2 Community Services in Pembrokeshire What we do Our aims and principles for community nursing services in Hywel Dda University Health Board are to provide patient focused holistic care closer to home by a flexible and skilled workforce, maximising patient independence, improving health outcomes and quality of life. Core services are provided seven days a week and include the promotion of self care, health and wellbeing, assessment of complex care needs, management of acute and chronic conditions, provision of palliative and terminal care, assessment and management of complex wound care, and administration of prescribed treatment regimes among others. Our community services South Pembrokeshire Hospital, Health & Social Care Resource Centre South Pembrokeshire Hospital, Health & Social Care Resource Centre (SPH) offers a variety of outpatient services and clinics to patients, including; continence, AAA screening, diabetic retinopathy, newborn audiology, post cardiac clinic, speech and language therapy, leg ulcers, dietetics, multiple sclerosis, smoking cessation, diabetes, lymphoedema, radiology, community occupational therapy, clinical musculoskeletal assessment and treatment (CMAT), community physiotherapy, ophthalmology wet AMD and clinical nurse specialists in heart failure, oxygen and palliative care. Sunderland Ward is a community ward within SPH, comprising a number of health and social care beds which continue to support patients requiring inpatient care admitted from the community and transfers from district general hospitals. Sunderland Ward accepts patients into healthcare beds for a variety of health conditions, including patients with cognitive impairment/dementia, frailty, palliative and end of life care needs, complex wound care management, orthopaedic, stroke, post operative and general active rehabilitation, step down medical care, respite/convalescence beds, as well as admit to assess and complex discharge planning care. Cleddau River Day Unit (CRDU) provides a multi-disciplinary team and environment, which aims to: 2

3 provide support, tailored to meet individual need; help people achieve maximum independence; provide advice, treatment and care. This integrated day care and rehabilitation services is available for adults five days per week and provides rehabilitation and re-ablement, with access to a range of therapies and support services. CRDU is a facility that is responsive to individual needs, incorporating the following elements: assessment; care management planning and implementation; evaluation of care; best practice. Tenby Cottage Hospital Tenby Cottage Hospital provides a wide range of outpatient services for patients, including; orthopaedic clinic, chest medicine, medical cardiac, Parkinsons, frailty clinic, gynaecology clinic, antenatal clinic, AAA screening, nurse-led pain clinic, CMAT, continence service, nurse-led respiratory clinic, dietician clinics (general and plus size), paediatric physiotherapy, diabetic retinopathy, plaster technical, leg ulcer clinic, podiatry, speech and language therapy, audiology, multiple sclerosis, epilepsy, smoking cessation, lymphoedema service, general physiotherapy and physiotherapy-led balance group satellite service from SPH, and also care provided by midwives, a cardiac nurse, heart failure nurse and paediatric specialist nurse. District Nursing Our community nursing services are provided by teams of community based registered nurses and health care support workers led by qualified district nurses, providing both assessment and care management for people that need unscheduled, long or short term care, as well as palliative and terminal care. This is provided principally within the patient s own home (which includes residential care settings) or where it is most appropriate, based on service provision/clinical judgment. 3

4 The service promotes healthier lifestyles, physical, psychological and social wellbeing, encouraging and supporting people with disability and chronic conditions to live as independently as possible. Services provided include: Palliative and terminal care Wound care management Complex needs assessment Pressure damage care management Continence and catheter care Leg ulcer clinics Community Clinical Nurse Specialists, Nurse Practitioners and Advanced Nurse Practitioners Within Pembrokeshire, we currently have a number of clinical nurse specialist (CNS), nurse practitioner (NP) and advanced nurse practitioner (ANP) roles working in community settings with the aim of optimising care management, admission avoidance and facilitating discharge in specific patient areas/specialities which include: Heart Failure Chronic conditions Frailty Palliative care Complex wounds and leg ulcer management Continence Multi-Assessment Support Team (MAST) The Multi-Assessment Support Team (MAST) based within the Emergency and Urgent Care Centre (A&E) at Withybush Hospital exists to facilitate discharge in patients identified with complex health and social care needs, but where acute admission is preventable through the provision of supportive measures in the community. This multi-disciplinary team includes district nurses, physiotherapists, occupational therapists and social workers. Acute Response Team (ART) Pembrokeshire s Acute Response Team (ART) operating from Withybush Hospital exists to prevent admission and facilitate discharge through the delivery of acute nursing interventions within the patient s home, e.g. intravenous antibiotics and fluids. 4

We provide support for staff development, including a 12 month structured preceptorship programme for newly qualified nurses, a specific community focused competency programme, support in developing into an experienced nurse, role model for others and advanced practice roles. There are also opportunities for developing areas of specific interest and actively contributing to practice development within clinical areas. We are located in an area that provides a great mix of both town and country living.
