stcharleshealthcare.org Responding to Roseburg St. Charles caregiver helps victims in traumatic events Alan Burke It wasn t long after the news broke that a mass shooting had taken place at Umpqua Community College in Roseburg that Alan Burke s phone began to ring. As manager of social work and spiritual care for St. Charles Health System, Burke has many years of experience helping patients, family members and caregivers deal with traumatic events. He s part of a statewide network of social workers and behavioral health professionals who are called on to respond to major tragedies. Within a matter of hours, Burke was on the road to Roseburg to provide what service he could to those in need. You have something of that magnitude in a relatively small community Roseburg is about the size of Redmond it impacts everybody on different levels, Burke said. Umpqua Community College is a vital part of that community. Burke s role was to work with staff members at two different businesses that were located close to the community college one had staff working on campus at the time of the incident. He met with employees whose children had been on campus during the shooting or who had family members who were first responders to the scene. There were people who knew people who were affected either wounded or killed, Burke said. There really isn t anybody who doesn t have, within one or two degrees of separation, a personal link. It was very personal to this community. In those first days after the shooting, Burke said most of the emotions of those he counseled were related to the shock that something so tragic could happen in their town. In addition to shock, many were suffering from grief. His goal in working with victims in these situations is to provide a listening ear, give them a chance to process the situation and help them understand that their reaction is a normal response to an abnormal situation. A gunman opened fire in a classroom at the community college on Oct. 1 killing nine people and injuring seven others before he entered into a shoot-out with police and ultimately took his own life. The incident was the largest mass shooting in Oregon s history. Different organizations try to provide support when you have communitywide disasters or trauma of this nature, Burke said. The Red Cross has its volunteers they bring in. It s not uncommon for local emergency services to have a team that does this. Many businesses, through their benefits packages, have access to resources for debriefing and crisis management. It breaks their sense of safety and they feel much more vulnerable, he said. Everybody has their own life history. These kinds of events tend to bring forward other traumatic or grief-related past experiences. When those previous events haven t been processed, it intensifies it and can be very overwhelming emotionally. Burke provides those in crisis with educational support material and referrals to local support services. The intent of this work is to create and sustain an environment of safety, calming, connectedness to others, empowerment and hopefulness, he said. 3
FLOURISH WINTER 2016 Ferris Hill Back to normal New stroke tool improves outcomes for some patients St. Charles Bend and Redmond achieve Baby Friendly status St. Charles Bend and Redmond have received the prestigious international recognition as Baby Friendly Designated birth facilities. The designation is important because it means a hospital has made a commitment to promote and support breastfeeding which has been shown to improve health outcomes for children over the longterm in its policies and practices. Breastfeeding is important in the prevention of obesity, diabetes and chronic illness, and can significantly change life-long health outcomes. When Beth Wiese, a registered nurse and stroke coordinator at St. Charles Bend, conducted an assessment on Ferris Hill after he suffered a major stroke last February, she said his stroke scale score was one of the worst she had ever seen. He was paralyzed, blind, couldn t swallow, couldn t talk, Wiese said. Dr. Tien (a Bend neurosurgeon) took him to the Cath Lab and retrieved a clot from his brain. The next morning, Wiese walked into Hill s room to check on him and was shocked at the change in his condition. I said, How are you doing? and he said, I m just fine, she said. I get chills just talking about it. The man went home the same as he was before the stroke. Hill s recovery is attributed to Tien s skill and the use of a new tool called the Trevo XP ProVue Retriever a device that can be inserted through an artery and maneuvered into the brain. From there, the neurosurgeon uses the retriever to grab a clot and remove it. The device is particularly useful for stroke patients who don t qualify for clot-busting medications, typically because their stroke has been underway for too long before they reach the emergency room. The next morning the entire hospital was buzzing with the news of (Tien s) accomplishment, Hill said. I was back to normal. All of the paralysis was gone and I was able to talk again. An MRI of his brain showed that Hill had virtually no damage from the stroke. He went home on Saturday two days after his stroke occurred and required no physical therapy for recovery. Hill s outcome is one of the most amazing of any St. Charles stroke patients to date. Wiese explained that while clots have been removed successfully each time the device has been used, some patients do experience deficits and require physical, speech and occupational therapy for recovery. Before Hill went into the Cath Lab for the procedure, Wiese said she talked with his wife, Leah, and explained the risks involved and the possibility that he wouldn t survive no matter what actions were taken. She said, I m not ready to let him go. We re going to go forward with this, Wiese recalled. After returning home, Hill wrote a letter to the staff at St. Charles Bend thanking his caregivers for saving his life. 4
stcharleshealthcare.org Support for new moms Breastfeeding support group in Redmond When Linsay McCullough first started attending the new breastfeeding support group at the St. Charles Center for Women s Health in Redmond, she and her infant son needed some help. He was tongue tied, McCullough said, explaining that due to a short band of tissue Brogan s tongue had a limited range of motion, which can cause problems with breastfeeding. I had a hard time finding anybody who knew how to help. Through the support group, McCullough found resources that made a difference and Brogan is now eating well. At each session, new moms weigh their babies then feed them and weigh them again to have an idea of how much breast milk they are consuming. The transfer weights are a good resource to see how much the baby is taking in at the breast, said Jessica Burke, a registered nurse with experience in labor and delivery who leads the support group. We are certified lactation consultants. If we see a problem, we can try to rectify it. Providing breastfeeding support is one of many focuses for St. Charles Redmond and St. Charles Bend. Both hospitals recently received Baby Friendly designations, a global program sponsored by the World Health Organization and the United Nations Children s Fund. The initiative encourages and recognizes hospitals and birthing centers that offer an optimal level of care for breastfeeding mothers and their babies. Offering these support groups is part of our Baby Friendly designation, said Tricia Clay, clinic manager for the St. Charles Center for Women s Health. When we moved into this space in Redmond we made the commitment we wanted to have more community education. This is one of the first things we wanted to accomplish. For Shandi Taylor, attending the group is a great way to connect with other moms with babies close to the same age as her daughter Harper. I like meeting moms who are going through the same thing, Taylor said. This is my second baby. I ve done it before. But I spaced them out far enough that it s nice to have the resources here, for mom s that need a refresher. The group meets every Tuesday from 12 to 2 p.m. and moms are encouraged to drop in and stay as long as they like. Moms and facilitators share information and stories with each other while ensuring that babies are latching correctly and getting the nutrition they need. I come to the support group with a big dose of empathy I had breastfeeding issues, Burke said. I try to give them hope. When I have a mom that is struggling I try to tell her, You can still be a good mom and feed your baby however you can. There is a lot of pressure with breastfeeding. With Baby Friendly, we are doing a lot to help those moms who are struggling. 5

FLOURISH WINTER 2016 Time-honored tradition Annual hospice Christmas auction a community affair Saints Gala nets more than $200,000 Thanks to many generous community donations, the annual St. Charles Foundation Saints Gala netted more than $200,000 this year. More than $80,000 came from the paddle raise and will go to support stroke rehabilitation services. At the event, Dr. John Zachem, a St. Charles Medical Group hospitalist, received the North Star Award. The award honors a local physician whose work has been defined by compassionate patient care, professionalism and a commitment to improving the lives of Central Oregonians. Jim Puckett brought 74 people to the annual hospice Christmas auction in Prineville this year. The event was in its 24th year and this was Puckett s 24th time attending. It s the best organization that s going, Puckett said simply of his dedication and support. I like their results. The auction is a heartwarming event featuring artfully decorated Christmas trees, handmade quilts and many other items for bid. Over the decades it has grown into a special night that many in the Prineville community look forward to each year. Two years ago it was 23 below zero, said Jeff Robberson, of Robberson Ford, which serves as title sponsor. I thought, This is just going to kill the attendance at the event. But no, that community just put on another layer of clothes and showed up. It amazes me how resilient they are. Looking around the sold-out room this year, Luanne Stephenson said along with representatives from the car dealership she saw tables with people from the agricultural industry, another with hospice staff members and several representing businesses throughout town. People always have a good time, she said. Everybody likes to get together for a party. As one of the volunteers helping to organize the auction, Stephenson said she got involved after her mother benefitted from hospice services. Once she retired, she wanted to give back to an organization that she knew provided an important service. I don t know of many people that haven t been touched in some way by hospice, she said. There is no socioeconomic barrier against the need for hospice and this event is basically to fund hospice care for those who can t afford it. This year, the event set a record by netting more than $100,000. The highest bid of the evening was $10,000 for one quilt honoring the work of Dr. Michael Knower. One of the decorated Christmas trees made in memory of Sharon Krider went for $9,000 and was generously donated back to the Krider family. Cost should never be a reason why a patient is denied hospice care, said Sharon Strohecker, director of St. Charles Hospice. For those in our community without insurance, this event provides funds that are life-changing for patients and their families during their most difficult times. We are all grateful for every dollar raised. 8
stcharleshealthcare.org Southern expansion St. Charles plans to build Family Care clinic in La Pine In La Pine, Sunriver, Gilchrist, Chiloquin, Chemult, Christmas Valley and Fort Rock, many people drive 30 minutes or more just to see their primary care provider for routine medical care. St. Charles Foundation and St. Charles Medical Group are hoping to change that in the near future. The Foundation is actively working on a fundraising campaign to support the opening of a St. Charles Family Care clinic in La Pine. We conducted a feasibility study earlier this year and found that because people don t have access to medical care in the area during the evening and weekend hours, even minor injuries or illnesses often result in unnecessary ambulance rides and emergency room visits, said Lisa Dobey, executive director of St. Charles Foundation. Our goal is to provide enhanced access to care for the 25,000 people living in the southern Deschutes and northern Klamath and Lake counties. A location has not yet been identified for the clinic, but the plan is to open by summer 2017 and provide primary care, immediate care, radiology/ imaging services, lab services and space for visiting specialists. From our study, we learned that far too many people go without preventive care or delay their treatment because of the difficulty in accessing services, said John Jepson, senior philanthropy officer for St. Charles Foundation. When they have to take time away from work and school it also results in high costs for individuals and affects the community as a whole. Corinne Martinez, the co-chair of the campaign and a St. Charles Foundation board member, said that as the owner of a local business she has employees with young children and if someone gets sick or injured it can take hours out of their day to seek care in Bend. It s a real issue down here for services, Martinez said. It affects the elderly too. In the wintertime they have to be so careful about driving that they don t go to the doctor. They will put it off. The clinic will follow the medical-home model adopted for all St. Charles Family Care locations. The model includes an embedded behavioral health specialist to work directly with patients and primary care providers on behavioral health needs along with physical health concerns. The model also includes pharmacists to help patients understand their medications and to consult with providers on potential medication conflicts, and nurse care coordinators to work with patients on chronic health needs. Community health workers also assist patients with social and environmental needs. We know that when patients have access to a multidisciplinary team of providers who all have a vested interest in keeping them healthy, it makes a difference, said John Weinsheim, CEO of St. Charles Medical Group. We are looking forward to bringing more services to La Pine and the surrounding communities. For more information or to donate, please contact John Jepson, senior philanthropy officer with St. Charles Foundation, at
FLOURISH WINTER 2016 Under construction St. Charles Madras renovation and expansion begins New Year After Loss workshop St. Charles Hospice is offering a free, half-day workshop for those entering the New Year in the midst of grief and loss. The workshop will take place from 9:30 to 11:30 a.m., Jan. 16 at St. Charles Redmond in the Ponderosa Conference Room. It is open to all who have lost a loved one. Space is limited. Please pre-register by calling Patients and visitors to St. Charles Madras may have started to notice the signs of a construction project in progress. The hospital is undergoing a $16 million renovation and expansion that will add 26,000 square feet to the facility and will enhance outpatient and primary care services. We are so excited that this project is underway because it will bring such an incredible benefit to our community and the patients we serve, said Jeanie Gentry, CEO of St. Charles Madras and St. Charles Prineville. We hope that our patients will be patient with us as we may have more noises in the halls than normal while we work to make our facility the best it can be. Patients will be given earplugs to help with any construction noise that may be bothersome as they recuperate from surgeries or illnesses. Signage will be posted at entrances to direct families and visitors around construction zones. And caregivers will be available to provide assistance if needed. The project will take place in multiple phases. The addition will include a new main entrance, a combined operating room and emergency department, a new imaging department and laboratory. HVAC, plumbing and electrical upgrades will also take place in the existing facility along with other improvements. Making a significant investment in the facility which was built in 1967 and does not meet current standards of care was part of the agreement between St. Charles Health System and Mountain View Hospital District when the two organizations completed an asset transfer in late The hospital became St. Charles Madras in January If we had not gone through with the asset transfer, we would simply not have the resources to make these important updates to our facility so that we can continue providing our community with the care it deserves, Gentry said. I m looking forward to sharing the upgrades with patients as they are complete and know that this investment will make a difference to our community for many generations to come. 10
stcharleshealthcare.org Dr. Natalia Usatii talks with a patient Pediatric sleep medicine St. Charles Sleep Center welcomes Dr. Natalia Usatii Sleep, says Dr. Natalia Usatii, is the sister of mood. Without enough high-quality sleep, parents will notice a difference in the behavior of their children. The newest member of the St. Charles Sleep Center team, Usatii focuses on pediatric patients and enjoys helping parents with lifestyle changes that can make a big difference in developing healthy sleep habits. It s very simple. Have a set bedtime and wake time, Usatii said. It should be set every day regardless of weekends or weekdays. This is a must. You have to make sure there is a routine. The bedtime routine for kids should last between 30 and 40 minutes and can include brushing teeth, putting on pajamas and reading stories. The television should be off during the bedtime routine, giving children time to relax. Usatii strongly recommends against families sharing a bed and says that diet plays a huge factor in the quality of sleep for children. Ingesting too much sugar or caffeinated beverages can make it difficult for the body to calm down and go to sleep. Sleep occurs in four stages and it is critical that all four stages are achieved for high-quality sleep to occur. Deep sleep is important for the body to completely relax and to restore energy level and physical capacity for the next day, Usatii explained. In addition, rapid eye movement or REM sleep is important for memory, concentration and high-level executive functions of the brain. Medications given to aid in sleeping are not designed to break sleep into the four stages, Usatii said. She believes that lifestyle changes and the ability to develop strong sleep habits may be more valuable than medication for treatment in many cases. If the child is irritable, starts to fail classes or is snoring and gasping for breath, parents should be concerned, she said. The child may be having some breathing issues during sleep. During the day, if their regular activities are impaired, go to your primary care provider for a referral to a sleep doctor. To learn more or to make an appointment, visit For kids in particular, sleep is important for the development of the brain, Usatii said. When they sleep enough, the quality of the sleep must also be good. 11
