing concerns about cost and quality of care through investments in telehealth, education and programs such

5 as ReConnect, which trains care transition coaches to help patients manage chronic conditions and navigate the changing health care climate. The success of CHAH s programs has created a need for an overarching system of coordinated care that links the practical advancements of telehealth with the human face of ReConnect. By expanding and linking these programs, Centura will improve the quality, consistency, availability and affordability of health care to communities throughout Colorado. In keeping with CHAH s ultimate vision of health care as evolving from centers of care (e.g., individual hospitals) to complete systems of care, the project will involve collaboration with numerous hospitals and community agencies throughout the state. CHAH hopes that successful implementation of Healthy Living Colorado will result in lower medical costs; improved health outcomes brought on by greater access to primary care; and increased adoption of patient self-management practices and use of non-medical social services. CHAH will conduct pre- and post-tests to measure patient understanding and building of new skills as the program progresses. Drew Weilage drewweilage@centura.org FAITHFUL FOOLS STREET MINISTRY Franciscan Sisters of Little Falls San Francisco, California Institute for Street Level Learning $183,900 over three years Since 1998, Faithful Fools Street Ministry has been fighting poverty and injustice in San Francisco and around the world through advocacy, community mediation and arts programs and educational street retreats. In recent years, the Faithful Fools have made education a more central goal of their work. The internships, fellowships, educational programs and research conducted by their Institute for Street Level Learning have helped create an environment that fosters reflection and a lifelong commitment to social justice.

6 With input from faculty and administrators at seminaries and schools with undergraduate and graduate programs in public policy, health and other disciplines, the Faithful Fools leadership realized that the Institute for Street Level Learning could provide the kind of supportive, structured internship that will benefit students enrolled in these programs. The formalization of the internship program will capitalize on the interdisciplinary, interinstitutional learning experience that the Institute for Street Level Learning can offer, which will help educate a new generation of advocates. The Faithful Fools hope to support and supervise three interns per semester. They will work with students undergraduate and graduate institutions on an individual basis to determine assessment methods and educational credit. Sarah Sam Dennison sam.denison.01@gmail.com FRANCISCAN FOUNDATION Tacoma, Washington Community Building Mental Health $182,000 over three years The Plateau community of Tacoma has one of the highest rates of frequent mental distress in the area. In the community s 2012 Healthy Youth Survey, more than 16% of area teens reported having seriously considered suicide in the past year; 28% reported experiencing serious depression. While the community is clearly struggling, Washington s mental health care system is struggling, too. With only 8.3 psychiatric beds per 100,000 residents the third lowest rate in the entire country the state lacks resources to address the complex issues of the Plateau community, whose relative isolation makes access to the state s limited mental health services even more difficult to obtain. The Franciscan Foundation will partner with Enumclaw Regional Healthcare Foundation, the Rainier Foothills Community Health Network (RFCHN), and the RFCHN s Mental Health Task Force to improve the overall mental and emotional health of the community. Their approach will include increasing access to local treatment and services, building mental health literacy and skills among those who work with individuals who experience mental health problems, and increasing the broader community s awareness of mental health problems and resources. Specific goals include decreasing the need for psychiatric hospitalizations among Plateau residents as well as the number of emergency department patients who present with mental health problems. The coalition also hopes to see lower reported rates of suicidal thoughts and depression in the state s annual Healthy Youth Survey. Data from additional communitywide surveys, including questions about mental health, will supplement the information gathered from these metrics. Kathy Schmidt kathyschmidt@fhshealth.org GOOD SAMARITAN HOSPITAL FOUNDATION Cincinnati, Ohio HOPE Program $200,000 over two years Since 2010, Good Samaritan Hospital has seen an increase of nearly 60% in the number of opiate-addicted women seeking maternity care. The number of women served by the Helping Opiate-Addicted Women Evolve (HOPE) program, which provides enhanced prenatal care to pregnant women, has increased 113% since it began in These patients require specialized support before, during, and after pregnancy to address the many risks associated with addiction and mental illness, and the HOPE program needs additional resources to fully address those needs. During the past six years, the HOPE program has established a multidisciplinary care team and built strong, collaborative relationships with local organizations, including medicationassisted treatment programs, inpatient and outpatient treatment programs, and maternal and infant health clinics. The 2014 Mission and Ministry Fund grant will fund an expansion of HOPE s Postpartum Care Enhancement Project, which seeks to ensure the best possible outcomes for mothers and infants through increased compliance with postpartum and high-risk infant follow-up care. Additional funding will be used to create a licensed social worker assistant position on HOPE s staff. During the next two years, program goals include establishing an 80% participation rate in HOPE s prenatal health education program, increasing participants breastfeeding rates from 17% to 50%, and increasing postpartum visit compliance from 10% to 75%. Specific goals for HOPE s postpartum care enhancement program are to achieve 75% compliance with requirements for high-risk infant follow-up visits, and to increase participants compliance with recovery treatment from 60% to 70%. 7 KENTUCKYONE HEALTH Louisville, Kentucky Health Connections $1,535,561 over three years ST. VINCENT INFIRMARY Little Rock, Arkansas Health Connections $1,040,474 over three years Data from the Agency for Healthcare Research and Quality show that the sickest 5% of U.S. patients account for more than half of health care costs. But, when these patients are provided with comprehensive and coordinated services, health outcomes improve and costs decrease. One of the first organizations to practice this was the Camden (NJ) Coalition of Health Providers, which used insurance claim data to identify hot spots of highrisk, high-utilization patients. The coalition found two buildings in Camden where residents accounted for nearly $30 million in medical spending. These patients had difficulty accessing transportation, poor social relationships and/or ineffective primary care. By coordinating care and providing social services, the coalition improved the population s health status, reduced their hospital visits and cut costs in half. In CHI s ministry, preferential treatment of those who are poor and vulnerable is an obligation. KentuckyOne Health, St. Vincent Infirmary, and CHI s national office will collaborate on a model of care that will deliver compassionate, coordinated, comprehensive and cost-effective care to vulnerable populations of high-utilization patients at four CHI hospitals. Identified using the hot spotting technique, these 876 superusers of care generated costs of $56.3 million in The initiative will monitor the health of these patients and address concerns such as prevention and management of chronic disease and patient access to affordable care. The goal is to create a model of coordinated care delivery that will improve health outcomes, be sustained by cost savings, and be replicable across CHI. The initiative will closely monitor improvement in the patients health outcomes, improvement in patient perception of care coordination, and reduction in participants total emergency department and inpatient costs. Alice Bridges KentuckyOne Health alicebridges@kentuckyonehealth.org Phyllis Romine St. Vincent Infirmary pdromine@stvincenthealth.com MERCY FOUNDATION Roseburg, Oregon Rural Action Dental Health $174,802 over three years According to the 2012 Oregon Smiles Survey, children in the lowest income brackets suffer almost twice the rate of tooth decay as their higher-income counterparts, but are 30% less likely to have seen a dentist in the past year. In Douglas County, where 28.3% of children live in poverty, close to 80% of eighth grade students reported having cavities in Mercy Foundation and its community partners will seek to improve the overall dental health of students in Douglas County, and to ultimately shift community focus from intervention to prevention. The Dental Health currently provides direct access to dental services for 3,500 students in preschool to sixth grade. The grant will support expansion of the program s school-based oral health clinics and the installation of learning labs in area middle schools. Using evidence-based methods established by the American Dental Association, the Mercy Foundation hopes to see 10% improvement in the following areas: a reduction in the number of students who have dental caries and an increase in the number of students who receive preventive dental services, the number of students who receive dental sealants, and the number of connections to dental care and established dental homes. Lisa Platt and Trina Gwaltney lisaplatt@chiwest.com MERCY MEDICAL CENTER Des Moines, Iowa Childhood Obesity Prevention Program $172,779 over three years Childhood obesity is a national epidemic with long-term and wide-ranging consequences. Children who are obese are more likely to experience social stigmatization and struggle with low self-esteem. They are also at greater risk for developing serious health problems as adults, including cardiovascular disease, type 2 diabetes, and many types of cancer. An estimated onethird of Catholic elementary school students in Des Moines are overweight or obese; a 2012 needs assessment found achieving healthy weight to be a top priority within the community. Mercy Medical Center will work with Diocese of Des Moines Catholic Schools to implement the research-driven CATCH MISSION & MINISTRY 2014 GRANTS 5

8 curriculum in 10 elementary schools. CATCH originated as a university study in Texas in the late 1980s, and its evidencebased practices have spread across the country. CATCH takes a coordinated approach to preventing obesity, emphasizing proper nutrition, physical activity, and education. The Des Moines program will serve an estimated 1,275 students over the three-year grant period. Annual goals include increasing student activity levels by 20%, increasing healthy food choices by 10%, and increasing parental knowledge of their children s BMI levels by 75%. Baseline BMI levels will be established by nursing students from Mercy College of Health Sciences, and outcomes assessed in accordance with current CATCH practices. Lisa Pagel lpagel@mercydesmoines.org MERCY MEDICAL CENTER Des Moines, Iowa Mental Health Crisis Stabilization Center $221,187 over two years In Iowa s rural Appanoose County, residents are in dire need of mental health crisis intervention services. There are no immediate outpatient services available, and inpatient beds are almost impossible to access in times of crisis. Due to this lack of local resources, it is not unusual for a patient who experiences a mental health crisis to be transported to a facility 100 to 200 miles away, even when his or her condition is not severe enough to warrant hospitalization under normal circumstances. A community needs assessment identified expanding mental health services as Appanoose County s greatest need. Mercy Medical Center hopes to establish a mental health crisis stabilization home treatment center in Appanoose County to serve individuals in crisis who require round-the-clock observation and assistance, but do not require hospitalization. The average patient stay is expected to be five to seven days. The treatment center will provide treatment in the form of medication management, intensive group and individual therapy, and referral and follow-up services. Goals of the program include helping patients achieve a stable mental health state, reducing hospital admissions and criminal arrests of those who suffer from mental illness. The program hopes to serve 215 individuals per year. Ann Young ayoung@mercydesmoines.org NEBRASKA AND SOUTHWEST IOWA NETWORK Omaha, Nebraska Plan for Behavioral Health Services Across the Network $130,100 The 2013 Community Health Needs Assessments conducted in the CHI Nebraska and Southwest Iowa Network showed a widespread need for better access to mental health services. The Network outlined an initiative that would draw on the behavioral health strengths of several Network hospitals and the findings of the recent Robert Wood Johnson Foundation s Commission to Build a Healthier America report. The report emphasized the effect of social factors on the nation s health and called for a broadening of the mission and definition of health care beyond treating illness to helping people lead healthy lives. The Commission to Build a Healthier America report urged health care professionals to prioritize early childhood education and involved communities as essential components of any plan for improving community health. The Network s Plan for Behavioral Health Services will be a collaborative effort from the planning stages on, drawing on the expertise of all members of the Network, the Creighton University School of Medicine Department of Psychiatry, regional public health departments, community mental health agencies, community social service agencies and the Behavioral Health Education Center of Nebraska. The objective is to develop a comprehensive, community-centric plan to address the concerns of Network patients and families who experience the effects of mental illness; and, to increase the preventive outreach, educational efforts, and resources that support the resiliency of all community members. Sheree Keely shereekeely@alegent.org SAINT FRANCIS MEDICAL CENTER Grand Island, Nebraska Community Health Workers Intervention Program $324,856 over three years Grand Island is a community that struggles with poverty and limited access to health care. Minority residents comprise more than one-third of the population, and many do not speak English as their first language. Grand Island s privately funded free clinic has been unable to keep up with the growing health needs of the community, and many low-income residents must resort to costly emergency department treatment. 6 MISSION & MINISTRY 2014 GRANTS

9 To create a new health delivery network for low-income patients, Saint Francis Medical Center will partner with The Physician Network, the Third City Community Clinic, the Mid- Plains Behavioral Center and the Heartland Health Center (a new federal clinic soon to open in Grand Island). The partners hope to improve quality of life for low-income, high-risk patients by connecting them with community health workers and lay ambassadors who can provide support and advice. Key goals include improved patient access to health care services, improved health outcomes, and decreases in emergency room visits and hospitalizations of at least 15% and 25%, respectively. 10 INTERNATIONAL GRANTS SISTERS OF CHARITY OF NAZARETH Nazareth, Kentucky Nazareth Hospital in Mokama, India $151,641 over three years In Mokama, India, part of the Patna district, the number of individuals above 80 years old is increasing, and 30% of the elderly currently live below the poverty line. Providing care to these patients is difficult for a number of reasons. More than 80% of the elderly in India live in rural areas, making service delivery a challenge. The area also reports a high incidence of elder abuse cases: in the Patna district, 56% of the elderly suffer from family abuse. The Sisters of Charity of Nazareth hope to increase the health services available to all in Mokama and especially to elderly residents, with specific goals of reducing pain and suffering for palliative care patients and giving terminally ill individuals the opportunity to have a dignified death. Additional goals include a decrease in disease through increased awareness of healthy lifestyle practices, proper diet, and early detection and treatment. Patsy O Toole patsy@scnfamily.org The Sisters of Charity of Nazareth are refocusing efforts at Nazareth Hospital in Mokama to meet the specific health needs of the area s aging population. During the next three years, the hospital will specialize in palliative care, geriatric care and preventive care through a wellness clinic. Partners in this project include existing social welfare and referral services, as well as local schools, international faith-based organizations and other nongovernmental organizations.

11 ST. ANTHONY HEALTH FOUNDATION Lakewood, Colorado Alma Mater Hospital, Haiti: Developing a Sister Hospital Relationship $140,000 Alma Mater Hospital, Gros-Morne, Haiti, is the only 24-hour health facility for a community of 135,000 residents. The next-closest hospital is an hour away via dirt roads that are impassable in the rainy season. Gros-Morne has little clean water and no sewage facilities, reliable electricity or communication with the rest of Haiti. The area suffers from the same health conditions as the rest of Haiti: 15% of children die before age five and only 30% of children are vaccinated; preventable diseases are major causes of death; and maternal mortality is the highest in the Americas. During 2011, Alma Mater Hospital admitted 4,296 patients, of whom 87 died, for a 2% patient mortality rate. Most deaths were from diseases and conditions that should be easily curable. relationship between Alma Mater Hospital and St. Anthony Hospital, Lakewood, CO. The grant will support nurse training for maternal-neonatal health, integration of a new outpatient clinic, and implementation of infection control measures, as well as shipping costs and much-needed improvements to Alma Mater s septic system. Representatives of Alma Mater and St. Anthony are collaborating with the University of Colorado s Health Administration Department, the Catholic Health Association s Campaign for Rebirth and Renewal, Catholic Relief Services and Haiti s regional health network. Key goals this year include a finalized five-year business plan to guide fundraising efforts; continued development of a stronger, more trusting relationship between Alma Mater and St. Anthony staff; and creation of strategies that will generate improvements in Alma Mater s operations and patient outcomes. 12 VIOLENCE PREVENTION DESIGN & START-UP GRANTS YEAR ONE FLAGET MEMORIAL HOSPITAL Bardstown, Kentucky $15,600 Statistics about violence and abuse in Nelson County come from various sources and are often difficult to reconcile. While it s apparent that the area suffers from an unusually high rate of binge drinking and a high incidence of drug arrests, more information is needed before effective violence prevention programming can be designed for the area. In the first year of this initiative, Flaget Memorial Hospital will conduct research consisting of asset mapping and needs assessments in order to clearly identify the region s most vulnerable populations and most pervasive types of violence. Hospital leaders hope to work with a collaborative network of community representatives to design and implement a violence prevention program that addresses the root causes of violence in the area. Shane Fitzgerald shanefitzgerald@catholichealth.net JEWISH HOSPITAL SHELBYVILLE Shelbyville, Kentucky $15,750 Compared to the rest of Kentucky, Shelby County reports higher incidence of violent crime. Shelby County also ranks poorly on national benchmarks for health factors that may precipitate violence, such as excessive drinking and poor mental health. The shifting demographics of Shelby County which now has the highest percentage of Hispanic individuals in the state make a collaborative, community-wide approach to violence prevention an even more important goal. Jewish Hospital Shelbyville will create a community-wide coalition to address the community health needs of Shelby County. The coalition will include representatives from local law enforcement agencies, public schools, Centro Latino, Shelby

13 County Emergency Medical Services, and a number of violence prevention experts, among others. Holly Husband hollyhusband@kentuckyonehealth.org SAINT ELIZABETH FOUNDATION Lincoln, Nebraska $58,952 Until now, violence prevention efforts in Lincoln have been earnest but disjointed. The Saint Elizabeth Foundation seeks to unite all community members who have an active interest in the health and safety of Lincoln in an effort to increase the efficacy of the area s violence prevention efforts. To best evaluate the needs of Lincoln s residents, the Saint Elizabeth Foundation plans to work with previously-identified community leaders to recruit representatives from a crosssection of the area to participate in the violence prevention coalition. The coalition will subsequently conduct an assessment as a first step to identifying the community s most pervasive forms of violence. Libby Raetz or Jean Stilwell jstilwell@stez.org SAINT JOSEPH BEREA Berea, Kentucky $15,750 Madison County is one of the fastest growing counties in Kentucky, with a 17% population increase in the past decade. But, population growth has led to an increase in violent crime in the community. A Community Health Needs Assessment revealed an increase of nearly 20% in arrests from 2005 to To address the rise of violence in Madison County, Saint Joseph Berea will partner with a number of community stakeholders to create a violence prevention coalition. The group will identify a

14 priority area for violence prevention and create an action plan to reduce the rate of violence in the community. Katie Heckman SAINT JOSEPH MARTIN Martin, Kentucky $15,750 Floyd County is an isolated Appalachian community with little diversity and limited road access. In addition to high incidences of teen pregnancy, poor mental health, and children born to mothers who smoke during pregnancy, Floyd County has an alarmingly high suicide rate. Suicide is a leading cause of death. Saint Joseph Martin plans to gather together key community stakeholders to evaluate, discuss and reach a consensus on the most pressing violence-related issue currently faced by Floyd County residents. Once identified, this problem will guide the coalition s development of a comprehensive, community-wide violence prevention program. Neva Francis nevafrancis@catholichealth.net SAINT JOSEPH MOUNT STERLING Mount Sterling, Kentucky $16,200 A 2013 Community Health Needs Assessment of Montgomery County revealed a number of health concerns that could potentially lead to increased violence in the community. The indicators of greatest concern included rates of alcohol and drug abuse in the area, the number of children living in poverty, and the high teen birth rate. The complexity of the health and social issues Montgomery County residents face requires a nuanced and informed plan of action. Saint Joseph Mount Sterling will bring together a number of community leaders, organizations and experts to create a community-wide violence prevention coalition. The group will identify a priority area of violence. ST. ANTHONY HOSPITAL Pendleton, Oregon $15,000 Umatilla County is a mostly rural and agricultural county located in Eastern Oregon. Little research has been conducted into the root causes of violence in the community. A thorough measurement of the physical and social well-being of Umatilla residents will require the cooperation of numerous subsections of the community. To address the causes of violence in Umatilla County, St. Anthony Hospital first plans to partner with a variety of community stakeholders to create a collaborative violence prevention coalition. The group will gather information and identify a focus area for violence prevention. The coalition will then develop an action plan to reduce the rate of violence in the community. ANTHONY SUMMIT MEDICAL CENTER and CHI COLORADO FOUNDATION, dba SUMMIT MEDICAL CENTER HEALTH FOUNDATION Frisco, Colorado $84,200 In Summit County, CO, community leaders know that many types of violence occur and must be addressed. But, the community has not yet had the time or resources to identify what is the most prevalent form of violence in Summit County. St. Anthony will expand the existing Violence Prevention Committee in Summit County and facilitate the community in conducting a needs assessment. 16 ST. LUKE S SUGAR LAND HOSPITAL Sugar Land, Texas $14,000 Sugar Land is one of the fastest-growing communities in the greater Houston area. It has a relatively high per-capita income, many master-planned communities and abundant opportunities for education, recreation and fitness. While the community has many assets, more than 4,000 cases of sexual assault are reported each year in Fort Bend County, indicating a need for violence prevention. To best evaluate the needs of community residents, St. Luke s Sugar Land Hospital plans to recruit community leaders and experts to participate in its collaborative Violence Prevention Coalition. In its first year, the coalition will conduct a needs assessment as a first step to identifying the community s top violence priorities. Sharon Galloway and Katina Scott sgalloway@stlukeshealth.org YEAR TWO CHI COLORADO FOUNDATION / CENTURA HEALTH SOUTH STATE REGION Colorado Springs, Colorado $164,546 Last year, a CHI grant enabled the communities of Colorado Springs, Pueblo, and Cañon City to form three distinct Violence Prevention Coalitions, and to hire two Violence Prevention Regional Coordinators to manage their efforts. In the second year of the grant, the coalitions in each community will establish individual Violence Prevention Steering Committees to identify community-specific violence prevention focus areas and develop evidence-based approaches. LUKE S THE WOODLANDS HOSPITAL The Woodlands, Texas $15,550 In the greater Houston area, the most violent areas are often the poorest, with incidences of violent crime most often reported by residents of South Montgomery County and North Harris County. Troubling data from the Montgomery County district attorney s office shows that the community has seen a rise in aggravated assaults during the past five years, as well as an alarming increase in school violence, including the stabbing death of a student at an area high school. To best evaluate the violence prevention needs of community residents, St. Luke s The Woodlands Hospital leaders plan to recruit community leaders and experts to participate in a collaborative Violence Prevention Coalition. 17 DESIGN AND START-UP EXTENSIONS Several CHI organizations in North Dakota are in the final stages of the design and start-up of community violence prevention efforts. 18 VIOLENCE PREVENTION IMPLEMENTATION GRANTS FRANCISCAN FOUNDATION Tacoma, Washington $467,000 over three years After identifying youth violence as a top concern in many Washington communities, the Franciscan Foundation has devoted the past few years to implementing comprehensive youth violence prevention strategies in two communities: Federal Way and Key Peninsula. Using a combination of school-centered and community-based strategies has been essential to program success, as has the Franciscan Foundation s efforts to seek continuous feedback from community members. During the next three years, the Franciscan Youth Violence Prevention initiative will continue working in Federal Way and Key Peninsula to support the institutionalization of violence prevention programs in those communities. The initiative will create a detailed transition plan for local leadership and ownership as the foundation turns its efforts toward expanding the initiative to a community in Highline Medical Center s service area. 19 FRANCISCAN SISTERS OF LITTLE FALLS Little Falls, Minnesota $273,851 over three years In rural Morrison County, children and teenagers have few distractions outside of sports and school. Local agencies and community members cite drug and alcohol use and a high suicide rate as serious concerns. By developing year-round programming at the St. Francis Music Center, the Franciscan Sisters of Little Falls hope to provide area youth with a creative outlet. Two years of planning allowed the St. Francis Music Center to build strong relationships with local agencies that work with children in the social services and judicial systems. With the support of their community partners, the sisters were able to create URock Against Violence, a program to provide free music lessons to at-risk youth. Music campaign using print and social media, as well as music. The goal of the campaign will be to show teens they have somewhere to turn in times of crisis and to raise awareness of helpful resources. The goals of the program are to use music to improve the quality of life for residents of Morrison County, and to prevent violence by providing youth in the community with a constructive and powerful channel for their energy and emotion. 20 MEMORIAL HEALTH CARE SYSTEM Chattanooga, Tennessee $88,398 over three years In 2011, a survey to assess risky behaviors among youth revealed that teens in Hamilton County were more likely to answer yes to the question Have you been hit, slapped, or physically hurt by your boyfriend or girlfriend in the past 12 months? than teens in the rest of Tennessee and other parts of the country. This statistic raised the community s interest in creating an initiative to address teen dating violence in Hamilton County. During two planning years, the Memorial Foundation created a strong partnership with the University of Chattanooga and identified three Hamilton County high schools to participate in the project and adopt violence prevention programming. The foundation has hired and trained Champion advocates for each of the schools, completed baseline surveys, and pretested programming for the Champions. In the coming year, the Memorial Foundation plans to introduce Champions at each of the three schools and conduct monthly meetings with the Champions and Teen Advisory Boards to assess progress. The foundation also hopes to launch a social media marketing campaign to raise awareness of the project and to identify a fourth school where they can replicate the program. The final goal of the program is to achieve a 5% decrease in affirmative responses to the Youth Risk Behavior Survey question indicating dating violence by the year Elizabeth Walden & Howard Roddy Elizabeth_walden@memorial.org ST. FRANCIS HEALTHCARE CAMPUS Breckenridge, Minnesota $284,463 over three years St. Francis began developing the Family Footprints program in response to the relatively high incidence of child abuse and neglect reported in Richland County (North Dakota) and Wilkin County (Minnesota). The first Family Footprints coordinator was hired in 2011, and since then the program has been working steadily to educate families identified as high-risk, providing these parents with the parenting skills and knowledge of child development that can make a difference in their children s lives. In the coming years, the Family Footprints program hopes to connect with more families at the pregnancy stage and provide support to families who can benefit from education prior to giving birth. Although they receive a steady stream of referrals from physicians whose patients deliver at St. Francis, rarely do these doctors refer prenatal patients. The program will also continue to search for sustainable sources of funding. Family Footprints hopes to obtain sustainable funding for its programs by The program will also advocate for preventive visiting services so that, by 2020, the services provided though the program will be reimbursable. JOSEPH REGIONAL HEALTH CARE NETWORK Reading, Pennsylvania $250,000 over three years The Reading Youth Violence Prevention Program began in 2009 as a community planning and coalition-building process that engaged more than 150 individual and organizational stakeholders in a plan to prevent youth violence. This effort included programs like a Coaching Boys into Men summer basketball league, and the student-led Project Peace group formed in 2010 in the aftermath of a series of shootings. As St. Joseph continues to expand existing violence prevention programming in Reading and introduce new initiatives, the organization will seek to maintain the community spirit that has been so integral to its success. This year, the coalition plans to re-engage its stakeholders in updating the Reading Youth Violence Prevention Program s infrastructure and longterm action plan. In addition, the coalition hopes to create programming to encourage increased interaction between city and county youth. The long-term goal of the Reading Youth Violence Prevention Project is to prevent and reduce incidents of youth-on-youth violence in Reading by 20% by Laura Welliver laurawelliver@catholichealth.net

JOSEPH S AREA HEALTH SERVICES Park Rapids, MN $175,454 over three years Upon learning that 56% of sixth graders in Hubbard County reported having been teased, bullied, or excluded at school every day to once or twice a week in 2007, St. Joseph s Area Health Services facilitated the development of a plan to reduce relational aggression in the lives of area youth. Together with their collaborators, St. Joseph s launched a three-pronged effort to prevent youth bullying, focusing on: the importance of fatherhood, mentoring, and family resources and activities. Saint Joseph s has since helped to develop or expand multiple programs to address each of these deterrents to aggression. St. Joseph s will continue work to decrease bullying reported among sixth graders in Hubbard County by 20% by In addition, the organization will work with community partners to develop means by which their work can be supported and maintained beyond the three-year grant period. MARY S COMMUNITY HOSPITAL Nebraska City, Nebraska $235,488 over three years A 2011 Mission and Ministry Fund grant enabled United Against Violence in Southeast Nebraska (UAV) to launch violence prevention programming in many area schools. These initiatives range from specific curriculum, such as Second Step (a curriculum to foster empathy in elementary students), to broader cultural changes epitomized by the ubiquity of UAV slogans such as Anger is Normal. Violence is Not. a phrase that has been adopted by local pastors as well as displayed on the walls of local bars. : By 2020, UAV hopes to reduce reported incidents of youth violence (including bullying, assault, and physical aggression) in Otoe County by 20%. GABRIEL S HOSPITAL Little Falls, MN $120,776 over three years Saint Gabriel s Hospital saw an opportunity to improve the way that Morrison County addresses issues of bullying and domestic violence. They successfully formed a collaborative Violence Prevention (VPI) with help from community partners and engaged in numerous projects to increase awareness of partner violence. During the next three years, the VPI will continue to support existing measures to prevent domestic violence, such as the establishment of an emergency fund available to victims of partner violence; and, giving presentations to schools, prenatal classes and the Boys and Girls Club about domestic violence and the resources available to victims. The VPI also plans to conduct another broad media campaign to reach the widest possible community audience. VPI would like to achieve a 10% reduction in domestic assaults by the end of the three-year grant period.
