3 Albury Wodonga Health Albury & Wodonga Campuses Affiliated with University of New South Wales About Albury Wodonga Health Albury Wodonga Health (AWH) is the regional referral hospital for North East Victoria and Southern New South Wales, comprising 280 beds over two acute sites 10 kilometres apart. AWH provides a broad range of clinical services with local resident specialists supported by Accredited Registrars in all specialties. Services are provided over two campuses. Our Health Service includes a 9 bed Intensive Care Unit, 6 Operating Theatres and two Emergency Departments with over 60,000 attendances per annum. Inpatient services include general medicine, ICU, paediatrics, obstetrics & gynaecology, internal medicine, general surgery, orthopaedics, urology, ENT, ophthalmology, psychiatry, medical oncology, geriatrics and rehabilitation. There is a large group of Senior Medical Staff supporting these services, providing both elective and emergency care. The specialists are supported by a full range of accredited registrar positions in general medicine, paediatrics, general surgery, orthopaedics, anaesthetics, urology, and medical oncology. On site pathology services and Imaging services (including CT and MRI) are available 24 hours. Graduate Medical Education There is a structured weekly education program delivered by consultant medical staff for Interns and HMOs at both Albury and Wodonga Hospitals. In addition there is an active education program which includes a weekly Emergency education session at both sites and a number of departmental education sessions, X-ray meetings and case presentations across AWH. Facilities and Amenities Albury Wodonga is 300 km from Melbourne and has a population of 100,000. It is serviced by multiple daily plane and train services, to either Melbourne or Sydney. First class sporting and recreational facilities are available in the district include skiing (at Falls Creek, Mt. Buffalo or Mt. Hotham), fishing, tennis, winery, squash, boating, golf, swimming and water skiing. The arts are well catered for with live theatre, cinema and frequent musical events. A key feature of our hospital is the active education program for HMOs and Registrars, including Clinical Review meetings, in addition to the formal intern HMO education program. The Alfred has over 398 adult inpatient beds including 45 intensive care beds with over 300,000 inpatient visits and over 94,983 emergency department attendances annually. Postgraduate medical education and training at Alfred Health is an integral part of the hospital s operations and philosophy. It is recognised that there is an inter dependence between the provision of high quality healthcare and the education of health professionals. Alfred Health supports postgraduate medical education by providing a wide range of postgraduate educational activities together with a strong commitment by the senior medical staff in the provision of appropriate clinical experience and education support. Senior medical staff foster an environment of enquiry and learning. The Medical Education Unit (MEU) supports the development, implementation and evaluation of training and education programs and organisational orientation programs for junior doctors. Education programs for PGY1 and PGY2s are mapped to the Australian Curriculum framework. The Manager of Learning & Development works closely with the Director of Clinical Training, Supervisor of Intern Training, Supervisor of Physician Training, and Supervisors of SET training programs in the development and implementation of relevant programs. Our Supervisors are dedicated to supporting our junior medical staff in their chosen career pathway, and their clinical and professional development. Our Medical Education programs consist of mandatory and recommended components and are delivered in different ways, varying from face to face programs, to workshops and online e- learning modules. The Learning Exchange website, LEX facilitates access to these training programs. In developing and implementing educational programs the Postgraduate Medical Education Unit regularly seeks feedback from junior medical staff to ensure continuous improvement and that programs are tailored to fit their needs. 7 Austin Health Austin Hospital, Heidelberg Repatriation Hospital, and Royal Talbot Hospital Affiliated with University of Melbourne About Austin Health Austin Health is the major provider of tertiary health services, health professional education and research in the North East of Melbourne. Austin Health is world renowned for its research and specialist work in many fields of medicine and provides state wide services in liver transplantation, spinal cord injuries, respiratory support services, toxicology and paediatric mental health. We are also the centre for veteran mental health services. Austin Health incorporates the Austin Hospital, Heidelberg Repatriation Hospital (HRH) and the Royal Talbot Rehabilitation Centre (RTRC) and is affiliated with eight universities, specifically the University of Melbourne, Medical School for undergraduate and postgraduate medical education. Originally built in 1941, the hospital became part of Austin Health in Today the Heidelberg Repatriation Hospital continues to treat veterans and war widows and the wider community with day surgery, palliative care, mental health services, aged care, and a range of outpatient services. With the opening of The Surgery Centre in 2013 we now have eight operating theatre suites at the Heidelberg Repatriation Hospital; it now fills a substantial elective surgery role. Royal Talbot Rehabilitation Hospital The Royal Talbot Rehabilitation Centre is a specialist provider of intensive rehabilitation programs, providing a comprehensive and coordinated range of medical, nursing, therapy and support services to people with a wide range of disabilities. Areas of specialty include acquired brain injury rehabilitation, amputee rehabilitation, neurological rehabilitation, spinal cord injury rehabilitation, orthopaedic rehabilitation and orthotic and prosthetic services. Services are provided on an inpatient and day patient basis. Austin LifeSciences Austin LifeSciences brings together internationally renowned research institutes and over 800 Austin Health researchers, with a focus on cancer, diabetes, respiratory disease, liver disease, heart disease, stroke, epilepsy and psychiatry. 8 General Information Conditions of Employment Conditions are as per the Victorian Public Health Sector (AMA Victoria) Doctors in Training (Single Interest Employers) Enterprise Agreement Post Graduate Medical Education There is a dedicated Clinical Education Unit at Austin Health which provides support and innovative training opportunities in a safe learning environment for doctors in training. These activities include orientation, mentoring, vocational guidance, feedback, assessment and professional development activities. Led by the Director, Clinical Education Unit and Clinical Lead, Medical education, the CEU is well supported by both clinical and non-clinical staff including a Director of Clinical Training, Supervisor of Intern Training, Surgical Supervisor of Training, Directors of Physician Training, GP Liaison Officer, Medical Clinical Educator, Medical Education Officer and Projects Coordinators. Orientation There is a 4 day comprehensive orientation program for interns at Austin health that includes a full day of shadowing with the former intern and an interactive clinical skills workshop. At Austin Health, we provide an Intern Liaison Officer for the first week of the new intern year; this role supports the interns after hours and over week end duties to ensure a supported and safe environment for all interns. For new HMOs and Registrars there is a half day orientation at both hospitals that delivers key organisational information on rostering, payroll, credentialing activities and education and support programs. Attendance at this session is required for all new DiT s. Unit Handbooks Comprehensive handbooks are available for each unit. These are designed to support on the job orientation provided by the units and to assist with the overall transition from one unit to another. Feedback, Assessment and Rotation Evaluation Mid term and end of term feedback for interns is provided both verbally and in writing by the Registrar and/or the Unit Consultant in line with the Performance Assessment and Feedback Guidelines for Junior Doctors. In addition to receiving feedback, all interns and DiT s are provided the opportunity to provide feedback about their experience in the unit. This information is de-identified for feedback to Heads of Unit annually. Rosters Yearly rotations are available and distributed prior to commencement. These are set after careful consideration of training requirements, past rotations, special interests and requests. Formal Training Programs Austin Health has in place formal training programs for interns, HMO2 s and HMO3 s. These programs provide a structured and supportive environment for learning, and are evidence of the importance placed on continuing education by Austin Health. Surgical Training Austin Health was the first hospital in Melbourne to run a series of hands on weekend workshops on Basic Surgical Skills for interns and basic and advanced trainees. The skills taught include open laparoscopy techniques, intra corporal and extra corporal knot tying and hand sewn end to end anastomosis. The workshops provide the opportunity for immediate feedback from experienced consultants. We provided training in interview techniques and have a high success rate of entry into Surgical Training Programs. General Training Austin Health has a proactive training program that is facilitated by our GP Liaison Officer and staff from the Clinical Education Unit. Included in this program is a series of workshop activities that take place throughout the year. The program is well supported by senior physicians in each practice. Critical Care Training In addition to HMO3 Critical Care Training programs, it is likely that Austin Health will also be offering Critical Care (ICU) Training at HMO2 level in Physician Training The Central North West Basic Physician Training Consortium incorporates Austin Health, Northern Health, Bendigo Health, and Wimmera Healthcare Group (Horsham Hospital). The Consortium is renowned for its physician training program and has an excellent pass rate in the FRACP Written and Clinical Exams. The Consortium runs a mock written exam in early February which emulates true exam conditions. A feedback session gives the opportunity for trainees to discuss the questions and answers. Directors of Physician Training and Senior Medical Registrars at both Austin Health and Northern Health organise an extensive series of topic tutorials, long and short case clinical tutorials and arrange Saturday morning practice clinical exams to adequately prepare trainees for the clinical exam. The practice clinical exams are held at Austin Health, Northern Health and Bendigo Health. A Professional Development program is available for Physician Trainees through the Consortium and covers topics such as Feedback, Teaching styles, Mentoring, Managing Conflict and Team building skills. ( computermatching.pmcv.com.au) Advanced Cardiac Life Support Course Austin Health Critical Care Staff run various courses throughout the year to improve doctors in training confidence with handling cardiac arrests. Annual Basic Life Support (BLS) credentialing is mandatory. Career Counselling There are many opportunities throughout the year to get advice from the staff of the Clinical Education Unit and the Medical Workforce Unit on your career prospects. Mentor and Peer Support Programs Austin Health is serious about nurturing a positive and professional culture for our medical workforce. 9 Elizabeth Austin Registrar The Elizabeth Austin Registrar is another resource for junior medical staff at Austin health. The Elizabeth Austin Registrar is available to provide support and advice relating to clinical situations, and also participates in delivery of education sessions for staff. International Medical Graduate (IMG) Austin Health has a supportive and formalised training program for International Medical Graduates. Monthly education sessions are conducted covering a broad range of clinical and non clinical topics. In addition to the monthly sessions, the Medical Clinical Educator conducts an annual training day on site. A dedicated Medical Clinical Educator supports the IMG training program. 10 Bairnsdale Regional Health Service About Bairnsdale Regional Health Service In 2018 Bairnsdale Regional Health Service will facilitate the placement of 2 interns in the region for the full year of internship. These placements combine with three East Gippsland Community based and five Eastern Health rotational placements comprising the intern workforce at BRHS. These rural pathway placements represent an opportunity to experience rural medicine, working alongside general practitioners, visiting and staff specialists within a community focused hospital. In Bairnsdale, procedural general practitioners provide all Anaesthetic and Obstetric services and admit medical patients under their care. There is a strong Medical Department over 3 wards, and Medical patients are managed by both GP VMO s, as well as on staff Physicians. From highly acuity medical patients, to our sub-acute rehabilitation patients, you will experience a wide variety of cases and presentations in your Medical rotation at Bairnsdale! Our Emergency Department draws from a large geographic catchment pool, and has over 20,000 presentations per year. Our Emergency Department is staffed by Senior Medical staff, and sees a broad range of presentations each day. You will also have the opportunity to see stabilisation and retrieval medicine in action, with BRHS being a heavy user of ARV. Our Sub-Acute/Geriatric Evaluation Medicine ward is staffed by a Geriatrician and relieved by a Rehabilitation Physician. Our Surgical Department has two Theatres which both run most days, with both our staff Surgeons and visiting specialist lists. Our on-site Consulting Suites have over 50 visiting specialists, who in addition to consulting have theatre lists and run education sessions for staff. 18 East Gippsland Community Based Internship Program About the East Gippsland CBI Program The East Gippsland Community Based Internship (EGCBI) program is a cooperative collaboration between The Central Gippsland Health Service, the Bairnsdale Regional Health Service, the Macleod St Medical Centre (Bairnsdale), Clocktower Medical Centre (Sale) and additional key community sites across the region. In 2017, we will facilitate the placement of five interns in the region for their full year of internship. This will involve three interns placed in the Bairnsdale region and two interns placed in the Sale region, but with an integrated and rurally oriented education program and a Director of Clinical Training to coordinate and oversee the training. Core rotations (surgery, emergency medicine, medicine) will be completed in either of the community focused hospitals and the remainder of the time spent in an integrated term in community sites. A community based internship program creates a rural pathway for intern training, which provides competitive career choices compared with metropolitan hospital based internships. Interns experience working in hospitals in which both specialists and general practitioners provide services, and in the community to increase the community responsiveness of the healthcare workforce and competencies in community oriented care. For further information go to Central Gippsland Health Service (CGHS) Central Gippsland Health Service is located in south east Victoria and is the major provider of health and community services in Wellington Shire. The total population of the Shire is approximately 42,000, making it the third largest local government area by size in Victoria. The main hospital site is located in Sale which is where prevocational medical training is situated. The CGHS Acute Care Division provides acute care as part of an integrated service with 97 acute beds across three campuses (Sale, Maffra and Heyfield), 209 nursing home and hostel beds and 98 independent living units. For further information go to Clocktower Medical Centre, Sale Clocktower Medical Centre has eight GPs and three practice nurses on duty. A number of the GPs provide anaesthetic and obstetric services and admit patients to the hospital GP ward. The practice is located in a large, modern, purpose built medical centre, centrally situated in Sale. There are eleven consulting rooms, a large nursing complex (including a casualty type area), an audio room and one procedure room. Gippsland Pathology has a collection centre on site, and around 750 patients are seen each week. The Centre is accredited for Registrar, medical student and PGPPP training. Tutorials are held regularly and usually rotate through the partners. Medical education is encouraged through educational meetings held weekly at the Hospital and many other local meetings including satellite broadcasts. For further information go to McLeod St Medical Centre, Bairnsdale McLeod St Medical Centre has 13 GPs, practice manager, two fulltime nurses, two part-time nurses, and seven reception and account staff. The GPs admit and manage medical patients, and provide obstetric and anaesthetic services for the hospital. The Centre is housed in a purpose built facility, consisting of a spacious waiting area, twelve consulting rooms, two procedure rooms, three nursing cubicles, an ECG room, modern kitchen/ dining area and a large meeting/library room. A pathology provider is on site. The Centre is accredited for Registrar, medical student and PGPPP training. Supervisors hold regular formal teaching sessions. 19 The diagram below summarises the structure of these rotations in both the Bairnsdale and Sale regions. The Community Based Interns have a 3hr evening face-toface session each month, which comprise hands on learning activities in the Skills and Simulation Laboratory. This allows for the enhancement of skill acquisition, and also provides a time for case presentations and catch-up/debrief time spent with the other Interns. This is in addition to the weekly educational sessions each Intern has in their health service or medical clinic. One-on-one teaching on the run moments and usual supervisory review are in addition to this dedicated off-line time. The Community Practice rotation is generally designed to extend over 20 weeks rather than the traditional 10 week rotation. A primary purpose of this extended rotation is to allow you to become familiar with the interface between primary and secondary care. In Bairnsdale, general practitioners provide all anaesthetic and obstetric services and admit medical patients under their care. In Sale, although there are more resident specialist medical practitioners, some general practitioners provide anaesthetic and obstetric services and there is a separate ward for patients who are under the care of their general practitioner. It is proposed that during the 20 week community experience, you will spend approximately half of your time in the general practice undertaking parallel consulting with a number of the general practitioners and the remaining time rotating through various sites and experiences (Aged care, Aboriginal health, anaesthetics, obstetrics etc) with your GP supervisor and other visiting specialists. 22 Echuca Regional Health About Echuca Regional Health Echuca Regional Health (ERH) is a newly renovated and expanded accredited public health facility which has been training medical students, junior medical staff and international medical graduates for over 20 years. A well-established and ongoing subregional training facility for rotating PGY1-3s from both the Austin and Bendigo, ERH established its own intern training program in 2015 (see below) with 100% of its graduates obtaining General Registration. Post internship training opportunities have also expanded and now include accredited medical registrar positions in both Medicine and Rehabilitation/Aged Care/Palliative Care. Rotating ACEM and SET registrars also train in the facility. The expanded hospital has 30 surgical, 30 medical, 6 high dependency and 8 maternity/paediatric acute inpatient beds. The 24 bed Rehabilitation Unit provides acute rehabilitation, geriatric evaluation and management, palliative care and transitional care. Telemedicine and teleconferencing is available and includes ViTCCU rounds with emergency/intensive care specialists daily through this video link. Community GPs and local primary care practices have won multiple awards for the teaching they provide to medical students and interns. The Education Centre has multiple lecture halls, dedicated computer room and simulation lab for medical, nursing and allied health trainees and practitioners. 26 Grampians Rural Community Intern Training Program About Grampians RCIT Program The Grampians Medical Training program is a partnership between East Grampians Health Service (Ararat), St John of God Ballarat Hospital and Maryborough District Health Service. It is a community based intern program where the intern can gain a relevant and thoroughly hands-on experience in the roles and procedures of the rural doctor. The Grampians Medical Training program provides the opportunity for career progression from undergraduate to general practitioner or specialist registrar. The program is designed to create a strong basis for a rural medical career whilst also developing skills transferrable to metropolitan practice. This is an innovative model which provides participants exposure to both public and private health services and regional and rural locations. 36 Murray to the Mountains (M2M) About Murray to the Mountains Intern Program The M2M program is a Rural Community Intern Training program providing a unique, hands-on training experience in general practice, small and large regional health services. The M2M program has been developed by a partnership between Alpine Health, Cobram District Health, Numurkah District Health Service and Yarrawonga Health. The M2M placement structure provides an outstanding experience in both rural and regional hospitals in some of Victoria s most beautiful locations such as Cobram, Mt Beauty, Falls Creek, Yarrawonga, Benalla, Kyabram, Mansfield and Mt Buller. The core medical, surgical and emergency medicine rotations are provided at Northeast Health Wangaratta or Albury Wodonga Health (Wodonga & Albury Campus). The aim of the M2M experience is for the interns to: maximize their exposure to generalist medicine; experience high levels of hands-on clinical experience & training experience medicine in a variety of settings ambulatory, hospital and residential care; have core rotations of 10 weeks in surgery, medicine and emergency medicine in the Hume region; be the patient s first contact and follow the patient journey; have the opportunity to work in rurally based procedural general practice and small rural teaching services for 20 weeks; participate in parallel consulting; be exposed to sports medicine through working in the ski fields or water sports along the Murray River; access a high quality education program that includes grand rounds, clinical review meetings, medical meetings geriatric & paediatric education sessions and skills workshops; be provided with opportunities for procedural based career pathways at PGY 2, 3 & 4 level; be supported in their career aspirations. 37 Description of Hospitals Moira, Alpine, Benalla, Mansfield and Campaspe Shires are in north east Victoria, about 200 and 300 kilometres from Melbourne. The attraction of the Murray River as a popular holiday resort makes tourism an important industry and increases population markedly during holiday times. The region is a major fruit growing, dairying and beef district. Alpine and Mansfield Shires are to the southeast of Victoria. They extend from numerous valleys to the mountains, snowfields and national parks. These regions include the Mount Hotham, Falls Creek and Mt Bulla ski resorts. Yarrawonga Health Yarrawonga is located in the north of the Moira Shire on the Murray River. Mulwala is a small town just across the Murray River from Yarrawonga, which looks to Yarrawonga for general practice and hospital services. Yarrawonga Health complex comprises acute and subacute facilities. There are 19 acute medical/surgical/obstetrics beds plus dialysis chairs and day procedure beds. There are delivery rooms, operating theatre facilities and emergency department staffed by local GP s. Other services include medical imaging and a range of visiting specialists including Gynaecology, Orthopaedic Surgery, General Surgery, Paediatrics, Urology, Geriatrics and Nephrology. There are three residential aged care facilities: Karana, with 30 high care beds; Warrina Hostel with 28 individual low care units, and Allawah Special Care Hostel with 30 dementia specific beds. Yarrawonga Community Health Centre is located across the road from the acute facility. Yarrawonga Denis Medical Group, one of the two general practices in Yarrawonga, has 5 full-time GPs, 2 part-time GPs and 2 full-time Registrars, Practice Nurses and Allied Health Professionals, with a mixture of skills including anaesthetics, obstetrics, women s health, minor surgery, emergency medicine and aged care. The Practice is an accredited teaching Practice in accordance with RACGP and ACRRM standards with 3 Supervising GPs, and also supervises Interns and Medical Students. Visiting Specialists include General Surgeon, Paediatrician, Psychologist, Nurse Practitioner, Mental Health Counsellor. With a Diabetic Educator and Dietitian they are able to run a Diabetes Clinic once a week.
