3 UNIVERSITY COLLEGE LONDON HOSPITALS NHS FOUNDATION TRUST 1. The Appointment In replacement of our current, retiring senior consultant we are looking to appoint a clinician to lead and develop our Pre-travel services at HTD. This is a unique, full-time post at the HTD within the Division of Infection at University College London Hospitals, providing a consultant led specialist service to clients and patients attending the travel clinic; it includes 40% WTE within the Clinical Research Department (at Associate Professor level, according to academic qualifications) at LSHTM to contribute to the delivery of the LSHTM teaching programme as well as the wider academic duties of the Department. The post-holder will hold a substantive employment contract at University College London Hospitals and an honorary associate professor contract at LSHTM. The service also provides advice and support to primary care and specialist clinical services on the management of their travellers with complex health needs and problems. The post-holder will run the travel medicine service mainly as outpatient based clinics at the HTD. They will oversee the delivery of travel medicine both through service and training nursing members of staff to deliver the same quality of service. The post requires the consultant to liaise with national and international travel medicine organisations to improve and enhance the delivery and quality of travel medicine. The consultant will, through research and training, implement best practices in the clinic and devise policies for protecting travellers locally, nationally and internationally. The post will have an integral role with the national Travel Health Network and Centre (NaTHNaC) in supporting them in training, policy development and strategic oversight. The consultant will be supported by senior nursing staff and will contribute to the teaching of travel medicine to undergraduates, specialist trainee doctors from within the Trust, London and South East England and participate in weekly postgraduate meetings of the Infection Division as well as audit, governance and training meetings. The post holder will be expected to take on a leadership role in leading and developing the travel medicine service. This leadership role will support the nursing and advisory activities currently undertaken at the HTD, will contribute to national and international training in travel medicine and continue to develop the international education and research role of the HTD and LSHTM. The post holder will have the opportunity to lead and support research linking with the NIHR, Bloomsbury Consortium and through LSHTM. The post holder will be expected to contribute, at LSHTM, to Masters and Diploma courses and run specialist short courses including the Travel Medicine Short Course. S/he will contribute to supervision of MSc students and participate in their evaluation and examination of students performance at all levels (Diplomas, MSc, RD), and facilitate research projects at the Hospital for Tropical Diseases and elsewhere. The travel clinic has a national role in providing training in pre-travel medicine for ID specialty registrars; there is a rolling programme of UK ID SpRs who will be coming through the clinic, for a week at a time, under supervision of the travel clinic consultant. 3

New Cancer Centre The Trust s brand new purpose-built University College Hospital Macmillan Cancer Centre houses outpatients and day care cancer services, a dedicated teenage and young adult cancer service, and a private patients facility run by HCA International. The five storey building has been designed with the needs of patients and modern cancer care in mind, with an abundance of natural light, open spaces and a rooftop garden that is open to all. 6 The services we provide UCLH provides high quality accident and emergency, general medical and general surgical services to the people who live, work and visit north central London. In addition, the hospital group is home to many specialist services, which attract patients from all over the UK. Some of the key services are: Cancer services Cardiac services Infectious diseases Neurosciences Oral health Women s health. Our Staff The Foundation Trust has a combined staff of over 8, 000. Around 19% are doctors and dentists, 34% nurses or midwives; 14% are allied healthcare professionals; 9% are scientific and technical staff, and the remainder clinical and general managers and support staff. Key services such as catering, portering and domestic services are provided by private sector partners. Our Patients Every year we treat over 100,000 patients in hospital, either as inpatients or day cases. We also see nearly 700,000 outpatients per year. The majority of our patients are from our closest health authority areas: Camden, Islington, Kensington, Chelsea, Westminster and Haringey though we provide nationwide services and many patients choose to come to us from outside the capital. Further information about the Trust, its strategic direction and hospitals can be found on our website at University College Medical School The Trust is closely associated with University College London (UCL), London s global, multifaculty university. UCL Medical School (UCLMS) is one of the highest rated medical schools in the country and is a Division within the Faculty of Medical Sciences, UCL School of Life and Medical Sciences (SLMS). The academic activity of SLMS is delivered by four Faculties: Brain Sciences (Dean, Professor Alan Thompson), Life Sciences (Dean, Professor Geraint Rees), Medical Sciences (Dean, Professor Mark Emberton) and Population Health Sciences (Dean, Professor Graham Hart). Activity is largely centred on three main sites - the Bloomsbury Campus, the Royal Free Campus and the Whittington Campus. Associated hospitals include Eastman Dental Hospital, Great Ormond Street Hospital for Children, Moorfields Hospital; University College Hospitals, including the National Hospital for Neurology and Neurosurgery; Royal Free Hospital and the Whittington Hospital. There are also strong academic links with many other NHS Trusts in Greater London and the South East. UCLMS provides the curriculum and overview of the teaching for the medical undergraduates. It also offers a wide range of graduate programmes and houses many interdisciplinary research teams. The Medical School has consistently received one of the highest ratings in the UK in the HEFCE research assessments undertaken during the last twelve years. Professor Deborah Gill is Director of UCL Medical School. Professor David 6

7 Lomas is the Vice-Provost (Health) and Head of the Medical School and Professor Anthony Schapira is Vice-Dean and Campus Director (Royal Free Campus). The Trust has an arrangement with the Medical School to provide clinical teaching for the MBBS undergraduate programme, for which purpose it receives NHS funds from the Service Increment for Teaching (SIFT). The successful candidate will have a joint appointment as Associate Professor at the LSHTM and will be expected to participate and contribute to research and to training and examination (Masters, Diploma and short courses programmes, as well as Research Degree students) in the field of Travel Medicine/Public Health/Infectious & Tropical Diseases, consistent with the interests of the LSHTM. The agreement to define the responsibilities and associated programmed activities in relation to teaching time and research will be negotiated jointly between the post holder, LSHTM and the NHS Trust. The annual consultant appraisal procedure will include review of teaching performance, and consideration will be given to joint academic appraisal of the consultant staff/honorary senior lecturers, in the same way as for the clinical academics/honorary consultants. 7

8 3. The Clinical Board, Division & Department The Board The delivery of clinical care at University College London Hospitals is through three clinical Boards: Medicine; Surgery and Cancer; and Specialist Hospitals. The Division of Infection, which includes the Hosiptal for Tropical Diseases and the Travel Medicine Service is within the Medicine Board and the Medical Director is Dr Charles House. Clinical governance is assuming ever greater importance within the NHS and there is strong emphasis on this area. The appointee will be expected to contribute and participate in governance activities in depth and breadth including attendance at the monthly Consultant meetings Division of Infection The specialist consultants at the Hospital for Tropical Diseases (HTD) provide the UK with diagnoses and treatment for all tropical diseases including: parasites, malaria, leprosy, and diseases of the skin, eye, and gut, including children. The hospital also houses a unique NHS based travel medicine service providing advice and prophylaxis to the travelling public, GP and tertiary referrals. The Hospital accepts direct referrals from general practitioners or referrals and transfers from other hospitals, and also has an emergency self-referral service for certain conditions. The Hospital serves long and short-term travellers, immigrants and refugee populations. It also provides the clinical infectious disease service for University College London Hospitals NHS Foundation Trust, including the Outpatient Antimicrobial Therapy (OPAT) service, and provides daily Infectious Diseases liaison support to inpatients across the Trust, and houses the National Travel Health Network and Centre. 4. The Role & Responsibilities The post-holder will be accountable to Dr Vanya Gant, Divisional Clinical Director for the Division of Infection. Job Plan Your job plan is anticipated to contain 6 Programmed Activities per week on average to be carried out on UCLH sites, and 4 programmed activities per week at LSHTM. This is an outline programme and you will be expected to discuss and agree a detailed job plan including your personal and professional objectives with your Clinical Director within 3 months of your start date. You will be required to undertake a prospective diary exercise to inform this process. There will be opportunities for the successful applicant to contribute to wider aspects of the UCLH ID service (clinics and ward cover) if appropriate. 9 service. Acceptance of the offer of employment will be interpreted as mutual agreement of this requirement between the consultant, his or her clinical manager and the Trust. The contract of employment for this position will reflect this requirement. Direct Clinical Care Activities & Other programmed activities: Clinics based at Mortimer market plus undergraduate, postgraduate, and nurse teaching and supervision and local grand rounds and audit meetings. LSHTM Citizenship roles: The post holder will be expected to play an active role in Department and Faculty activities, including involvement in Centres relevant to their research interests, and to encourage and supervise junior colleagues wishing to gain research and clinical experience. Where the post holder does not hold a higher qualification, will be able to, if accepted, join the staff PhD programme. To contribute to the life of the Department, Faculty and the School by participating in other academic duties (Citizenship); To facilitate any other activities that will help to achieve the Department s and Faculty s objectives, as deemed appropriate by the HoD or Dean of Faculty; To contribute to reputation of the School by participating in national/international committees or groups relevant to the individual s expertise, specifically travel medicine. England. Clinical Governance, and other duties and obligations Clinical governance is assuming an ever greater importance within the NHS, and within the Division and Trust. All consultants are expected to contribute to the development of governance in its widest sense. The new post-holders will have an overriding duty of care to patients, and are expected to comply with best practice standards and Trust policies for patient and personal safety. This includes in particular a requirement for compliance with Trust policies on hand hygiene, IV line insertion, taking blood cultures, and other policies for the prevention of healthcareassociated infection (HCAI). In line with HSC 2003/11 it is UCLH Trust policy that clinicians who wish to introduce an interventional procedure that the clinician has not performed before or the clinician has only performed outside the NHS, must seek approval from their Divisional and Medical Directors. The intention to introduce the procedure must then be notified to the Clinical Effectiveness Steering Group. The committee will require assurance that the Divisional Clinical Director has reviewed the safety and efficacy of the procedure if it is not subject to NICE guidance, and that the Lead Clinician has discussed any equipment or staff training issues associated with it with the relevant general manager(s). If the proposed interventional procedure does not appear on the NICE website, the clinician should register it with the Interventional Procedure Programme on the NICE website. The Clinical Effectiveness Coordinator in the Governance department is available to help and advise clinicians wishing to introduce a new procedure and can be contacted on ext On Call Rota If the post-holder undertakes duties involving contribution to the inpatient infectious diseases service she/he will be required to participate in the consultant on-call rota (Category X) for weekdays and weekends. Emergency work arising from on-call duties will be taken into account in assessing the number of programmed activities for on-call in the agreed job plan. 9

10 Category A: this applies where the consultant is typically required to return immediately to site when called or has to undertake interventions with a similar level of complexity to those that would normally be carried out on site, such as telemedicine or complex telephone consultations; Category B: this applies where the consultant can typically respond by giving telephone advice and/or by returning to work later. The successful candidate is also supported by shared use of a divisional Personal Assistant. At the LSHTM (Keppel Street building), the job holder will have access to a desk, computer and administrative support provided by the Departmental Operations Office. 5. Terms and Conditions of Employment The post will be offered under the terms and conditions of service for Consultants [England 2003]. Basic Pay and Pay Thresholds: Basic pay is 76,761 and, subject to satisfactory job planning and review, will rise through annual threshold increases to 103,490 pa. London Weighting is currently 2,162 per annum. Part-time Consultants will be paid pro rata to the thresholds described above, based on the number of agreed weekly Programmed Activities. The starting salary for new consultant appointments will be at the minimum of the payscale in accordance with the terms and conditions. Previous service at Consultant level or equivalent may be taken into account when determining the starting salary; please contact the Consultant Recruitment Adviser for further information. The Trust is entitled at any time to deduct from the post-holder s salary, or any other monies payable to the post-holder by the Trust, any overpayment of salary or wages, underdeduction of charges, over-payment of holiday entitlement or any other sum which the postholder may owe to the Trust and, if this is insufficient, the Trust may require payment of the balance. The Workforce Directorate is responsible for ensuring that the Trust complies with its legal obligation, which includes any prohibition on offering an appointment, and with any requirements stipulated by regulatory bodies. This will include participating in clinical audit and review of outcomes, working towards achievement of national and local performance management targets, complying with risk management policies, and participating in the consultant appraisal process. The post-holder will also be responsible for maintaining satisfactory patient notes and, when relevant, for entering data onto a computer database in accordance with the rules and regulations of the Data Protection Act. Confidentiality All employees and honorary appointees are required to exercise discretion and maintain confidentiality at all times. Conflict of Interest All applicants to any post within the Trust are required to declare any involvement directly with any firm, company or organisation, which has a contract with the Trust. Failure to do so may result in an application being rejected or, if discovered after appointment, that such information has been withheld, this may lead to dismissal. Consultant Terms and Conditions The Terms and Conditions referred to in the contract can be viewed in full on the Trust s intranet site (Human Resources/New consultant contract) or via the NHS Employers website ( Criminal Record In view of the nature of the work this post is exempt from the provision of Section 4 (2) of the Rehabilitation of Offenders Act 1974 by virtue of the Rehabilitation of Offenders Act 1974 (Exemption order 1975). Applicants are, therefore, not entitled to withhold information about convictions including those which for other purposes are spent under the provisions of the Act. You are, therefore, required to declare any pending prosecutions or convictions you may have, even if they would otherwise be regarded as spent under this Action and any cautions. In the event of employment, failure to disclose such convictions could result in dismissal or disciplinary action by the Authority. Any information given will be completely confidential and will be considered only in relation to an application for positions to which the order applies. 11

12 The Trust will check with the police for the existence and content of any criminal record of the successful applicant. Information received from the police will be kept in strict confidence. The disclosure of a criminal record, or other information, will not debar you from appointment unless the selection panel considers that the conviction renders you unsuitable for appointment. In making the decision the Trust will consider the nature of the offence, how long ago and what age you were when it was committed and any other factors which may be relevant, including appropriate considerations in relation to the Trust s published Equal Opportunities Policy. Failure to declare a conviction, caution or bind-over may, however, disqualify you from appointment, or result in summary dismissal/disciplinary action if the discrepancy comes to light. Data Protection In accordance with the Data Protection Act (1998), the Trust is authorised, if required to do so, to obtain, process and/or use information held on a computer in a fair and lawful way. It is further authorised to disclose data only to authorised organisations as instructed. Health and Safety Employees must be aware of the responsibilities placed on them under the Health and Safety at Work Act (1974), to ensure that agreed safety procedures are carried out to maintain a safe environment for employees, patients and visitors. Hepatitis B All employees who perform exposure prone procedures should be immunised against Hepatitis B. Antibody response should be checked on a regular basis. If successful in being appointed to a post, all employees MUST report to the Occupational Health Department within two weeks of starting in post. Failure to comply with this, or the new regulations pertaining to Hepatitis B, may result in an employee being suspended from duty. For further information, either contact the HR Department or the Occupational Health Department. All matters discussed will remain confidential. Insurance Policy The Trust accepts no responsibility for damage to or loss of personal property with the exception of small valuables handed to their officials for safe custody. Staff are therefore recommended to take out an insurance policy to cover personal property. Ionising Radiation Use 12

13 Under the Ionising Radiation (Medical Exposure) Regulations 2000 the Trust is obliged to maintain a register of all persons entitled to act as Practitioners or Operators (i.e. to justify or to carry out a medical exposure) and to keep records of their training. If your post includes the responsibilities of either Practitioner or Operator as defined by these regulations you must provide the Trust with evidence of training. This should include evidence of completion of an approved training course plus details of practical experience. Please note that if, during the course of your duties, you refer a person for a medical exposure you are obliged to provide sufficient relevant clinical information to the Practitioner to justify the use of Ionising radiation. You are expected to follow any guidelines for such referrals, which the Trust provides. Job Sharing UCLH has a job sharing policy under which all posts are open to job sharing, with or without a partner. Health and wellbeing UCLH is committed to proactively consider how we can best encourage and safeguard the health and wellbeing of all our staff. We operate a no smoking policy which does not allow smoking in any of our property or in ground in which it is located.. We have a comprehensive staff support programme for staff who wish to give up smoking and our occupational health team will offer dedicated personal support to any smoker who wishes to give up smoking and/or improve their wellbeing. Private Practice All consultants should adhere to the national Code of Conduct for Private Practice. Private Practice should as far as possible, be undertaken within UCLH facilities, always ensuring that the needs of the practitioner can be met. In this way income generated can be used to further develop patient care within the Trust. The appointee must follow Trust agreed procedures when seeking to commence private practice. Any private practice commitments must not prejudice the basic service requirements contained in the job plan. Appointees expressing an interest in a part time post in order to undertake private practice will normally be expected to hold a 6 programmed activity [PA] contract. Public Transport and Car Parking Our bases are within 5-10 minutes walk of Warren Street, Great Portland Street or Euston stations. Our central London base limits our ability to offer car parking, yet spaces in secure car parking may be available and secure cycle spaces can also be allocated. Residential Criteria A consultant is required to reside within a distance of 30 minutes average traveling time or ten miles by road from their principal place of work unless an employing organisation agrees that they may reside at a greater distance. The Trust continually strives to improve internal communication systems and encourage staff involvement in organisational and service developments including business planning to enable staff to influence discussions which affect them and their working conditions. Staff Nursery/Crèche UCLH has childcare provision at two nurseries for employees. The nurseries are based in the following areas: The Mousehole nursery (23 Devonshire St.) The Fig Tree (4 St. Pancras Way) Places at these nurseries are available for babies and children aged 4 months 5 years. Contribution rates for members vary according to their level of pensionable pay. Contribution rates for an individual on the consultant contract would currently start at 9.9%, but the real cost is lower because of the tax relief on contributions and the lower rate of National Insurance contributions paid as members opt out of the State Earnings Related Pension Scheme.
