3 The Health System in Muskoka : Collaborative Partner Steering Committee Formed : Community outreach, engagement and input into Business Plan Early 2014: Implementation commenced with approval by MOHLTC and NSM LHIN Target: Better coordinate care to meet the needs of individuals living with complex and chronic conditions that require higher volumes and costs of care Muskoka Community Health Link Muskoka Community Health Hubs : Community outreach, engagement and insight to shape a Made in Muskoka Solution 2013: Linked and partnered with Health Link Business Plan submission with formation of Muskoka Health Hubs Implementation Committee Late 2014: Approval received by MOHLTC, 3 year demonstration 2015: Implementation and Operationalization commenced Target: Enhance and expand access to primary care, community and social services across Muskoka, reaching more isolated and vulnerable populations : Planning, community engagement, outreach, & completion of Master Plan / Master Program Oct. 2015: MAHC Pre- Capital Submission endorsed by NSM LHIN Nov. 2015: NSM LHIN approved establishment with first meeting of the MAHC Capital Planning Task Force Nov. May 2016: Task Force continued to meet for purpose of defining a common future vision for acute care services Muskoka Algonquin Healthcare

6 A System Solution Ambitious project to redesign the local health care system serving Muskoka and Area. Unique opportunity to generate a person-centred model for health care and a plan to implement those changes designed by people who live in Muskoka, experience the current system and best understand local needs MAHST Council has been appointed by NSM LHIN to oversee a grassroots design process that engages health, social services and community stakeholders across the continuum of care
